Bilateral primary spontaneous pneumothoraces postcaesarean section--another reason to avoid general anaesthesia in pregnancy.

A 36-year-old, healthy, primiparous female underwent a caesarean section under general anaesthetic. She had previously had a severe reaction to dye during a myelogram and therefore, had declined epidural analgesia or regional anaesthesia. Induction and maintenance of anaesthesia was uneventful, but on emergence, and before tracheal extubation, the patient coughed on the endotracheal tube and almost immediately developed right-sided subcutaneous emphysema of the face and neck. At this point her oxygen saturation began to fall and she was noted to be difficult to ventilate. Clinically and radiol

Etomidate vs. propofol to carry out suspension laryngoscopies.

Patients about to undergo a suspension laryngoscopy were randomly assigned to one of two groups (n = 10). They were given either etomidate 1 mg kg-1 followed by 1 mg kg-1 h-1, or propofol 2.5 mg kg-1 followed by 9 mg kg-1 h-1. Alfentanil 10 micrograms kg-1 was given to provide analgesia with a further half dose if necessary. Duration of apnoea, quality of anaesthesia, times between stopping hypnotic administration and the moment when the patients opened their eyes, gave their name, date of birth and the day's date were noted. Complications in Equine Anesthesia.

General anesthesia in equine patients is associated with a higher risk of mortality despite improved monitoring equipments and anesthetic techniques most likely due to their large body size, unique physiology and anatomy, behavioral tendencies, and therefore the challenging recovery. Larger body mass, increased age, higher American Society of Anesthesiologists (ASA) status, out-of-hours surgery, and longer duration of anesthesia have been identified as contributing factors increasing the risk of perioperative mortality in equine patients. Especially the recovery period remains a critical time

Association of amisulpride and recovery room length of stay among patients with postoperative nausea and vomiting following outpatient surgery.

STUDY OBJECTIVE: Postoperative nausea and vomiting (PONV) is a common sequela of surgery in patients undergoing general anesthesia. Amisulpride has shown promise in its ability to treat PONV. The objective of this study was to determine if amisulpride is associated with significant changes in PACU efficiency within a fast-paced ambulatory surgery center. METHODS: This was a retrospective cohort study of 816 patients at a single ambulatory surgery center who experienced PONV between 2018 and 2023. The two cohorts analyzed were patients who did or did not have amisulpride among their anti-emeti

Ketamine and military anaesthesia. The effect of heavy papaveretum premedication and Althesin induction on the incidence of emergence phenomena.

An anaesthetic technique suitable for military field anaesthesia was sought which would attenuate the psychic side effects of ketamine. Heavy premedication with 30 mg of papaveretum and induction of anaesthesia with Althesin were evaluated. A combination of the two lowered the incidence of side effects, but they still occurred frequently enough to pose problems. Ketamine sequelae are considered incompatible with the battlefield environment.

Remimazolam in neuroanesthesia.

PURPOSE OF REVIEW: Remimazolam is a novel ultrashort-acting benzodiazepine that has recently gained attention for its potential benefits in neuroanesthesia. This review aims to critically assess the latest evidence regarding its hemodynamic properties, neurophysiological monitoring compatibility, and recovery profile while highlighting gaps in research and clinical uncertainties. RECENT FINDINGS: Several randomized controlled trials and case reports suggest that remimazolam may reduce episodes of hypotension, minimize vasopressor use, and facilitate quicker emergence, especially with flumazen

Spontaneous recovery from neuromuscular block after a single dose of a muscle relaxant in pediatric patients: A systematic review using a network meta-analytic and meta-regression approach.

BACKGROUND: Age-related differences in the pharmacokinetics and pharmacodynamics of neuromuscular blocking agents (NMBAs) and the short duration of many surgical procedures put pediatric patients at risk of postoperative residual curarization (PORC). To date, the duration of neuromuscular blocking agent effect in children has not been analyzed in a quantitative review. The current meta-analysis aimed to compare spontaneous recovery following administration of various types and doses of neuromuscular blocking agents and to quantify the effect of prognostic variables associated with the recovery

Functional near-infrared spectroscopy guided mapping of frontal cortex, a novel modality for assessing emergence delirium in children: A prospective observational study.

INTRODUCTION: Despite an 18%-30% prevalence, there is no consensus regarding pathogenesis of emergence delirium after anesthesia in children. Functional near-infrared spectroscopy (fNIRS) is an optical neuroimaging modality that relies on blood oxygen level-dependent response, translating to a mean increase in oxyhemoglobin and a decrease in deoxyhemoglobin. We aimed to correlate the emergence delirium in the postoperative period with the changes in the frontal cortex utilizing fNIRS reading primarily and also with blood glucose, serum electrolytes, and preoperative anxiety scores. METHODS: A

The impact of inhalation anesthetics on early postoperative cognitive function and recovery characteristics in Down syndrome patients: a randomized, double - blind study.

BACKGROUND: Down syndrome (DS) is associated with intellectual disability. DS patients may be unable to cooperate and often require general anesthesia even for minor surgeries. Rapid recovery significantly contributes to fast-tracking. This prospective randomized, double - blind study investigates the impact of desflurane and sevoflurane on recovery and early postoperative cognitive function of these patients. METHODS: Forty-four patients undergoing dental surgery, were randomized to receive desflurane (DES-group) or sevoflurane (SEVO-group) for anesthesia maintenance. The primary outcome was

[Problem of neuromuscular block reversion: neostigmine and sugammadex].

The review covers the history of neuromuscular block reversal. It states that indications for decurarization have been changed due to the extremely high rate of residual curarization even after the use of average-acting myorelaxants. The paper provides the pharmacological characteristics of sugammadex, a new selective relaxant-binding agent for aminosteroids (rocuronium and vecuronium) with the unique mechanism of action. It also gives data on the efficacy and safety of sugammadex versus anticholinesterase agents and estimates the prospects for its clinical application.

[The use of cardiac troponins (T or I) measurement in cardiology and various clinical settings].

Measurement of cardiac troponin I or T in serum (highly specific for the myocardium) have replaced classical markers, such as creatine kinase MB. Cardiac troponins are preferred markers because of their high specificity and sensitivity. This had led to modifications of the original World Health Organization criteria for acute myocardial infarction. Furthermore, the place of the troponins as superior markers of subsequent cardiac risk in acute coronary syndrome has now become firmly established, for both diagnostic and risk stratification purposes. The use of C-reactive protein and/or other inf

The Lambert-Eaton myasthenic syndrome: a cause of delayed recovery from general anesthesia.

A 70-year-old man required prolonged ventilation after surgery to remove a rectal neoplasm. The cause of the slow recovery from the effects of neuromuscular blocking agents used during his anesthetic was the Lambert-Eaton myasthenic syndrome (LEMS). Before surgery, he had no neuromuscular symptoms, even in retrospect. LEMS should be considered in the diagnosis of prolonged recovery from neuromuscular blockade, even in previously asymptomatic patients.
