We present the successful use of perioperative non-invasive mechanical ventilation in a morbidly obese pregnant woman with bronchial asthma, severe preeclampsia and pulmonary edema undergoing an emergency cesarean delivery with spinal anesthesia. The combination of non-invasive mechanical ventilation with neuraxial anesthesia may be of value in selected parturients with acute or chronic respiratory insufficiency requiring surgery.

Spinal anesthesia is a safe and effective anesthetic technique for cesarean section, considering its simplicity, rapidity, accompanied maternal awareness and distribution of anesthetic agents. The problems of spinal anesthesia, hypotension, postdural puncture headache, failed spinal anesthesia, and its duration, have been investigated. Intravenous fluid therapy may reduce the incidence and severity of the hypotension. Colloid administration is one of the interventions for prevention of hypotension. Low dose phenylephrine is effective without fetal acidosis in healthy mother. The fluid and vaso

A 46-year-old man, with a history of atopic dermatitis and bronchial asthma, underwent surgery for an inguinal hernia. Forty-three minutes subsequent to spinal anesthesia, the patient complained suddenly of dyspnea with wheezing. Blood pressure decreased and skin eruption was observed on his chest. Postoperative laboratory tests revealed high IgE concentration, and a skin test confirmed an allergy to latex. The patient's allergic reaction was easily overlooked because of his history of bronchial asthma and the possibility that the hypotension was caused by the high spinal anesthesia. Latex all

Intracranial subdural hematoma is a serious complication of spinal anesthesia. We report the case of a 31-year-old woman who presented with headache and neurologic localizing signs after spinal anesthesia for cesarean delivery. She was subsequently diagnosed with a right temporoparietal subdural hematoma. The hematoma was evacuated, and she had good recovery.

A male patient developed neurological deficits after an uneventful spinal anesthesia. After 2 months without any improvement an epidural hematoma was presumed. Magnet resonance imaging detected inflammatory tissue and destruction at lumbar levels L2/3. The inflammatory tissue had to be removed via laminectomy. Histology of the excised tissue revealed a plasma cell myeloma that was not diagnosed prior to spinal anesthesia 2 months previously.

BACKGROUND AND OBJECTIVES: Intracranial subdural hematoma is a rare but potentially fatal complication of spinal anesthesia (SA). This case is intended to highlight the importance of careful follow-up of patients with a chronic headache that develops after SA.
CASE REPORT: A 38-year-old woman underwent saphenous vein ligation for varices under SA. On the first postoperative day, she complained of severe postural headache that was controllable with oral analgesics. Two weeks later, bilateral abducens nerve palsy with diplopia developed. Brain magnetic resonance imaging (MRI) showed small bilat

UNLABELLED: It has been postulated that regional anesthesia, when feasible, is the best anesthetic approach in asthmatic patients. However, there are reports of severe bronchospasm during regional anesthesia. In the present study, we developed an experimental model of spinal (subarachnoid) anesthesia in guinea pigs and studied respiratory system responsiveness to aerosolized methacholine. The animals received sodium pentobarbital (50 mg/kg intraperitoneally), a tracheotomy, and mechanical ventilation. Four groups of animals were studied: guinea pigs that received spinal anesthesia with lidocai

BACKGROUND AND OBJECTIVES: Cauda equina syndrome (CES) is a well-known complication of spinal and epidural anesthesia. Previous reports have implicated lidocaine, chloroprocaine, and procaine in its etiology, but not bupivacaine.
METHODS: A 63-year-old man underwent transurethral resection of the prostrate for which he received bupivacaine with glucose intrathecally. Postoperative, he had difficulty in urination and defecation, and magnetic resonance imaging revealed spinal stenosis at the L1-L2 level. The second patient was a 70-year-old woman who underwent hip replacement surgery using a co

BACKGROUND AND OBJECTIVES: A case of delayed respiratory arrest in the combined spinal-epidural anesthesia (CSEA) is described. This event was likely due to morphine injected through the epidural catheter, unintentionally entering into the subarachnoid space through the hole in the dura that was made previously by the spinal needle in the needle-through-needle technique.
METHODS: The CSEA is a popular new regional anesthesia that combines the benefits of the spinal and epidural injections. The needle-through-needle technique, which is only one of the various techniques to accomplish CSEA, may

