BACKGROUND: Trigger finger (TF), a painful condition involving a finger flexor tendon, is a common problem with a prevalence of ~2-3% in the general population. However, the TF prevalence is higher among diabetic patients-ranges from 6.7% to 10%. We have analyzed the expression of the extracellular matrix, inflammation, and epigenetic related genes in diabetic and non-diabetes TF. We hypothesized that Diabetes condition induces alter the expression of epigenetic modification genes in diabetic patients and one of the underlying determinants for more prevalence of TF in diabetic patients.

BACKGROUND: The aim of this study was to evaluate and compare the cross-sectional area (CSA) of flexor digitorum tendons and the thickness of first annular (A1) pulleys between contralateral normal digits and trigger digits (TDs) at positions of finger flexion and extension using a noninvasive ultrasound system.
METHODS: Seventeen affected fingers of 17 patients (6 men and 11 women) with TDs in one hand, and 17 contralateral normal digits without trigger finger symptoms were examined. The sonographic appearances of flexor digitorum tendons and A1 pulleys were observed at two positions of meta

PURPOSE: The use of routine physical therapy (PT) and occupational therapy (OT) after certain hand procedures, such as carpal tunnel release, remains controversial. The objective of this study was to evaluate baseline use, the change in use, variation in prescribing patterns by region, and costs for PT/OT after common hand procedures.
METHODS: Outpatient administrative claims data from patients who underwent procedures for carpal tunnel syndrome, trigger finger, carpometacarpal arthritis, de Quervain tenosynovitis, wrist ganglion cyst, and distal radius fracture were abstracted from the Truve

Background: Open surgical release of the A1 pulley is the definitive treatment for the common hand condition of trigger finger, or inflammatory stenosing tenosynovitis. Anecdotal evidence among hand surgeons has questioned whether or not recent steroid injection may be related to complications following open trigger finger release, particularly wound infection, but no studies have primarily studied this connection to date. We aimed to determine whether recent steroid injection was associated with postoperative surgical infections. Methods: We performed a retrospective chart review of 780 adult

Trigger finger is a common cause of hand pain in the adult population. Studies in the past have suggested that ring finger and thumb are the most prevalent trigger fingers. Risk factors, such as diabetes and hypothyroidism, have been reportedly linked to trigger fingers. This observational prospective study was carried out to identify the most commonly affected trigger finger and observe associated comorbidities. At a single clinical site, a total of 46 patients with 54 trigger fingers on 49 hands were identified over a 7-week period. Ring finger, thumb, and long finger were observed to be the

PURPOSE: Because pediatric trigger finger is much less common than pediatric trigger thumb, there is no consensus on the efficacy of splinting, owing to both the rarity of the condition and a lack of natural history and comparative therapeutic data. We performed the present retrospective study on 47 fingers to compare pediatric trigger finger treatment by splinting and nonsplinting.
METHODS: We included 24 children with a total of 47 trigger fingers. Affected fingers included 4 index, 28 middle, 11 ring, and 4 little fingers. Patient age at initial examination ranged from 1 month to 9 years (

We report a case of keloid formation on the palm of the hand after trigger finger release. A 58-year-old male with trigger finger of the right index, middle and ring fingers was treated with open release at a nearby hospital. Three months after this operation, a progressively enlarging skin lesion formed at the surgical site. A diagnosis was made of keloid formation after trigger finger release, and keloid excision with full thickness skin graft and postoperative radiation therapy was done. One year postoperatively, the patient was asymptomatic and had not experienced a recurrence. This is a f

Ulnar superficialis slip resection is a procedure initially performed to treat flexor tenosynovitis in the rheumatoid finger. It was first described for treating trigger digits of children with mucopolysaccharide storage disorders and for more complex trigger digits in children. The procedure for adults with trigger finger was described for triggering with flexion contracture at the proximal interphalangeal joint level persisting after release of the A1 pulley and for trigger finger in diabetic patients. This article describes a case of chronic flexor tenosynovitis treated with ulnar superficia

The terminology used to describe most common tendon disorders in the hand and wrist suggests that they are inflammatory in nature, although current evidence indicates that mechanical and degenerative factors are more important. Corticosteroid injections provide relief in 60% or more of cases; however, the duration of their effectiveness remains uncertain. Surgical release of the stenotic pulley or sheath is curative in well over 90% of cases; complications of surgery are rare, and relief is long-lasting. Enlightened management of these common problems demands evidence-based guidelines defining

Trigger finger is a condition that can result from a stenosed digital flexor tendon sheath. The condition was first described by French physician Alphonse Notta in 1850 in a report that consisted of 4 case studies of adult patients. Notta's and other reports written in the 19th century document the early views on this condition's etiology and management. The 20th century saw the development of modern-day treatment methods for the condition, namely corticosteroid injections and the surgical release of the A1 pulley.

