[Right hemiasomatognosia and sensation of amputation caused by left subcortical lesion. Role of callosal disconnection].

A 72-year-old right-handed hypertensive man presented with a right brachial monoplegia, and hypesthesia of the right half of the body to touch and pricking, sparing the face. A CT scan 2 weeks later showed a spontaneous hyperdense area corresponding to a left subcortical parietal hematoma. The patient used his spontaneous language to express body image disturbances: intense prolonged feeling of amputation related to the upper limb and foot on the right side, hemiasomatognosia without anosognosia, autotopagnosia. Also associated were a right-sided visual negligence and a more general inability

Interrupted Time Series Analysis of the Efficacy of Bilingual Abstract Semantic Associative Network Training.

PURPOSE: In response to the need for more appropriate anomia treatment materials for bilingual persons with aphasia (BPWAs), we have created linguistically and culturally appropriate versions of AbSANT (Abstract Semantic Associative Network Training) for several languages that we call BAbSANT (Bilingual Abstract Semantic Associative Network Training). BAbSANT has now been conducted with 15 BPWAs in six different language combinations. The purposes of this study were to determine whether BAbSANT produces (a) similar within-language treatment effects as AbSANT and (b) cross-language treatment ef

Protocol for Cerebellar Stimulation for Aphasia Rehabilitation (CeSAR): A randomized, double-blind, sham-controlled trial.

In this randomized, double-blind, sham-controlled trial of Cerebellar Stimulation for Aphasia Rehabilitation (CeSAR), we will determine the effectiveness of cathodal tDCS (transcranial direct current stimulation) to the right cerebellum for the treatment of chronic aphasia (>6 months post stroke). We will test the hypothesis that cerebellar tDCS in combination with an evidenced-based anomia treatment (semantic feature analysis, SFA) will be associated with greater improvement in naming untrained pictures (as measured by the change in Philadelphia Picture Naming Test), 1-week post treatment, co

Treating lexical retrieval using letter fluency and tDCS in primary progressive aphasia: a single-case study.

BACKGROUND: In early stages, individuals with Primary Progressive Aphasia (PPA) report language symptoms while scoring within norm in formal language tests. Early intervention is important due to the progressive nature of the disease. METHOD: We report a single case study of an individual with logopenic variant PPA (lvPPA). We tested whether letter fluency, used as a therapy task, can improve lexical retrieval when combined with tDCS to either the left inferior-frontal gyrus (IFG) or the left inferior parietal lobe (IPL), administered in two separate therapy phases separated by a wash-out per

A machine-learned analysis suggests non-redundant diagnostic information in olfactory subtests.

BACKGROUND: The functional performance of the human sense of smell can be approached via assessment of the olfactory threshold, the ability to discriminate odors or the ability to identify odors. Contemporary clinical test batteries include all or a selection of these components, with some dissent about the required number and choice. METHODS: Olfactory thresholds, odor discrimination and odor identification scores were available from 10,714 subjects (3662 with anomia, 4299 with hyposmia, and 2752 with normal olfactory function). To assess, whether the olfactory subtests confer the same infor

The error of Broca: From the traditional localizationist concept to a connectomal anatomy of human brain.

Neurology, especially aphasiology, was mainly built on behavioral-structural correlations ("lesion method"). In this setting, Broca's area has been considered as the "speech area"; moreover, this observation led to localizationism. However, advances in brain mapping techniques, as functional neuroimaging and direct electrical stimulation in patients undergoing awake surgery for gliomas, has resulted in a paradigmatic shift regarding models of neural architecture. In fact, the brain is organized in distributed complex networks underpinning sensorimotor, visuospatial, language, cognitive and emo

Between thought and expression, a magnetoencephalography study of the "tip-of-the-tongue" phenomenon.

"Tip-of-the-tongue" (TOT) is the phenomenon associated with the inaccessibility of a known word from memory. It is universally experienced, increases in frequency with age, and is most common for proper nouns. It is a good model for the symptom of anomia experienced much more frequently by some aphasic patients following brain injury. Here, we induced the TOT state in older participants while they underwent brain scanning with magnetoencephalography to investigate the changes in oscillatory brain activity associated with failed retrieval of known words. Using confrontation naming of pictures o

Right unilateral jargonagraphia as a symptom of callosal disconnection.

We report the case of a right-handed patient who exhibited right unilateral jargonagraphia after a traumatic callosal hemorrhage. The lesions involved the entire corpus callosum, except for the lower part of the genu and the splenium. The patient's right unilateral jargonagraphia was characterized by neologisms and perseveration in kanji and kana, and was more prominent in kana than kanji. The jargonagraphia was similar to that observed in crossed aphasia, except that agraphia occurred only with the right hand. The patient also showed right unilateral tactile anomia and right tactile alexia, a

The acoustic cortex in frontal dementia.

Frontal dementia is a clinical entity of cognitive impairment, characterized mostly by progressive loss of fluency in speech, eventually resulting in aphasia or anomia, associated frequently with early loss of insight and many forms of inappropriate behavior. Hyperphosphorylation of the isoforms of tau protein, a microtubule-associated protein, which plays an important role in the pathogenetic mechanisms of Alzheimer's disease, is mainly involved in the pathogenesis of frontal dementia. In the present study, the morphological alterations of the acoustic cortex are described in three cases of d

Callosal syndrome: implications for understanding the neuropsychology of stroke.