A case is reported of spinal subarachnoid haematoma occurring after spinal anaesthesia. The patient was given prophylactic pre and postoperative low molecular weight heparin. On the second postoperative day, he complained of backache and got a partial cauda equina syndrome. Magnetic resonance imaging carried out on the 6th day confirmed the presence of a spinal haematoma, which was removed surgically. One year later, the bilateral motor palsy which had involved several roots, had completely regressed. It remained a severe sensory loss associated with a loss of sphincter control. This case high

BACKGROUND AND OBJECTIVE: Although continuous spinal anesthesia with microcatheters has a number of advantages, there are also some drawbacks: technical problems in advancing the catheter, the possibility of traumatizing neural structures, the development of cauda equina syndrome, and maldistribution of the local anesthetic.
METHODS: Spinaloscopy was performed with a 2-mm-diameter endoscope in fresh cadavers to visualize the fate of the catheters, as well as the distribution of the local anesthetic administered through these fine-bore catheters. Midline and paramedian approach achieved an eas

BACKGROUND AND OBJECTIVES: The use of sedatives during regional anesthesia can lead to life-threatening hypoxemia. Older patients particularly are prone to enhanced effects of these drugs. We studies whether oral premedication with benzodiazepines produced hypoxemia during spinal anesthesia in elderly patients.
METHODS: In a prospective, double-blind, and randomized study, we evaluated the effect of oral benzodiazepine premedication on the incidence of hypoxemia measured by pulse oximetry (arterial oxygen saturation less than 90% for 30 seconds or longer) during surgery under spinal anesthesi

There is a tendency to perform increasingly severe operations on patients of advanced age. Because geriatric patients are of greater risk when under anesthesia during operation, it is necessary to be aware of the possible anesthesia-related and operation-related complications. In a retrospective study the complication rate with regard to patients (age, multimorbidity), operation (e.g. number and duration of procedures) and anesthesia was analyzed. Of particular interest were the kind and degree of certain complications at a particular time. Directly following the induction of anesthesia and th

To test the effectiveness of adrenergic agonists in correcting the vascular sequelae of spinal anesthesia, we used venous reservoir volume (RV) and mean arterial pressure (MAP) as indices of the changes in venous capacitance and arterial resistance produced by adrenergic agonists in dogs anesthetized with pentobarbital and undergoing cardiopulmonary bypass (CPB). A CPB-based technique was chosen both to prevent drug and reflex effects on the heart from influencing the results and to provide a convenient means by which to monitor venous capacitance. Total spinal anesthesia significantly decreas

BACKGROUND: This study aimed to compare the personality belief scores of patients who chose either general or spinal anesthesia during cesarean delivery (CD) and explore the relationship between personality beliefs, and anxiety levels.
METHODS: This prospective study included expecting mothers, aged 18-45 years, who were classified as ASA II-III, scheduled for elective CD. Anesthesia (general or spinal) was selected based on the patient's preference. Patients with emergency CD indications, contraindications to either anesthesia type, or inability to complete the evaluation scales were exclude

BACKGROUND: Inguinal hernia repair is one of the most commonly performed surgical procedures. To date, there is no consensus on which anesthesia should be used. The objective of this meta-analysis was to assess the efficacy of spinal anesthesia (SA) vs. general anesthesia (GA) in inguinal hernia repair in adults.
METHODS: Eligible studies were identified before January 2020 from PubMed, Embase, ScienceDirect, Cochrane Library, Scopus database as well as reference lists. Outcomes included surgery time, the time in the operation room, the length of hospital stay, pain scores, patient satisfacti

Cauda equina syndrome is a well-known but rare complication of spinal anesthesia. An 80-year-old man was scheduled for both herniorrhaphy. Spinal anesthesia was performed at the L3-4 interspinous space with 0.5% hyperbaric bupivacaine 12 mg. Eight hours after anesthesia, the patient complained bilateral sensorimotor deficits of the lower extremities and peroneal region. Urinary and fecal incontinence were also observed. MRI and myelography showed severe central spinal stenosis at L3-4 and L4-5. EMG showed cauda equina syndrome. Seven weeks after the procedure, left decompressive subtotal lamin

The effect of epidural morphine anesthesia on NO-ergic mechanisms was studied during primary and secondary sensitization. Nitro-oxide synthase (NOS) activity was evaluated by the method of Home and Vincent (1989) (identification of NADPH diaphorase). NOS and NADPH diaphorase are two double enzymes in cells with synchronously changing activity. NOS activity corresponds to the level of NO secretion. The enzyme was measured on a microdensitometer. The findings evidence active involvement of NO-ergic mechanisms in the formation of primary and secondary hyperalgesia, and injection of morphine cause
