MSC secretomes as a cell-free therapeutic platform: Molecular heterogeneity, disease-specific bioactivity, delivery strategies, and regulatory translation across six major disease areas.

The mesenchymal stromal/stem cell (MSC) secretome is emerging as a cell-free pharmacological agent with multitarget activity in regenerative and inflammatory diseases. Rather than durable engraftment or differentiation, most therapeutic effects of MSC-based therapies are mediated by a complex mixture of soluble factors and extracellular vesicles (EVs) that can, in principle, be formulated as standardized biological drugs. This review appraises current evidence on MSC secretome composition, mechanisms of action, and pharmacological profile, emphasizing its potential as a scalable off-the-shelf

Neurosurgery as an immune anchor point: a translational framework for perioperative immunoengineering.

Neurosurgical diseases-including brain tumors, hemorrhage/trauma, ischemia, infection, epilepsy, and spinal cord injury-share convergent neuro-immune mechanisms. In the acute phase, sterile inflammation and barrier disruption trigger innate immune cascades. During the subacute phase, immune resolution and clearance determine the quality of tissue repair. In the chronic phase, persistent immune-glia interactions and synaptic remodeling influence epileptogenesis and long-term cognitive outcomes. Recent discoveries-such as the meningeal immune niche, meningeal lymphatic system, and glymphatic cle

Hydrogel dressings for diabetic foot ulcers: microenvironment-informed design, clinical scenario matching, and translational challenges.

Diabetic foot ulcers (DFUs) are difficult-to-heal chronic wounds characterized by ischemia, biofilm-associated infection, persistent inflammation, oxidative stress, protease-rich exudate, and abnormal biomechanics. Although hydrogel dressings have been widely explored for DFU management, many reviews remain material-centered and provide limited guidance on clinical scenario matching. This review proposes a microenvironment-informed framework for DFU hydrogel design. We organize hydrogel strategies according to dominant pathological barriers, including impaired angiogenesis, chronic inflammatio

Exploring and translating novel approaches targeting the choroid plexus for the treatment of hydrocephalus: a literature review.

Hydrocephalus is common with a mixed phenotype and aetiology. Its treatment is predominantly surgical, with temporary or permanent cerebrospinal fluid (CSF) diversion, which does not address the underlying disease mechanism. The choroid plexus (ChP), a secretory epithelium found in all four ventricles of the brain, produces most CSF within the central nervous system and has been investigated extensively over the past century to understand its function and potential role as a therapeutic target. Past attempts at either medically or surgically controlling its rate of secretion have not significa

Factor XII in Thrombosis and Thromboinflammation: From Molecular Biology to Clinical Translation.

Factor XII (FXII) is a central mediator at the intersection of coagulation, fibrinolysis, inflammation, and immunity. It is activated upon contact with negatively charged surfaces, triggering the intrinsic coagulation pathway and driving thrombus formation and stabilization. Beyond clotting, FXII contributes to activation of the kallikrein-kinin system, generation of bradykinin, and modulation of inflammatory and immune responses. Congenital FXII deficiency does not increase bleeding risk, highlighting its unique role and making FXII inhibition an attractive strategy for anticoagulation and im

Xanomeline and Trospium Chloride: Mechanism of Action, Clinical, and Translational Science.

Xanomeline-trospium chloride (Cobenfy, formerly KarXT) is a first-in-class, oral central M1/M4 muscarinic receptor agonist approved for the treatment of schizophrenia in adults. Cobenfy represents a paradigm shift by avoiding direct dopamine D2 receptor blockade and thus significantly reduces the burden of metabolic, extrapyramidal, and sedative side effects observed in standard first and second-generation antipsychotics. Trospium chloride is a peripheral muscarinic antagonist with negligible blood-brain barrier penetration. Cobenfy is co-formulated with trospium to minimize peripheral choline

Antibody-Drug Conjugates in Solid Tumor Oncology and the Frontier of Precision Immunosuppression: A Mechanistic, Translational, and Clinical Review.

