Income-related inequalities in ACSC hospitalization in Canada: evidence from 2005-2019 surveys linked with hospitalization and tax data.

This study examines income-related inequalities in hospitalizations due to ambulatory care-sensitive conditions (ACSCs) in Canada, using linked data from the Canadian Community Health Surveys (2005-2019), the Discharge Abstract Database, and the T1 Family File across nine provinces. We quantify inequalities in ACSC hospitalizations using the Erreygers concentration index (ECI) and ACSC hospitalization costs using the standard concentration index (CI). The recentered influence function-ordinary least squares estimator (RIF-CI/ECI-OLS) was used to decompose income inequalities in ACSC hospitaliz

Why are voluntary health insurance holders willing to pay tax towards public healthcare? A qualitative study from Sweden.

BACKGROUND: Voluntary health insurance (VHI) is increasing in many European healthcare systems, including the tax-funded public healthcare systems found in Northern Europe. As private alternatives to publicly financed health services expand, there is a risk that the willingness to pay tax towards redistributive public healthcare systems will diminish. This study investigated how employer-funded VHI holders accounted for their continued willingness to pay income tax to suppor the public healthcare system and the motivation underlying their tax compliance. METHODS: A qualitative approach based

Linking the Nursing and Residential Care Facility Survey to tax and census data: A new data resource for health workforce research.

Calls for comprehensive data collection on the health workforce and long-term care sector have been voiced by policy makers, researchers, and public health experts following the aftermath of the COVID-19 pandemic and ongoing health care worker shortages. This paper introduces a novel linked data resource that fills these critical data gaps and establishes a foundation for evidence-informed policy making and workforce planning to strengthen the nursing and residential care system in Canada. This data resource integrates the Nursing and Residential Care Facility Survey, the Canadian Employer-Emp

Take-up of the 2021 child tax credit expansion among disadvantaged households in California: Continuing barriers to health equity.

BACKGROUND: In 2021 Congress expanded the Child Tax Credit (CTC), one of the largest US economic supports for working families with proven benefits for health. Information on CTC take-up among eligible families is lacking. Understanding barriers to anti-poverty programs is an urgent policy issue, as low take-up is an established barrier to health equity. METHODS: Among a longitudinal cohort of primarily Latino and Black California families with low income (N = 380), we used data from individuals' 2019 and 2021 tax returns to compare CTC take-up (i.e., receipt among eligible individuals) pre-

Understanding the use of media analysis in public health research through food tax debates (HEALTHEI Project): a scoping review.

BACKGROUND: Poor diet is a major public health concern. In 2021, 63·8% of adults and 22·2% of reception-age children were either overweight or obese in England. Fiscal interventions have become a popular policy measure to reduce obesity and encourage healthy eating. Such measures are highly controversial, leading to media debate promoting pro-tax and anti-tax arguments. To better understand food tax debates and the use of media analysis in public health research, we conducted a scoping review of media analyses using food taxes as a case study. METHODS: In this scoping review, we searched SCOP

The Association between the Australian Alcopops Tax and National Chlamydia Rates among Young People-an Interrupted Time Series Analysis.

A national tax increase, which became known as the "alcopops tax", was introduced in Australia on the 27th April 2008 on ready-to-drink alcoholic beverages, which are consumed predominantly by young people. The affordability of alcohol has been identified as the strongest environmental driver of alcohol consumption, and alcohol consumption is a well-known risk factor in the spread of sexually transmitted infections via its association with sexual risk-taking. We conducted a study to investigate whether there was any association between the introduction of the tax and changes in national chlamy

Tax and Semaphorin 4D Released from Lymphocytes Infected with Human Lymphotropic Virus Type 1 and Their Effect on Neurite Growth.

Human lymphotropic virus type 1 (HTLV-1) is a retrovirus causing HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP), a neurodegenerative central nervous system (CNS) axonopathy. This virus mainly infects CD4(+) T lymphocytes without evidence of neuronal infection. Viral Tax, secreted from infected lymphocytes infiltrated in the CNS, is proposed to alter intracellular pathways related to axonal cytoskeleton dynamics, producing neurological damage. Previous reports showed a higher proteolytic release of soluble Semaphorin 4D (sSEMA-4D) from CD4(+) T cells infected with HTLV-1. S

LTR point mutations in the Tax-responsive elements of HTLV-1 isolates from HIV/HTLV-1-coinfected patients.

