Physiologically Based Pharmacokinetic Modeling and Therapeutic Drug Monitoring for Ertapenem Dose Optimization in Hypoalbuminemia Elderly Patients.

OBJECTIVE: As a highly protein-bound, time-dependent antibiotic, the duration for which the unbound concentration of ertapenem remains above the minimum inhibitory concentration (%T>MIC) is a critical pharmacokinetic/pharmacodynamic (PK/PD) parameter. In our clinical TDM practice, most enrolled patients were elderly (mean age 62±21 years), with hypoalbuminemia present in 23/39 (59.0%), and the protein binding rate in these patients was significantly lower than literature-reported values. This study aimed to (1) establish a method for determining total and unbound ertapenem concentrations; (2)

Improving SARS-CoV-2 RNA Detection in Wastewater: Comparison of Concentration Methods and the Effect of PCR Inhibitor Removal on Detection Sensitivity.

Wastewater-based epidemiology (WBE) has emerged as a valuable tool for monitoring community-level infections, capturing both symptomatic and asymptomatic cases, as demonstrated during the SARS-CoV-2 pandemic. However, WBE implementation is challenged by the complex nature of wastewater, which contains PCR inhibitors and varies significantly between wastewater treatment plants (WWTPs) due to differences in influent sources, industrial discharge, and environmental conditions. Furthermore, the lack of standardized protocols for virus concentration limits the practical and reliable use of WBE. Tim

Identification, Characterization and Multi-Target Mechanism of Novel ACE Inhibitory Peptides from Idesia polycarpa Seed Cake Protein with Ultrasound-Assisted Extraction.

Angiotensin-converting enzyme (ACE) inhibitory peptides are promising bioactive components for developing blood pressure-regulating functional foods. Idesia polycarpa meal (IPM), an underutilized protein-rich byproduct from oil processing, is currently restricted to low-value applications such as animal feed, and systematic research on its ACE inhibitory peptides remains largely absent. This study hypothesized that controlled enzymatic hydrolysis of IPM protein could release novel ACE inhibitory peptide candidates with high activity. Fourier-transform infrared spectroscopy and differential sca

Fluid restriction after pediatric cardiac surgery is independently associated With shorter mechanical ventilation: a retrospective cohort study With adjustment for cardiopulmonary bypass time.

BACKGROUND: Cardiopulmonary bypass (CPB) for pediatric cardiac surgery triggers a systemic inflammatory response driving postoperative fluid retention, prolonged ventilation, and acute kidney injury (AKI). The Starship Hospital guideline provides a structured framework for postoperative fluid restriction, but North American observational validation adjusting for CPB time has been lacking. OBJECTIVE: To evaluate the association between a Starship-derived fluid restriction protocol and clinical outcomes, with adjustment for operative complexity. METHODS: Retrospective single-center cohort of 3

ADAM19 participated in peritoneal fibrosis by regulating M2 macrophage polarization in peritoneal dialysis patients.

Peritoneal fibrosis, driven by M2 macrophage polarization, limits the long-term application of peritoneal dialysis (PD). Although ADAM19 is known to mediate fibrosis in other organs, its specific role in PD-associated peritoneal fibrosis remains unclear. PD patients were enrolled in a single center and divided into three groups depending on the PD time. Demographic and clinical data were collected. We detected the expressions of ADAM19, Notch1, Fibrosis-associated protein, chemokines and inflammatory factors in the peritoneum dialysis effluent by real-time PCR and western-blot assays. Macropha

Haemodynamic effects of low versus high dialysate temperature in haemodialysis patients: a single-blinded, randomised, cross-over study.