A case is presented of callosal syndrome following cerebral hemorrhage and amphetamine abuse in a 26-year-old right-handed man. There were few hemispheric findings but a full callosal syndrome including left apraxia to verbal commands, left tactile anomia, left agraphia, right constructional apraxia, failure of blindfolded side-to-side hand replication and form-board testing showing loss of interhemispheric transmission including intermanual interference. The "draw-a-clock" test showed left inattention when drawn with the right hand but not the left, and perseveration was noted for spatial tas

Severe anterograde amnesia with onset in childhood as a result of anoxic encephalopathy.

Our patient (M.S.) had an abrupt onset of amnesia due to a respiratory arrest at the age of 8 years and has been followed by one of us (A.L.R.) for 19 years. A specially designed MRI study indicated that the neuroanatomical localization of his lesion is restricted to the hippocampal formation bilaterally. Comparison of M.S.'s present IQ and academic scores with earlier scores revealed that his literacy skills, certain basic language functions and vocabulary development were arrested by his memory disorder. In contrast, development of mathematical skill was less curtailed, and verbal and nonver

Alternating antagonism between two languages with successive recovery of a third in a trilingual aphasic patient.

The recovery of the languages of an aphasic patient premorbidly almost equally proficient in Farsi and German, with a good knowledge of English, is described. For the first week post-trauma, the patient was in no condition to speak. He then spoke only a little Farsi for 5 days, after which he spoke only German for the next 3 weeks, even when addressed in Farsi by his unilingual relatives. The patient then switched back to using only Farsi for 4 days, after which he regained voluntary control over each of his languages, including English. The patient was repeatedly assessed with equivalent vers

Axial movements in ideomotor apraxia.

Non-symbolic axial movements were examined and compared to oral and limb movements in a group of 60 aphasic patients (15 of each major subgroup) with exclusively left-sided brain damage. The contention in the literature that axial movements are preserved in patients with ideomotor limb apraxia was not confirmed.

Script training treatment for adults with apraxia of speech.

PURPOSE: Outcomes of script training for individuals with apraxia of speech (AOS) and mild anomic aphasia were investigated. Script training is a functional treatment that has been successful for individuals with aphasia but has not been applied to individuals with AOS. Principles of motor learning were incorporated into training to promote long-term retention of scripts. METHOD: Three individuals with AOS completed script training. A multiple-baseline, across-behaviors design examined acquisition of client-selected scripts. Errors and speaking rates were also analyzed. Random practice and de

'I know your name but not your face': explaining modality-based differences in access to biographical knowledge in a patient with retrograde amnesia.

In current functional models of person recognition it is proposed that there exists a single common body of semantic information concerning people accessible from all modalities (e.g. names, faces). In this paper, we present evidence that challenges this view. Two experiments are discussed investigating residual autobiographical and public knowledge in a patient suffering from retrograde amnesia. Knowledge about people was investigated in each case by asking a series of questions, ranging from the very general to the very specific. Experiment 1 examined knowledge about famous people. The resul

Activation of the phonological lexicon for reading and object naming in deep dyslexia.

Poor oral reading in some cases of deep dyslexia could be due to difficulty in inhibiting the phonological lexical entries of words semantically related to the correct reading responses. If this is the case, then additional activation of the correct phonological entries should improve reading performance, whereas additional activation of competing entries should lead to errors. This should hold true for object naming as well as for reading, since both depend on a semantically mediated lexical route. These predictions were borne out with an "output" deep dyslexic patient, who made many semantic

Errors resembling semantic paralexias in Spanish-speaking aphasics.

Forty-one Spanish-speaking left-hemisphere-damaged patients were selected and divided into seven groups (transcortical, Broca's aphasia, conduction aphasia, Wernicke's aphasia, anomic aphasia, alexia without agraphia, and global aphasia). A reading battery composed of eight different subtests was given to each patient (reading of letters, reading of syllables, reading of pseudowords, reading of words, reading of sentences, understanding commands, reading and comprehension of texts, and logographic reading). Different types of reading errors were analyzed. Only in the logographic reading subtes

Neural correlates of recognition and naming of musical instruments.

OBJECTIVE: Retrieval of lexical (names) and conceptual (semantic) information is frequently impaired in individuals with neurological damage. One category of items that is often affected is musical instruments. However, distinct neuroanatomical correlates underlying lexical and conceptual knowledge for musical instruments have not been identified. METHOD: We used a neuropsychological approach to explore the neural correlates of knowledge retrieval for musical instruments. A large sample of individuals with focal brain damage (N = 298), viewed pictures of 16 musical instruments and were asked

Semantic Feature Analysis: Further Examination of Outcomes.

PURPOSE: The purpose of this investigation was to systematically examine outcomes associated with Semantic feature analysis, which is an established treatment for word-retrieval deficits in aphasia. Attributes of the experimental design and stimuli were manipulated to evaluate generalized naming of semantically related and unrelated items. In addition, the study was designed to examine changes in production of semantic information. METHOD: Semantic feature analysis was applied in the context of multiple-baseline designs with 5 persons with chronic aphasia. Experimental items were controlled f
