Insights into Accelerated MRI Protocols for Pediatric Brain Assessment in Emergency Cases.

Two accelerated magnetic resonance imaging (MRI) protocols for pediatric brain imaging, GOBrain and Deep Resolve Swift Brain, developed by Siemens Healthineers (Erlangen, Germany), were evaluated in a series of clinically relevant pediatric cases at 3 Tesla. Pediatric patients are particularly prone to motion, may be uncooperative, and often require sedation, especially in emergency settings. Consequently, there is a persistent clinical demand for fast brain MRI protocols that provide diagnostically sufficient image quality while minimizing examination time. Contemporary turbo spin-echo (TSE)-

Endoscopic Posterior Cervical Fusion for the Treatment of Atlantoaxial Pathologies.

BACKGROUND AND OBJECTIVES: Surgery for the upper cervical spine (atlantoaxial region) is a significant challenge because of its complex anatomy and proximity to vital neurovascular structures. Traditional posterior approaches require extensive muscle dissection, leading to considerable tissue trauma. Although endoscopic techniques have revolutionized minimally invasive spine surgery, their application for simultaneous decompression, fusion, and internal fixation in the atlantoaxial region has not been previously documented. This study aims to preliminarily investigate the technical feasibility

Development and Validation of Machine Learning Models to Optimize Imaging and Referrals for Dizziness in the Emergency Department.

BACKGROUND: Dizziness and vertigo are common emergency department (ED) presentations, but only 2%-5% receive a serious diagnosis, such as stroke or transient ischemic attack (TIA). Due to the lack of reliable validated prediction tools, many undergo unnecessary imaging and consultations, highlighting the need for improved risk stratification. OBJECTIVE: To develop machine learning (ML) models that predict serious diagnoses in ED patients presenting with dizziness or vertigo. METHODS: This multicenter cohort study included 6637 ED patients with dizziness, vertigo, or imbalance from September

Validation of the Sudbury Vertigo Risk Score to risk stratify for a serious cause of vertigo.

INTRODUCTION: In 2022, nearly 0.5 million Canadians visited an emergency department (ED) for dizziness, accounting for over 3.5% of all ED visits. Of these patients, only 2%-5% received a serious diagnosis. The cost of ED and inpatient care for dizziness in Canada exceeds $200 million per year, of which neuroimaging accounts for a large proportion. Over one-third of dizziness patients undergo a CT scan of the head, 96% of which are negative. Despite extensive investigation, patients discharged with a benign dizziness diagnosis have a 50-fold increased risk of being admitted to the hospital wit

Chiari type I presenting as left glossopharyngeal neuralgia with cardiac syncope.

Glossopharyngeal neuralgia is an uncommon craniofacial pain syndrome that is occasionally associated with cardiac syncope. However, we relate Chiari I syndrome as a cause of this clinical picture for the first time in the literature. The authors analyze the relevant literature and discuss the pathogenesis and treatment of associated syndromes. We describe the case of a 45-year-old female patient who presented with a 3-year history of left glossopharyngeal neuralgia with occasional dysphagia and episodes of syncope when eating or swallowing. The pain was not disseminated to the right side and w

Comparing Clinical Characteristics and Outcomes of Tenecteplase Versus Alteplase in Young Adult Patients With Stroke: A Secondary Analysis of the AcT Trial.

BACKGROUND: With the increasing use of tenecteplase for acute ischemic stroke, it is important to determine its safety in young adults. We aimed to characterize the clinical characteristics and imaging findings of young adults (≤45 years) in the AcT trial (Intravenous Tenecteplase Compared With Alteplase for Acute Ischaemic Stroke in Canada) and to compare their safety and efficacy outcomes. METHODS: This is a secondary analysis of the AcT trial, a phase 3 pragmatic multicenter registry-linked randomized controlled trial. We describe clinical and imaging characteristics of young adults with a

A supraomohyoidal plexus block designed to avoid complications.

Interscalene blocks of the brachial plexus are used for surgery of the shoulder and are frequently associated with complications such as temporary phrenic block, Horner syndrome or hematoma. To minimize the risk of these complications, we developed an approach that avoids medially directed needle advancement and favors spread to lateral regions only: the supraomohyoidal block. We tested this procedure in 11 cadavers fixed by Thiel's method. The insertion site is at the lateral margin of the sternocleidomastoid muscle at the level of the cricoid cartilage. The needle is inserted in the axis of

[Intermediate medullary infarction: a case report].

