Regulation and adaptation of endocrine axes at high altitude.

As a model of extreme conditions, eight healthy women, part of a 40-member Nepal mountain-climbing expedition, were monitored for dynamic endocrine adaptations. Endocrine measurements were made at frequent intervals over a 6-10-h period at four altitudes: 450 m, 4,800 m (base camp), 6,050 m, and again at 4,800 m (on descent) after an acclimatization (A) period (4,800 mA). Quantified hormones were growth hormone (GH), prolactin (PROL), cortisol (Cort), thyroid-stimulating hormone (TSH), and free thyroxine. These hormones are important to the anabolic/catabolic balance of the body, and are vital

The Effects of Acetazolamide on Exercise Performance at Sea Level and in Hypoxic Environments: A Review.

Lowlanders rapidly ascending to high altitude (>2500 m) often develop acute mountain sickness (AMS). While acclimatization is the most effective method of reducing symptoms of AMS (ie, headache, fatigue, nausea, gastrointestinal distress, etc.), it may take several days to become fully acclimated. Prophylactic use of acetazolamide (AZ), a carbonic anhydrase inhibitor, has become a popular alternative to staged acclimatization because it can be a less time-consuming method of reducing symptoms of AMS. While numerous studies have shown the effectiveness of AZ in mitigating the symptoms of AMS, a

Central Sleep Apnea at High Altitude.

The discovery of central sleep apnea (CSA) at high altitude is usually attributed to Angelo Mosso who published in 1898. It can occur in susceptible individuals at altitude above 2000 m, but at very high altitude, say above 5000 m, it will occur in most subjects. Severity is correlated with ventilatory responsiveness, particularly to hypoxia. Theoretically, it should spontaneously improve with time and acclimatization. Although the time course of resolution is not well described, it appears to persist for more than a month at 5000 m.It occurs due to the interaction of hypocapnia with stages 1

Central and obstructive sleep apnoea during ascent to high altitude.

OBJECTIVE: The aim of the study was to investigate the relationship between central sleep apnoea (CSA) at high altitude and arterial blood gas tensions, and by inference, ventilatory responsiveness. METHODOLOGY: Fourteen normal adult volunteers were studied by polysomnography during sleep, and analysis of awake blood gases during ascent over 12 days from sealevel to 5050 m in the Nepal Himalayas. RESULTS: Thirteen subjects developed CSA. Linear regression analysis showed tight negative correlations between mean CSA index and mean values for sleep SaO2, PaCO2 and PaO2 over the six altitudes (

Reduced hyperpnea-induced bronchospasm following repeated cold air challenge.

This study assessed reduction in expiratory function in 12 asthmatic subjects both after 5 min of cold air provocation (CAP) with dry air conditioned to approximately 0 degrees C and after exercise (to 85% of predicted maximum heart rate) while breathing ambient room air (approximately 21 degrees C and 40% relative humidity). These assessments were done both before and after the following training protocol. Three 5-min periods of isocapnic cold air hyperpnea separated by 5-min rest periods were performed breathing 0 degrees to -10 degrees C air, for 36 sessions over 12 wk. As expected, pretrai

Carotid Body Chemoreceptors: from Mountain to Space.

Human acclimatization to both high-altitude and microgravity is crucial for terrestrial and space exploration. Although these extreme environments elicit several similar physiological responses, including autonomic imbalance, metabolic stress, and reduced exercise capacity, the underlying central mechanisms remain unclear. However, these effects appear to be linked to chemoreflex control. This review summarizes current evidence and proposed potential similarities and bioconvergent mechanisms underlying human responses to both terrestrial and extraterrestrial environments.

Consensus recommendations on training and competing in the heat.

Exercising in the heat induces thermoregulatory and other physiological strain that can lead to impairments in endurance exercise capacity. The purpose of this consensus statement is to provide up-to-date recommendations to optimize performance during sporting activities undertaken in hot ambient conditions. The most important intervention one can adopt to reduce physiological strain and optimize performance is to heat acclimatize. Heat acclimatization should comprise repeated exercise-heat exposures over 1-2 weeks. In addition, athletes should initiate competition and training in a euhydrated

Acute mountain sickness: pathophysiology, prevention, and treatment.

Barometric pressure falls with increasing altitude and consequently there is a reduction in the partial pressure of oxygen resulting in a hypoxic challenge to any individual ascending to altitude. A spectrum of high altitude illnesses can occur when the hypoxic stress outstrips the subject's ability to acclimatize. Acute altitude-related problems consist of the common syndrome of acute mountain sickness, which is relatively benign and usually self-limiting, and the rarer, more serious syndromes of high-altitude cerebral edema and high-altitude pulmonary edema. A common feature of acute altitud

Coping with extreme heat: current exposure and implications for the future.

