The Clinical Significance of Streptococcus mitis Bacteremia.

BACKGROUND: Except for their role in bacterial endocarditis, nonpneumococcal Streptococcus mitis group (SMG) bacteria have traditionally been regarded as commensals of the oropharynx. Widespread adoption of matrix-assisted laser desorption/ionization time-of-flight (MALDI-TOF) mass spectrometry has increased identification of these organisms and has led to increased concern about their clinical significance. We evaluated SMG bacteremia in a large multicenter cohort to characterize associated clinical syndromes and better define their clinical relevance. METHODS: We performed a retrospective r

Variability in Hospital-Onset Bacteremia in Ontario.

IMPORTANCE: Hospital-onset bacteremia and fungemia (HOB) is being proposed as a new hospital quality metric for health care-associated infections. There is a lack of data on the ability of HOB to discriminate between hospitals from a large regional perspective. OBJECTIVE: To describe and compare the HOB rates across hospitals for the province of Ontario in Canada. DESIGN, SETTING, AND PARTICIPANTS: This population-based retrospective cohort study used ICES Ontario databases. Participants were adult patients admitted to an acute care hospital from 2017 to 2024 in Ontario, Canada. EXPOSURE: L

Pasteurella multocida Bacteremia in a Young Infant.

Pasteurella multocida is a rare cause of infection in the pediatric age group that can lead to severe systemic disease and is associated with significant morbidity and mortality. This bacterium is found in the normal nasopharyngeal flora of domestic animals. Transmission can occur through direct traumatic contact or indirectly via animal saliva. We present the case of a 30-day-old male infant who attended a neonatology follow-up visit with no parental concerns. On physical examination, the infant showed intermittent periods of whining, ill appearance, reduced vitality, and poor peripheral perf

Management of Staphylococcus aureus Bacteremia: A Review.

IMPORTANCE: Staphylococcus aureus, a gram-positive bacterium, is the leading cause of death from bacteremia worldwide, with a case fatality rate of 15% to 30% and an estimated 300 000 deaths per year. OBSERVATIONS: Staphylococcus aureus bacteremia causes metastatic infection in more than one-third of cases, including endocarditis (≈12%), septic arthritis (7%), vertebral osteomyelitis (≈4%), spinal epidural abscess, psoas abscess, splenic abscess, septic pulmonary emboli, and seeding of implantable medical devices. Patients with S aureus bacteremia commonly present with fever or symptoms from

Treatment of Francisella philomiragia bacteremia in a dog.

To describe the diagnosis and successful treatment of systemic francisellosis in a dog. An 11-year-old female spayed Labrador retriever presented for progressive lethargy, hyporexia, and cough. The dog was febrile with a neutrophilia, nonregenerative anemia, thrombocytopenia, and had increased activity in serum of liver-derived enzymes. Francisella philomiragia was isolated from aerobic blood culture. The dog was treated for 6 weeks with enrofloxacin orally. Repeated aerobic blood cultures after 2 and 6 weeks of antibiotic therapy were negative. The dog was clinically normal 7 months after dia

Tissierella Bacteremia in the Setting of Polymicrobial Osteomyelitis.

We present a case of Tissierella bacteremia in the setting of polymicrobial osteomyelitis. Tissierella is a Gram-variable bacterium that has been rarely documented as the etiologic organism in human infections such as septic arthritis or otitis media, and even more rarely reported as an organism associated with bacteremia. The patient presented with septic shock and the physical exam was notable for gangrene of bilateral feet. Infectious workup was significant for acute osteomyelitis and septic arthritis of his feet in the setting of Tissierella bacteremia, which was successfully treated with

Staphylococcus argenteus bacteremia in the Republic of Korea.

In 2015, Staphylococcus argenteus and Staphylococcus schweitzeri were proposed as new species, distinct from Staphylococcus aureus and collectively referred to as the S. aureus complex. However, no clinical reports of these new species exist in Korea. Upon the application of matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) for all bloodstream isolates since September 2022, S. argenteus was identified in one patient. Therefore, we aimed to search for new species among the archives of the S. aureus bacteremia cohort and describe their clinical and micro

Myroides cellulitis and bacteremia: A case report.

Formerly classified under the genus Flavobacterium, Myroides species are common gram-negative, environmental bacterium ubiquitous in soil and water. While infrequent, infections of human hosts can result in devastating consequences due the bacteria's intrinsic multidrug resistance, particularly in those who are immunocompromised. The pathogenicity and mechanisms for resistance remain poorly understood at this time. The case presented in this report details Myroides bacteremia secondary to a soft tissue infection of the lower extremities and adds to the 60 documented infections to date, of whic

Pasteurella multocida Bacteremia in an Immunocompromised Patient.

We present the case of a 61-year-old Caucasian gentleman who presented with a one-day history of fever, chills, and altered mental status. His symptoms were initially thought to be secondary to cellulitis. Blood cultures grew Pasteurella multocida, a rare pathogen to cause bacteremia. Our patient was treated with ciprofloxacin for two weeks and made a complete and uneventful recovery. Our patient's uncontrolled diabetes mellitus and chronic kidney disease put him at a higher risk for developing serious P. multocida infection. The patient's dog licking the wounds on his legs was considered as t

Bacteremia in hemodialysis patients with hepatitis C.

BACKGROUND: Hepatitis C virus (HCV) infection and bacteremia are common comorbidities in hemodialysis patients. A specific relationship between HCV infection and bacteremia has not been defined; however, there is evidence of immune compromise in both HCV-infected and uremic patients, suggesting that this group may be at higher risk for infection. METHODS: We investigated risk factors and mortality associated with bacteremia in HCV-infected hemodialysis patients from the United States Renal Data System. RESULTS: During the 4-year study period, HCV was present in 2.1% of 355,084 patients initi

Are vital signs indicative for bacteremia in newborns?

OBJECTIVE: Neonatal systemic infection is a leading cause of morbidity and mortality both in industrialized and developing countries. The aim of this prospective study was to evaluate if vital signs had a predictive power in neonatal sepsis as an early marker. METHODS: This study was designed as a matched case-control study. Vital signs were monitorized prior to infection in newborns that had healthcare-associated blood stream infection (BSI). Maximum and minimum values of the vital signs (blood pressure, heart rate, respiratory rate and temperature) of the babies at rest were recorded from t

Shigella bacteremia in a patient with visceral leishmaniasis.

Bacteremia due to Shigella is rare. A 26-year-old HIV-negative male presented with a persistent high-grade fever of two months duration to the Leishmaniasis Research and Treatment Center of University of Gondar Hospital. He was anorexic and had lost significant weight (from 76 to 57 kg in 4 months, BMI = 17.2 kg/m(2)). He also complained of headache, chills, and rigor. In the last one year, he was experiencing a few episodes of acute bloody diarrhea, the last episode being two months ago. Microscopy from splenic aspiration showed Leishman-Donovan bodies with parasite load of +3. The blood cult

Predictors of mortality in Staphylococcus aureus Bacteremia.

Staphylococcus aureus bacteremia (SAB) is an important infection with an incidence rate ranging from 20 to 50 cases/100,000 population per year. Between 10% and 30% of these patients will die from SAB. Comparatively, this accounts for a greater number of deaths than for AIDS, tuberculosis, and viral hepatitis combined. Multiple factors influence outcomes for SAB patients. The most consistent predictor of mortality is age, with older patients being twice as likely to die. Except for the presence of comorbidities, the impacts of other host factors, including gender, ethnicity, socioeconomic stat
