Predictors of retention and attrition in a study of an advanced upper limb prosthesis: implications for adoption of the DEKA Arm.

PURPOSE: The purpose was to identify factors associated with completion of the VA home study of the DEKA Arm. Design and methodological procedures used: Differences between groups were examined using chi-square and t-tests. A multivariable logistic regression model predicting completion was generated and odds ratios (OR) for significant variables calculated. Post-hoc analysis was performed to plot the receiver operating characteristics (ROC) curve. RESULTS: Participants who completed were more likely to be prosthesis users at study onset (p = .03), and less likely to have a history of musculo

Blood transfusion is associated with increased risk of perioperative complications and prolonged hospital duration of stay among patients undergoing amputation.

OBJECTIVE: We evaluated the outcomes of patients undergoing major lower-extremity amputation who received packed red blood cell transfusion. METHODS: Using the dataset of the National Surgical Quality Improvement Program (2005-2011), we examined 5,739 above-knee and 6,725 below-knee amputations. Patients were stratified by perioperative (preoperative, intraoperative, or postoperative) blood transfusion. Outcomes included perioperative mortality, myocardial infarction (MI), thromboembolism, and duration of stay (DOS) at the hospital. Adjusted comparisons of outcomes between transfused and not-

Experiences of lower limb prosthetic users in a rural setting in the Mpumalanga Province, South Africa.

BACKGROUND:: Ambulation with a prosthesis is the ultimate goal of rehabilitation for a person with a major lower limb amputation. Due to challenges with prosthetic service delivery in rural settings, many patients with amputations are not benefitting from prosthetic interventions. Inaccessibility to prosthetic services results in worse functional outcomes and quality of life. Learning from the experiences of current prosthetic users in this setting can assist to improve prosthetic service delivery. OBJECTIVES:: To explore the experiences of lower limb prosthetic users and to understand the im

Lower limb amputation in England: prevalence, regional variation and relationship with revascularisation, deprivation and risk factors. A retrospective review of hospital data.

OBJECTIVE: We describe the prevalence of major lower limb amputation across England and its relationship with revascularisation, patient demography and disease risk factors. DESIGN: Retrospective cohort study. SETTING: England 1 April 2003 to 31 March 2009. PARTICIPANTS: Patients aged 50-84 years. MAIN OUTCOME MEASURES: Age standardised prevalence rates were calculated using Hospital Episode Statistics as the numerator with census data as the denominator. The outcome measure 'amputation with revascularisation' was created if an amputation could be linked with a revascularisation. Logistic

Multimodal psychoanalytically informed aid work with children traumatized by the Chechen war.

As demonstrated in three cases, this paper illustrates how psychoanalytically informed multimodal care was an essential element of effective medical treatment of children traumatized by the Chechen war. Multimodal psychoanalytically informed aid work involves holding a variety of psychoanalytic viewpoints, including but not restricted to those represented by the Freudian, Interpersonal, and Relational orientations; its purpose is to allow for greater clarity in conceptualizing the traumatized child's response to war in order to provide the necessary care during the therapeutic process. Among t

Rehabilitation outcome: comparative analysis of different patient types.

Postdischarge progress in 11 types of patients was analyzed with regard to eating, dressing, transfers, bladder management, ambulation, cognition, language comprehension and speech, placement (living arrangement), and mortality rates. Of 273 patients admitted, 263 were available for foillow-up, which disclosed that patients with lower bladder management and cognition scores are more likely to be directed into restrictive discharge placements (p < 0.05), and that CVA patients with right and left cerebral hemisphere involvement, cancer patients, and patients with brain stem infarctions make sign

Acute kidney injury in non-renal medical and surgical admissions in a secondary hospital in Cameroon: recognition and outcomes.

INTRODUCTION: there is a paucity of data on the burden of acute kidney injury (AKI) in non-renal medical and surgical admissions where renal function monitoring is not routinely done. This study evaluated the incidence and outcomes of AKI in non-renal medical and surgical admissions at risk of AKI. METHODS: we prospectively assessed non-renal medical and surgical admissions at the Buea Regional Hospital during a 6-week period for AKI risk factors. Consenting participants with AKI risk factors were then screened for AKI using the modified KDIGO (Kidney Disease Improving Global Outcomes) criter

Structural and molecular characteristics of weight-bearing volar skin can be reconstituted by micro skin tissue column grafting.

For patients with lower limb amputations, prostheses are immensely helpful for mobility and the ability to perform job-related or recreational activities. However, the skin covering the amputation stump is typically transposed from adjacent areas of the leg and lacks the weight-bearing capacity that is only found in the specialized skin covering the palms and soles (a.k.a. volar skin). As a result, the skin tissue in direct contact with the prosthesis frequently breaks down, leading to the development of painful sores and other complications that limit, and often preclude, the use of prosthese

Is lower limb salvage worthwhile after severe open tibial fractures in a developing country? An analysis of surgical outcomes, quality of life and cost implications.

