Reduced left vmPFC-STN-related white matter integrity is associated with pre-surgical apathy in Parkinson's disease.

INTRODUCTION: Apathy is a common and disabling non-motor symptom in Parkinson's disease (PD) that can diminish the therapeutic benefit of subthalamic nucleus deep brain stimulation (STN-DBS). We previously demonstrated that co-stimulation of the left ventromedial prefrontal cortex (vmPFC) can alleviate apathy following levodopa withdrawal in PD patients treated with STN-DBS. Building on these findings, we investigated whether presurgical white matter integrity of the vmPFC-STN pathway constitutes a structural correlate of clinically significant apathy prior to DBS. METHODS: Preoperative T1-we

Apathy and Depression Among People Aging With Traumatic Brain Injury: Relationships to Cognitive Performance and Psychosocial Functioning.

OBJECTIVE: Apathy and depression are both common after moderate to severe traumatic brain injury (TBI) and may be especially important to distinguish in older adults with TBI. The authors examined apathy and depression in relation to cognitive performance domains and their potentially unique contribution to psychosocial functioning in this patient population. METHODS: A total of 106 participants (mean±SD age=64±8 years) with chronic moderate to severe TBI (≥1 year) completed questionnaires assessing severity of apathy (Frontal Systems Behavior Scale-apathy subscale) and depression (Geriatric

Cognitive Deficits, Apathy, and Hypersomnolence Represent the Core Brain Symptoms of Adult-Onset Myotonic Dystrophy Type 1.

Myotonic dystrophy type 1 is the most common form of muscular dystrophy in adults, and is primarily characterized by muscle weakness and myotonia, yet some of the most disabling symptoms of the disease are cognitive and behavioral. Here we evaluated several of these non-motor symptoms from a cross-sectional time-point in one of the largest longitudinal studies to date, including full-scale intelligence quotient, depression, anxiety, apathy, sleep, and cerebral white matter fractional anisotropy in a group of 39 adult-onset myotonic dystrophy type 1 participants (27 female) compared to 79 unaff

Dominantly inherited apathy, central hypoventilation, and Parkinson's syndrome: clinical, biochemical, and neuropathologic studies of 2 new cases.

We describe 2 new patients from a family in which 10 persons in 3 successive generations had a dominant neuropsychiatric disorder characterized by apathy, central hypoventilation, and parkinsonism. Neuropathologically, both patients showed severe neuronal loss and reactive gliosis in the substantia nigra. Neurochemical studies showed a marked depletion of dopamine in substantia nigra, putamen, and caudate nucleus, as well as reduction in serotonin content in the substantia nigra. Glutamate contents were low in frontal cortex and thalamus, and gamma-aminobutyric acid (GABA) contents were low in

The social dimension of apathy: evidence for a distinct domain from 11,243 individuals across health and neurocognitive disorders.

Apathy is a highly prevalent and disabling neuropsychiatric syndrome, but its multi-dimensional structure is a challenge for progress towards better identification and treatment. A crucial unresolved question is whether social disengagement reflects a distinct deficit in social motivation or a by-product of diminished initiative or emotional blunting. Previous studies have been constrained by modest sample sizes and limited use of apathy-specific instruments or phenotypically narrow cohorts. Here, we analysed item-level data from 11,243 individuals recruited across multiple centres, including

A Longitudinal and Reproducible Anti-Coactivation Pattern Between the Cerebellum and the Ventral Tegmental Area Is Related to Apathy in Schizophrenia.

BACKGROUND: Negative symptoms of schizophrenia (SZ) lack effective treatments. Anomalies in the reward system and cerebellum have been linked to these symptoms. The cerebellum modulates reward circuitry via the ventral tegmental area (VTA). The cognitive dysmetria theory posits that reduced cerebellar inhibition in SZ may underlie striatal hyperdopaminergia. However, the role of cerebellum-VTA connectivity in negative symptoms remains unclear. METHODS: From 427 individuals screened, 146 were recruited, including 90 with SZ and 56 healthy control participants (HCs). At 3 months (T2), 65 indivi

Temporal Sequence of Cognitive Function and ADLs and Mediation Effect of Apathy in Parkinson's Disease: Cross-Lagged Analyses.

OBJECTIVES: To explore the temporal sequence between cognitive function and activities of daily living (ADLs) in patients with Parkinson's disease (PD) and investigate whether apathy mediates these effects. METHODS: We recruited 486 patients from the Parkinson's Progression Marker Initiative. We constructed a cross-lagged panel model to explore the temporal sequence between cognitive function and ADLs and analyzed the longitudinal mediating effect of apathy on the relationship between cognitive function and ADLs. RESULTS: The slopes of ADLs and cognitive function were significant (p < .001),

The Role of Cognition, Affective Symptoms, and Apathy in Treatment Adherence with Noninvasive Home Mechanical Ventilation in Myotonic Dystrophy.

BACKGROUND: Chronic respiratory failure often occurs in myotonic dystrophy type 1 (DM1) and can be treated with noninvasive home mechanical ventilation (HMV). Treatment adherence with HMV is often suboptimal in patients with DM1, but the reasons for that are not well understood. OBJECTIVE: The aim of this exploratory study was to gain insight in the prevalence of mild cognitive impairment, affective symptoms, and apathy and to investigate their role in HMV treatment adherence in DM1. METHODS: The Montreal Cognitive Assessment (MoCA), the Hospital Anxiety and Depression Scale (HADS), and the

A Biopsychosocial Framework for Apathy Following Moderate to Severe Traumatic Brain Injury: A Systematic Review and Meta-analysis.

