Characterization of Opioid Use in the Intensive Care Unit and Its Impact Across Care Transitions: A Prospective Study.

Purpose: The objective of this study is to characterize opioid intensity in the intensive care unit (ICU) and its association with opioid utilization across care transitions. Methods: This is a prospective cohort study. Medically ill ICU patients with complete medication histories who survived to discharge were included. Opioid intensity was characterized based on IV morphine milligram equivalents (IV MME). Primary outcomes were opioid prescribing upon ICU and hospital discharge. Results: Opioids were prescribed to 34.1% and 31.1% of patients upon ICU and hospital discharge. Within the ≥50 mea

Transition of Care from Pediatric to Adult Care in Eosinophilic Esophagitis: Insights From a Patient Perspective Survey.

Eosinophilic esophagitis (EoE) is a chronic immune-mediated disease. Patients with a childhood diagnosis require ongoing disease management in adulthood; however, knowledge of the patient experience during pediatric to adult healthcare transition is lacking. Here, an online survey captured patient perceptions of the challenges faced by patients with EoE in the United States during transition to adult healthcare, and which resources, if implemented, could better support transition. Of 67 respondents, 91% (n = 61) were under adult care at the time of survey completion. Aspects that respondents s

Participant feedback on a health care transition conference for youth with medical complexity.

INTRODUCTION: Each year in the United States, an estimated 1.2 million youth with chronic conditions or functional limitations reach adulthood and must transition from pediatric to adult health care, a process often fragmented by service loss, limited adult providers, and complex waiver and financial systems. The University of Alabama at Birmingham developed the Staging Transition for Every Patient (STEP) Program which includes an annual Healthcare Transition Conference. Here we describe the conference and analyze two years of attendee feedback. METHODS: Conference planning followed an iterat

Transitions of care following acute kidney injury: a systems-based analysis.

BACKGROUND: People affected by Acute Kidney Injury (AKI) experience poor short and long-term outcomes following discharge from hospital. Attempts to improve care have recognised the complexity of the healthcare system. We applied systems methods to explore everyday work and illuminate areas important for success, challenges faced and how people adapted to these challenges. METHODS: Twenty-six patients who had recently been discharged from six hospitals after an admission complicated by AKI were interviewed. Focus groups were conducted with healthcare staff across six sites in England. Using t

Exploring transitions in care for individuals living with and beyond cancer: A qualitative study.

OBJECTIVE: Cancer care involves multiple healthcare providers across various settings. Each time the responsibility of care is transferred from one healthcare provider or setting, referred to as transitions in care (TiC), there is an increased risk of medical error, patient dissatisfaction with care and poor outcomes. The objective of this study is to explore patient perspectives and experiences with TiC during cancer care, and to identify perceived gaps in care during TiC among individuals living with and beyond cancer. DESIGN: A qualitative study using the interpretive descriptive approach

Transition of care for children with complex congenital surgical conditions as they age into adulthood.

INTRODUCTION: Advances in neonatal care have created an expanding population of adults living with congenital surgical conditions, yet structured transition of care (TOC) pathways between pediatric and adult surgical systems are poorly defined. This study aims to understand the current national landscape of TOC for congenital surgical conditions. METHODS: A survey exploring TOC for short bowel syndrome (SBS), esophageal atresia (EA), Hirschsprung disease (HSCR), anorectal malformation (ARM), and congenital diaphragmatic hernia (CDH) was distributed to program directors of all ACGME-accredited

Social transition in young trans people accessing gender-affirming care.

BACKGROUND: In-depth quantitative study of how young people undertake social transition is lacking. We aimed to provide a comprehensive profile of social transition among young people accessing gender-affirming care. METHODS: In this cross-sectional study (n = 482), young people aged 8-17 years completed online questionnaires before their first appointment at a pediatric gender clinic. Measures captured social transition in different contexts (home, school, online, friends, and extended family) by assessing extent of social transition, and changes to presentation, name and pronouns or intent

Antithrombosis stewardship across the care spectrum: optimizing transitions in venous thromboembolism.

As global burden of venous thromboembolism (VTE) increases, clinicians are managing more patients across diverse and often fragmented care settings. VTE care transitions are more frequent and complex than those seen in stable anticoagulation conditions like atrial fibrillation. Each transition presents opportunity for optimizing antithrombotic therapies and mitigating associated risks. In this review, we use case-based scenarios to highlight common care transition challenges and offer evidence-based, stewardship-guided strategies to optimize antithrombotic use and outcomes throughout the VTE c

Transitioning to midwifery models of care: lessons from case studies in India.

