Percutaneous transcatheter closure of a descending aorta to vertebral venous plexus fistula using an Amplatzer Vascular Plug 2: a case report.

A descending aorta to vertebral venous plexus fistula is an extremely rare form of arteriovenous fistula. A 10-month-old infant was referred to the hospital for evaluation of a murmur. On examination, a continuous murmur was heard in the entire back. Transthoracic echocardiography revealed left heart chamber dilatation in the presence of preserved left ventricle systolic function. Computerized tomography angiography with 3-dimensional reconstruction, and lateral projection aortography revealed a descending aorta-vertebral venous plexus fistula measuring 4.8 mm in the aortic orifice. The fistul

Endovascular repair of an aortic arch pseudoaneurysm by an atrial septal defect occluder combined with a chimney stent.

An aortic arch pseudoaneurysm distal to the left common carotid artery (LCCA) was repaired endovascularly with an atrial septal defect occluder. Aortography showed complete closure of the pseudoaneurysm but delayed blood flow in the LCCA. A chimney stent was then placed into the LCCA to salvage it. Completion angiography suggested free flow in the LCCA and a minor perioccluder leak, which disappeared at 1-month follow-up. The patient was free from any symptoms related to the pseudoaneurysm at 1-year follow-up, with the atrial septal defect occluder in a satisfactory position, absence of leakin

Remnant of a non-patent ductus arteriosus mimicking traumatic thoracic aorta transection: a case report.

We present an interesting case of a 53-year-old man with a non-patent ductus arteriosus erroneously diagnosed as acute thoracic aorta transection after a car accident. The aortography revealed a "rupture" of the linear inner curve of the aorta in the isthmus area, as well as a protrusion of the aortic lumen in the corresponding area. During the followed thoracotomy an intact thoracic aorta and the remnant of a non-patent ligamentum arteriosum were found. It is the first reported case and we review all the possible entities which may give a false-positive image of traumatic aortic transection.

[Coarctation of the aorta presenting as a pulsatile occipital mass].

A healthy 18 months old boy, is referred to our hospital for a thrilling pulsatile mass behind the left ear. Aortography in the ascending aorta shows a severe coarctation with almost interruption of the aorta between the left carotid and left subclavian artery. Both external carotid arteries provide an important collateral pathway through occipital arteries to two dilated vertebral arteries. Descending aorta is feeding by a reverse blood flow into vertebral and subclavian arteries. The child has been operated. This is an original presentation of severe coarctation of the aorta with development

Huge aneurysm of the sinus of Valsalva following infective endocarditis in Behçet's disease.

A huge aneurysm of the sinus of Valsalva with conduction disturbance as a consequence of infective endocarditis in Behçet's disease is reported. The aneurysm extended not only into the ventricular septum but also to the right atrium and ventricle, with a complicated cavity formation. We speculate that complete atrioventricular block occurred due to an enlargement of the aneurysm into the ventricular septum, leading to a direct conduction system injury. Preoperative echocardiography and aortography were insufficient to recognize the extent of the lesion; subsequent operative examination reveale

Newer diagnostic measures and emergency management.

Evolving technology provides new diagnostic and emergency management options for the surgeon caring for patients with thoracic trauma. Abdominal ultrasound performed by surgeons in the emergency department makes pericardiocentesis and subxyphoid pericardiotomy obsolete. Transesophageal echocardiography supplements aortography in the diagnosis of thoracic aortic injury. New terminologies replace the overused term myocardial contusion. Pulmonary tractotomy is an innovative approach to through-and-through pulmonary penetrating injuries. Centrifugal pumps add new options to thoracic great-vessel i

[Quadricuspid aortic valve: a case report and the review of the literature].

We report a case of rare congenital anomaly of quadricuspid aortic valve and coexisting right coronary displacement. A 70-year-old man was admitted to our hospital for heart murmur and ECG abnormality. The trans-esophageal echocardiography and aortography showed moderate aortic stenosis and regurgitation. At the operation, a quadricuspid aortic valve with three equal cusps and one smaller cusp that showed marked calcification and displacement of right coronary orifice were noted. Successful aortic valve replacement and coronary artery bypass grafting were performed and postoperative course was

Arterioenteric fistulae: diagnosis and treatment by angiography.

Two cases of massive gastrointestinal haemorrhage caused by arterioenteric fistulae are presented. In both cases, bleeding was controlled by interventional angiography. In the first case, a fistula between an aberrant right subclavian artery and a reconstructed oesophagus was temporarily occluded with a balloon catheter as a pre-surgical measure. In the second case a communication between the external iliac artery and the colon in a patient with invasive cervical cancer was treated by embolization. An arterioenteric fistula should be considered as a possible cause of acute gastrointestinal hae

Management of closed injuries of the innominate artery.

Two cases of closed traumatic rupture of the innominate artery are described. Both patients underwent surgical reconstruction, for which one patient was cooled to 15 degrees C on cardiopulmonary bypass. In the management of these rare and severe injuries, which almost invariably need surgical repair, protection of the cerebral circulation is vital; cerebral blood flow and function should be monitored during the operation, for which cardiopulmonary bypass, hypothermia, or local shunting should be available. Failure to recognise the features of these injuries lead to a high death rate, and aorto

Bilateral complete ureteral obstruction secondary to an abdominal aortic aneurysm with perianeurysmal fibrosis: diagnosis by computed tomography.

The sixteenth case of bilateral ureteral obstruction with anuria secondary to an abdominal aortic aneurysm with perianeurysmal fibrosis is presented. The details of this case clearly indicate the need for a combined vascular and urologic approach to patients with this disorder. A complete preoperative evaluation, including excretory urography, retrograde pyeloureterography, aortography and, when indicated, venacavography, may determine renal and vascular abnormalities that will allow a more definitive and successful management of this difficult problem. Furthermore, we believe that the use of

Traumatic haemorrhage into the thyroid simulating major vessel damage from deceleration injury.

