Lung capillary injury and repair in left heart disease: a new target for therapy?

The lungs are the primary organs affected in LHD (left heart disease). Increased left atrial pressure leads to pulmonary alveolar-capillary stress failure, resulting in cycles of alveolar wall injury and repair. The reparative process causes the proliferation of MYFs (myofibroblasts) with fibrosis and extracellular matrix deposition, resulting in thickening of the alveolar wall. Although the resultant reduction in vascular permeability is initially protective against pulmonary oedema, the process becomes maladaptive causing a restrictive lung syndrome with impaired gas exchange. This pathologi

Changes in shape of posterior parts of upper jaws after extraction of teeth and prosthetic treatment.

Changes in the sagittal and transverse profiles of the anterior and posterior parts of the alveolar process after extraction of all teeth and insertion of immediate dentures were studied during a five-year period using casts of 14 patients. The casts were oriented in a uniform way with the help of reference points in the hard palate. The desired profiles could then be reproduced with a special tracing apparatus. Four profiles made during the five-year period were compared for each patient. Changes in shape of the upper jaws in the median sagittal plane agreed well with the results obtained in

Assessment of a Novel Alveolar Ridge Preservation Concept in Dehiscence Sockets: A Pilot Study.

In this prospective pilot study on ridge preservation (RP), a collagen sponge was placed to fill the bottom half of the socket, followed by a sequence of bone graft, collagen membrane, and a sponge placed on top. Twelve patients with 13 hopeless posterior teeth were included. Changes in bone dimension (including variations of horizontal ridge width [HRW] and bone height [BH]) between the time immediately postextraction (T0) and 6 months later (T6M) were evaluated through CBCT. The soft tissue was assessed using a wound healing index (WHI) at 2 weeks (T2W), 2 months (T2M), and 6 months (T6M) po

Three-dimensional bone and soft tissue requirements for optimizing esthetic results in compromised cases with multiple implants.

Achieving an esthetic outcome in tooth replacement and implant treatment requires a proper tooth shape and stable surrounding soft tissue profiles. Bone augmentation is considered vital to support the esthetic soft tissue profile around definitive restorations. To prevent recession of the peri-implant soft tissue in cases with multiple implants, buccal bone augmentation of more than 2 mm from the implant platform is necessary to overcome the normal pattern of bone remodeling. Drawing an imaginary horizontal line spanning the space between the remaining healthy interproximal bone peaks is the m

Anterior alveolar segmental osteodistraction with a bone-borne device: clinical and radiographic evaluation.

PURPOSE: Anterior alveolar osteodistraction is a common method for enlargement of the dentoalveolar process, and bone-borne distraction devices are hypothesized to avoid the risk of dental tipping and periodontal impairment during distraction. The aim of this study was to objectify this thesis and to determine the reliability of bone-borne osteodistraction of the anterior alveolar process. PATIENTS AND METHODS: The study group consisted of 18 consecutive patients who underwent anterior alveolar segmental distraction with a bone-borne distraction device for the treatment of dental crowding or

Alveolar-capillary membrane dysfunction in heart failure: evidence of a pathophysiologic role.

Chronic heart failure (CHF) increases the resistance to gas transfer across the alveolar-capillary interface. Recent reports highlight the pathophysiologic relevance of changes in the lung leading to impaired fluid and gas exchange in the distal airway spaces. Under experimental conditions, an acute pressure or volume overload can injure the alveolar blood-gas barrier. This may disrupt its anatomic configuration, cause the loss of regulation of fluid-flux, and thereby affect alveolar gas conductance properties. These ultrastructural changes have been identified under the term of stress failure

Giant Complex Odontoma Occurring Independently Within the Maxillary Sinus.

Previous reports on complex odontomas in the maxillary sinus describe cases that are associated with the alveolar process and extend into the maxillary sinus. This report presents a rare case of giant complex odontoma in the maxillary sinus, arising entirely within the maxillary sinus and unassociated with the alveolar process. In this case, the lesion was surgically removed through Caldwell-Luc operation. under general anesthesia, and the bony window was closed with titanium mesh.

The remodeling of alveolar bone supporting the mandibular first molar with different levels of periodontal attachment.

The objective of this study was to investigate alveolar bone remodeling of the mandibular first molar with differing levels of periodontal attachment under mastication loading. Three-dimensional finite element models of the mandibular first molar with differing levels of periodontal attachment were established. The stress distributions and bone density changes were analyzed under mastication loading to simulate the remodeling process of mandibular bone based on the theory of strain energy density. The results showed that the alveolar buccal, lingual ridges and root apex areas experienced highe

Position of upper permanent central incisors prior to eruption in unilateral cleft lip and palate.

The position and degree of eruption of permanent central incisors within the premaxilla were measured on x-ray films obtained in 102 patients with unilateral cleft lip and palate and in 52 normal individuals aged 5 years. The patients were subdivided according to sex and to the method of surgical repair (bone grafting or periosteal flap surgery). Individuals with rotated incisors were assessed separately. The results showed that maxillary depth was not significantly reduced prior to palate surgery while the alveolar process was markedly retroclined. An unerupted central upper incisor on the si

[Hyperactivity of the muscles of the lip and chin and indications for surgery to reduce it].

Hyperactivity of the muscles controlling the lower lip and chin (muscles of the chin, buccinator, orbicularis, etc.) can be demonstrated at different levels of the muscular complex and may act as an elastic force against the mandibular alveolar process. Dysfunction in this region may lead to reduction of the alveolar bone quantity, reducing the tooth-bone equilibrium. If this balance is disturbed, periodontal lesions, a lower incisal crowding and a retrognathic mandibular process may result. Surgery to correct overactivity of the chin muscles is described here step by step. Resection of the mu

Quantitative characterizations of the Sharpey's fibers of rat molars.

BACKGROUND AND OBJECTIVE: The Sharpey's fibers of periodontal ligament (PDL) anchor the PDL to alveolar bone and cementum and are essential for the function of PDL. While qualitative analyses of the Sharpey's fibers have been widely explored, a comprehensive quantitative characterization of the Sharpey's fibers is not available. In this work, we selected rat molars as a model and comprehensively characterized the PDL Sharpey's fibers (diameter, density, length, embedding angle, and insertion angle). MATERIALS AND METHODS: A total of 24 rat mandibular molars, eight maxillary first molars, and

Alveolar band in patients with cleft lip and cleft alveolar deformities.

A complete cleft of the lip is often interrupted by a Simonarzt band. Very rarely, a similar fibrous soft tissue band can be seen interrupting the complete cleft of the alveolus. This alveolar band has not been previously described in the literature.