A preview of how effective behavioral, biological and technological responses might be in the future, when outdoor conditions will be at least 2°C hotter than current levels, is available today from studies of individuals already living in extreme heat. In areas where high temperatures are common-particularly those in the hot and humid tropics-several studies report that indoor temperatures in low-income housing can be significantly hotter than those outdoors. A case study indicates that daily indoor heat indexes in almost all the 123 slum dwellings monitored in Kolkata during the summer were

Periodic breathing, arterial oxyhemoglobin saturation, and heart rate during sleep at high altitude.

The aim of this study was to investigate the effects of acclimatization to high altitude on periodic breathing (PB), arterial oxygen saturation (Sao(2)), and heart rate (HR). Nine male elite climbers, age 24-52 years underwent overnight cardiorespiratory monitoring at sea level and at Everest North Base Camp (5180 m), during the first (BC1) and the tenth (BC2) nights. PB was commonplace in all subjects at high altitude. PB cycle duration increased (p<0.0001) from BC1 (21.7±1.9 s) to BC2 (26.7±2.1 s). Mean Sao(2) from BC1 to BC2, significantly increased during wakefulness (77.4±3.4% vs. 82.5±2.

Altered structure and function of the carotid body at high altitude and associated chemoreflexes.

The ventilatory response to hypoxia is complex. First contact with hypoxia causes an increase in ventilation within seconds that reaches full intensity within minutes because of an increase in carotid sinus nerve (CSN) input to the brain stem. With continued exposure, ventilation increases further over days (ventilatory acclimatization). Initially, it was hypothesized that ventilatory acclimatization arose from a central nervous system (CNS) mechanism. Compensation for alkalosis in the brain and restoration of pH in the vicinity of central chemoreceptors was believed to cause the secondary inc

Transient and Recurrent Vision Loss in a High-Altitude Porter from Pakistan on a Polish Winter Karakoram Expedition.

Visual sensations appear in most migraine auras, but binocular blindness is uncommon. We described a case of multiple transient losses of vision in a man on a winter expedition to K2. His symptoms were later diagnosed as recurrent visual auras without pain. Sojourns at altitude can induce migraine attack; therefore, susceptible individuals should avoid factors that might provoke migraines at high altitude, such as improper acclimatization, dehydration and an inadequate sleep regime.

Renal reactivity: acid-base compensation during incremental ascent to high altitude.

KEY POINTS: Ascent to high altitude imposes an acid-base challenge in which renal compensation is integral for maintaining pH homeostasis, facilitating acclimatization and helping prevent mountain sicknesses. The time-course and extent of plasticity of this important renal response during incremental ascent to altitude is unclear. We created a novel index that accurately quantifies renal acid-base compensation, which may have laboratory, fieldwork and clinical applications. Using this index, we found that renal compensation increased and plateaued after 5 days of incremental altitude exposure,

Alpine travel. Mountain sickness, the unwelcome companion.

A personal observation and two cases from the literature are described to draw attention to the dangers of mountain sickness and to the rapid progression from the milder form, termed acute mountain sickness, to the more serious conditions of high-altitude pulmonary or cerebral oedema and their possible fatal outcome. The pathogenesis and treatment of the disorders are briefly reviewed. It is stressed that there are no substitutes for acclimatization through gradual ascent and that the best treatment is to descend to a lower altitude.

Higher growth temperatures compound heatwave recovery of photosynthesis in different populations of Corymbia calophylla.

Global temperature rise is leading to increased stress and mortality in forests globally and is driving more frequent and intense heatwaves; however, the interaction between growth temperatures and heatwave events on tree populations remains poorly understood. We aimed to determine if gas exchange performance during and after heatwaves was determined by growth temperature, climate origin, and heatwave intensity in Corymbia calophylla saplings from four contrasting temperature-rainfall climate regions. Saplings were grown under warm and cool growth temperatures and exposed to a moderate or extr

In Vitro Propagation of Plants via Organogenesis in Bambusa vulgaris Schrad. ex Wendl Using Temporary Immersion Systems.

The low multiplication and ex vitro survival rates during acclimatization in the culture house limit the in vitro mass propagation of B. vulgaris. Several scientific studies have described the development of different protocols for bamboo; however, not all of them address the effects of these systems on plant morphology, physiology, and biochemistry in vitro. In this chapter, a complete and optimized protocol is described for plants propagated via organogenesis in temporary immersion systems. In addition, the morphophysiological and biochemical characterization of the plants as well as the sur

When Short Stature Is an Asset in Trees.

With their imposing grandeur, the small number of very tall tree species attract a disproportionate amount of scientific study. We right this bias by focusing here on the shorter trees, which often grow in the shade of the giants and many other places besides. That tall trees are so restricted in distribution indicates that there are far more habitats available for small trees. We discuss some leading candidates for the mechanisms that limit maximum plant height in any given habitat, as well as why every habitat has a range of plant sizes. At least two attributes - greater adaptation capacity
