OBJECT: Artificial disc replacement (ADR) devices are unlike implants used in cervical fusion in that they are continuously exposed to stress not only within the implant site but also at their site of attachment to the adjacent vertebra. An imaging technique with higher accuracy than plain radiography and with the possibility of 3D visualization would provide more detailed information about the motion quality and stability of the implant in relation to the vertebrae. Such high-accuracy studies have previously been conducted with radiostereometric analysis (RSA), which requires implantation of

BACKGROUND: Cervical total disc replacement (TDR) is intended to preserve motion and reduce adjacent segment degeneration compared to anterior cervical discectomy and fusion (ACDF). The Simplify Cervical Disc (Globus Medical, Audubon, Pennsylvania), a polyetheretherketone (PEEK)-on-ceramic device with titanium-coated endplates, was approved following encouraging short-term outcomes. However, emerging registries and the Food and Drug Administration (FDA) Manufacturer and User Facility Device Experience (MAUDE) reports suggest an increase in device failures and revision surgeries, raising concer

BACKGROUND: Cervical disc arthroplasty (CDA) has been demonstrated, in clinical trials, as an effective and safe treatment for patients diagnosed with radiculopathy and/or myelopathy. However, the current CDA indication criteria, based on the preoperative segmental range of motion (ROM), comprises a wide range of variability. Although the arthroplasty level preserved ROM averages 7°-9° after CDA, there are no clear guidelines on preoperatively limited or excessive ROM at the index level, which could be considered as suitable for CDA.
METHODS: This was a retrospective study of patients who und

There is no consensus on whether anterior cervical arthroplasty or anterior cervical discectomy and fusion (ACDF) is the optimal treatment for single-level cervical radiculopathy or myelopathy. We conducted a meta-analysis of randomized controlled trials to compare the safety and efficacy of anterior cervical arthroplasty with ACDF. Eight studies met the inclusion criteria. Overall, there were significant differences between these two treatment approaches in the arm visual analog scale (VAS) scores [mean difference (MD)=-4.86, 95% confidence interval (CI)=-6.42 to -3.30], neck VAS scores (MD=-

OBJECTIVE: Cervical artificial disc replacement (C-ADR) has been shown to have potential advantages over anterior cervical discectomy and fusion for certain indications. However, currently published data have been extrapolated from the postoperative outcomes of C-ADR at 1 or 2 levels, and there is no extensive literature regarding patients who have had multilevel (≥ 3 levels) C-ADR. The aim of this paper was to characterize how patients who have undergone multilevel C-ADR procedures fare in terms of their outcomes and complications. In addition, the authors present an illustrative case of a pa

BACKGROUND CONTEXT: Cervical disc arthroplasty (CDA) has been advocated as an alternative to anterior cervical discectomy and fusion (ACDF) with the added potential to reduce the risk of adjacent level disc degeneration and segmental instability. However, the long-term adverse events of arthroplasty have yet to be fully reported.
PURPOSE: To investigate the 10-year follow-up adverse events rates between CDA and ACDF.
STUDY DESIGN/SETTING: The study was a randomized, prospective, multicenter Investigational Device Exemption (IDE) trial and its continued follow-up as a postapproval study (PAS)

We report a case of allergic reaction after total cervical disc arthroplasty. A 52-year old woman was operated on for right C6 cervicobrachial neuralgia secondary to C5-C6 disc disease with foraminal stenosis. A cobalt-chromium-molybdenum total disc prosthesis had been implanted two years earlier. The patient was referred to our institution for recurrence of axial neck pain associated with abdominal patches of erythematous itching rash and swallowing disorder. Allergy tests confirmed type-4 allergic reaction to chromium. Symptoms decreased after removal of the prosthesis with secondary fusion.

Lumbar artificial disc technology has been commercially available outside the United States for nearly 2 decades. With the Food and Drug Administration approval of the Charité Artificial Disc in October 2004, an entirely new spinal surgeon population will be able to offer this technology to their patients as a treatment option. As with other techniques in spinal surgery, indications for lumbar total disc replacement are paramount to the success of the procedure. The correct surgical technique is also important to a successful outcome. This article describes the technique for placement of the C
