[The significance of the results of crash-tests with the use of the models of the pedestrians' lower extremities for the prevention of the traffic road accidents].

The disabling injuries inflicted during road traffic accidents (RTA) create a serious challenge for the public health services and are at the same time a major socio-economic problem in the majority of the countries throughout the world. The injuries to the lower extremities of the pedestrians make up the largest fraction of the total number of the non-lethal RTA injuries. Most of them are responsible for the considerable deterioration of the quality of life for the participants in the accidents during the subsequent period. The objective of the present study was to summarize the currently ava

Paroxysmal sympathetic hyperactivity: Autonomic instability and muscle over-activity following severe brain injury.

BACKGROUND: Children who suffer from moderate-to-severe brain injury can develop a complicating phenomenon known as paroxysmal sympathetic hyperactivity (PSH), characterized by autonomic instability and identified clinically as a cluster of symptoms that can include recurrent fever without a source of infection, hypertension, tachycardia, tachypnea, agitation, diaphoresis and dystonia. Studies with adults have demonstrated that this cluster of symptoms is associated with poorer clinical outcomes (prolonged hospitalizations, poorer cognitive and motor function). However, there have been limited

Compliant flooring to prevent fall-related injuries: a scoping review protocol.

INTRODUCTION: Fall-related injuries can have serious consequences for older adults, including increased risk of dependence in daily activities and mortality. Compliant flooring is a passive intervention that may reduce the incidence and severity of fall-related injuries in healthcare settings, including acute and long-term care, but few sites have implemented compliant flooring, in part because synthesised evidence about key performance aspects has not been available. METHODS AND ANALYSIS: We will conduct a scoping review to address the question: what is presented about the biomechanical effi

Limb salvage of severely injured extremities after military wounds.

Severely wounded extremities following battlefield injuries present a surgical dilemma to military surgeons of whether to attempt salvage or amputate the limb. There are many considerations to be made, including local and systemic patient factors, other wounded personnel and logistical constraints. Attempts have been made previously to form objective scoring criteria so as to remove possible subjectivity in this decision-making process. Furthermore, paediatric patients present their own particular dilemmas. This paper examines these various matters and, with contemporaneous evidence, presents

Reducing chest injuries in automobile collisions: rib fracture timing and implications for thoracic injury criteria.

The purpose of this study was to quantify the biomechanical response of the human thorax during dynamic shoulder belt loading representative of that seen in a severe automotive collision. Two post-mortem human surrogates (PMHSs) (one male and one female) were instrumented with 26 single-axis strain gages on the ribs, sternum, and clavicle. The thorax of each PMHS was placed on a custom spine support bracket designed to support the thorax on either side of the spinous process, thereby allowing free motion at the costovertebral joints. In addition, the support bracket raised the thorax above the

Management guidelines for penetrating abdominal trauma.

PURPOSE OF REVIEW: Patients with penetrating abdominal trauma are at risk of harboring life-threatening injuries. Many patients are in need of emergent operative intervention. However, there are clearly patients who can be safely managed nonoperatively. This review evaluates the literature to identify management guidelines for patients with penetrating abdominal trauma. RECENT FINDINGS: Accumulating evidence supports nonoperative management of patients with stab wounds to the thoracoabdominal region, the back, flank, and anterior abdomen. Furthermore, select patients with gunshot wounds can b

"Stingers" in adolescent athletes.

Brachial plexus injuries commonly occur in athletes participating in contact sports. The incidence of transitory brachial plexus injury is approximately 30% to 50% over the course of a high school, college, or professional football player's career. These injuries are called "stingers" or "burners" because of the associated tingling that occurs in the upper extremity after the injury. Brachial plexus injuries are poorly understood and sometimes are difficult to manage. Appropriate knowledge and understanding of these injuries along with prompt recognition, diagnosis, and treatment are essential

Effect of increased rear row occupancy on injury to seat belt restrained children in side impact crashes.

Previous work identified a similar risk of injury for children seated on the struck side and center rear in side impact crashes in passenger cars. In order to further explain this finding, we investigated the effect of sharing the rear row with other occupants on injury risk and delineated differences in injury patterns among the seat positions. These analyses, conducted from a large child specific crash surveillance system, included: children 4-15 years old, rear seated, seat belt restrained, in a passenger car, and in a side impact crash. Injury risk was compared among each rear seat positio

[Forensic assessment of injuries to the subaxial part of the cervical spine column].

An analysis of the treatment outcomes of 65 patients with non-fatal subaxial cervical spine (CS) injuries was conducted. Injuries were classified according to the four-column biomechanical classification model for injuries to these structures (STSG, 2006). It was established that the most frequently injured structures are the anterior column of the C4-C7 vertebrae, where complicated compression, burst fractures, fracture-dislocations, and subluxations are most often observed. Uncomplicated fractures of the transverse processes, arches, and articular processes are commonly observed in the later

Outcome measures used in clinical research evaluating prehospital blood component transfusion in traumatically injured bleeding patients: A systematic review.

BACKGROUND: Trial outcomes should be relevant to all stakeholders and allow assessment of interventions' efficacy and safety at appropriate timeframes. There is no consensus regarding outcome measures in the growing field of prehospital trauma transfusion research. Harmonization of future clinical outcome reporting is key to facilitate interstudy comparisons and generate cohesive, robust evidence to guide practice. The objective of this study was to evaluate outcome measures reported in prehospital trauma transfusion trials. METHODS: Data Sources, Eligibility Criteria, Participants, and Inter

Road traffic crashes at night-time: characteristics and risk factors.

The study was to examine the nature and risk factors associated with road traffic crashes at night in Ghana and identify potential measures to control them. Crash and injury data for the period 2013-2017 were analyzed. The fatality index and fatal crash ratio measures were employed to assess the severity of injuries among traffic participants. Statistical analysis was carried out for the variables using contingency tables and the chi-square (χ2) tests to assess statistical significance. Generally, night-time traffic crashes resulted in severer injury outcomes than crashes in the hours of dayli

Psychological Characteristics in Acute Mild Traumatic Brain Injury: An MMPI-2 Study.

The psychological characteristics of acute traumatic brain injury (TBI) have received limited research focus, despite empirical evidence of their relevance for subsequent psychological adjustment and early therapeutic intervention. This study addressed a wide range of psychological features in 47 individuals who were hospitalized as a result of acute mild TBI (mTBI). Participants were screened from amongst consecutive TBI admissions for moderate to severe brain injury, and for pre-injury neurological, psychiatric, or substance abuse histories. Clinical and content scale scores on the MMPI-2 we

Early intramedullary nailing of femoral shaft fracture on outcomes in patients with severe chest injury: A meta-analysis.

Early intramedullary nailing (IMN) within the first 24 hours for multiply injured patients with femoral fracture and concomitant severe chest injury is still controversial. This review aimed to investigate the association between early IMN and pulmonary complications in such patients. We searched the literature up to Jan 2016 in the main electronic databases (PubMed, Web of Science, Cochrane library databases) to identify eligible studies. Data were extracted and analyzed using a Mantel-Haenszel method with random-effects model to estimate pooled odds ratio (OR) and 95% confidence intervals (C