A 68-year-old man presented with right eye pain and vertigo. Thereafter, he gradually leaned rightward, then laid down. He felt nausea and vomited. His right upper eyelid drooped and he felt dysethesia of the right hand. On neurological examination, ptosis of his right eye with slightly miotic right pupil, paresis of the right soft palate and hoarseness were noted. Arm deviation test demonstrated rightward deviation. He presented sensory ataxia of the right upper and lower extremities: finger nose test showed mild dysmetria of the right upper extremity, heel knee test demonstrated dysmetria of

Three-dimensional computerized tomography angiography-guided surgery of acutely ruptured cerebral aneurysms.

OBJECT: The aim of this study was to assess whether aneurysm surgery can be performed in patients with ruptured cerebral aneurysms by using three-dimensional computerized tomography (3D-CT) angiography alone, without conventional catheter angiography. METHODS: In a previous study, 60 patients with subarachnoid hemorrhage (SAH) from ruptured aneurysms were prospectively evaluated using both 3D-CT and conventional angiography, which resulted in a 100% accuracy for 3D-CT angiography in the diagnosis of ruptured aneurysms, and a 96% accuracy in the identification of associated unruptured aneurysm

The angiosomes of the mammals and other vertebrates.

This is a comparative study of the vasculature of the integument and underlying deep tissues of a range of mammals and other vertebrates. The investigation was conducted in the pig, monkey, dog, cat, possum, guinea pig, rat, rabbit, duck, and toad. The results from each are compared not only to each other, but also to previously performed human studies. The arterial network of the fresh animal cadaver was injected with a mixture of lead oxide and gelatin. The vascular anatomy of the skin, deep tissues, and individual muscles was defined by dissection, cutaneous perforator counts, photography,

A Novel Approach to Atlanto-Occipital (AO) Joint Injection: A Cadaveric Study.

BACKGROUND: Atlanto-occipital (AO) joint instability and arthrosis are significant contributors to cervicogenic headaches and related disability. Injection and radiofrequency treatment of the AO joint are innovative approaches. However, the literature providing guidance on safe access to these joints remains sparse. The AO joint presents a high-risk target for interventions due to its proximity to the vertebral artery (VA), potentially compromising vascular supply to the cord, brainstem, cerebellum, and occipital lobe. OBJECTIVES: We describe the technical details of a novel fluoroscopy-guide

Stent angioplasty in patients with vertebral artery ostial stenosis: clinical and angiographic outcomes in 525 patients.

BACKGROUND: The optimal treatment approach for patients with severe atherosclerotic vertebral artery ostial stenosis (VAOS) is not yet supported by evidence from large-scale studies. Even with optimal medical treatment and effective risk factor management, substantial numbers of patients experience symptoms or recurrent strokes. Endovascular treatment is a potential solution, particularly when medicinal therapy alone is insufficient for symptom and stroke risk reduction. This study was aimed at assessing the safety and effectiveness of stent angioplasty in patients with VAOS. METHODS: This si

Resection of a Posterior Fossa Endodermal Cyst With Exoscopic Assistance: 2-Dimensional Operative Video.

Neurenteric cysts are rare benign congenital tumors of endodermal origin that most commonly occur in the cervical and upper thoracic spine, with only about 10% to 18% of the reported cases occurring intracranially.1 A definitive preoperative diagnosis is complicated by the variable appearance of neurenteric cysts on magnetic resonance (MR) imaging.2 The recommended treatment of neurenteric cysts is complete surgical resection when possible.3,4 We present a case of a posterior fossa neurenteric cyst. A 33-yr-old man without medical history presented with left-sided headache and mild left-sided

Neurological Complications After Thoracic Endovascular Repair (TEVAR): A Narrative Review of the Incidence, Mechanisms and Strategies for Prevention and Management.

Background: Thoracic endovascular aortic repair (TEVAR) has evolved the management of descending thoracic aortic disease, but neurological complications-particularly spinal cord ischemia (SCI), stroke, and postoperative delirium-remain among the most feared adverse events, adversely affecting survival, quality of life, and functional independence. Objectives: The aim of this study was to provide a contemporary narrative synthesis (2000-2025) of the incidence, mechanisms, risk factors, prevention, and management of neurological complications after TEVAR, emphasizing how current evidence support

A radio-anatomical study of median arcuate ligament syndrome: unveiling the morphology and morphometry of median arcuate ligament, celiac trunk, and superior mesenteric artery.

PURPOSE: In the context of medical literature, a significant lacuna exists in understanding median arcuate ligament syndrome (MALS). While clinical aspects are well documented, literature lacks a robust exploration of the anatomical relationship between the celiac trunk and the median arcuate ligament (MAL). METHODS: Morphometric parameters, including the vertebral level of MAL origin, MAL thickness, celiac trunk (CeT) origin level, diameter, and distances between CeT/Superior Mesenteric Artery (SMA) and the MAL center were observed on 250 CT angiograms. Cadavers (n = 11) were dissected to ex