OBJECTIVE: To further assess the safety, efficacy, and results of the percutaneous trigger finger release, using the tip of an 18-gauge needle.
METHODS: At King Hussein Medical Center, Amman, Jordan, from January 2004 to January 2006, 42 patients with trigger fingers were treated with percutaneous A1 pulley release. The operations were performed using the tip of an 18-gauge hypodermic needle, to release the A1 pulley, by an experienced orthopedic hand surgeon. There were 24 women and 18 men. The mean age of the patients was 56 years (range 32-75 years). The mean duration of follow-up was 18 w

Trigger finger is stenosing tenosynovitis that occurs in A1 pulley. It usually occurs idiopathically in patients' 40s and 50s. On the other hand, pediatric trigger finger usually occurs before 8 years old in pediatric patients. Even though being rare, a tumor occurred in the soft tissue or bone near flexor tendons can cause a trigger finger. Trigger finger due to osteochondroma is very rare. Furthermore, most cases of trigger finger due to osteochondroma occur in pediatric patients with hereditary multiple osteochondromatosis (HMO). The authors report this case of a trigger finger caused by a

OBJECTIVES: The aim of this study was to investigate the correlation between hypervascularization of the first annular (A1) pulley on color Doppler imaging of trigger finger and patients' backgrounds.
METHODS: A total of 148 trigger digits from 144 patients were studied with ultrasound at the time of initial diagnosis. We observed the A1 pulley at the level of the metacarpophalangeal joint in a transverse image and noted the presence or absence of a signal in the A1 pulley on color Doppler imaging. Patients' ages, sexes, clinical grades, symptom durations, prevalence of interphalangeal joint

PURPOSE: Open release of the A1 pulley is a widely known procedure for the treatment of trigger finger. A subset of patients presents with both trigger finger and a positional contracture of the proximal interphalangeal (PIP) joint. These patients usually have a long history of trigger finger or have already undergone a surgical release of the annular pulley. This study is a retrospective review of the outcomes of resection of the flexor digitorum superficialis (FDS) for patients whose trigger finger was associated with a positional contracture of the PIP joint.
METHODS: Thirty-six patients (

BACKGROUND: Trigger finger is most common in the ring finger, but the reason for this is not known. We hypothesized that the compliance of the A1 pulley might be one of the factors responsible for this phenomenon. The purpose of this study was therefore to compare the compliance of the normal A1 pulley of the thumb, index, middle, ring and little fingers using human cadavers.
METHODS: Eight normal thumbs, index, middle, ring and little fingers from eight fresh frozen human hand cadavers were used in this experiment. The compliance of the A1 pulley was measured by the resistance when passing a

The literature generally supports the safety of epinephrine injection in the digits, but recent case reports describe ischemic adverse events associated with the use of lidocaine and epinephrine in which phentolamine rescue was not performed. We present a case of finger necrosis and subsequent amputation in a patient after 1% lidocaine with 1:100,000 epinephrine was injected in the fat and flexor sheaths in the palm for a 3-finger trigger release. Phentolamine rescue was not performed. All surgeons who use epinephrine in the finger should be prepared to reverse vasoconstriction with phentolami

The purpose of this systematic review was to determine the outcome of interphalangeal (IP) joint motion in children undergoing open surgical release, splinting, and passive exercising therapy for the treatment of paediatric trigger thumb. We conducted an online literature search of seven major databases. Only studies with a mean follow-up of at least 12 months were considered for inclusion. Seventeen retrospective studies and one prospective study met all the inclusion criteria. They reported on the results of surgery (634 children, 759 thumbs), splinting (115 children, 138 thumbs), and passiv

Carpal tunnel syndrome (CTS) impairs sensation of a subset of digits. Although the effects of CTS on manipulation performed with CTS-affected digits have been studied using precision grip tasks, the extent to which CTS affects multi-digit force coordination has only recently been studied. Whole-hand manipulation studies have shown that CTS patients retain the ability to modulate multi-digit forces to object mass, mass distribution, and texture. However, CTS results in sensorimotor deficits relative to healthy controls, including significantly larger grip force and lower ability to balance the

The trigger thumb is an uncommon condition in infants and children which occurs due to pathology of flexor pollicis longus or A1 pulley. The objectives in our study were to determine the treatment outcome of trigger thumb. The study included total of 45 patients (24 males and 21 females) with trigger thumb. The mean age of onset was 28.5 months (3 months -7 years). There was overall success rate of 72.41% following conservative treatment and the success rate appears to be higher in the younger age group. The outcome of children who underwent surgery was 91.66% with recurrence rate of 8.33% and
