[An update on giant cell arteritis].

Giant cell arteritis (GCA) is a primary granulomatous systemic vasculitis involving the aorta and its main branches that affects people aged over 50 years with a genetic predisposition. Its main phenotypes are cranial and extracranial involvement, with or without symptoms of polymyalgia rheumatica. These phenotypes can overlap. The extracranial form can be oligosymptomatic and must be sought directly. The main complications of the disease are ischemia of essential territories such as the optic nerve or cerebral circulation, and aneurysmal dilations of the aorta and its large branches. Clinicia

The diagnosis of giant cell arteritis.

Giant cell arteritis (GCA) is the most common primary vasculitis in adults older than 50 years. The potential of GCA to cause bilateral, sequential vision loss makes it often a true neuro-ophthalmic emergency. Approximately one fifth of patients with GCA will present with ophthalmic complaints alone. The diagnosis of GCA requires a high index of suspicion and a systematic approach to diagnostic testing. The combination of abnormal laboratory markers of systemic inflammation and unilateral temporal artery biopsy is usually diagnostic. Additional testing with other diagnostic modalities may be r

Temporal arteritis: report of a case.

Temporal arteritis is a rheumatic disease that affects large and medium-sized arteries. It is a severe arteritis involving both the intima and media of the vessel and is a cause of headache that is frequently diagnosed erroneously as "atypical migraine." The patients have a burning or throbbing type of pain. Ultimately, there is localized inflammation or cellulitis over the swollen, tortuous artery. Jaw claudication, eye pain, photophobia, diplopia, and even blindness may accompany the temporal symptoms. As many as 20% to 60% of inadequately treated or untreated patients will lose their vision

[Stroke in the course of cerebral arteritis].

In this paper we wanted to present cases of stroke occurring in the course of cerebral vasculitis. Cerebral vasculitis occurs most often in patients with infections, tumours and connective tissue diseases. Idiopathic vasculitis is observed very rarely and making the diagnosis meets great difficulties. Three patients (2 women and 1 man) with idiopathic cerebral arteritis aged 28-41 who underwent treatment in our department are prevented. There were no known stroke risk factors in anamnesis and diagnostic investigations. All patients had headaches before the occurrence of stroke. Electrocardiogr

[Takayasu arteritis and the algodystrophic syndrome].

Algodystrophic syndrome represents a group of symptoms involving pain and oedema of the affected extremity, with movement disorders, vegetative and vasomotor disturbances and focal osteoporosis development. The key role in the pathogenesis of the syndrome plays intensive and prolonged stimulation of pain receptors. The authors present a case report of 47 years old woman with Takayasu's arteritis complicated by skin defects on right instep and sole with consequent development of algodystrophic syndrome on both lower extremities. Combined influence of both types of nociceptive stimuli is demonst

An unusual presentation of temporal arteritis.

Serious ocular and systemic complications may result from inflammatory processes of the orbit. Temporal arteritis is one such process that can affect the eye. The patient described herein is unusual because she presented initially with signs and symptoms similar to orbital cellulitis. As her disease evolved, she displayed marked, hemorrhagic chemosis with some limitation of abduction and resistance to retropulsion on the involved side. Laboratory and radiologic findings eliminated orbital cellulitis, limited Wegener's granulomatosis, and other systemic inflammatory disorders and led to the dia

Temporal arteritis and gangrene of the tongue.

A 69-year-old woman, suspected of having polymyalgia rheumatica, was biopsied from the temporal artery to establish the diagnosis. Six hours after the biopsy, she developed progressive gangrene of the tongue and the floor of the mouth, and intensive steroid therapy was initiated. The histological examination of the biopsy revealed giant-cell arteritis. Biopsy or other manipulation of the artery is suggested to be the cause of exacerbation of the arteritis locally. Steroid treatment is proposed in all cases prior to the biopsy. Other studies have not shown that the possibility of obtaining a po

Asymptomatic arteritis of the uterine cervix.