BACKGROUND: In Virology Journal 2011, 8:535, Neto et al. described point mutations into Tax-responsive elements (TRE) of the LTR region of HTLV-1 isolates from asymptomatic carriers from Sao Paulo, Brazil, and hypothesized that the presence of the G232A mutation in the TRE-1 increase viral proliferation and consequently the proviral load (PvL), while the A184G mutation in the TRE-2 do not have such effect. FINDINGS: We performed the real-time PCR assay (pol) and sequenced LTR region of HTLV-1 isolates from 24 HIV/HTLV-1-coinfected patients without HTLV-1-associated diseases from the same geog

c-Maf suppresses human T-cell leukemia virus type 1 Tax by competing for CREB-binding protein.

Latent infection of human T-cell leukemia virus type 1 (HTLV-1) is considered to be preferentially associated with CCR4(+) CD4(+) T cells. Here we report that c-Maf, one of the critical transcription factors for Th2 differentiation, suppresses the transcriptional activity of HTLV-1 Tax by competing for CREB-binding protein. Notably, c-maf expression is selectively induced in a fraction of CCR4(+) CD4(+) T cells upon activation. Furthermore, c-Maf significantly decreases Tax-induced HTLV-1 envelope gp46 gene expression from an infectious HTLV-1 molecular clone and tax expression in a cell-free

The HTLV-1 tax protein cooperates with phosphorylated CREB, TORC2 and p300 to activate CRE-dependent cyclin D1 transcription.

Adult T-cell leukemia/lymphoma is a fatal malignancy etiologically linked to infection with the human T-cell leukemia virus (HTLV-1). The virally encoded oncoprotein Tax activates the transcription of HTLV-1 and cellular genes by cooperating with cellular transcription factors. Cyclin D1 is a pivotal regulator of cell cycle progression, and increased expression strongly correlates with malignant transformation. Here, we characterize the mechanism of Tax transactivation of cyclin D1. We find that cyclin D1 transcript levels are elevated in HTLV-1 infected cells and that Tax physically associate

HTLV-1 Tax protects against CD95-mediated apoptosis by induction of the cellular FLICE-inhibitory protein (c-FLIP).

The HTLV-1 transactivator protein Tax is essential for malignant transformation of CD4 T cells, ultimately leading to adult T-cell leukemia/lymphoma (ATL). Malignant transformation may involve development of apoptosis resistance. In this study we investigated the molecular mechanisms by which HTLV-1 Tax confers resistance toward CD95-mediated apoptosis. We show that Tax-expressing T-cell lines derived from HTLV-1-infected patients express elevated levels of c-FLIP(L) and c-FLIP(S). The levels of c-FLIP correlated with resistance toward CD95-mediated apoptosis. Using an inducible system we demo

Transcriptional activation of survivin through the NF-kappaB pathway by human T-cell leukemia virus type I tax.

Survivin, a unique member of the inhibitor of apoptosis protein family, is overexpressed in many cancers and considered to play an important role in oncogenesis. We previously reported the survivin expression profile in ATL, a CD4-positive T-cell malignancy caused by HTLV-I. HTLV-I Tax is thought to play an important role in immortalization of T cells. We have shown also that the expression of Tax protected the mouse T-cell line CTLL-2 against apoptosis induced by deprivation of IL-2 and converted its growth from being IL-2 dependent to being IL-2 independent through the NF-kappaB pathway. Functional inactivation of p53 by human T-cell leukemia virus type 1 Tax protein: mechanisms and clinical implications.

Human T-cell leukemia virus type 1 (HTLV-I) has been implicated with the etiology of adult T-cell leukemia (ATL) and certain other clinical disorders. Although the leukemogenic mechanism of HTLV-1 is not fully understood yet, the viral Tax protein is widely regarded as a key factor in this mechanism. Tax can modulate the synthesis or function of many regulatory factors which control a wide range of normal and oncogenic cellular processes and therefore, it acts as a potent oncoprotein. In the last few years, special attention has been attracted to Tax interference with the transactivation funct

Interleukin-13 overexpression by tax transactivation: a potential autocrine stimulus in human T-cell leukemia virus-infected lymphocytes.

