Long-term follow-up study on obstructive hypertrophic cardiomyopathy patients treated with disopyramide: evidences of a notable trend in symptom control within a real-world clinical setting.

BACKGROUND: In obstructive hypertrophic cardiomyopathy (HOCM), disopyramide is used in patients who remain symptomatic despite β-blockers or verapamil. However, effectiveness of disopyramide therapy has not been clearly established due to inconsistent definition of responders and the insufficient length of follow-ups reported in literature. To address these shortcomings, we have conducted a retrospective analysis from detailed databases with long follow-up, from two HCM Referral Centers. METHODS: 62 symptomatic HOCM patients (43% women, age 52 ± 14 years) with left ventricular (LV) outflow tr

Effects of Renin-Angiotensin-Aldosterone System Inhibition on Left Ventricular Hypertrophy, Diastolic Function, and Functional Status in Patients With Hypertrophic Cardiomyopathy: A Systematic Review.

The renin-angiotensin-aldosterone system (RAAS) plays a vital role in cardiovascular homeostasis by regulating blood pressure, salt, and water balance. The kidneys produce renin which converts angiotensinogen to angiotensin-1 (AT-I) and angiotensin-converting enzyme (ACE) to angiotensin-II (AT-II). AT-II binds to receptors in the adrenal cortex to release aldosterone. AT-II and aldosterone promote water and salt retention, vascular tone, and myocardial contractility. These physiological changes raise blood pressure and circulation. Reduced renal perfusion pressure sensed by baroreceptors and t

Timing of invasive septal reduction therapies and outcome of patients with obstructive hypertrophic cardiomyopathy.

BACKGROUND: Whether early vs. delayed referral to septal reduction therapies (SRT, alcohol septal ablation or surgical myectomy) bears prognostic relevance in hypertrophic obstructive cardiomyopathy (HOCM) is unresolved. We analyzed the impact of SRT timing on the outcome of HOCM patients. METHODS: We followed 126 patients for 5 ± 4 years after SRT (mean age 53 ± 15 years; 55 post-ASA and 71 post-SM). Based on time-to-treatment (TTT; from HOCM diagnosis to SRT), patients were divided into three groups: "<3" years, N = 50; "3-5" years, N = 25; ">5" years, N = 51. RESULTS: Patients with TTT >

Pacing for drug-refractory or drug-intolerant hypertrophic cardiomyopathy.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is a genetic disease with an autosomal-dominant inheritance for which negative inotropes are the most widely used initial therapies. Observational studies and small randomised trials have suggested symptomatic and functional benefits using pacing and several theories have been put forward to explain why. Pacing, although not the primary treatment for HCM, could be beneficial to patients with relative or absolute contraindications to surgery or alcohol ablation. Several randomised controlled trials comparing pacing to other therapeutic modalities ha

The complex genetics of hypoplastic left heart syndrome.

Congenital heart disease (CHD) affects up to 1% of live births. Although a genetic etiology is indicated by an increased recurrence risk, sporadic occurrence suggests that CHD genetics is complex. Here, we show that hypoplastic left heart syndrome (HLHS), a severe CHD, is multigenic and genetically heterogeneous. Using mouse forward genetics, we report what is, to our knowledge, the first isolation of HLHS mutant mice and identification of genes causing HLHS. Mutations from seven HLHS mouse lines showed multigenic enrichment in ten human chromosome regions linked to HLHS. Mutations in Sap130 a

Mavacamten in Patients With Hypertrophic Cardiomyopathy Referred for Septal Reduction: Week 56 Results From the VALOR-HCM Randomized Clinical Trial.

IMPORTANCE: There is an unmet need for novel medical therapies before recommending invasive therapies for patients with severely symptomatic obstructive hypertrophic cardiomyopathy (HCM). Mavacamten has been shown to improve left ventricular outflow tract (LVOT) gradient and symptoms and may thus reduce the short-term need for septal reduction therapy (SRT). OBJECTIVE: To examine the cumulative longer-term effect of mavacamten on the need for SRT through week 56. DESIGN, SETTING, AND PARTICIPANTS: This was a double-blind, placebo-controlled, multicenter, randomized clinical trial with placeb

Transseptal Transcatheter Mitral Valve Replacement for Mitral Annular Calcification: ENCIRCLE MAC Registry 1-Year Outcomes.

