Aortic dimensions in relation to bicuspid and tricuspid aortic valve pathology.

BACKGROUND AND AIM OF THE STUDY: The relationship between aortic valve pathology and the aortic root and ascending aortic dimensions in cardiac surgery patients is unclear, and its clarification was the objective of this study. METHODS: The severity of valve pathology, whether aortic valve stenosis (AS) or aortic valve regurgitation (AR), and the aortic dimensions (aortic root and ascending aorta) were prospectively evaluated with echocardiography in 500 consecutive patients with tricuspid aortic valve (TAV) or bicuspid aortic valve (BAV) who had undergone surgery due to aortic valve and/or a

Genotype-phenotype correlation in patients with bicuspid aortic valve and aneurysm.

OBJECTIVES: Bicuspid aortic valve is the most common congenital cardiac abnormality, occurring in 1% to 2% of the population, and often associates with ascending aortic aneurysm. Based on familial studies, bicuspid aortic valve with aneurysm segregates in an autosomal dominant manner with incomplete penetrance. NOTCH1 mutations have been reported in 6 families with prominent valve calcification and dysfunction and low penetrance of aneurysm. We sought to determine the contribution of NOTCH1 mutations to the more common phenotype of highly penetrant aneurysms with low penetrance of bicuspid aor

A tetrad of bicuspid aortic valve association: A single-stage repair.

We report a 27 years old male who presented with a combination of both congenital and acquired cardiac defects. This syndrome complex includes congenital bicuspid aortic valve, Seller's grade II aortic regurgitation, juxta- subclavian coarctation, stenosis of ostium of left subclavian artery and ruptured sinus of Valsalva aneurysm without any evidence of infective endocarditis. This type of constellation is extremely rare. Neither coarctation of aorta with left subclavian artery stenosis nor the rupture of sinus Valsalva had a favorable pathology for percutaneus intervention. Taking account in

Repair of congenitally dysplastic aortic valve by bicuspidization: midterm results.

BACKGROUND: Repair of congenital aortic valve (CAV) lesions may be achieved by creation of either tricuspid or bicuspid valve morphology. To define feasibility and outcome of CAV repair by bicuspidization a 10-year experience was reviewed. METHODS: Between January 2002 and December 2011, 147 consecutive patients underwent operation for CAV insufficiency; 58 had valve or root repair (group 1) and 89 had valve or root replacement (group 2). Patients having repair were younger (42.9 vs 51.3 years, p=0.001), with lesser prevalence of severe insufficiency (72% vs 90%, p=0.002). In patients having

Predictors of rapid aortic dilatation in adults with a bicuspid aortic valve.

BACKGROUND AND AIM OF THE STUDY: The role of atherosclerosis and atherosclerotic risk factors in predicting progressive aortic dilatation in patients with bicuspid aortic valve (BAV) is not well defined. The study aim was to assess the role of these risk factors in progressive aortic dilatation in patients with this condition. METHODS: Adult patients were identified with BAV who displayed rapid aortic dilatation, and the association of the condition with hemodynamic and atherosclerotic risk factors was assessed. By using the Dartmouth-Hitchcock and Hartford Hospital echocardiographic database

Endocarditis due to Neisseria bacilliformis in a patient with a bicuspid aortic valve.

We report a case of endocarditis due to the rod-shaped Neisseria species Neisseria bacilliformis. The phenotypic characterization of this recently characterized bacteria is difficult, and the identification requires the sequencing of the 16S rRNA gene. The resolution of the disease was complete after appropriate antibiotic therapy, and surgery was not required.

Approach to the patient with bicuspid aortic valve and ascending aorta aneurysm.

Bicuspid aortic valve (BAV) disease is a common congenital heart valve abnormality accounting for a large number of valve replacements in the United States. Although still incompletely understood, the natural history of BAV disease is severe aortic stenosis and associated ascending aortic dilatation. In addition to the increased risk of endocarditis, aortic dissection and severe aortic valve dysfunction are responsible for most fatal complications. Thus, early and precise recognition of this condition is mandatory. The new American College of Cardiology/American Heart Association recommendatio

Frequency of aortic root dilation in children with a bicuspid aortic valve.

Bicuspid aortic valve (BAV) is associated with aortic root dilation and dissection in adults, but the age and conditions when dilation begins are unknown. Using echocardiographic data, we compared the aortic root dimensions and valve hemodynamics of 76 children with BAV with valves derived from 41 normal children. The children with BAV had significantly larger aortic roots (p <0.001) regardless of the presence of aortic stenosis or regurgitation.

Feasibility of the ross procedure: its relationship with the bicuspid aortic valve.

BACKGROUND AND AIM OF THE STUDY: The feasibility of the Ross procedure, and which patients benefit most from its performance, have not yet been fully established. The study aim was to analyze the relationship between the etiology of aortic valve disease, the feasibility of performing the Ross procedure, and late pulmonary autograft performance. METHODS: Between June 1995 and June 2001, 117 patients (77 males, 40 females; mean age 37+/-12 years) underwent the Ross procedure at the authors' institution. Of these patients, 53 (45.3%) had severe aortic stenosis, 53 (45.3%) had significant aortic

[A case of aortic dissection associated with congenital bicuspid aortic valve].

