Acquired alexia with agraphia syndrome in childhood.

The acquired alexia with agraphia syndrome is a conspicuous disorder of reading and writing in the absence of significant other language impairments that has mainly been recorded in adults. Pure cases are rare, with most patients displaying mild aphasic deficits. In children, acquired reading and writing disorders are generally reported as part of more encompassing aphasic syndromes affecting oral and written language equally, for example, Broca or Wernicke aphasia. Documented instances of predominant acquired reading and writing disorders in childhood are exceptional. We report an 11-year-old

Evaluation of cerebral infarction with iodine 123-iomazenil SPECT.

UNLABELLED: This study evaluates ischemic damage to central benzodiazepine (BZD) receptor binding in the brain with [123]iomazenil SPECT in relation to CT hypodense lesions and blood flow abnormalities. METHODS: Nine patients with middle cerebral artery territory infarction were studied. Iomazenil images obtained 180 min postinjection were analyzed for BZD receptor binding. The cortical infraction, visualized as CT hypodense area on CT, the peri-infarct area, visualized as normodensity surrounding the infarction on CT, the intrahemispheric remote area and the cerebellum were analyzed by takin

Sporadic Creutzfeldt-Jakob Syndrome Misdiagnosed as Recurrent Stroke: A Case Report.

While sporadic Creutzfeldt-Jakob disease (sCJD) typically presents with neurological symptoms such as cognitive impairment, ataxia, and myoclonus, its clinical manifestations can be diverse. We report about a 70-year-old woman with sCJD who was misdiagnosed with recurrent stroke. She initially showed hemiplegia and high-intensity signals on brain diffusion-weighted magnetic resonance imaging (DWI), which corresponded to the symptoms of a stroke. She was diagnosed with recurrent stroke as her muscle weakness showed stepwise deterioration, with the appearance of additional high-intensity signals

[Mixed transcortical aphasia due to multiple deep white matter infarction in the dominant cerebral hemisphere: a case report].

We report a case of mixed transcortical aphasia (MTA) due to multiple cerebral infarction in the dominant hemisphere in an 80-year-old right-handed woman without hemiplegia. Her spontaneous speech was markedly reduced and auditory comprehension, reading and writing were severely disturbed. Although the repetition of sentences (at most 3 words) was relatively preserved, her speech was echolalic. Brain MRI showed bilateral multiple deep white matter infarction and subcortical infarction of the left parietal lobe, including left angular gyrus, but no abnormal signal intensities were detected in e

Functional lateralisation of pitch accents and intonation in Norwegian: Monrad-Krohn's study of an aphasic patient with altered "melody of speech".

The present article provides a linguistic analysis of Monrad-Krohn's famous description of a patient with deviant prosody (1947). Monrad-Krohn found it particularly striking that the patient's "melody of speech" had been damaged although her musical abilities showed no impairment. The patient had suffered a traumatic motor aphasia as the result of a shrapnel wound in Broca's area. In East Norwegian, accented syllables are associated with one of two pitch patterns, either a fall in pitch or a low-level pitch. The patient had trouble producing the distinction between these two accents. Monrad-Kr

Frontotemporal and dopaminergic control of idea generation and creative drive.

This article presents a three-factor anatomical model of human idea generation and creative drive, focusing on interactions between the temporal lobes, frontal lobes, and limbic system. Evidence is drawn from functional imaging, drug studies, and lesion analysis. Temporal lobe changes, as in hypergraphia, often increase idea generation, sometimes at the expense of quality. Frontal lobe deficits may decrease idea generation, in part because of rigid judgments about an idea's worth. These phenomena are clearest in verbal creativity, and roughly parallel the pressured communication of temporal lo

Plasticity in language cortex and white matter tracts after resection of dominant inferior parietal lobule arteriovenous malformations: a combined fMRI and DTI study.

OBJECTIVE: The dominant inferior parietal lobe (IPL) contains cortical and subcortical structures that serve language processing. A high incidence of postoperative short-term aphasia and good potential for language reorganization have been observed. The authors' goal was to study the plasticity of the language cortex and language-related fibers in patients with brain arteriovenous malformations (BAVMs) located in the IPL. METHODS: A total of 6 patients who underwent microsurgical treatment of an IPL BAVM were prospectively recruited between September 2016 and May 2018. Blood oxygen level-depe

[Amusia and aphasia of Bolero's creator--influence of the right hemisphere on music].

