Prepubertal periodontitis: a report of 2 cases.

PURPOSE: Prepubertal periodontitis is a term suggested for a form of periodontitis that starts soon after the eruption of primary teeth. There is increased awareness that the disease entity known as "periodontitis" is in all probability a family of related but reasonably discrete diseases. The purpose of this report is to describe the clinical, radiographic, and laboratory findings in 2 patients, as well as treatment of prepubertal periodontitis. METHODS: This study presents 2 cases of generalized prepubertal periodontitis: a 5-year-old girl and a 13-year-old girl. RESULTS: Case 1 presented

A long term study of root retention in the treatment of maxillary molars with furcation involvement.

A study was done to evaluate long-term results of treating 387 maxillary molars with furcation involvement in 100 patients with chronic destructive periodontal disease. Results showed a favorable long term functional survival rate of 341 teeth (88%) 5 to 24 years after treatment, despite the fact that many teeth had at least one root with 50% or less of bone support before treatment. Of the 46 teeth (12%) that were ultimately extracted, 25 were present for a significant length of time (6 to 18 years) before extraction. The radiographs of 292 teeth (75%) showed no significant change in bone sup

Timing of exchange of the maxillary deciduous and permanent teeth in boys with three types of orofacial clefts.

Timing of exchange of the deciduous and permanent maxillary teeth was investigated using dental plaster casts of 163 boys with total unilateral cleft (UCLP), 82 boys with bilateral cleft (BCLP), and 97 boys with isolated cleft palate (CP). All patients were treated at the Prague Plastic Surgery Clinic. The results were compared with a control group of 294 schoolboys. To evaluate the course of eruption, the proportion of each erupted teeth in each year of age was employed. In boys with UCLP, eruption of the permanent maxillary lateral incisors and the permanent maxillary second molar was retard

Dental characteristics of patients with four different types of skeletal dysplasias.

OBJECTIVE: Skeletal dysplasia (SD) comprises more than 450 separate disorders. We hypothesized that their dental features would be distinctive and investigated the tooth characteristics of four patients with different SDs. MATERIAL AND METHODS: Four SD patients with molecularly confirmed diagnoses, Pt-1 acromicric dysplasia, Pt-2 hypophosphatasia and hypochondroplasia, Pt-3 cleidocranial dysplasia, and Pt-4 achondroplasia, were recruited. A tooth from each patient was evaluated for mineral density (micro-computerized tomography), surface roughness (surface profilometer), microhardness, minera

Persistence of deciduous molars in subjects with agenesis of the second premolars.

The purpose of the present study was to investigate persistent primary second molars in a group of young people in their late twenties with agenesis of one or two second premolars. In 1982-83 it was decided, in connection with the orthodontic evaluation of 25 patients, to allow 35 primary molars (one or two in each patient) to remain in situ. All patients had mixed dentitions and agenesis of one or two premolars. The primary teeth were generally in good condition, although root resorption and infra-occlusion (compensated by occlusal composite onlays) occurred. In 1997, 18 of the 25 patients wi

Contemporary space maintenance for the pediatric patient.

Primary teeth play a critical role in the growth and development of a child. In addition to their role in esthetics, eating, speech, and to encourage normal function and resultant expected growth, the other main function of a primary tooth is to hold space for the permanent successor until it is ready to erupt. Space maintenance forms an integral part of preventive and interceptive orthodontics. After the premature loss of a tooth, not only do space maintainers maintain function and preserve arch length, they also maintain esthetics and eliminate any potential psychological damage a child coul

Comparison of stem cells from human exfoliated deciduous posterior teeth with varying levels of root resorption.

BACKGROUND: Stem cells from human exfoliated deciduous teeth (SHED) are regarded as an attractive cell source for tissue regeneration. However, the effect of different levels of root resorption on the characteristics of SHED remains less understood. Thus, the tooth source that is most suitable for the isolation of SHEDs needs to be determined. To compare cellular and biological characteristics of stem cells from human exfoliated deciduous posterior teeth with varying levels of root resorption. METHODS: The pulp was obtained from the deciduous posterior teeth depending on the level of root res

Ankylosis of primary molars--a future periodontal threat to the first permanent molars?

Most infra-occluded and ankylosed primary molars with a permanent successor will exfoliate normally. Continuous supervision of occlusal development and radiographic control of normal root resorption have been recommended. The decreased height of the alveolar bone level at the site of the infra-occluded primary molar has been reported to normalize after the eruption of the permanent successor. However, opinions that infra-occlusion of primary molars entails a risk of future periodontal damage to the first permanent molars have been presented. The aim of this study was to perform a long-term fol

The Hall technique may be an effective treatment modality for caries in primary molars.

SUBJECTS: From 2001 to 2004, 17 general dental practitioners (GDPs) in Tayside, Scotland, recruited 132 children aged 3 to 10 years at enrollment who had caries affecting matched pairs of asymptomatic primary molar teeth for participation in this split-mouth randomized controlled trial. Of these 264 study teeth with caries lesions, 42% were radiographically more than halfway into dentin, and 67% required Class II restorations. In 2000, the regional decayed/missing/filled teeth (DMFT) number was 2.47 (d3 1.71; mt 0.54; ft 0.22). Ninety-one patients (69%) had 48 months minimum of follow-up, or b

A Novel DSPP Mutation in Dentinogenesis Imperfecta Shields Type II: Clinical, Genetic and Stem Cell Perspectives.

INTRODUCTION AND AIMS: To identify the genetic cause of dentinogenesis imperfecta type II (DGI-II) in a family, analyse tooth microstructure and evaluate functional defects in patient-derived stem cells (SHED) to explore genotype-phenotype links. METHODS: A 13-member family with DGI-II underwent clinical evaluation. Whole-exome sequencing (WES) and Sanger sequencing identified pathogenic variants. Exfoliated tooth was assessed via micro-CT, microhardness testing and SEM. SHEDs from patients and controls were isolated, validated for stemness (flow cytometry, adipogenic induction) and tested fo

Acquired enamel pellicle protects gastroesophageal reflux disease patients against erosive tooth wear.

The objective of this study was to compare the protein profile of the acquired enamel pellicle (AEP) formed in vivo in patients with or without gastroesophageal reflux disease (GERD), and with or without erosive tooth wear (ETW). Twenty-four volunteers were divided into 3 groups: 1) GERD and ETW; 2) GERD without ETW; and 3) control (without GERD). The AEP formed 120 min after prophylaxis was collected from the lingual/palatal surfaces. The samples were subjected to mass spectrometry (nLC-ESI-MS/MS) and label-free quantification by Protein Lynx Global Service software. A total of 213 proteins w

MEPE-derived ASARM peptide inhibits odontogenic differentiation of dental pulp stem cells and impairs mineralization in tooth models of X-linked hypophosphatemia.

Mutations in PHEX (phosphate-regulating gene with homologies to endopeptidases on the X-chromosome) cause X-linked familial hypophosphatemic rickets (XLH), a disorder having severe bone and tooth dentin mineralization defects. The absence of functional PHEX leads to abnormal accumulation of ASARM (acidic serine- and aspartate-rich motif) peptide - a substrate for PHEX and a strong inhibitor of mineralization - derived from MEPE (matrix extracellular phosphoglycoprotein) and other matrix proteins. MEPE-derived ASARM peptide accumulates in tooth dentin of XLH patients where it may impair dentino