It is known that bicuspid aortic valve with dilatation of the ascending aorta is one of risk factors of the aortic dissection. A case of acute aortic dissection (DeBakey type-II) associated with bicuspid aortic valve who underwent successfully operation 8 hours after onset is reported. This patient went into cardiogenic shock because of cardiac tamponade and aortic valve regurgitation immediately after onset. Aortic valve and the ascending aorta were replaced using composite graft (#23 SJM prosthetic aortic valve and 26 mm woven Dacron vascular graft) combined with coronary artery reconstructi

Bicuspid aortic valve in an adult human cadaver: a case report.

We encountered a "bicuspid" aortic valve in an 83-year-old man. This valve consisted of two semilunar leaflets, anterior and posterior, though they were extensively calcified. The right and left coronary arteries independently arose from the anterior aortic sinus. The infundibular ligament ran from the pulmonary trunk at the commissure of the right and left semilunar cusps to the aorta at the mid-point of the anterior sinus. These findings strongly suggests the present bicuspid aortic valve may be formed by fusion of usual right and left (coronary) cusps. No other congenital anomalies were not

Bicuspid homograft reconstruction of the right ventricular outflow tract in infants.

BACKGROUND: The use of homograft tissue in cardiac surgery has progressively increased so that supply is limited, especially in the sizes required for neonatal and infant cardiac operations. METHODS: We developed in recent years a technique for surgically reducing the size of more readily available large-diameter homografts, making them suitable for right ventricular outflow tract reconstruction in small infants. This is achieved by means of a longitudinal incision of the oversized conduit, excision of one homograft cusp, and oversewing of the "bicuspid" homograft wrapped around a dilator of

Coronary artery anomalies and bicuspid aortic valves in the Syrian hamster.

The condition of coronary arteries and aortic valves was studied in 552 Syrian hamsters belonging to a single family subjected to high endogamous pressure. The study was carried out using a corrosion-cast technique. In 178 hamsters the aortic valve was bicuspid. In 138 specimens, 54 of them with normal aortic valves and 84 with bicuspid aortic valves, anomalies in the origin of the coronary arteries could be classified in three morphologic types: left coronary artery from the pulmonary trunk (36 cases); single right coronary artery (84 cases); left coronary artery from the dorsal aortic sinus

Bicuspid aortic valve morphology when associated with coarctation of the aorta.

Thirteen children and young adults with coarctation of the aorta as their principal cardiovascular abnormality, 11 with bicuspid aortic valves, were evaluated by orifice-view aortography to evaluate their aortic valvular morphology. For comparison 30 individuals with aortic valvular deformities but without coarctation of the aorta were similarly studied. Two distinct forms of bicuspid valves could be identified characterized by either the appearance of gross inequality of size of the two valve leaflets or an appearance wherein each leaflet closely approximated the size of the other, thus equal

Extreme distal migration of the mandibular second bicuspid. A variant of eruption.

A five-year study of 26,264 panoramic radiographs found distal positioning of the mandibular second bicuspid associated with a missing adjoining first molar in one of every 505 patients. Following early removal of the first molar, there is a 5% to 10% chance that the second bicuspid will migrate distally. It usually continues until it contacts the root of the second molar, and then rotates up into occlusion adjacent to the parallel with that tooth. Mesial drift seldom occurs in this circumstance; the resulting space between lower first and second bicuspids remains. Early conservative managemen

Differentiating Cardiac (Mal)adaptation in 2 Athletes With Bicuspid Aortic Valves.

BACKGROUND: Distinguishing left ventricular (LV) remodeling due to early decompensation from chronic aortic regurgitation (AR) from physiologic adaptation to intensive endurance training is challenging. CASE SUMMARY: Two elite endurance athletes (and best friends) presented with bicuspid aortic valves complicated by moderate AR and marked LV remodeling. Despite near-identical valvular findings, LV volumes, and exceptional cardiorespiratory fitness at baseline, their clinical courses diverged. One developed exertional symptoms and underwent a Ross procedure, followed by rapid and profound reve

Haemodynamic Early Outcomes of Sinus Plication for Bicuspid Aortic Valve Repair.

OBJECTIVES: Postoperative stenosis negatively affects durability after bicuspid aortic valve (BAV) valvuloplasty. This study was performed to evaluate the haemodynamic efficacy of sinus plication (SP) to avoid stenosis after asymmetrical BAV repair. METHODS: Between 2018 and 2023, 41 patients with BAV (33.3 ± 11.5 years, 40 males) with moderate-severe aortic regurgitation (AR) underwent aortic valve repair with (SP group, n = 14) or without (NSP group, n = 27) SP. SP was performed to reduce the circumference of the fused sinus and restore sinus contour symmetry. Preoperative and postoperative
