Contemporary relevancy of carotid-subclavian bypass defined by an experience spanning five decades.

BACKGROUND: The contemporary impact of and indications for carotid-subclavian bypass (CSB) are essential considerations in decision making for brachiocephalic reconstruction. METHODS: We analyzed operative outcomes, long-term graft patency, and the extended epidemiological impact of the primary disease process in 287 consecutive patients (mean age, 60.6 years; 43.2% male) who received CSB for symptomatic brachiocephalic disease. RESULTS: Technical success was achieved in each patient. Operative mortality was 1.0% (3/287) and total (ipsilateral [1.4%, 4/287] plus contralateral [0.7%, 2/287])

Long-term safety and efficacy outcomes of the Acellular Tissue Engineered Vessel (ATEV) in extremity arterial trauma repair.

BACKGROUND: In cases of arterial injury, use of autologous vein for arterial repair is sometimes not feasible. We previously reported the short-term (ie, 30-day) outcomes of the Acellular Tissue Engineered Vessel (Symvess) in treating extremity vascular injuries. This study evaluates the long-term safety, efficacy, and mechanical durability of Symvess in patients with arterial trauma. METHODS: This prospective analysis focused on patients from the CLN-PRO-V005 clinical study who sustained noniatrogenic vascular injuries to the extremities, and for whom saphenous vein was not feasible for use.

Hemodialysis patients have worse outcomes after infrageniculate revascularization procedures.

BACKGROUND: Hemodialysis (HD) has been shown to be an independent predictor of poor outcomes after femoropopliteal revascularization procedures in patients with chronic limb-threatening ischemia. However, HD patients tend to have isolated infrageniculate disease, an anatomic risk factor for inferior patency. We aimed to compare outcomes for HD versus non-HD patients after infrageniculate open lower extremity bypass (LEB) and endovascular peripheral vascular interventions (PVIs). METHODS: Data from the Society for Vascular Surgery Vascular Quality Initiative database (2008-2014) were analyzed.

Radio-median cubital / radiocephalic arteriovenous fistula at elbow to prevent vascular steal syndrome associated with brachiocephalic fistula: Review of 320 cases.

AIM: Radiocephalic arteriovenous fistula (AVF) at wrist is the vascular access of choice for dialysis. In the absence of a suitable vein at the wrist, a brachiocephalic fistula at elbow is usually constructed. In order to avoid the complication of vascular steal syndrome associated with the brachiocephalic fistula, an alternative operative technique involving the creation of radio-median cubital vein / radiocephalic fistula at elbow was evaluated. SETTINGS AND DESIGN: Retrospective study. MATERIALS AND METHODS: Between January 1990 and October 2005, 320 patients underwent creation of radio-m

Perivascular administration of metformin through 3D minichannels improved adventitia ingrowth and endothelialization for electrospinning vascular grafts.

Achieving rapid endothelialization to maintain the patency of small-diameter vascular grafts remains a significant clinical challenge. Conventional surface modification strategies provide limited biochemical signals for recruiting circulating endothelial cells and fail to mitigate perivascular scarring and fibrosis. Emerging evidence underscores the critical role of regenerated adventitia in facilitating fallout endothelialization. Based on this rationale, we hypothesized that pharmacological administration around vascular grafts could promote rapid adventitia ingrowth and accelerate fallout e

Engineered small-diameter vascular graft with tailored degradation profile transform into a living blood vessel after implantation in a murine model.

Current vascular replacements available on the market often require frequent interventions. In situ tissue engineering (TE), which utilizes biodegradable grafts that transform into autologous vascular tissue at the implantation site, presents a promising solution to these limitations. In this study, electrospun small-diameter scaffolds (2 mm ø) composed of polycaprolactone (PCL) and polydioxanone (PDO), reinforced with a 3D-printed PCL anti-kinking coil, were implanted in a rat abdominal aortic replacement model. Grafts were explanted after 3 and 12 months, and the results were compared with n

Impact of preoperative arterial stiffness index on graft patency following peripheral arterial bypass surgery.

INTRODUCTION: We aimed to investigate the impact of preoperative Arterial Stiffness Index (ASI) values on primary graft patency following peripheral arterial bypass surgery. METHODS: This retrospective cohort study included 92 patients who underwent femoro-popliteal bypass surgery between January 2019 and November 2024. Preoperative ASI values were measured using the Vasolab 5000 device. Patients were divided into two groups based on an ASI cut-off value of 10.0. Primary graft patency rates, Kaplan-Meier survival analysis, and multivariate Cox regression were performed to assess the relations

Elbow fistulas using autogeneous vein: patency rates and results of revision.

BACKGROUND: The provision and maintenance of vascular access remains a major cost to end stage renal failure programmes. There are few reports regarding the surgical revision of the failing native elbow arteriovenous fistula (AVF). PATIENTS AND METHODS: A retrospective case note review was performed on all patients identified from the hospital vascular access database as having undergone construction of an autogeneous vein elbow AVF. Over a seven year period 282 autogeneous vein AVFs were fashioned in 232 patients using the brachial artery as the in-flow conduit. Of these 208 were brachioceph

[A study of the long-term graft patency using a saphenous vein graft following aorto-coronary bypass surgery].

Long term graft patency after aorto-coronary (A-C) bypass surgery has been reported in the West and the data shows a lower patency rate in saphenous vein grafts (SVG) than internal mammary artery grafts. We studied the long term SVG patency in cases of Japanese patients at our institute, and we have compared our results with those in the West. The subjects, including children, were 211 cases who received A-C bypass surgery using at least one SVG from Jan., 1975 to Sept., 1989. 182 of these cases (examination rate: 86%) received a postoperative angiography one or three months after surgery. Effects of antiplatelet agents in combination with endothelial cell seeding on small-diameter Dacron vascular graft performance in the canine carotid artery model.

The purpose of this study was to assess the success of endothelial cell-seeded and non-seeded small-diameter vascular grafts in dogs medicated with antiplatelet agents. Eighty dogs underwent bilateral carotid artery replacements with 6 cm lengths of 4 mm I.D. double-velour Dacron grafts. In each dog one graft was seeded with enzymatically derived autologous endothelial cells; the contralateral graft was nonseeded. The following anti-platelet medications were administered beginning 4 days preoperatively: aspirin (5 grains every day); dipyridamole (50 mg twice a day); aspirin plus dipyridamole (

Early Outcomes of Complex Vascular Reconstructions in Lower Extremities Using Spiral and Panel Vein Grafts.

BACKGROUND: Spiral saphenous vein grafts (SSVG) or paneled vein grafts (PVG) can be used when the diameter of the autologous great saphenous vein does not match the vessel that needs to be repaired. This study aimed to present early results of complex vascular reconstruction with SSVGs and PVGs in the lower extremities. METHODS: From May 2019 through January 2021, 6 SSVGs and 3 PVGs were used for vascular reconstruction in 9 patients. Patient data were collected retrospectively, including age, gender, cause of vascular pathology, target vessels, concomitant injury, surgical method, additional

Migrainous spasm simulating carotid dissection: a pitfall in MR arteriographic findings.

An MR arteriogram obtained in a patient with migraine headaches during prodrome showed features of carotid dissection. A repeated study conducted within minutes demonstrated complete patency. Vascular spasm was proposed to be the cause. Migraine occurs in more than 10% of the population. The possibility of misdiagnosing a vascular spasm as carotid dissection is real and should be guarded against since there are great prognostic and treatment implications.
