Occupational repetitive strain injury.

Repetitive strain injuries include a group of disorders that most commonly develop in workers using excessive and repetitious motions of the neck and upper extremity. A careful occupational history, physical examination and specific diagnostic maneuvers can help distinguish these musculoskeletal injuries from rheumatologic diseases, psychologic disorders, acute joint or tendon inflammation from other causes and single-event traumatic sprains and strains. Repetitive strain injury may be manifested by cervical syndrome, tension neck syndrome, thoracic outlet syndrome and frozen shoulder syndrome

Carpal tunnel syndrome caused by synovial osteochondromatosis of the finger flexor tendon: A case report.

RATIONALE: Carpal tunnel syndrome (CTS) is the most common peripheral nerve neuropathy resulting from compression of the median nerve as it traverses the carpal tunnel. The pathophysiology of this condition is multifactorial, and majority of cases of CTS are idiopathic. We report cases of CTS caused by synovial osteochondromatosis (SOC), which has rarely been reported. PATIENT CONCERNS: A 45-year-old female was admitted to the clinic due to right hand tingling sensation for 4 months. On physical examination, the patient's symptoms and signs corresponded to the median nerve entrapment at wrist

Non-traumatic paralysis of the posterior interosseous nerve.

We treated 31 patients with non-traumatic paralysis of the posterior interosseous nerve over 15 years. There were 10 men and 21 women of mean age 40.3 years (17 to 71). Six were managed conservatively, and 25 by operation. In 14 patients entrapment occurred at the supinator, including three who had double compression at both the entrance and exit from the muscle. In four it was caused by a ganglion, in one by a lipoma, in one by a dislocated radial head and in two by a marked constriction in the nerve of unknown cause. The remaining three patients were retrospectively diagnosed as having neura

Posttreatment Imaging of the Wrist and Hand: Update 2022.

Common indications for surgical procedures of the wrist and hand include acute fractures or fracture-dislocations; nonunited fractures; posttraumatic, degenerative, and inflammatory arthritides and tendinopathies; injuries to tendons, ligaments, and the triangular fibrocartilage complex; and entrapment neuropathies. Soft tissue or osseous infections or masses may also need surgical treatment. Several of these procedures require surgical hardware placement, and most entail clinical follow-up with periodic imaging. Radiography should be the first imaging modality in the evaluation of the postope

Incarceration of the Posterior Tibial Tendon in an Isolated Comminuted Medial Malleolus Fracture.

Isolated medial malleolar fractures are a less common presentation of an ankle fracture. Treatment is not universally accepted, although many have agreed that any displacement warrants anatomic reduction and fixation. We present a case of an isolated, comminuted medial malleolar fracture that was displaced secondary to entrapment of the posterior tibial tendon between the fracture fragments requiring surgical intervention. The patient was treated with prompt open reduction and internal fixation and had an excellent functional outcome at 1 year. When open reduction and internal fixation of the

Ultrasonography of the hand and wrist.

Musculoskeletal ultrasonography (US) of the hands and wrist has recently been increasing in popularity. Recent rapid technical advances in the US, such as new ultra-high frequency probes and smaller probe sizes, have led to improved image quality. This, in turn, has accelerated the growth of musculoskeletal US. Known advantages of US are its lack of ionising radiation, noninvasiveness, portability and low cost. Dynamic and real-time assessment and Doppler imaging are additional benefits of this modality, especially in the imaging of the hands and wrist. Superficial structures of the hands and

Anterior tarsal tunnel syndrome.

Anterior tarsal tunnel syndrome is a rarely reported entrapment neuropathy of the deep peroneal nerve under the extensor retinaculum at the ankle. The roof of the tunnel is the inferior extensor retinaculum. The floor is the fascia overlying the talus and navicular. Within the tunnel are four tendons, an artery, a vein, and the deep peroneal nerve. Two patients with foot pain and dysesthesias had prolonged peroneal distal latencies with reduced amplitudes from the extensor digitorum brevis (EDB). Electromyographic (EMG) abnormalities were confined to the EDB. Both patients underwent surgical d

High-Resolution US of the Radial Wrist: Anatomy, Variants, and Clinical Correlates.

US has emerged as a valuable imaging tool for evaluating musculoskeletal abnormalities, including radial wrist conditions. It offers high-resolution real-time assessment of joints, ligaments, tendons, nerves, and other soft-tissue structures in the region. Yet, the complex anatomy of the wrist, frequent anatomic variants, and often nonspecific clinical symptoms present substantial challenges for the clinician and the US practitioner alike. This comprehensive review provides a structured and practical approach to US evaluation of the radial wrist, designed to enhance diagnostic confidence and a

Shoulder ultrasound: current concepts and future perspectives.

