Extrinsic and Intrinsic Modulators of Anaphylaxis.

The severity of anaphylaxis is determined by many factors. The allergenic source as well as the age of the affected individual and the route of allergen exposure encompass the major contributors of the clinical outcome. Moreover, the severity can be modulated further by intrinsic and extrinsic factors. Among these, the genetic predisposition, certain comorbidities such as uncontrolled asthma, and hormonal fluctuations have been proposed as intrinsic and antihypertensive medications or physical activity as extrinsic factors. Recent advances have highlighted immunologic pathways that may exacerb

Approach to Idiopathic Anaphylaxis in Adolescents.

Anaphylaxis is a potentially-life threatening condition. Adolescents are particularly vulnerable due to increased risk-taking behaviors, poor disease management, and minimized perception of risk. Although most anaphylaxis can be attributed to food, drug, or venom allergy via a detailed history and confirmatory studies, in nearly 1 in 5 cases, the cause may not be obvious. Clinical differentials including rare allergens, cofactors, mast-cell disorders, and mimic disorders can increase the likelihood of discovering of the cause of anaphylaxis.

New arrivals in anaphylaxis to foods.

PURPOSE OF REVIEW: More people are excluding wheat from their diet, or turning to a more sustainable diet in which includes meat substitutes or is mainly or wholly plant-based. This increases the availability of new foods and with it the increasing likelihood of novel allergens. RECENT FINDINGS: There is a growing body of evidence which suggests that allergies to seeds and legumes are increasing potentially due to their use in concentrated form in vegan or health foods. Insects can be a sustainable source of protein, but mealworm could provoke symptoms in individuals sensitized or allergic to

Recent update on the management of anaphylaxis.

Anaphylaxis is a life-threatening systemic allergic reaction presenting various clinical manifestations. Its prevalence has increased in almost all age groups and both sexes. Food, venom, and drugs are major causes in both children and adults; a higher prevalence of food-induced anaphylaxis is noted in children, while a higher prevalence of drug-induced anaphylaxis is noted in adults. The pathogenic mechanism is mediated by immunologic and nonimmunologic mechanisms, where mast cells and basophils are key cells that release mediators. A diagnosis of anaphylaxis is mainly based on clinical sympt

The Role of KIT Mutations in Anaphylaxis.

PURPOSE OF REVIEW: Gain of function KIT mutations are detected in clonal mast cell diseases, namely mastocytosis and monoclonal mast cell activation syndrome. Timely diagnosis and treatment of these disorders are crucial because of their association with severe and life-threatening anaphylaxis. KIT mutations also have implications for targeted therapies of mast cell disorders. This review article strives to serve as an overview of the role of clonal mast cell disorders in anaphylaxis while elucidating current and future therapies. RECENT FINDINGS: Clonal mast cell disease has been increasingl

An unexpected cause of anaphylaxis: potato.

Immediate reactions against contact to raw potato has been reported in adults with generally being in the form of an oral contact dermatitis or contact urticaria, but it may also manifest as rhinitis symptoms, wheezing or even anaphylaxis. Cooked or raw potato allergy has been rarely reported in children as some is being immediate and others being late reactions, and it usually results from ingestion. Herein, we report two cases with a background of allergic diseases developed anaphylaxis one with cooked potato and the other one with raw potato.

Sugammadex hypersensitivity-a case of anaphylaxis.

We describe a case of a patient undergoing open abdominal aneurysm surgery who developed a severe, life-threatening allergic reaction immediately after administration of sugammadex. The manifestation was purely a cardiovascular collapse. The mainstay of treatment was administration of high-dose adrenaline and fluid resuscitation. The diagnosis of anaphylaxis was supported by a positive serum mast cell tryptase (93 µg/l) at one hour post-event. Sugammadex was confirmed as the cause of the anaphylaxis by a positive intradermal allergy test (25 mm diameter response to 1:100 dilution), with a norm

Management of anaphylaxis: a systematic review.

To establish the effectiveness of interventions for the acute and long-term management of anaphylaxis, seven databases were searched for systematic reviews, randomized controlled trials, quasi-randomized controlled trials, controlled clinical trials, controlled before-after studies and interrupted time series and - only in relation to adrenaline - case series investigating the effectiveness of interventions in managing anaphylaxis. Fifty-five studies satisfied the inclusion criteria. We found no robust studies investigating the effectiveness of adrenaline (epinephrine), H1-antihistamines, syst

Glucocorticosteroids for the treatment and prevention of anaphylaxis.

PURPOSE OF REVIEW: To review recent evidence on the effectiveness of glucocorticosteroids in the treatment and prevention of anaphylaxis. RECENT FINDINGS: Glucocorticosteroids are often used in the management of anaphylaxis in an attempt to reduce the severity of the acute reaction and decrease the risk of biphasic/protracted reactions. A recent Cochrane systematic review failed to identify any randomized controlled or quasi-randomized trials investigating the effectiveness of glucocorticosteroids in the emergency management of anaphylaxis. In contrast, randomized controlled trials have been

Anaphylaxis after the injection of buprenorphine.