The human T-cell leukemia virus type 1 (HTLV-1) Tax oncoprotein induces growth transformation and is critical for the pathogenesis of the HTLV-1-induced adult T-cell leukemia (ATL). It stimulates the cell cycle and transactivates cellular genes. Here we show that the expression of interleukin-13 (IL-13) is upregulated as a consequence of Tax in HTLV-1-transformed T cells and ATL-derived cultures. IL-13 exerts proliferative and antiapoptotic functions and is linked to leukemogenesis, since it stimulates Hodgkin lymphoma cells by an autocrine mechanism. Overexpression of IL-13 RNA and protein wa

Cross-reactivity between immunodominant human T lymphotropic virus type I tax and neurons: implications for molecular mimicry.

Human T lymphotropic virus type I (HTLV-I)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) is associated with immunoreactivity to HTLV-I tax. Antibodies isolated from patients with HAM/TSP and monoclonal antibodies (MAbs) to HTLV-I tax stained neurons. In neuronal extracts, HAM/TSP immunoglobulin G identified heterogeneous nuclear ribonucleoprotein A1 (hnRNP A1) as the autoantigen. Importantly, tax MAbs reacted with hnRNP A1. To identify the epitope recognized by the tax MAbs, the fine epitope specificity of the antibodies was determined using overlapping peptides. This analysis i

Rat primary T cells expressing HTLV-I tax gene transduced by a retroviral vector: in vitro and in vivo characterization.

We prepared a recombinant retroviral vector expressing the human T-lymphotropic virus type-I tax gene. Infection of WKA/H rat splenocytes yielded T-cell lines which proliferated continuously in media supplemented with exogenous interleukin-2 (IL-2) after the control cells ceased to grow. The phenotype of these cells closely resembled that of typical adult T-cell leukemia cells and tax-immortalized human T cells; i.e., positive for CD3, CD4 and IL-2 receptor alpha-chain. Chromosomal analysis revealed that about 10% of the tax-transduced T cells had several chromosomal abnormalities. We also per

Developing a novel conceptual model of how food and drink tax policy impacts on consumption, health, environmental and economic outcomes.

BACKGROUND: While food taxes are proposed to promote healthier dietary choices and mitigate diet-related diseases, addressing rising UK obesity requires navigating a highly complex web of causal mechanisms. Economic evaluations of these interventions often lack a documented understanding of this systemic complexity. To bridge this gap, this study aims to comprehensively map the system by identifying the specific components affected, the causal pathways (mechanisms), and the individual and system-level factors (context) influencing the ultimate impact of food taxes. METHODS: A diagrammatic rep

Cost-effectiveness of CPAP treatment in a tax-funded healthcare system - a scoping review and suggestions for cost reduction of CPAP treatment.

AIMS: Obstructive sleep apnoea (OSA) is an increasing global public health problem. The primary treatment option is continuous positive airway pressure (CPAP). The cost-effectiveness of CPAP in patients with OSA is incompletely characterized. METHODS: For this scoping review, we conducted a PubMed search for cost-effectiveness analysis of CPAP treatment in European countries with tax-funded healthcare systems and for clinical practices to reduce costs of CPAP without reducing treatment quality. OSA severity was classified by the overnight apnoea-hypopnea index (AHI). The primary outcome was t

Lobbying against tobacco tax increases in France: arguments and strategies of the tobacco industry and tobacconists analysed through their trade press.

INTRODUCTION: Raising taxes is one of the most cost-effective measures to reduce tobacco use. France has a unique profile: it has high tobacco use prevalence and a state monopoly on tobacco sales for tobacconists who are both agents of the customs administration and a recognised tobacco industry (TI) front group. In this paper, we investigate the lobbying tactics and arguments against tobacco taxation mobilised by the TI and tobacconists in France. METHODS: We conducted a quantitative and qualitative analysis of the two leading French tobacco trade media outlets (La Revue des Tabacs and Le Lo

"It's confusing as hell": a qualitative study of barriers accessing poverty alleviation benefits through the US Earned Income Tax Credit.

BACKGROUND: The Earned Income Tax Credit (EITC) is the largest US poverty alleviation program, with positive effects on health equity. Over 20% of eligible households do not receive benefits. METHODS: In this qualitative study, we explored benefits of and barriers to EITC receipt among EITC-eligible families through analysis of 40 semi-structured interviews with EITC-eligible parents in California using the immersion-crystallization method. RESULTS: We identified 3 categories of findings: (1) perceived benefits of EITC; (2) administrative barriers leading to low EITC take-up among eligible f