A necrotizing arteritis in the uterine cervix is described as an incidental finding in surgical material from 10 patients. The histological features are compared with those seen in polyarteritis nodosa. Subintimal hyaline deposition and a relative paucity of neutrophil and eosinophil polymorphs characterized the lesion but the histological appearance was never sufficiently specific to exclude confidently the possibility of polyarteritis nodosa. None of the patients had evidence of multisystem disease either at the time of operation or at subsequent follow-up assessment.

Takayasu Arteritis Complicated by Superior Mesenteric Artery Stenosis and Bilateral Renal Arteritis: A Clinical Case Report.

Takayasu arteritis (TA), a rare large-vessel vasculitis, primarily affects women of childbearing age, causing granulomatous inflammation in the aorta and its major branches. This inflammation can lead to stenosis, aneurysms, or occlusion, with the abdominal aorta, subclavian, and brachial arteries commonly involved. We present the case of a 26-year-old female with TA with a rare involvement of the superior mesenteric artery (SMA). The patient presented with progressive shortness of breath and exertional pain in the left upper limb, suggesting claudication. Echocardiography showed an ejection f

Analysis of pregnancies in women with Takayasu arteritis: complication of Takayasu arteritis involving obstetric or cardiovascular events.

AIMS: The incidence of Takayasu arteritis during child-bearing years is relatively high. The management of pregnancies in patients with this disease is of great importance in clinical obstetrics. Here we analyzed pregnancies of women with Takayasu arteritis with and without complications. MATERIAL AND METHODS: We retrospectively identified 27 pregnancies in 20 women with Takayasu arteritis seen between 1983 and 2005 at the National Cardiovascular Center, in Osaka, Japan. The incidences of obstetric events, steroid dose increase in pregnancy, and cardiovascular events were compared between gro

Tocilizumab for the treatment of large-vessel vasculitis (giant cell arteritis, Takayasu arteritis) and polymyalgia rheumatica.

OBJECTIVE: The interleukin-6 pathway is up-regulated in giant cell arteritis (GCA), Takayasu arteritis (TA), and polymyalgia rheumatica (PMR). We retrospectively assessed the outcomes of 10 patients with relapsing/refractory GCA, TA, or PMR treated with tocilizumab (TCZ). METHODS: Patients with GCA (n = 7), TA (n = 2), and PMR (n = 1) received TCZ. Seven subjects had failed at least 1 second-line agent. The outcomes evaluated were symptoms of disease activity, inflammatory markers, ability to taper glucocorticoids, and cross-sectional imaging when indicated clinically. RESULTS: The mean foll

Non-giant cell arteritis of the temporal artery presenting as the polymyalgia rheumatica-temporal arteritis syndrome.

We describe a heterogeneous group of 4 elderly patients with atypical vasculitis of the temporal artery who presented clinically with the polymyalgia rheumatica-temporal arteritis syndrome. The first had apparently isolated eosinophilic necrotizing vasculitis without peripheral blood eosinophilia, history of asthma, or pulmonary vasculitis (so called limited form of Churg-Strauss syndrome). Temporal artery biopsy in the 2nd patient revealed small vessel necrotizing vasculitis in the fragment of skeletal muscle surrounding a normal temporal artery (i.e., negative for giant cell arteritis, GCA).

The clinical pictures of giant cell arteritis. Temporal arteritis, polymyalgia rheumatica, and fever of unknown origin.

In a prospective study, 68 hospitalized patients were diagnosed as having giant cell arteritis. Temporal artery biopsy was performed in all patients and showed histologic evidence of arteritis in 42 (62%). Twenty-six patients had a negative biopsy but met the clinical criteria for the diagnosis. Four different clinical pictures were recognized. Thirteen patients (19%) had symptoms of localized temporal arteritis without muscular discomfort. The polymyalgia rheumatica syndrome without signs of localized arteritis was seen in 33 patients (49%). Seventeen (25%) had symptoms of both polymyalgia rh