BACKGROUND: Patients with symptomatic mitral valve dysfunction (MVD) and mitral annular calcification (MAC) represent one of the most challenging populations in structural heart disease. Advanced age, multiple comorbidities, extensive annular calcification, and unfavorable anatomy often preclude surgical mitral valve replacement or transcatheter repair, leaving many patients with limited treatment options. Whether fully percutaneous transseptal transcatheter mitral valve replacement (TMVR) can provide a meaningful treatment pathway for patients with MAC-related MVD remains uncertain. OBJECTIV

Long non-coding RNAs as molecular hubs integrating inflammatory and osteogenic pathways in calcific aortic valve disease.

SIGNIFICANCE: Cardiovascular disease remains the leading cause of morbidity and mortality worldwide. Among valvular pathologies, CAVD is the most prevalent and poses a growing burden on the aging population. Once considered a passive degenerative process, aortic stenosis (AS) is now understood to be an actively regulated disease characterized by progressive leaflet fibrosis, calcification, and inflammation, ultimately leading to left ventricular outflow obstruction and heart failure. Current treatment options are limited to surgical or transcatheter valve replacement, as no pharmacological the

Evolution of aortic valve replacement in children: a single center experience.

BACKGROUND: The availability of an ideal prosthesis for aortic valve replacement (AVR) in children remains controversial due to an early degeneration of xenografts and the potential risks related to anticoagulation with mechanical prostheses. This has led many surgeons to the Ross procedure. This study outlines the evolution of our 30-year experience with AVR in children. METHODS: One hundred and forty-seven children, aged 10 days to 18 years (mean 11.9+/-5.7 years), underwent AVR between 1974 and June 2005. Preoperative diagnosis included aortic insufficiency (n=39), aortic stenosis (n=14),

Reversible myocardial perfusion defects in patients not suffering from obstructive epicardial coronary artery disease as assessed by coronary angiography.

In approximately 10-30% of patients presenting with angina complaints, normal or non-obstructive coronary arteries are found on angiography. In this review paper, available literature on the underlying pathophysiological substrate explaining these discrepancies is reviewed. Both histological studies as well as studies using intravascular ultrasound e.g. the PROSPECT trial, show that epicardial coronary vessel significant lumen stenosis may be delayed until a plaque occupies 40% of the internal elastic lamina area. Limited available data suggest that these angiographically undetectable plaques

Early and midterm outcomes of a modified single-patch technique for repairing complete atrioventricular septal defect in children and adults.

OBJECTIVES: The "modified single-patch" technique (Nunn's technique) has become more widely adopted, especially in small infants with common atrioventricular septal defect (CAVSD). In this study, we retrospectively reviewed our surgical experiences of the "modified single-patch" technique with CAVSD patients, including newborns and adults, to reveal the early and midterm clinical results. METHODS: There were 233 cases diagnosed with CAVSD undergoing biventricular repair in our hospital from June 2009 to 2016. A total of 197 cases (84.5%) with the "modified single-patch" technique were enrolle

Point-of-care ultrasound in emergency department sepsis and septic shock: a pragmatic framework for source-oriented assessment and physiologic reassessment.

In emergency department (ED) sepsis and septic shock, early management is often dominated by two bedside uncertainties: identifying a clinically plausible infection source and determining whether additional fluid is likely to improve perfusion or worsen pulmonary or systemic congestion. Point-of-care ultrasound (POCUS) may contribute to both questions when it is used as a trigger-based, time-boxed adjunct rather than as a comprehensive screening test or stand-alone diagnostic adjudicator. This article presents a pragmatic narrative review of multiorgan POCUS in adult ED sepsis and septic shock

Pre-perfusion coronary wedge pressure and microvascular obstruction in anterior ST elevation myocardial infarction (STEMI): An analysis from the EUROICE study.

BACKGROUND: Microvascular injury (MVI), commonly assessed on cardiac magnetic resonance (CMR) as microvascular obstruction (MVO), is frequently observed in patients with ST-elevation myocardial infarction (STEMI) despite timely primary percutaneous coronary intervention (PPCI), and is key to adverse left ventricular (LV) remodelling and poor outcomes. Established invasive indices, such as the index of microvascular resistance (IMR), are obtained after reperfusion and therefore do not capture microvascular compromise present during coronary occlusion.
