Local Anesthetics in Diabetic Retinopathy Procedures: A Comprehensive Review With a Focus on Lidocaine-Based Pain Control.

This review investigates commonly used local anesthetic agents and administration methods in diabetic retinopathy (DR) procedures, such as pan-retinal photocoagulation, intravitreal injections, and vitrectomy, focusing on pain control and procedural outcomes. A systematic review of PubMed was conducted to identify studies examining local anesthetic use in DR procedures. Studies were screened for relevance, full-text availability, and methodological rigor. Fourteen studies met the inclusion criteria and were evaluated for anesthetic type, dosage, application route, co-administration, and patien

Preventive but Not Therapeutic Topical Application of Local Anesthetics Can Inhibit Experimental Epidermolysis Bullosa Acquisita in Mice.

Epidermolysis bullosa acquisita (EBA) is an autoimmune blistering disorder characterized and caused by autoantibodies against type VII collagen (COL7). Although it has been noticed that EBA in both patients and mice is associated with an increased scratching, it is not clear whether and how the scratching contributes to disease manifestation. Hence, we here aimed to validate this clinical observation and also to investigate the potential contribution of increased scratching in EBA pathogenesis in mice. Longitudinal assessment of scratching behavior revealed an increased frequency of scratching

[Interactions between opioids and local anesthetics at the level of the dorsal horn of the spinal cord].

For many years, local anaesthetic drugs, given spinally, have been used to control pain after surgery or during parturition. For nearly 20 years, we have known that opioids given by the same way have an analgesic effect. However the later group do not provide satisfactory analgesia in several pain contexts but also give troublesome side effects; local anaesthetics are associated also with side effects. Experimental and clinical studies have shown that the combination of local anaesthetics with opioids provides some degree of synergism, associated with a reduction of side effects. Further studi

[Local anesthetics. VXII. Preparation and effectiveness of dibasic alkylesters of 2-, and 3-alkoxy-substituted phenylcarbamic acids].

In a series of 24 compounds of the type of dibasic alkylesters of 2-, and 3-alkoxy substituted phenylcarbamic acids, esterified alcohols 1-dipropylamino-3-pyrrolidino (piperidino or perhydroazepino)-2-propanol were used. It follows from the results of pharmacological evaluation that these compounds possess a high index of effectiveness in surface and infiltration anaesthesia compared to the standards of cocaine and procaine. Maximum effectiveness in surface anaesthesia was shown by [1-dipropylaminomethyl)-2- (piperidino)ethyl]-ester of 3-butoxyphenylcarbamic acids whose activity is as much as

[Mechanism of inhibition by local anesthetics of electron transport at the donor site of the photosystem II].

The mechanism of inhibition by local anaesthetics of the procaine group of electron transport at the donor site of photosystem II (PS II) from pea chloroplasts was investigated. It was found that besides the inactivation of the O2 release system the anaesthetics used at one order of magnitude lesser concentration exert an uncoupling effect. With a rise in pH the inhibiting activity increases; however, this process is not coupled with the protonophore effect but is due to the generation of a neutral form of the amine. The increment of the inhibiting activity of the anaesthetics in the course of

[Micelle formation as the origin of the cut-off phenomenon in a homologous series of local anesthetics].

On the example of the surface anaesthetic homologous series of 4-alkoxybenzoic-diethylaminoethylester-HCl (parethoxycaine) the so-called "cut off"-effect was studied. The "cut off"-effect is the phenomenon, that a weaker efficiency or an inefficiency is observed on the higher homologues of therapeutic substances. It is demonstrated that the "cut off"-effect is referred to the micellisation and aggregation of the higher homologues of the local anaesthetics in aqueous solution. This has the consequence, that the higher homologues cannot diffuse on the pain receptors and so the efficiency is redu

[The effect of local anesthetics and injuries on the angiographic picture of the cat mandibular blood vessels].

The authors examined the effects of local anesthetics and traumata (traumatic dental extractions) on the angiographic picture of the mandibular vessels of the cat in the 10th, 70th and 120th minutes. The application of local anaesthetics results in a reduced filling of the inferior alveolar artery in the 10th and 70th minutes. The picture returns to normal in the 120th minute. The traumatic extraction produces more marked changes up to complete spasm of the vascular system of the common carotid artery. In the 120 th minute, the blood supply is restored; and the normal number of vessels appears

Local Anesthetic Resistance: Pathophysiology, Clinical Recognition, and Management Strategies.

