Clinical Outcomes After Cervical Transcorporeal Microdecompression and Vertebral Body Access Channel Repair.

BACKGROUND: Although anterior cervical decompression and fusion (ACDF) can be performed using minimally invasive techniques, the extensive removal of anatomical keystones during decompression requires a segmental fusion to restore biomechanical stability. Treatment with arthrodesis techniques may result in a prolonged recovery time, loss of motion, and the need for further treatment if a pseudarthosis or adjacent segment disease occur. Transcorporeal micro decompression (TCMD) is a newly developed motion sparing, minimally invasive anterior cervical spine decompression procedure that utilizes

Vertebral body split fracture after a single-level cervical total disc replacement.

Cervical total disc replacement (TDR) is a viable option for the surgical treatment of degenerative disc disease. This 67-year-old nonsmoking male patient underwent single-level ProDisc-C cervical TDR at C5-6 without any intraoperative problem. His radicular pain improved and he had no neck pain immediately after the operation. However, on postoperative Day 3, a radiograph demonstrated a vertical split fracture of the C-5 vertebra. This fracture was managed conservatively, and 2 years postoperatively a follow-up CT scan demonstrated stable device position and fusion of the fracture. Although t

Transpedicle body augmenter for vertebral augmentation in symptomatic multiple osteoporotic compression fractures.

BACKGROUND: Multiple osteoporotic vertebral compression fractures (VCFs) have been treated with polymethylmethacrylate augmentation; however, there are cement complications and long-term fracture healing that are unknown. Transpedicle body augmenter (a porous titanium spacer) has been reported as an internal support to reconstruct the vertebral body combining short-segment fixation in burst fracture and Kümmell's disease with cord compression. Transpedicle body augmenter for vertebral augmentation (TpBA) also has been reported successfully in treating single painful VCF and vertebral metastasi

Post-radiation reactive changes in a single vertebral body mimicking metastatic pineoblastoma.

This 18-year-old woman presented with headache and diplopia over several months and was found to have an enhancing pineal tumor with resultant obstructive hydrocephalus. Following standard preoperative diagnostic tests, including spinal axis imaging, the patient was taken to the operating room for an endoscopic third ventriculostomy to relieve hydrocephalus and then subsequently underwent a craniotomy for gross-total resection of the pineal mass. The patient was discharged after an uneventful hospital course and received standard adjuvant cranial-spinal radiation and chemotherapy as an outpati

[Effect of vertebral body stiffness before and after vertebroplasty on intradiscal pressure].

Fractures of osteoporotic vertebral bodies are increasingly stabilized with bone cement. The effects of vertebral-body stiffness before and after augmentation with bone cement and of wedge-shaped vertebral body fractures on intradiscal pressure are insufficiently known. In a finite element model of the lumbar spine the elastic modulus of cancellous bone as well as the amount and the elastic modulus of bone cement were varied and the dependency of intradiscal pressure on these parameters was calculated. In addition, a wedge-shaped vertebral-body fracture was simulated. The bulge of the vertebra

Finite-element stress analysis of the normal and osteoporotic lumbar vertebral body.

A finite-element model of an isolated elderly human L3 vertebral body was developed to study how material properties and loading conditions influence end-plate and cortical-shell displacements and stresses. The model consisted of an idealized geometric representation of an isolated vertebral body, with a 1-mm-thick end plate and cortical shell. For uniform compression, large tensile stresses occurred all around the cortical shell just below the end plate as a result of bending of the cortical shell as it supported the end plate. Large tensile bending stresses also developed in the inferior sur

Cervical vertebral body fracture with ankylosing spondylitis treated with cervical pedicle screw: A fracture body overlapping reduction technique.

We describe a patient with ankylosing spondylitis (AS) with cervical spinal fracture treated with cervical pedicle screw placement (CPS) through a single posterior approach. A 43-year-old male patient with AS visited our emergency centre due to paralysis following a trauma. Coronal reconstructed cervical spine computed tomography (CT) scan showed a C5 oblique fracture, and the bilateral pedicles were separated superiorly and inferiorly. The sagittal reconstructed CT image revealed bamboo spine and C5 vertebrae body fracture. Hyperextension between the fractured segments of the C5 body was note

Cervical vertebral body metastasis from a gonadotroph pituitary neuroendocrine tumor: illustrative case.

BACKGROUND: Metastatic dissemination of pituitary neuroendocrine tumors (PitNETs) is uncommon and portends a poor prognosis. Within this spectrum, osseous involvement of the vertebral body is particularly rare, and, specifically for the gonadotroph subtype, cervical vertebral body metastasis has not, to the authors' knowledge, been documented. OBSERVATIONS: The authors report a clinically nonfunctioning gonadotroph PitNET with histologically confirmed C5 vertebral metastasis. A 40-year-old man, who had undergone multiple surgeries for recurrent pitNET, presented with severe axial neck pain an

Fat Body Mass and Vertebral Fracture Progression in Women With Breast Cancer.

IMPORTANCE: Women with early breast cancer (EBC) exposed to aromatase inhibitors (AIs) may experience fragility fractures despite treatment with bone-active drugs. Risk factors for fractures in patients receiving AIs and denosumab have not been explored to date. OBJECTIVES: To evaluate whether an association exists between dual x-ray absorptiometry (DXA)-measured fat body mass (FBM) and vertebral fracture (VF) progression in postmenopausal women with EBC undergoing adjuvant therapy with AIs in combination with denosumab and to examine whether VF was associated with common risk factors for bon

The different applications of Vertebral Body Tethering - Narrative review and clinical experience.

BACKGROUND: Vertebral body tethering (VBT) has been originally developed as a growth modulation technique for the surgical management of skeletally immature patients with adolescent idiopathic scoliosis (AIS). Given the positive results obtained in this setting, the use of VBT is gradually expanding to other patient categories, such as those with no or limited remaining growth or with non-idiopathic scoliosis. Aim of this manuscript is to offer an overview over the current applications of VBT, along with imaging and comments derived from the clinical experience. The work was based on a literat

Vertebral body tethering: An alternative to posterior spinal fusion in idiopathic scoliosis?

INTRODUCTION: Skeletally immature patient with adolescent idiopathic scoliosis (AIS) whose curves continue to progress despite bracing should be treated surgically. Vertebral body tethering (VBT) is a non-fusion, compression-based, growth preserving alternative to posterior spinal fusion (PSF) based on the concept of 'growth modulation' to prevent possible functional complications secondary to fusion while correcting scoliotic deformity. This review aims to shed light on the indications of VBT, short- and medium-term outcomes, describe the surgical technique and associated complications, and t

Spontaneous Lumbar Curve Correction Following Vertebral Body Tethering of Main Thoracic Curves.

BACKGROUND: Growth modulation through anterior vertebral body tethering (AVBT) has emerged as a fusionless option for the treatment of progressive scoliosis. When tethering the main thoracic curve, the compensatory thoracolumbar/lumbar curve must correct indirectly as a result. The present study evaluated the response of these lumbar curves following AVBT of the main thoracic curves. METHODS: Patients who underwent thoracic AVBT and who had a minimum follow-up of 2 years were included. Magnitudes of the thoracic and lumbar curves were recorded preoperatively and at the first-erect and 2-year