Antibody-drug conjugates (ADCs) have transitioned from clinically marginal agents into a defining therapeutic class for solid tumor oncology. In DESTINY-Breast03, trastuzumab deruxtecan achieved a four-fold progression-free survival advantage over trastuzumab emtansine, attributable not to antibody engineering but to the linker-payload axis: a cleavable peptide linker and a topoisomerase I payload with bystander activity. Sacituzumab govitecan extends the same logic to Trop-2-positive disease via extracellular payload release, and the framework now spans breast, urothelial, gynecologic, lung,

Modeling Negative Symptom Domain Neurobiology: Protocol for an Observational, Transdiagnostic, Translational Study.

BACKGROUND: Negative symptoms (NS) such as anhedonia (reduced pleasure), avolition (reduced motivation), asociality (social withdrawal), blunted affect (diminished emotional expression), and alogia (poverty of speech) are associated with poor functional outcomes in psychiatric and neurological disorders and are an unmet treatment need. Current medication primarily targets positive or affective symptoms, leaving NS neurobiology unaddressed. A critical research gap exists in understanding whether these symptoms share a common biological architecture across different diagnoses or whether they eme

Experimental Models of Autoimmune Hepatitis: Disease Fidelity and Translational Relevance.

Autoimmune hepatitis (AIH) remains difficult to study mechanistically in patients because disease initiation is rarely observed directly, the relevant autoantigens differ across subsets, and clinically meaningful outcomes such as chronic inflammation, fibrosis, relapse, and treatment response evolve over time. Animal models therefore remain indispensable. At the same time, the field has become increasingly heterogeneous. Acute immune-mediated hepatitis systems, especially concanavalin A (ConA), dominate the recent literature because they are rapid, inexpensive, and experimentally tractable. Ho

Extracellular Vesicle Therapies for Diabetic Skin Lesions: Mechanisms, Engineering, and Clinical Translation.

Diabetic foot ulcers and other chronic diabetic skin lesions persist because vascular insufficiency, neuropathy, infection, oxidative stress, dysregulated inflammation, and impaired stromal and epithelial repair act concurrently. Extracellular vesicles (EVs), including exosome-enriched preparations, can deliver proteins, lipids, and regulatory RNAs to multiple wound-resident cell types and therefore offer a cell-free strategy for this multifactorial pathology. This review examines recent evidence for EV-based therapy across inflammatory resolution, angiogenesis, fibroblast and keratinocyte rec

High-Throughput Automated Multiplex Immunofluorescence Assays for Translational Research.

Multiplex immunofluorescence (mIF) is an advanced technique that allows for detailed, spatially resolved analysis of tissue samples by visualizing multiple protein biomarkers within a single section. However, most mIF technologies face significant tradeoffs between sensitivity and throughput when multiplexing, posing a critical limitation for robustly detecting low-abundance biomarkers across multiple tissue sections. A new approach for mIF uses unique DNA barcodes linked to antibodies, which are amplified through a parallel single-molecule amplification mechanism. This method achieves stainin

A roadmap for transformative translational research on gambling disorder in the UK.

The UK has one of the highest rates of recreational gambling in the world. Some vulnerable individuals progressively lose control over gambling and develop at-risk gambling or gambling disorder (GD), characterised by the compulsive pursuit of gambling. GD destroys lives and incurs massive costs to societies, yet only a few treatments are available. Failure to develop a wider range of interventions is in part due to a lack of funding that has slowed progress in the translational research necessary to understand the individual vulnerability to switch from controlled to compulsive gambling. Diaphragm blood flow: new avenues for human translation.

The rhythmic contraction of the diaphragm facilitates continuous pulmonary ventilation essential for life. Adequate blood flow to the diaphragm is critical to continuously support contractile function, as an imbalance in nutritive supply and demand can lead to diaphragm insufficiency, patient morbidity, and mortality. Given oxygen supply to the diaphragm is key to its function, it is no surprise that more than 200 animal studies have investigated diaphragm blood flow ([Formula: see text]) regulation over the past century. This work has advanced our understanding of the diaphragm's circulatory