BACKGROUND: Cooling of the dialysate may improve intradialytic haemodynamics. This study compared low dialysate temperature (LDT) of 35 °C with high dialysate temperature (HDT) of 37 °C (standard) in terms of blood pressure (BP), intradialytic haemodynamics, and orthostatic BP. METHODS: In a single-blind, randomised, controlled, crossover study fourteen haemodialysis (HD) patients underwent a 4-h HD session with LDT (35 °C) or HDT (37 °C) separated by a 1- week interval. BP was monitored throughout HD. Intradialytic haemodynamics were assessed with ultrasound dilution technique (Transonic). O

Spontaneous pregnancy in a woman with diminished ovarian reserve following dietary supplementation with major royal jelly proteins: A case report.

RATIONALE: Royal jelly (RJ), a natural secretion produced by the hypopharyngeal and mandibular glands of worker honeybees, is a nutrient-rich substance that contains proteins, lipids, vitamins, and bioactive compounds. Among its key components, major royal jelly proteins (MRJPs) constitute the predominant fraction and have been linked to various physiological benefits. Our previous studies on female mice have shown that MRJPs can increase uterine and ovarian indices, estradiol levels, and endometrial thickening. Overexpression of MRJPs in female Drosophila can increase estrogen levels and acti

Fenofibrate-associated rhabdomyolysis complicated by dialysis-requiring acute kidney injury presenting with atypical clinical manifestations: A case report and literature review.

RATIONALE: Rhabdomyolysis (RM) may cause acute kidney injury (AKI), but the classic triad of myalgia, muscle weakness, and dark-colored urine is often absent. Fenofibrate-associated RM is rare and may be overlooked in older patients with chronic kidney disease (CKD), reduced renal reserve, or polypharmacy. PATIENT CONCERNS: A 70-year-old man with CKD was admitted with nausea, poor appetite, and a marked increase in serum creatinine for 1 week. Two months earlier, atorvastatin had been discontinued, and micronized fenofibrate had been started for hypertriglyceridemia. He had no obvious myalgia

Cardiac Tamponade Secondary to Severe Uremic Pericarditis in the Modern Dialysis Era: A Case Report.

Uremic pericarditis is currently an uncommon complication of end-stage renal disease (ESRD) in developed countries due to improved access to renal replacement therapy and earlier initiation of hemodialysis. Nevertheless, delayed or interrupted dialysis may still result in life-threatening manifestations, including massive pericardial effusion and cardiac tamponade. We present the case of a 44-year-old man with ESRD requiring hemodialysis, insulin-dependent type 2 diabetes mellitus, morbid obesity, hypertension, and heart failure with preserved ejection fraction who was admitted because of prog

Biological Activity of Plant-Derived Peptides, Hydrolysates and Digests in the Caco-2 Intestinal Epithelial Model: A Systematic Critical Review.

Plant-derived bioactive peptides are widely investigated for their effects on intestinal epithelial function using Caco-2 cells, with reported antioxidant, anti-inflammatory, antihypertensive and antidiabetic activities. However, this literature remains heterogeneous and has rarely addressed whether the tested material corresponds to the actual bioactive species. This PRISMA-compliant systematic review searched PubMed and Scopus for studies on plant-derived peptides, hydrolysates or digests in Caco-2 cells, identifying 34 eligible studies. The material tested in each study was classified a pri

In vivo measurement of pediatric extracorporeal oxygenator insensible losses; a single center pilot study.

INTRODUCTION: Fluid overload on Extracorporeal Membrane Oxygenation (ECMO) is associated with worse outcomes. Previous in vitro studies have attempted to quantify oxygenator-related insensible losses, as failure to account for this fluid loss may lead to inaccurate fluid balance assessment and potentially harmful clinical management, such as unnecessary exposure to diuretics, slow continuous ultrafiltration (SCUF), or continuous kidney replacement therapy (CKRT). We performed a novel in vivo study to measure insensible fluid losses in pediatric ECMO patients. METHODS: Pediatric ECMO patients

Lacticaseibacillus rhamnosus CRL 2244 secreted metabolites display killing and antibiotic synergistic activity against multi-drug resistant pathogens.