PURPOSE OF REVIEW: Local anesthetics (LAs) are a foundational component of surgical pain management and play a critical role in multimodal analgesia, particularly in opioid-sparing perioperative strategies. While generally effective, rare instances of LA resistance have been reported, in which standard doses fail to provide adequate analgesia. This review aims to consolidate current evidence on local anesthetic resistance, including its proposed pathophysiology, diagnostic considerations, and management strategies. RECENT FINDINGS: Most cases of apparent LA failure are attributable to technica

Evaluating the efficacy of local anesthetic techniques during scalp microneedling.

INTRODUCTION: An increasing interest in minimally invasive procedures hassled to a demand for an effective local anesthetic algorithm. The scalp presents a challenge in achieving effective anesthesia due to the presence of hair shafts. This study aims to evaluate the efficacy of different methods during a microneedling procedure, including 25 mg lidocaine and 25 mg prilocaine cream, skin spray with 10% lidocaine, and cold gel compresses. MATERIALS AND METHODS: Sixty-two men aged between 20 and 50 years underwent three microneedling treatments, each using a different method of anesthesia. The

The Role of Multilevel Intercostal Nerve Block in Local Anesthetic Thoracoscopy.

BACKGROUND: Intercostal nerve block (ICNB) has long been used in thoracic surgery. Local anesthetic thoracoscopy (LAT) is performed under conscious sedation with local anesthesia at the port insertion site. This alone, however, does not anesthetize the parietal pleura from where biopsies are taken and patients can experience pain. OBJECTIVES: To compare LAT with multilevel ICNB versus standard care to determine whether it reduces pain during and post-LAT, its effect on analgesia use, the hospital length of stay (LOS), and related complications. METHODS: Prospective analysis of patients under

Local Anesthetic Cardiac Toxicity Is Mediated by Cardiomyocyte Calcium Dynamics.

BACKGROUND: Long-lasting local anesthetic use for perioperative pain control is limited by possible cardiotoxicity (e.g., arrhythmias and contractile depression), potentially leading to cardiac arrest. Off-target cardiac sodium channel blockade is considered the canonical mechanism behind cardiotoxicity; however, it does not fully explain the observed toxicity variability between anesthetics. The authors hypothesize that more cardiotoxic anesthetics (e.g., bupivacaine) differentially perturb other important cardiomyocyte functions (e.g., calcium dynamics), which may be exploited to mitigate dr

Dental Negligence: Accidental Administration of Formalin Instead of Local Anesthetic.

A solution like formalin used as a fixative has to be handled with lot of care and needs to be stored with utmost care. Negligence on part of the dental professional by accidentally injecting formalin instead of the local anesthetic can lead to permanent tissue damage and also may induce a life threatening situation. The operating dentist injected formalin accidentally instead of the local anesthetic and the patient experienced severe pain and burning sensation. The general practice of formalin being stored in the local anesthetic bottles and being assisted by untrained assistant is the most c

Incidence of the "Adrenaline Rush" and Vasovagal Response with Local Anesthetic Injection.

BACKGROUND: Many patients feel an "adrenaline rush" or a vasovagal reaction when injected with lidocaine and epinephrine during wide awake surgery. The incidence of these reactions is not well documented in the literature. METHODS: In total, 387 patients were prospectively injected with lidocaine and epinephrine for minor procedures without sedation between July 1, 2019 and November 1, 2020. A concentration of epinephrine with 1:100,000 in 2% lidocaine was injected, with most patients getting less than 20 mL of volume. RESULTS: Eight (2.2%) of the patients had adrenaline rush symptoms, which

Mannitol Enhances the Antinociceptive Effects of Diphenhydramine as an Alternative Local Anesthetic.

Mannitol has recently been reported to be effective in enhancing the antinociceptive efficacy of lidocaine. No single study to date, however, has compared diphenhydramine with and without mannitol for nociceptive processing as an alternative local anesthetic. In this study, we examined the antinociceptive efficacy enhancements of diphenhydramine when combined with mannitol. Male Sprague-Dawley rats weighing 230-260 g were used in a hot plate test to evaluate the antinociceptive effects of diphenhydramine. All chemicals were dissolved in isotonic normal saline and administered subcutaneously in
