Simulated stand tests and centrifuge training to prevent orthostatic intolerance on Earth, moon, and Mars.

One proposed method to overcome postflight orthostatic intolerance is for astronauts to undergo inflight centrifugation. Cardiovascular responses were compared between centrifuge and gravitational conditions using a seven-compartment cardiovascular model. Vascular resistance, heart rate, and stroke volume values were adopted from literature, while compartmental volumes and compliances were derived from impedance plethysmography of subjects (n=8) riding on a centrifuge. Three different models were developed to represent the typical male subject who completed a 10-min postflight stand test ("mal

Cardio-postural deconditioning: A model for post-flight orthostatic intolerance.

Post-flight astronauts experience temporary but sometimes severe postural control dysfunction and decreased orthostatic tolerance. Research points to a possible link between cardiovascular and postural controls and orthostatic tolerance [Claydon,V.E., Hainsworth, R., 2006. Postural sway in patients with syncope and poor orthostatic tolerance. Heart 92, 1688-1689], for which a neurophysiological model has been presented [Souvestre, P.A., Blaber A.P., Landrock C.K., 2008. Re-evaluating pharmaceutical stability in space: A biomedical and regulatory science framework for long-duration missions.

Pharmaceutical stability is a critical determinant of crew health and mission success in long-duration spaceflight. Current regulatory frameworks governing drug stability are primarily based on terrestrial testing paradigms, including International Council for Harmonisation (ICH) guidelines, which assume controlled environmental conditions. However, spaceflight introduces unique stressors such as microgravity, ionizing radiation, exposure to the vacuum of space, and operational constraints that are not accounted for in existing models. This study systematically evaluates the limitations of curr

Pharmacological countermeasures for long-duration space missions: addressing cardiovascular challenges and advancing space-adapted healthcare.

Future long-duration crewed space missions beyond Low Earth Orbit (LEO) will bring new healthcare challenges for astronauts for which pharmacological countermeasures (pharmacological countermeasures) are crucial. This paper highlights current pharmacological countermeasures challenges described in the ESA SciSpacE Roadmap, with a focus on the cardiovascular system as a model to demonstrate the potential implication of the challenges and recommendations. New pharmacological approaches and procedures need to be adapted to spaceflight (spaceflight) conditions, including ethical and reglementary c

Spaceflight Associated Neuro-ocular Syndrome (SANS) and its countermeasures.

Astronauts can develop a distinct collection of neuro-ophthalmic findings during long duration spaceflight, collectively known as Spaceflight Associated Neuro-ocular Syndrome (SANS). These clinical characteristics include optic disc edema, hyperopic refractive shifts, globe flattening, and chorioretinal folds, which may pose a health risk for future space exploration. Obtaining knowledge of SANS and countermeasures for its prevention is crucial for upcoming crewed space missions and warrants a multidisciplinary approach. This review examines the potential causes and countermeasures of SANS, in

Defining spaceflight associated dry eye syndrome (SADES): Mechanisms, complications, and countermeasures.

There is a high prevalence of dry eye-related symptoms among astronauts during spaceflight. We propose naming this phenomenon: Spaceflight Associated Dry Eye Syndrome (SADES). In this study, we review the potential mechanisms of dry eye in the spaceflight environment, categorizing them into external (radiation, environmental hazards) and microgravity-related (meibum outflow) mechanisms. One key mechanism is that microgravity impedes the secretion of meibum, the primary contributor to the tear film lipid layer. SADES may impact the ocular surface due to the positive feedback loop of inflammatio

Sleep-Monitoring Technology Progress and Its Application in Space.

INTRODUCTION: Sleep is an indispensable physiological phenomenon. The complexity of sleep and the time it occupies in human life determine that its quality is positively correlated with human health. Since polysomnography was used in spaceflight in 1967, the sleep problem during astronaut flight has been studied in depth for more than 50 yr, and many solutions have been proposed, but astronauts have always had sleep problems during orbital flight. Insufficient sleep and changes in the rhythm of human sleep-wake activity will lead to disturbance of the human body's internal rhythm indicators, w

Cosmic chronometers: Is spaceflight a catalyst for biological ageing?

Astronauts returning from space missions often exhibit health issues mirroring age-related conditions, suggesting spaceflight as a potential driver of biological ageing and age-related diseases. To unravel the underlying mechanisms of these conditions, this comprehensive review explores the impact of the space "exposome" on the twelve hallmarks of ageing. Through a meticulous analysis encompassing both space environments and terrestrial analogs, we aim to decipher how different conditions influence ageing hallmarks. Utilizing PubMed, we identified 189 studies and 60 meet screening criteria. Re

Cardiac function, structural, and electrical remodeling by microgravity exposure.

Space medicine is key to the human exploration of outer space and pushes the boundaries of science, technology, and medicine. Because of harsh environmental conditions related to microgravity and other factors and hazards in outer space, astronauts and spaceflight participants face unique health and medical challenges, including those related to the heart. In this review, we summarize the literature regarding the effects of spaceflight on cardiac structure and function. We also provide an in-depth review of the literature regarding the effects of microgravity on cardiac calcium handling. Our r

Neuro-consequences of the spaceflight environment.

As human space exploration advances to establish a permanent presence beyond the Low Earth Orbit (LEO) with NASA's Artemis mission, researchers are striving to understand and address the health challenges of living and working in the spaceflight environment. Exposure to ionizing radiation, microgravity, isolation and other spaceflight hazards pose significant risks to astronauts. Determining neurobiological and neurobehavioral responses, understanding physiological responses under Central Nervous System (CNS) control, and identifying putative mechanisms to inform countermeasure development are

Integrated physiological characteristics and proteomes of Bacillus licheniformis response to 6-month spaceflight.

