Extracorporeal Off-Pump Antegrade Cerebral Perfusion in Reconstructive Surgery for Type A Aortic Dissection With Cerebral Malperfusion.

Introduction Type A aortic dissection (TAAD) is a life-threatening condition that often leads to cerebral malperfusion (CM), a severe complication that can result in permanent neurological damage. Traditionally, a cardiopulmonary bypass (CPB) with selective antegrade cerebral perfusion (ACP) is employed during aortic arch reconstruction to protect cerebral circulation. However, the use of CPB carries inherent higher risks, including embolic events, hypothermia, and interrupted cerebral perfusion, especially in patients with CM. This study evaluates an innovative off-pump extra-corporeal ACP te

Percutaneous transluminal angioplasty of intracranial artery stenosis: clinical results in 24 patients.

Circumscribed stenotic lesions of the intracranial arteries can cause cerebral ischemia by hemodynamic and/or thromboembolic mechanisms. Anticoagulation therapy, antiplatelet therapy, and bypass surgery are treatment strategies that have no direct impact on the underlying lesion. This study summarizes the experience of a single institution at which percutaneous transluminal angioplasty (PTA) of intracranial atherosclerotic stenoses was performed. The authors performed a retrospective analysis of 24 consecutive patients. Their medical histories (cardiovascular risk factors, current clinical sig

Increased prevalence of hyperhomocysteinemia in cervical artery dissection causing stroke: a case-control study.

BACKGROUND: Spontaneous cervical artery dissection (sCAD) is a nonatherosclerotic vascular disease of unknown etiology. Mild elevation of total plasma homocysteine (tHcy) levels may be a risk factor for sCAD, but the precise mechanism remains unknown. On the other hand, mild hyperhomocysteinemia is also associated with ischemic stroke related to atherothrombotic or small artery disease. We undertook a case-control study to compare the prevalence of mild hyperhomocysteinemia and tHcy levels between patients with a first ischemic stroke due to sCAD and healthy volunteers, as well as patients wit

[Management of subarachnoid hemorrhages without detectable aneurysm].

In this study, 21 patients with subarachnoid hemorrhage (SAH) but negative angiography were evaluated. Angiography was performed twice on each patient, that is, on admission and at 2 weeks following admission. All patients had severe headache of sudden onset, a characteristic manifestation of SAH. Clinical grades on admission (Hunt and Kosnik classification) were generally good: 17 patients were in grade I or II and 4 patients were in grade III. SAH was confirmed by the presence of subarachnoid clot on CT in all cases. Based on the distribution of SAH, CT findings were classified into two patt

Native valve endocarditis caused by Corynebacterium striatum without underlying structural heart disease or indwelling cardiovascular medical devices: a case report.

BACKGROUND: Corynebacterium striatum (C. striatum) is a gram-positive, anaerobic bacillus found both environmentally and in human skin and nasal mucosa flora. It is reportedly the etiologic agent of community-acquired and nosocomial diseases and is significantly associated with bacteremia and medical endovascular devices. This is the rare case of mitral valve native valve endocarditis (NVE) caused by C. striatum occurring in a young adult without underlying structural heart disease or indwelling cardiovascular medical devices successfully treated with multidisciplinary therapy. Anatomical and histological characterization of the filum terminale in dogs.

The filum terminale (FT) remains poorly characterized in the veterinary literature, limiting understanding of its role in spinal cord and nerve root pathologies such as tethered cord syndrome. This study aimed to establish baseline anatomical and histological features of the FT in neurologically normal dogs. Eight adult canine cadavers euthanized for non-neurological reasons were examined. Dissection was performed via dorsal laminectomy (n = 4) and midline sectioning (n = 4). Histological analysis (n = 4) included hematoxylin and eosin, Klüver-Barrera, Masson's trichrome, and Verhoeff-Van Gies

Initial Outcomes of the Gore TAG Thoracic Branch Endoprosthesis for Endovascular Repair of Blunt Thoracic Aortic Injury.

BACKGROUND: Endovascular repair of blunt thoracic aortic injury (BTAI) has dramatically reduced the morbidity and mortality of intervention. Injuries requiring zone 2 coverage of the aorta traditionally require left subclavian artery (LSA) sacrifice or open revascularization. Furthermore, these injuries are associated with an increased risk of in-hospital mortality and long-term morbidity. Here we report 1-year outcomes of total endovascular repair of BTAI with the GORE® TAG® Thoracic Branch Endoprosthesis for LSA preservation. METHODS: Across 34 investigative sites, 9 patients with BTAI requ

Anatomical impact of C1 transverse process resection on inferior and inferolateral exposure at the craniocervical junction: a cadaveric study.

BACKGROUND: Surgical exposure of the craniocervical junction (CCJ) is challenging because of the narrow osseous corridor and complex neurovascular anatomy. Although the far lateral approach is well established, the relative anatomical contribution of sequential osseous modifications used in far lateral surgery remains incompletely understood. This study aimed to provide a stepwise anatomical comparison of retrosigmoid, far lateral, and extended far lateral approaches, focusing on neurovascular exposure before C1 transverse process resection. METHODS: Four formalin-fixed, silicone-injected adu

A Diagnostic Approach to Stroke in Young Adults.

Optimal diagnosis and management of stroke in young adults benefit from a multidisciplinary team, including a vascular neurology specialist. In addition to the "standard" vascular risk factors including smoking, hypertension, diabetes, and hyperlipidemia, one needs to consider alternative etiologies including substance abuse, carotid/vertebral artery dissections, and rare genetic conditions among others. Once a young patient is determined to have had a stroke, the next question a clinician should ask is why did this patient have a stroke? A "heart to head" diagnostic approach is recommended. A

AI-assisted detection of an initially missed infrarenal abdominal aortic injury in a polytrauma patient: A case report.

BACKGROUND: Blunt abdominal aortic injury (BAAI) is rare but carries high mortality. Traditionally, hemodynamically unstable polytrauma patients are rushed directly to the operating room; however, because BAAI is frequently missed during exploratory laparotomy, definitive diagnosis requires computed tomography (CT). The Hybrid Emergency Room System (HERS), which integrates resuscitation, surgery, interventional radiology (IVR), and CT imaging, enables rapid CT scanning even in hemodynamically unstable patients. Nevertheless, the risk of missing BAAI persists when resources for image interpreta

Diagnosis and management of zone III carotid injuries.

The management of patients with extracranial carotid injury at the base of the skull (zone III) is challenging due to inaccessibility, severity, and associated injuries. In an effort to formulate a systematic approach to the evaluation and management of zone III carotid injuries, the records of 13 consecutive patients with such injuries were reviewed: nine sustained penetrating injuries and four had blunt injuries. A total of 16 arteries were injured: internal carotid (11), external carotid (four), and vertebral (one). Neurological examinations revealed a central nervous system deficit in 1/9