BACKGROUND: Limited evidence exists on how midwives, other healthcare providers, and their local communities in low- and middle-income countries experience the transition to a midwifery model of care. OBJECTIVES: This study aimed to identify opportunities, enablers, and barriers to integrating midwifery into existing public health systems, from the perspectives of key stakeholders representing health systems and communities. METHOD: We conducted two case studies - community-based and public health facility-based - in Gujarat and Telangana, comprising 63 in-depth interviews and five focus gro

Interdisciplinary transition of care for congenital gastrointestinal malformations: analysis of a standardized program.

OBJECTIVES: Adolescents with congenital gastrointestinal malformations, such as esophageal atresia, anorectal malformations, and Hirschsprung's disease, frequently face long-term physical and psychological sequelae. Despite increasing recognition of the need for structured transition of care (TOC) programs, standardization remains limited. The aim of this study was to evaluate the effectiveness of a structured interdisciplinary TOC program for adolescents with gastrointestinal malformations, focusing on patient well-being, gastrointestinal quality of life, transition competence, and satisfacti

Acute Heart Failure: Transitioning From Symptom-Based Care to Remission.

During the past century, the characteristics of patients with heart failure (HF) and acute HF (AHF) have shifted from patients with severe pump failure due to rheumatic, hypertensive and ischemic heart disease to older and more obese patients with multiple severe comorbidities. The pathophysiology of AHF has shifted, in parallel, from that of advanced, end-stage pump failure caused by severe left ventricular dysfunction to age, obesity and comorbidity-related cardiovascular dysfunction combined with neurohormonal and inflammatory dysregulation or "inflammaging." With the advent of neurohormona

The In-between: Time to Talk About Bronchiectasis in Adolescents and Their Transition to Adult Care.

Paediatric and adult bronchiectasis patients have been addressed in the literature as two different populations due to several differences, but there is insufficient evidence to understand how and when disease characteristics really change along patients' lifespan. This lack of knowledge is evident in all aspects of the transition: insufficient data is available about radiology, lung function, microbiology and treatment, and only limited information is currently available about changes in clinical presentation and psychosocial aspects. For instance, symptoms seem to improve during the third an

Pharmacists Act on Care Transitions in Stroke (PACT-Stroke): A Systems Approach.

PURPOSE: Stroke is one of the leading causes of disability in the United States. Prompt secondary prevention decreases the risk for recurrence. Pharmacists have taken a more active role in poststroke care and are initiating prompt follow-up after hospital discharge. There are, however, limited descriptions providing insight on how to implement programs that address this need. The purpose of this article is to provide insights into the development of a pharmacist-driven transitions of care program for patients with stroke. METHODS: This study was a single-center, retrospective assessment of pa

Bridging Care Transition After Hospitalization for Atrial Fibrillation and Coronary Interventions.

Treatment fragmentation between hospitals and the community can result in catastrophic outcomes; uninterrupted treatment with anticoagulant and platelet aggregation inhibitors is particularly important. We assessed the proportion and characteristics of patients who did not visit their primary community-based physician within 1 week of discharge from our department of cardiovascular medicine and the proportion that failed to procure essential drugs at the community pharmacy. We prospectively studied 423 patients who were discharged from our department. They were provided detailed explanations,

The Transition of Care for Patients with Anorectal Malformations and Hirschsprung Disease: A European Survey.

This study aimed at evaluating how transition of care is currently being organized in the European Reference Networks (ERNs) health care providers (HCPs) in pediatric areas and in the Anorectal Malformation Network (ARM-Net) Consortium hospitals. An online questionnaire was sent to a total of 80 surgeons, members of or affiliated members of three networks: ARM-Net Consortium, ERN eUROGEN, and ERN ERNICA. Complete information were obtained for 45 HCPs, most of which deal with transition and still see a few adult patients (ca. 10%). Gynecological, gastroenterological, urological, colorectal, and

Resilience factors in youth transitioning out of foster care: A systematic review.

OBJECTIVE: Youth who emancipate from the foster care system often experience poor outcomes during their transition into independent living (e.g., criminal justice involvement and homelessness). Yet, some youth are resilient and achieve positive outcomes. The purpose of this study is to review the resilience factors identified in the literature for youth transitioning out of care. METHOD: A systematic review of the literature was conducted using PsycINFO, PubMed, and EMBASE databases. A total of 12 studies met our inclusion criteria. RESULTS: A total of 38 different resilience factors were id

Transitioning across professional boundaries in midwifery models of care: A literature review.

BACKGROUND: High-risk pregnancy, or one with escalating complexities, requires the inclusion of numerous health professions in care provision. A strategy of midwife navigators to facilitate the smooth transition across models of care and service providers has now been in place in Queensland, Australia, for over twelve months, and a formal review process will soon begin. Navigators are experienced nurses or midwives who have the expertise and authority to support childbearing women with chronic or complex problems through the health system so that it is co-ordinated and they can transition to s
