PURPOSE: To evaluate whether 6 mm long by 4 mm wide dental implants could be an alternative to implants at least 10 mm long placed in bone augmented with bone substitutes in posterior atrophic jaws.
MATERIALS AND METHODS: A total of 20 patients with bilateral atrophic mandibles and 20 patients with bilateral atrophic maxillae, having 5 to 7 mm of bone height above the mandibular canal or below the maxillary sinus, had each side of the jaws randomly allocated according to a split-mouth design. They were allocated to receive one to three 6 mm long and 4 mm wide implants, or implants at least 10

OBJECTIVES: The rising demand for zygomatic implants (ZIs) in contemporary dental implantology underscores the need for an anatomy-based categorization system and a surgical decision-making panel that takes into account anatomical, biomechanical, and prosthetic factors to determine the final surgical strategy. This article proposes a new Z-Classification, the anatomical division of the maxillo-zygomatic complex.
MATERIALS AND METHODS: The classification was developed from the authors' clinical experience of 882 ZIs placed in 234 patients across two centres over a ten-year period. According to

Sinus floor elevation is the standard procedure that allows dental implant insertion in the atrophic posterior maxilla. Instead of autogenous bone, tissue-engineered bone grafts can be used, but clear comparative clinical studies also assessing the influence of the biomaterial are missing. In six patients, tissue-engineered bone grafts were used in eight sinus floor elevations. After culturing osteoblast-like cells from biopsies of the maxilla, they were seeded on scaffolds made either from demineralised bovine bone matrix (DBBM) or from solvent-dehydrated mineralised bone (SDBB), and grafted.

PURPOSE: Dental implant placement in atrophic alveolar ridges often necessitates grafting procedures, followed by immediate or delayed implant placement. This study assessed the survival of immediately loaded dental implants placed in deficient alveolar bone sites at bone grafting.
MATERIALS: From 1999 to May 2002, 1 operator (A.P.) inserted 1065 implants (607 in mandibles, 458 in maxillae) into 338 partially edentulous patients. Most implants were placed into compromised residual ridges or prepared tooth extraction sockets. Implants placed in augmented areas were splinted to implants in nona

The objective of this study was to compare marginal bone loss, surgical and clinical complications, and dental implant survival rate in bilateral maxillary sinus augmented by autologous or porcine xenograft. A randomized controlled clinical trial using split-mouth design enrolled 12 consent adult patients (59.7 ± 8.7 years), who received bilateral maxillary sinus floor augmentation for oral rehabilitation with implant-supported prosthesis. Each patient received both the autologous bone from the mandible (control) or porcine xenograft (test) during the random bilateral sinus lift surgery. A tot

The aim of this study was clinical and radiographic evaluation of sinus floor lifting from the alveolar crest with trephine bur and autogenous bone graft. In this research, a press-fit dowel bone technique with application of an autogenous bone block was used for elevating the sinus floor. Ten closed sinus lift surgeries were performed on 10 patients; all patients possessed atrophic ridges in an edentulous maxilla. Bone height of the ridge ranged between 3 and 5 mm. Surgery included forming a bony cylinder with a trephine bur to reach the sinus cortical floor and then elevating the sinus floor

PURPOSE: The purpose of the study was to obtain information on the possibility of using distraction osteogenesis (DO) to increase the height of posterior atrophic maxillary alveolar ridges by distracting very small alveolar crest bone segments and, secondly, to determine if it is possible to distract the bony antral floor itself, producing new bone between the sinus membrane and the alveolar crest.
MATERIALS AND METHODS: Three adult Papio anubis baboons were rendered edentulous in the posterior maxilla. Bilateral osteotomies were performed to produce a thin margin of crestal alveolar bone and

This retrospective cohort study was performed to evaluate the original Brånemark scheme vs the quad zygoma scheme for treating severe maxillary atrophy. Zygomatic implant surgeries performed between 1998 and 2021 at Ziekenhuis Oost Limburg, Belgium were analysed. Patients with ≥2 years of follow-up were included. Data on implant survival, prosthesis survival, and surgical techniques were assessed using Kaplan-Meier survival curves and multivariable Cox proportional hazards models adjusted for age, sex, and smoking; statistical significance was set at P ≤ 0.05. Overall, 203 patients with 675 zy

Longstanding partial edentulism in the posterior segment of the maxilla is a challenging treatment and typically involves extensive and invasive sinus floor augmentation. Various methods have been employed for sinus floor elevation, including the application of hydraulic pressure using the balloon technique. This case report describes a modified ballooning technique to elevate the sinus floor using a Foley catheter to apply the hydraulic pressure method to elevate the Schneiderian membrane prior to the placement of bone grafts and an endosseous implant in a 37-year-old male patient who present