Cause of death rulings in cases when the concentration of a drug or drugs is higher than observed following therapeutic use are generally straightforward "drug deaths." However, when toxicology testing identifies drug concentrations consistent with therapeutic use or detects no drugs at all, then the cause of death determination is more complicated. Given the rapidity and protean manifestations of anaphylaxis, it should be considered in deaths where no other cause of death is apparent in a suspected drug death. This article reports two cases where an anaphylactic reaction was observed followin

Factors associated with shock in anaphylaxis.

BACKGROUND: The aim of this study was to investigate the clinical characteristics of anaphylactic shock and the factors associated with anaphylactic shock in anaphylaxis. METHODS: Data were retrospectively collected from patients with anaphylaxis for 10 years. Study subjects were searched with broad disease codes including anaphyla-, adverse, angioedema, allergy, insect bite, bee, and hypersensitivity to prevent omission. All the 294 study subjects were divided into shock and nonshock groups. RESULTS: The mean age of the subjects was 43 years old, and males comprised 162 patients (55%). Ther

Causes and risk factors for anaphylaxis.

Anaphylaxis is in most cases an IgE-dependent immunologic reaction. Mast cells are activated and release several mediators. Recent data about possible triggers of anaphylaxis indicate a clear age-dependency. The most frequent triggers of anaphylaxis in children are foods; in adults venom and drugs predominate. In 2006 an anaphylaxis registry was established in German-speaking countries. In the registry the triggers, circumstances, and treatment measures are collected from patients with anaphylaxis. However, the registry cannot supply epidemiological data like prevalence or incidence rates sinc

Role for basophils in systemic anaphylaxis.

For more than 100 years since the discovery of basophils by Paul Ehrlich, the functional significance of this rare leukocyte as compared to mast cells has remained an enigma. Studies on basophils have long been hampered by their rarity (less than 1% of peripheral blood leukocytes) and the lack of useful analytical tools such as model animals deficient only in basophils. Recent studies have now defined previously-unrecognized roles for basophils in both allergic responses and immune regulation, and markedly changed our image of basophils, from a neglected minority to a key player in the immune

[Recognizing anaphylaxis in the emergency department].

Relatively little is known about the characteristics of patients who visit the emergency department for acute allergic reaction. The earliest symptoms are swelling of tissues, sweating, restlessness, itching, nausea, vomiting, diarrhoea, coughing or wheezing. Hypotension, circulatory failure and cardiac arrest may occur suddenly, often associated with bronchial spasm or laryngeal oedema. The objective of this review is to describe the child with severe acute allergic reaction in emergency department.

Anaphylaxis to rabbit: a case report.

BACKGROUND: While rabbits are common as pets, severe allergic reactions to domestic rabbits in homes are unusual. Typically, allergic manifestations are mild to moderate allergic rhinitis, conjunctivitis, pruritus and/or asthma in laboratory animal caretakers with frequent exposure. METHODS: We report an atopic child with a severe allergic reaction following inhalant exposure to a rabbit. We performed percutaneous skin tests and determined serum-specific IgE to commercial preparation of rabbit epithelium and extracts of rabbit fur and serum. RESULTS: Percutaneous skin test was positive to ra

[Intraoperative anaphylaxis to latex in pregnancy].

CASE REPORT: A 31-year-old pregnant woman had to undergo emergency abdominal surgery due to acute intestinal obstruction. The patient's preoperative history demonstrated multiple allergies as well as abdominal trauma several years before. The physical examination--including sonography of the two fetuses--showed no pathological signs. Anaesthesia was induced intravenously with the operating table tilted to the left side, using routine precautions pre-oxygenation, and rapid sequence intubation, and was maintained unproblematically. About 20 min after the onset of surgery, hypotension, tachycardi

Contact urticaria and anaphylaxis to latex.

Contact urticaria and anaphylaxis to latex is reported in two patients. One case was associated with oral and vaginal exposure to a condom and the other with rectal exposure to a latex glove. Both patients had positive reactions to scratch or prick tests to pieces of latex, as well as latex radioallergosorbent test (RAST). The diagnosis of contact urticaria to latex is difficult to make on the basis of history alone.

Anaphylaxis after contact with a jellyfish.

We report a case of an anaphylactic reaction to a jellyfish sting. The episode was manifested by hypotension and bronchospasm. The patient's basophils released histamine in response to nematocyst venom from the Chesapeake Bay sea nettle; this sensitivity could be passively transferred by a heat labile serum factor. This appears to be the first case report of such a reaction.