Lawton, G. (1974).Thorax, 29, 607-608. Traumatic haemorrhage into the thyroid simulating major vessel damage from deceleration injury. Haemorrhage into the thyroid gland is a well recognized condition but there are few cases in the literature of severe haemorrhage associated with trauma. A case is described of a patient involved in a road traffic accident where the clinical features strongly suggested a diagnosis of great vessel rupture associated with deceleration injury. The importance of considering the possibility of haemorrhage into a retrosternal thyroid gland in a closed chest injury is

[Pulsed Doppler echocardiographic detection of regurgitant blood flow in the ascending, descending and abdominal aorta of patients with aortic regurgitation].

To ascertain whether the presence of retrograde holo-diastolic flow (RHF) in the ascending, descending or abdominal aorta is indicative of the severity of aortic regurgitation (AR), ascending, descending and abdominal aortic flow velocities were measured by pulsed Doppler echocardiography in 35 patients with AR and in 18 patients without AR, confirmed by aortography. Among the 35 patients with AR, 15 had mitral regurgitation, 11 had mitral stenosis, eight had aortic stenosis, five had prosthetic mitral valves, four had prosthetic aortic valves and two had aorto-pulmonary shunts. Coarctation of distal thoracic aorta--the middle aortic syndrome in an elderly female with severe coronary artery disease.

Coarctation of the distal aorta or middle aortic syndrome is a segmental stenosis of the middle portion of the aorta, between the arch and the terminal bifurcation. Middle aortic syndrome is a rare disease of infants and young adults presenting with hypertension, lower limb claudication and renal insufficiency and is diagnosed by aortography or magnetic resonance angiography. Our case is unique because this condition was an incidental finding diagnosed during cardiac catheterization in a 70-year-old patient with hypertension in the absence of lower-limb claudication. Recognition of this rare c

Syphilitic aortitis: an uncommon cause of acquired aortopulmonary fistula.

Cardiovascular syphilis has become a medical curiosity with the advent of widespread use of penicillin for the treatment of early syphilis. We report a case of a 37-year-old man who presented with sudden onset breathlessness that worsened insidiously for 1 year. Diagnosis of syphilitic aortopulmonary fistula was confirmed by aortography, contrast computed tomography, and histopathology. The patient underwent successful surgical correction. A high level of suspicion and awareness is needed for the diagnosis of this now rare disease. This entity is only amenable to surgical correction, even as e

Agenesis of the internal carotid artery associated with aortic arch anomaly in a patient with congenital Horner's syndrome.

We report a rare case of agenesis of the left internal carotid artery (ICA) and an aortic arch anomaly that presented with ipsilateral congenital Horner's syndrome. Digital subtraction angiography revealed left ICA agenesis shortly after its origin and anastomosis between the left maxillary artery and the supraclinoid segment of the left ICA. Aortography of the aortic arch revealed the right subclavian artery arising as the first branch of a left aortic arch, followed by a bicarotid trunk and a left subclavian artery. High resolution CT of the skull base revealed the absence of the left caroti

Additional aortopulmonary collaterals in patients referred for coil occlusion of a patent ductus arteriosus.

Coexisting aortopulmonary collaterals in patients diagnosed with a patent ductus arteriosus (PDA) are rare findings. Percutaneous transcatheter closure of PDA and requisite aortography offer an unique opportunity to identify and treat these systemic arterial anomailes, which would be missed by echocardiographic evaluation alone. The significance of these collaterals is unclear, but it may contribute to left heart dilation from additional left to right shunting in patients with an otherwise isolated small PDA. Of 18 patients undergoing transcatheter occlusion of a PDA with Gianturco coils, 2 we

Endovascular stenting of an aortopulmonary fistula presenting with hemoptysis. A case report.

We present a 45 year old man with massive hemoptysis due to an aortopulmonary fistula. Our patient had a history of a previous patent ductus arteriosus repair which was complicated by a previous aortopulmonary fistula. Computed tomography of the chest and aortography made the diagnosis of a recurrent aortopulmonary fistula. Because of the history of previous surgical aortic procedures, repair of the fistula was completed through a retroperitoneal aortotomy with intravascular insertion of an expandable stainless steel stent covered by a polyester graft. The patient has had no hemoptysis or comp

[Endoluminal occlusion, using a catheter, of patent ductus arteriosus in an adult].

The risk of surgical closure of the ductus arteriosus in the adult is greater than in children. The ductus arteriosus can now be occluded by venous catheterisation using a Rashkind umbrella. This procedure vas performed in a 63 year old woman. The diagnosis was confirmed and the anatomy of the lesion defined by catheterisation with aortography. The patent ductus was then occluded with a balloon catheter to assess the reversibility of the pulmonary hypertension. A 17 mm Rashkind umbrella was then used to completely occlude the ductus. The advantages of the method over surgical closure are: abse

[Aortobronchopulmonary fistula following a pulmonary contained aortic rupture in patients with a thoracic aortic aneurysm and severe aortic sclerosis].

Rupture of the thoracic aorta is usually widespread and fatal. Rupture limited by the lung and aorto-bronchopulmonary fistula are rare. We report two patients with a contained perforation of the thoracic aorta, who presented with haemoptysis. Chest radiographs demonstrated an aortic aneurysm and pulmonary haemorrhage in the first, while in the other perforation of an atherosclerotic aorta with a pulmonary haematoma was misinterpreted as bronchial carcinoma. In both cases contrast enhanced CT clearly demonstrated the abnormality in the aorta and the pulmonary haemorrhage. Since aortography ofte