Apathy, the deficit of goal-directed behaviour, is well recognised as one of the most debilitating syndromes after moderate-to-severe traumatic brain injury (TBI). However, mechanisms underlying apathy, or at least factors associated with apathy, are sporadically reported. Based on a biopsychosocial framework, this systematic review and meta-analysis synthesised evidence regarding neurobiological, socio-environmental and individual factors associated with apathy. Our searches identified 21 studies satisfying inclusion and exclusion criteria. Results showed that the majority of work has focused

Is Depression or Apathy Playing a Key Role in Predicting Financial Capacity in Parkinson's Disease with Dementia and Frontotemporal Dementia?

(1) Background: Depression and apathy both affect cognitive abilities, such as thinking, concentration and making decisions in young and old individuals. Although apathy is claimed to be a "core" feature of Parkinson's disease (PD) and frontotemporal dementia (FTD), it may occur in the absence of depression and vice versa. Thus, the aim of this study is to explore whether depression or apathy better predict financial capacity performance in PD and FTD as well as in nondemented participants. (2) Methods: Eighty-eight participants divided into three groups (PD, FTD and non-demented participants)

Apathy, not depressive symptoms, as a predictor of semantic and phonemic fluency task performance in stroke and transient ischemic attack.

OBJECTIVES: This study examined the relationship between apathy and cognition in patients with cerebrovascular disease. Apathy may result from damage to frontal subcortical circuits causing dysexecutive syndromes, but apathy is also related to depression. We assessed the ability of apathy to predict phonemic fluency and semantic fluency performance after controlling for depressive symptoms in 282 individuals with stroke and/or transient ischemic attack. METHOD: Participants (N = 282) completed the Phonemic Fluency Test, Semantic Fluency Test, Center for Epidemiologic Studies Depression Scale,

Suppression of neuronal eEF2K alleviates cognitive deficits and apathy-like behavior in APP/PS1 AD model mice.

Alzheimer's disease (AD) is a complex neurodegenerative disorder characterized by synaptic failure, cognitive impairment and neuropsychiatric symptoms (NPS). Apathy is the most common NPS seen in AD patients, and its underlying mechanisms remain unknown. Here, we investigated the roles of neuronal eukaryotic elongation factor 2 (eEF2) phosphorylation (by its kinase eEF2K) in AD-associated cognitive deficits and NPS. We performed a series of experiments using a multidisciplinary approach including genetics, behavioral assays, synaptic electrophysiology, and unbiased proteomics. The results demo

Targeting central immune signaling enhances the effects of methylphenidate in alleviating apathy-like behavior in 5xFAD mice.

Beyond cognitive impairment, Alzheimer's disease (AD) is frequently accompanied by apathy, the most prevalent and burdensome neuropsychiatric symptom (NPS). Apathy significantly impacts AD onset and progression, yet its molecular underpinnings remain unclear. Our previous RNA-sequencing analysis revealed abnormal immune gene expression uniquely associated with apathy in AD patients. In this study, we investigated whether changes in these immune related genes are also linked to apathy-like behavior, and whether administration of C3a receptor antagonist SB290157, alone or with methylphenidate, m

Co-stimulating the left vmPFC compensates for apathy after levodopa withdrawal in Parkinson's patients with STN DBS.

INTRODUCTION: Subthalamic nucleus deep brain stimulation (STN DBS) improves motor symptoms of Parkinson's disease (PD), but its effect on motivation is controversial. Apathy, the lack of motivation, commonly occurs in PD and is often exacerbated after surgery and its concomitant levodopa reduction. Apathy and reward processing are associated with the ventromedial prefrontal cortex (vmPFC), which standard targeting strategies avoid by targeting the dorsolateral STN. Since apathy can be a levodopa-responsive PD symptom, levodopa withdrawal could unmask apathy without sufficient stimulation of no

Synthesis and pharmacological evaluation of a series of fluoro-substituted 3-phenoxy-azetidines as cortical enhancers for the treatment of apathy.

We here report the discovery of a series of fluoro-substituted 3-phenoxy-azetidines useful for modulating levels of monoamines in cerebral cortical areas of the brain. The compounds were characterized by in vivo studies using, in particular, behavioural, and microdialysis methods. Furthermore, the compounds were tested in binding assays for the human serotonin (5-HT), noradrenaline (NA), and dopamine (DA) transporters in order to assess the risk for addiction/dependency. Optimization of this class of compounds for improving cognitive functions while minimizing the effect on monoamine transport

Apathy's hidden pathways: Psychosocial mediators of quality of life among people with early-onset Parkinson's disease.

INTRODUCTION: Apathy is a highly prevalent non-motor symptom of Parkinson's disease (PD) and a major determinant of health-related quality of life (HRQoL). Its impact may be especially pronounced in early-onset PD (EOPD), yet the psychosocial mechanisms linking apathy to HRQoL are poorly understood. This study tested whether perceived stress, meaning in life (MiL), and psychological capital (PsyCap) mediate the relationship between apathy and HRQoL. METHODS: Between November 2022 and February 2023, 262 adults with self-reported EOPD (diagnosed ≤50 y) completed an online survey comprising the