OBJECTIVES: The experience with cortical localization (BA 44, 45, 22) of language (Broca, Wernicke and others) in the left hemisphere has been repeatedly tested over the last 150 years and is now generally accepted. A single case report with autopsy findings (Leborgne, Tan tan), has enabled to localize the seat of spoken language in the left third frontal convolution. As music and language have a lot in common and even share the same hearing system, it is logical to try to localize the cognitive centers for music too. METHODS: The disabling neurological disease illness of Maurice Ravel (1875-

Dominique Jean Larrey (1766-1842) and His Contributions to Military Medicine and Early Neurosurgery.

Dr. Dominique Jean Larrey was a pioneer in the field of military surgery. His creative innovation and drive to improve the quality of medical services available to those injured during war guided his achievements in medicine. Dr. Larrey has often been referred to as "the father of emergency medical services" and "the father of modern military medicine." His contributions to medicine continue to live on in common procedures and healthcare systems today, such as aspiration of pericardial effusion and drainage of hemothorax and empyema. Based on his own writings, he treated multiple cases of intr

HaNDL syndrome in a 14-year-old girl.

Transient headache and neurological deficits with cerebrospinal fluid lymphocytosis (HaNDL syndrome) consists of recurrent headaches with focal neurological signs, which can include motor, sensory and aphasic symptoms. Although considered rare, it is becoming increasingly recognised in clinical practice due to the accumulation of case reports. The pathophysiology remains unclear although changes in the neurovascular resemble those found in migraine, which are thought to be triggered by an infectious process. HaNDL can mimic various serious, including life-threatening, diseases, such as stroke

Emergence delirium with transient associative agnosia and expressive aphasia reversed by flumazenil in a pediatric patient.

Multiple factors may contribute to the development of emergence delirium in a child. We present the case of a healthy 12-year-old girl who received preoperative midazolam with the desired anxiolytic effect, underwent a brief general anesthetic, and then exhibited postoperative delirium, consisting of a transient associative agnosia and expressive aphasia. Administration of flumazenil led to immediate and lasting resolution of her symptoms. We hypothesize that γ-aminobutyric acid type A receptor-mediated effects, most likely related to an atypical offset of midazolam, are an important subset of

[Clinical considerations and EEG-CT correlations in lacunar infarcts].

56 cases of lacunar stroke were reviewed retrospectively. The patients showed a clinic picture which could be classified within nine of the lacunar syndromes recognized by Fisher. The localization of the lacunes was considered with respect to the clinical symptoms and the characteristics of the EEG and the TC and their different correlations were examined. The EEGrafic patterns of the lacunar strokes with normal TC were compared with those showing an altered one. Our study suggests that the absence of EEGrafic alterations may be a useful characteristic for the diagnosis of lacunar stroke, even

A cocaine-associated quadriplegia and motor aphasia after first use of cocaine.

UNLABELLED: A 31-year-old female who have snorted one "line" of cocaine hydrochloride (approximately 35 mg), for the first time in her life, was admitted to the hospital because of acute onset of right hemiplegia and left hemiparesis evolving into quadriplegia. Motor aphasia, right eye-ball divergent strabismus and right mouth recess lowering were also observed. CONCLUSIONS: A first time mucosal administration of cocaine hydrochloride even in low dose can cause severe neurological complications like quadriplegia and aphasia. Cocaine-associated stroke can be a diagnostic problem in the emergen

Unambiguous generalization effects after treatment of non-canonical sentence production in German agrammatism.

Agrammatism is-among others, characterized by a deficit in producing grammatical structures. Of specific difficulty is the utilization of complex, non-canonical sentence structures (e.g. object-questions, passives, object-clefts). Several studies have documented positive effects when applying a specific treatment protocol in terms of increasingly correct production of target complex sentence structures with some variance in generalization patterns noted across individuals. The objective of this intervention study was to evaluate an intervention program focussing on the production of non-canoni

An EMA analysis of the effect of increasing word length on consonant production in apraxia of speech: a case study.

The effect of increasing word length on the articulatory dynamics (i.e. duration, distance, maximum acceleration, maximum deceleration, and maximum velocity) of consonant production in acquired apraxia of speech was investigated using electromagnetic articulography (EMA). Tongue-tip and tongue-back movement of one apraxic patient was recorded using the AG-200 EMA system during word-initial consonant productions in one, two, and three syllable words. Significantly deviant articulatory parameters were recorded for each of the target consonants during one, two, and three syllables words. Word len

Application of the ICF in aphasia.

The aim of this article is to describe aphasia using the framework provided by the World Health Organization's International Classification of Functioning, Disability and Health (ICF). The key constructs of ICF are described in relation to the ultimate goal of intervention in aphasia-maximizing quality of life. Aphasic impairments as well as activity limitations and participation restrictions are discussed. In addition, the impact of contextual factors on the experience of aphasia and participation in life are addressed. Finally, a case example is presented to depict the use of the ICF as an o