Ultrasonography is an established and effective imaging technique that can be used to evaluate articular and periarticular structures around the shoulder. It has been shown to be useful in a wide range of rotator cuff diseases (e.g. tendon tears, rotator cuff calcific tendinopathy and bursitis) as well as non-rotator cuff abnormalities (instability, synovial joint diseases and nerve entrapment syndrome). A scanning protocol is highly recommended to reduce the rate of operators' errors by following a standardized scheme including a list of main structures. Shoulder ultrasound has several advant

Editorial Commentary: Proximal Hamstring Syndrome: Another Pain in the Buttock.

Proximal hamstring injuries cause buttock pain and may result in significant weakness and sciatic neuralgia. Avulsion ruptures involving 2 or more tendons with >2-cm retraction may benefit from early open surgical repair. More chronic equivalents may benefit from Achilles allograft reconstruction. Chronic proximal hamstring syndrome causing posterior thigh and sit pain may occur from less severe, often repetitive injury to the proximal hamstring origin with secondary sciatic neuralgia from local adhesions or scar entrapment. Recalcitrant cases may benefit from surgical intervention, but the mo

Changes in tissue substance P levels in patients with carpal tunnel syndrome.

BACKGROUND: Although carpal tunnel syndrome (CTS) is the most common entrapment neuropathy in adults, its etiology is not completely known. Chronic inflammation, fibrosis of the transverse carpal ligament (TCL), and altered sensory response contribute to the symptoms. OBJECTIVE: Because substance P (SP) is known to be involved in neuropathic pain, chronic inflammation, and fibrosis, the present study evaluated changes in SP levels in patients with CTS. METHODS: TCL, median nerve adventitia, and synovial connective tissue of the middle flexor digitorum superficialis tendon samples from patien

Open knee dislocation, triple intra-articular fractures and patellar tendon rupture: case report of a knee disaster treated with aggressive irrigation/debridement, early anatomic reduction and internal fixation.

BACKGROUND: Open knee fracture-dislocation is a rare orthopedic injury. However, the importance of its correct management could not be overstated. To the best of our knowledge, this is the fifth study reporting a case with simultaneous Hoffa fracture and knee dislocation and the 1st study describing a patient with open plateau fracture-dislocation accompanied with Hoffa fracture, patella fracture, and patellar tendon tear. In addition, this report is noticeable as our case had no gross ligament injury unlike frequent association of knee dislocation with knee collateral ligament damage. CASE P

The quantitative evaluation of the relationship between the forces applied to the palm and carpal tunnel pressure.

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy occurring in upper limbs. The etiology, however, has not been fully understood yet. Median nerve could be compressed by either increase of carpal tunnel pressure (CTP) or direct impingement when it is forced toward to carpal ligament especially in wrist flexion leading to CTS development. Thus, the increase of carpal tunnel pressure is considered an important role in CTS development. It has been identified that forces applied to the palm would affect the CTP. However, the quantitative relationship between palmar contact forc

A radiological sign (which we are calling the 'tongues of flame' sign) in irreducible trimalleolar fractures of the ankle.

BACKGROUND: The irreducible trimalleolar fracture case we describe is different from the Bosworth fracture, defined as a fixed posterior fracture-dislocation of the distal part of the fibula, in which the proximal fibular shaft fragment locks behind posterior tibial tubercle. This fracture is frequently irreducible by closed means as well. METHODS: We describe a rare case of trimalleolar fracture/dislocation that cannot be reduced by closed means, and review the relevant literature. RESULTS: The peroneal tendons were entrapped in a triangle between a postero-lateral displaced distal fibular

Ischiofemoral impingement in an 11-year-old girl.

Ischiofemoral impingement (IFI) is the entrapment of the quadratus femoris muscle (QFM) between the trochanter minor of the femur and the ischium-hamstring tendon. Patients with IFI generally present with hip pain, which may radiate toward the knee. Although there is no specific diagnostic clinical test for this disorder, the presence of QFM edema/fatty replacement and narrowing of the ischiofemoral space and the quadratus femoris space on magnetic resonance imaging (MRI) are suggestive of IFI. The optimal treatment strategy of this syndrome remains obscure. Patients may benefit from a conserv

[Sonography of the hand and wrist].

In the recent years US has gained wide diffusion as a useful imaging tool in the evaluation of musculoskeletal disorders. Recognized advantages of US over the other imaging modalities are low costs, non-invasiveness, possibility to perform a dynamic examination and readiness. Unfortunately, US has a long learning curve and allows good results only when performed by well-trained and experienced examiners. Specifically, utilization of the recent small transducers allows to perform dynamic examination of the hand during flexion-extension movements of the fingers, while their high frequency (till

Secondary Carpal Tunnel Syndrome Due to Flexor Tenosynovitis in a Diabetic Patient.

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy of the upper limb. Although many cases are idiopathic, structural or inflammatory pathology of the flexor tendon sheath can result in secondary median nerve compression. Diabetes mellitus predisposes to such changes due to chronic connective tissue alterations, but secondary CTS arising solely from noninfective flexor tenosynovitis remains uncommon. A 49-year-old woman with recently diagnosed type 2 diabetes presented with a 2-month history of progressive right-wrist pain, paresthesia, and volar swelling. Magnetic resonance