Bacillus licheniformis (BL) is used for clinical treatment of acute and chronic enteritis and diarrhea. The present study explored the physiological characteristics and proteomes of BL response to 6-month spaceflight aboard the China Space Station. 6-month spaceflight delayed BL lag phase, reduced cell wall thickness, and impaired BL biofilm formation and adhesion capacity to Caco-2 cells. After 6-month spaceflight, activity of β-amylase, alkaline protease and lipase in BL was increased by 21.6%,67.3% and 42.5%. The alkaline resistance of BL at pH 11.0 was increased by 29.6%. Response of Staphylococcus aureus physiology and Agr quorum sensing to low-shear modeled microgravity.

Staphylococcus aureus is commonly isolated from astronauts returning from spaceflight. Previous analysis of omics data from S. aureus low Earth orbit cultures indicated significantly increased expression of the Agr quorum sensing system and its downstream targets in spaceflight samples compared to ground controls. In this current study, the rotary cell culture system (RCCS) was used to investigate the effect of low-shear modeled microgravity (LSMMG) on S. aureus physiology and Agr activity. When cultured in the same growth medium and temperature as the previous spaceflight experiment, S. aureu

Microgravity-induced changes in skeletal muscle and possible countermeasures: What we can learn from bed rest and human space studies.

Despite exercise countermeasures to sustain health and performance in spaceflight, complete maintenance of muscle mass and functions in microgravity is still not possible for most astronauts. The principal cause of the limited effectiveness of existing exercise countermeasures is the difficulty in achieving full loading forces in space. The implementation of countermeasures which require small devices and simulate Earth-like loading forces to maintain muscle mass, strength and endurance is therefore highly desirable. At present, the cellular mechanisms that induce muscle atrophy in weightlessn

Ocular Matrix Metalloproteinases in Spaceflight.

INTRODUCTION: Spaceflight presents unique challenges to ocular health, as visual disturbances such as dry eye symptoms have been frequently reported by astronauts. Matrix metalloproteinases (MMPs), a family of extracellular matrix-degrading enzymes, are known mediators of ocular surface inflammation and tissue remodeling on Earth, particularly in dry eye disease. This review explores the potential role of MMPs in spaceflight-associated ocular conditions, including spaceflight-associated dry eye syndrome and spaceflight-associated neuro-ocular syndrome. METHODS: We searched literature on topic

Cerebrovascular pulsatility following long duration spaceflight is associated with changes in pulse pressure and carotid artery stiffness.

Central artery stiffening increases the haemodynamic pulsations transmitted downstream towards target organs, including the brain. While recent evidence suggests that long duration spaceflight is associated with reduced common carotid artery (CCA) distensibility, cerebrovascular pulsatility has not been extensively characterized in astronauts. This study investigated changes in pulsatility from pre-flight to after 6 months in space, using a secondary analysis of data from four separate experiments. Middle cerebral artery blood velocity (MCAv) was measured during supine rest in 27 astronauts (2

Multimodal Imaging-Based Cerebral Blood Flow Prediction Model Development in Simulated Microgravity.

Background: Abnormal alterations in cerebral blood flow (CBF) have been implicated in cognitive decline and neurodegeneration. Maintaining adequate CBF in astronauts during long-duration microgravity is therefore crucial for the success of manned spaceflight. However, the quantitative assessment of CBF during space missions remains challenging. Methods: Thirty-six participants underwent a 90-d -6° head-down tilt bed rest (HDTBR) protocol, a well-established ground-based analog of microgravity. Multimodal imaging data, including internal carotid artery Doppler ultrasound and brain magnetic reso

Celastrol and Cblin peptide activation of IGF-1 signaling prevents microgravity-induced atrophy in rat L6 myotubes.

This study investigated the efficacy of two natural compounds-celastrol, a heat shock protein (HSP) inducer, and Cblin peptide, a ubiquitination inhibitor-in counteracting muscle atrophy under real microgravity conditions. Both agents independently attenuated microgravity-induced reductions in myotube thickness, myosin heavy chain protein levels, and atrogene expression. Celastrol primarily enhanced HSP expression, whereas Cblin peptide inhibited insulin receptor substrate-1 degradation, thereby promoting insulin-like growth factor-1 signaling. Despite their distinct molecular actions, no syne

Pharmacological and non-pharmacological countermeasures to Space Motion Sickness: a systematic review.

INTRODUCTION: Space Motion Sickness (SMS) is a syndrome that affects around 70% of astronauts and includes symptoms of nausea, dizziness, fatigue, vertigo, headaches, vomiting, and cold sweating. Consequences range from discomfort to severe sensorimotor and cognitive incapacitation, which might cause potential problems for mission-critical tasks and astronauts and cosmonauts' well-being. Both pharmacological and non-pharmacological countermeasures have been proposed to mitigate SMS. However, their effectiveness has not been systematically evaluated. Here we present the first systematic review

Long-term dose response of trabecular bone in mice to proton radiation.

Astronauts on exploratory missions will experience a complex environment, including microgravity and radiation. While the deleterious effects of unloading on bone are well established, fewer studies have focused on the effects of radiation. We previously demonstrated that 2 Gy of ionizing radiation has deleterious effects on trabecular bone in mice 4 months after exposure. The present study investigated the skeletal response after total doses of proton radiation that astronauts may be exposed to during a solar particle event. We exposed mice to 0.5, 1 or 2 Gy of whole-body proton radiation and