A growing increase in the number of serious infections caused by multidrug resistant bacteria (MDR) is challenging our society. Despite efforts to discover novel therapeutic options, few antibiotics targeting MDR have been approved by the Food and Drug Administration (FDA). Lactic acid bacteria have emerged as a promising therapeutic alternative due to their demonstrated ability to combat MDR pathogens in vitro. Our previous co-culture studies showed Lacticaseibacillus rhamnosus CRL 2244 as having a potent killing effect against carbapenem-resistant Acinetobacter baumannii (CRAB) strains. Here

Soluble urokinase plasminogen activator receptor is associated with cardiovascular calcification in peritoneal dialysis patients.

BACKGROUND: Cardiovascular disease (CVD) is an important cause of morbidity and mortality in peritoneal dialysis (PD) patients. Cardiovascular calcification (CVC) is highly prevalent in PD patients and could predict their cardiovascular mortality. Soluble urokinase plasminogen activator receptor (suPAR) is closely associated with coronary artery calcification in hemodialysis patients and is an important predictor of CVD. However, the role of suPAR in PD patients is poorly understood. We investigated the relationship between serum suPAR and CVC in PD patients. METHODS: Abdominal aortic calcifi

Restrictive versus Liberal Rate of Extracorporeal Volume Removal Evaluation in Acute Kidney Injury (RELIEVE-AKI): a pilot clinical trial protocol.

INTRODUCTION: Observational studies have linked slower and faster net ultrafiltration (UFNET) rates during kidney replacement therapy (KRT) with mortality in critically ill patients with acute kidney injury (AKI) and fluid overload. To inform the design of a larger randomised trial of patient-centered outcomes, we conduct a feasibility study to examine restrictive and liberal approaches to UFNET during continuous KRT (CKRT). METHODS AND ANALYSIS: This study is an investigator-initiated, unblinded, 2-arm, comparative-effectiveness, stepped-wedged, cluster randomised trial among 112 critically

Pharmacokinetics and Pharmacodynamics of Fungal Defensin NZX Against Staphylococcus aureus-Induced Mouse Peritonitis Model.

Staphylococcus aureus (S. aureus) is one of the most common pathogenic bacteria responsible for causing a life-threatening peritonitis disease. NZX, as a variant of fungal defensin plectasin, displayed potent antibacterial activity against S. aureus. In this study, the antibacterial and resistance characteristics, pharmacokinetics, and pharmacodynamics of NZX against the S. aureus E48 and S. aureus E48-induced mouse peritonitis model were studied, respectively. NZX exhibited a more rapid killing activity to S. aureus (minimal inhibitory concentration, 1 μg/ml) compared with linezolid, ampicill

Simple and fast determination of tetrodotoxin in human plasma based on hydrophilic-interaction/ion-exchange mixed-mode solid phase extraction combined with liquid chromatography-tandem mass spectroscopy.

In this study, we developed and validated a simple, fast and sensitive LC-MS/MS method for the measurement of tetrodotoxin (TTX) in human plasma. Three HILIC-type solid phase extraction (SPE) carriers (PSA, silica, Siphila i HILIX) with different stationary phase functional groups were compared. The Siphila i HILIX SPE plate containing multi-carboxyl groups was finally selected due to obviously better extraction recovery of TTX (about 80% of recovery from plasma samples) than the other two and no significant matrix effects were observed, which was speculated to have mixed-mode synergistic effe

Practice Patterns and Outcomes of Online Hemodiafiltration: A Real-World Evidence Study in a Russian Dialysis Network.

BACKGROUND: Evidence suggests that online hemodiafiltration (OL-HDF) is associated with improved survival. Whether the dose-response relationship between convective volume and mortality may be confounded by selection bias or descends from practice patterns is not clear. We sought to evaluate the role of patients' characteristics and practice patterns on OL-HDF dose and mortality in a large private dialysis network in the Republic of Russia. METHODS: In this multicenter, historical cohort study, we included adult incident patients on OL-HDF with at least 90 days of survival on renal replacemen