INTRODUCTION: Sinus floor augmentation is a routinely used surgical technique for increasing the bone height/volume of the atrophic posterior maxilla. Optimal integration of the implanted augmentation material within the newly formed bone will-at least partly-depend on adequate vascularization to ensure sufficient recruitment of osteoblast and osteoclast precursor cells.
METHODS: The present technical note describes a modification intended to facilitate increased blood inflow into the augmented space. After preparation of the lateral window and elevation of the Schneiderian membrane, the cort

In the atrophic posterior maxilla, successful implant placement is often complicated by the lack of quality and volume of available bone. In these cases, sinus floor augmentation is recommended to gain sufficient bone around the implants. Sinus elevation can be performed by either an open lateral window approach or by a closed osteotome approach depending on available bone height. This case series demonstrates the feasibility and safety of minimally invasive antral membrane balloon elevation, followed by bone augmentation and implant fixation in 20 patients with a residual bone height of 2 to

BACKGROUND: There is a lack of evidence regarding success of implants placed in atrophic premaxilla using the nasal floor elevation technique.
PURPOSE: This study aimed to compare implants placed in augmented bone in the anterior maxilla using the nasal floor elevation technique with implants placed in the maxillary sinus region using the sinus lift technique.
MATERIALS AND METHODS: A within-patient controlled clinical trial was performed on 14 patients receiving 78 implants. The implants were assigned to one of two study groups on the basis of implant location. A total of 37 implants were p

BACKGROUND: Implant placement in the posterior maxilla is often complicated by the insufficient bone volume. While transalveolar sinus floor elevation (TSFE) has been proven as a predictable surgical procedure to increase the bone height in the posterior maxilla, questions in regard to the necessity of the bone grafting during the sinus lift and the question of whether TSFE could be performed when the residual bone height is below 5 mm are still debated. Furthermore, high-quality evidence comparing the clinical outcome of transalveolar sinus floor elevation with osteotome and modified sinus fl

PURPOSE: Multiple bone augmentation techniques are available to allow implant placement in the atrophic maxilla. However, questions remain, regarding which methods are most predictable and have the best dental implant survival rate (SR) in grafted bone. The aim of this systematic review was to evaluate literature from the last 30 years to determine predictability of bone grafting of the edentulous maxilla for implant placement as well as for implant SR.
MATERIALS AND METHODS: A systematic review was performed of studies conducted during the period 1980 to 2014, specifically focusing on the ed

A biologically guided, time-efficient analog workflow for anterior implant rehabilitation is presented. Two implants were placed in a horizontally deficient anterior maxilla with simultaneous guided bone regeneration and a de-epithelialized connective tissue graft to restore ridge contour and increase mucosal thickness. After healing, the transmucosal contour was shaped extraorally on a silicone index and transferred intraorally using custom interim restorations with controlled soft-tissue refinement. The emergence profile was captured with a custom impression coping, and multilayer zirconia c

Zygomatic implants (ZIs) have become a predictable graftless treatment option for rehabilitation of the severely atrophic edentulous maxilla. Recent Global Consensus for Clinical Guidelines (GCCG) recommendations have emphasized prosthetically guided treatment planning and advanced surgical expertise due to the anatomical complexity associated with ZI placement. This case report describes the digital planning workflow of a navigation-assisted hybrid rehabilitation approach in a 58-year-old man presenting with severe posterior maxillary atrophy and sufficient anterior residual bone for conventi

INTRODUCTION: Sinonasal anatomical anomalies and inflammatory conditions may reduce success rates of maxillary sinus elevation (MSE) procedures used to allow implant placement in the atrophic posterior maxilla. Approaches combining endoscopic sinus surgery (ESS) and MSE were firstly proposed by our group and have already been described in the literature. This article aims to re-evaluate the procedure in a larger sample of patients comparing results and indications with the pertaining literature.
MATERIALS AND METHODS: Thirty-three patients (19 men and 14 women, mean age 52.79 ± 11.95 years) u

Parry-Romberg syndrome (PRS) is a relatively rare degenerative disorder that is poorly understood. PRS is characterized by slowly progressing atrophy affecting one side of the face, and is frequently associated with localized scleroderma, especially linear scleroderma, which is known as en coup de sabre. This is a report of the author's experiences with PRS accompanying en coup de sabre, and a review of the ongoing considerable debate associated with these two entities. Case 1 was a 37-year-old woman who had right hemifacial atrophy with unilateral en coup de sabre for seven years. Fat graftin