INTRODUCTION: Open tibial fractures are rare and difficult-to-treat injuries because of the involvement of bony, skin and neuromuscular injury along with co-morbidities. Often, during the management of very severe cases these injuries, the question arises, should we amputate or salvage the limb? This question has been explored previously in civilian and military contexts in the US and UK but remains unstudied in the alternative sociocultural and economic context of the developing world. METHODS: We studied 78 adult patients with severe open tibial fracture that presented to our institution, a

Common Synaptic Input to Motor Neurons and Neural Drive to Targeted Reinnervated Muscles.

We compared the behavior of motor neurons innervating their physiological muscle targets with motor neurons from the same spinal segment whose axons were surgically redirected to remnant muscles (targeted muscle reinnervation). The objective was to assess whether motor neurons with nonphysiological innervation receive similar synaptic input and could be voluntary controlled as motor neurons with natural innervation. For this purpose, we acquired high-density EMG signals from the biceps brachii in 5 male transhumeral amputees who underwent targeted reinnervation of this muscle by the ulnar nerv

Heterotopic ossification following traumatic and combat-related amputations. Prevalence, risk factors, and preliminary results of excision.

BACKGROUND: Although infrequently reported in amputees previously, heterotopic ossification has proven to be a common and problematic clinical entity in our recent experience in the treatment of traumatic and combat-related amputations related to Operation Enduring Freedom and Operation Iraqi Freedom. The purpose of the present study was to report the prevalence of and risk factors for heterotopic ossification following trauma-related amputation as well as the preliminary results of operative excision. METHODS: We identified 330 patients with a total of 373 traumatic and combat-related amputa

Does Transhumeral Osseointegration After Amputation Improve Patient-reported Outcomes and Prosthesis Use?

BACKGROUND: Individuals who have undergone transhumeral amputation often have difficulty wearing conventional socket-based prostheses, leading to frequent dissatisfaction and abandonment of the prosthesis. Osseointegration surgery may alleviate or mitigate many of these issues by providing a firm, reliable attachment between the humerus and the prosthesis, potentially leading to better functionality and quality of life; however, it is unclear whether the risks inherent to transdermal bone-anchored implants outweigh the potential functional benefits. QUESTIONS/PURPOSES: (1) Were there improvem

Effects of physical exertion on trans-tibial prosthesis users' ability to accommodate alignment perturbations.

BACKGROUND: It has long been reported that a range of prosthesis alignments is acceptable in trans-tibial prosthetics. This range was shown to be smaller when walking on uneven surfaces. It has also been argued that findings on gait with prostheses that were obtained under laboratory conditions are limited in their applicability to real-life environments. OBJECTIVES: This study investigated the hypothesis that efforts to compensate for suboptimal alignments by active users of trans-tibial prostheses become less effective when levels of physical exertion increase. STUDY DESIGN: A 2 × 2 repeat

Fluid collections in amputations are not indicative or predictive of infection.

BACKGROUND: In the acute postoperative period, fluid collections are common in lower extremity amputations. Whether these fluid collections increase the risk of infection is unknown. QUESTIONS/PURPOSES: The purposes of this study were to determine (1) the percentage of patients who develop postoperative fluid collections in posttraumatic amputations and the natural course of the collection; (2) whether patients who develop these collections are at increased risk for infection; and to ask (3) are there objective clinical or radiologic signs that are associated with likelihood of infection when

Training with virtual visual feedback to alleviate phantom limb pain.

BACKGROUND: Performing phantom movements with visual virtual feedback, or mirror therapy, is a promising treatment avenue to alleviate phantom limb pain. However the effectiveness of this approach appears to vary from one patient to another. OBJECTIVE: To assess the individual response to training with visual virtual feedback and to explore factors influencing the response to that approach. METHODS: Eight male participants with phantom limb pain (PLP) resulting from either a traumatic upper limb amputation or a brachial plexus avulsion participated in this single case multiple baseline study

Prosthetic outcomes after amputation and the impact of mobility level on survival.

OBJECTIVE: Lower extremity amputation continues to be necessary in a significant number of patients with peripheral vascular disease. The 5-year survival following lower limb loss is markedly reduced. Many of these patients are never fitted with a prosthesis, and there is a dearth of knowledge regarding the barriers to prosthetic attainment. The goal of this study was to identify the risk factors for not receiving a prosthesis and the effect of mobility level on survival following major amputation. METHODS: This was a retrospective analysis of all patients that underwent lower extremity amput

Limb Salvage Strategy Amendment for a Better Future in the Era of Bone Cancer Therapy: A Cross-Sectional Study in North India.

BACKGROUND: Bone tumors remain a formidable challenge for orthopedic surgeons. In developing countries, the challenge is exacerbated by limited diagnostic, therapeutic, and management facilities and ignorance. Patients with upper and lower-extremity muscle and skeletal tumors are candidates for amputation or surgical rescue of the limbs. Traditionally, limb rescue surgery by neo-adjuvant chemotherapy is the preferred surgery method for localized carcinoma. Amputations are usually reserved for patients with increased tumor size. The purpose of this study is to investigate health-related quality
