Clinical and radiographic outcome in the management of posterior circulation aneurysms by use of direct surgical or endovascular techniques.

OBJECTIVE: Posterior circulation aneurysms can be difficult lesions to treat surgically, and they have potential for high morbidity and mortality, particularly in elderly patients or those in poor neurological condition. In an effort to improve outcomes, our combined neurosurgical and neuroendovascular unit has used both surgical clipping and endovascular coiling techniques to treat posterior circulation aneurysms. Patients considered at high risk for surgery were managed with endovascular treatment. METHODS: From 1990 to 1998, 197 posterior circulation aneurysms in 189 patients were treated

Balloon-in-stent technique for the constructive endovascular treatment of "ultra-wide necked" circumferential aneurysms.

INTRODUCTION: Circumferential aneurysms, which incorporate >180 degrees of the circumference of the parent vessel, present a unique therapeutic challenge, particularly in circumstances in which a deconstructive treatment strategy is not feasible. We detail a novel technique for endovascular parent vessel reconstruction with aneurysm embolization. METHODS: We performed a retrospective review of the prospectively maintained databases of our two institutions to identify cases in which a balloon-in-stent technique had been used to treat circumferential aneurysms. During the first stage of this te

A two-dimensional operative video of a midline-sparing para-articular approach for resection of a calcified cervical synovial cyst: A variant of the minimally invasive left C5/C6 foraminotomy.

BACKGROUND: Synovial cysts are rare degenerative lesions arising from facet joints that likely arise due to hypermobility, trauma, and inflammation. These lesions can calcify, making resection challenging when adjacent to critical neurovascular structures. Operative intervention classically warrants a two-level partial or complete laminectomy, or a unilateral "open" or minimally invasive medial facetectomy/foraminotomy; notably, the latter is largely equivalent to the procedure described here as a "midline sparing para-articular approach." CASE DESCRIPTION: This two-dimensional operative vide

Plasma homocysteine, MTHFR C677T, CBS 844ins68bp, and MTHFD1 G1958A polymorphisms in spontaneous cervical artery dissections.

Mild hyperhomocysteinemia is a probable risk factor for atherosclerotic diseases and stroke. Recently, associations of elevated plasma homocysteine concentrations in the acute phase and of MTHFR 677 TT genotype with spontaneous cervical artery dissections (sCAD) have been reported. The purpose of this study was to test this hypothesis in the currently largest sample of patients with sCAD, taking into account known factors influencing plasma homocysteine levels. Ninety-five patients with past sCAD were compared with 95 age- and sex-matched healthy individuals. Homocysteine, vitamin B6, B12, fol

Operative simulation of anterior clinoidectomy using a rapid prototyping model molded by a three-dimensional printer.

BACKGROUND: As the anatomical three-dimensional (3D) positional relationship around the anterior clinoid process (ACP) is complex, experience of many surgeries is necessary to understand anterior clinoidectomy (AC). We prepared a 3D synthetic image from computed tomographic angiography (CTA) and magnetic resonance imaging (MRI) data and a rapid prototyping (RP) model from the imaging data using a 3D printer. The objective of this study was to evaluate anatomical reproduction of the 3D synthetic image and intraosseous region after AC in the RP model. In addition, the usefulness of the RP model

A Comprehensive Review of the Role of the Latest Minimally Invasive Neurosurgery Techniques and Outcomes for Brain and Spinal Surgeries.

Minimally invasive neurosurgical techniques offer reduced surgical trauma, lower complication rates, and faster recovery compared to traditional open procedures. Approaches such as keyhole craniotomy, endoscopic skull base surgery, endovascular neurosurgery, and percutaneous spinal techniques have advanced the management of cranial and spinal pathologies. These methods aim to minimize tissue disruption, blood loss, and hospital stay while maintaining surgical efficacy. This narrative review explores their evolution, applications, and advantages in modern neurosurgery.

Posterior minimally invasive percutaneous short-segment fixation combined with mini-open anterior L5 corpectomy for metastatic tumor: a case report and literature review.

BACKGROUND: Metastatic tumors of the lumbar spine are a common complication of advanced malignancies. They often present with severe pain, neurological deficits, and spinal instability. Surgical removal of L5 vertebral body metastases is challenging due to the complex anatomy and biomechanical characteristics of the lumbosacral junction. Corpectomy using a combined approach is a common surgical strategy for achieving pain relief, adequate tumor resection, spinal cord and root decompression, and spinal stabilization. However, this approach may be associated with significant surgical trauma and

Surgery of the tumors of the spine.

Spinal tumors encompass a heterogeneous group of primary and metastatic lesions requiring individualized management strategies based on tumor biology, patient prognosis, and mechanical stability. Decision-making frameworks, such as NOMS, SINS, and Tokuhashi score, provide structured guidance for selecting metastatic patients for surgery and defining individualized treatment strategies. En bloc resection remains the gold standard for selected primary malignant tumors, whereas separation surgery combined with stereotactic radiosurgery represents the cornerstone of modern metastatic spine disease

Complication Avoidance in Surgical Management of Vertebral Column Tumors.

The surgical management of spinal tumors has grown increasingly complex as treatment algorithms for both primary bone tumors of the spine and metastatic spinal disease have evolved in response to novel surgical techniques, rising complication rates, and additional data concerning adjunct therapies. In this review, we discuss actionable interventions for improved patient safety in the operative care for spinal tumors. Strategies for complication avoidance in the preoperative, intraoperative, and postoperative settings are discussed for approach-related morbidities, intraoperative hemorrhage, wo

Vertebral fracture and splenomegaly in a head and neck cancer producing granulocyte colony-stimulating factor: A case report of systemic complications associated with a cytokine-producing solid tumor.

Granulocyte colony-stimulating factor (G-CSF)-producing tumors are rare and are associated with a poor prognosis when they occur in the lungs and the head and neck region. Positron emission tomography/computed tomography has been reported to show systemic specific accumulation of fluorodeoxyglucose in these cases, but the systemic complications associated with the cytokines produced are not well known. We herein present the case of a G-CSF-producing maxillary sinus squamous cell carcinoma in a 73-year-old Japanese woman with a vertebral fracture and splenomegaly. These findings are known sever

Metastatic spinal tumor.

In accordance with extending survival periods of cancer patients, number of consecutively developing metastatic spinal tumor is also increasing. There have been improvements in the treatment results of metastatic spine tumor by virtue of the developments in diagnostic radiology, chemotherapy, adjuvant treatment, operative device and technique, discrete preoperative plan, and standardized operation. Accordingly, surgical indication has also increased. Clinically, in case of metastatic spine tumor, treatment of tumor itself should be focused on pain relief, preservation of neurologic function, p

Vertebroplasty and delayed subdural cauda equina hematoma: Review of literature and case report.

Vertebroplasy is considered an alternative and effective treatment of painful oncologic spine disease. Major complications are very rare, but with high morbidity and occur in less than 1% of patients who undergo vertebroplasty. Spinal subdural hematoma (SDH) is an extremely rare complication, usual developing within 12 h to 24 h after the procedure. We report the case of a tardive SDH in an oncologic patient who underwent VP for Myxoid Liposarcoma metastasis. Trying to explain the pathogenesis, we support the hypothesis that both venous congestion of the vertebral venous plexus of the vertebra
