Exercise Thermoregulation with a Simulated Burn Injury: Impact of Air Temperature.

UNLABELLED: The U.S. Army's Standards of Medical Fitness (AR 40-501) states: "Prior burn injury (to include donor sites) involving a total body surface area of 40% or more does not meet the standard." However, the standard does not account for the interactive effect of burn injury size and air temperature on exercise thermoregulation. PURPOSE: To evaluate whether the detrimental effect of a simulated burn injury on exercise thermoregulation is dependent on air temperature. METHODS: On eight occasions, nine males cycled for 60 min at a fixed metabolic heat production (6 W·kg) in air temperatu

Nongrafted Skin Area Best Predicts Exercise Core Temperature Responses in Burned Humans.

UNLABELLED: Grafted skin impairs heat dissipation, but it is unknown to what extent this affects body temperature during exercise in the heat. PURPOSE: We examined core body temperature responses during exercise in the heat in a group of individuals with a large range of grafts covering their body surface area (BSA; 0%-75%). METHODS: Forty-three individuals (19 females) were stratified into groups based on BSA grafted: control (0% grafted, n = 9), 17%-40% (n = 19), and >40% (n = 15). Subjects exercised at a fixed rate of metabolic heat production (339 ± 70 W; 4.3 ± 0.8 W·kg) in an environmen

Ontogeny of integumental calcium in relation to surface area and skin water content in the perinatal rat.

Calcium is an important regulator of epidermal differentiation and skin biomechanics in many vertebrate species. In this study, we measured total epidermal calcium in the perinatal Sprague-Dawley rat. Values ranged from 12 to 15 mg per 100 g of tissue. These levels were elevated compared with dermis and other soft (nonbone) organs, including brain, kidney, heart, and liver. Administration of radioactive calcium to the pregnant rat resulted in high rates of 45Ca2+ localization in the fetal epidermis 24 h later. From gestational day 20 to postnatal day 3, the epidermis showed progressive dehydra

The depth to the epidural space.

Two hundred and ten patients, 145 males and 65 females, aged 20 to 72 years old, ready to receive extracorporeal shock wave lithotripsy under epidural anesthesia were studied to evaluate if any correlation existed between body weight, body surface area, body mass index and body height and the depth to the epidural space. The mean value for the depth to the epidural space was 4.77 +/- 0.55 cm for males, 4.25 +/- 0.55 cm for females and the overall average was 4.61 +/- 0.60 cm. The range of the depth to the epidural space was 3.0-7.0 cm. Linear regression analysis revealed significant correlatio

Hyperpyrexia in major burn injury: describing the epidemiology and evaluating the effectiveness of an intravascular temperature management device.

INTRODUCTION: Hyperpyrexia is a complication of burn injury, likely caused by systemic inflammation and infection, the pathophysiology of which is poorly understood. This study aimed to describe the patient cohort who developed hyperpyrexia (≥39.5°C) and evaluate the effectiveness of the Thermogard XP Icy®, an intravascular temperature management device, in restoring normothermia. METHODS: This was a retrospective cohort study of adult patients admitted to Glasgow Royal Infirmary's intensive care unit with major burns (TBSA ≥20%) between January 2016 and October 2024. A central database and e

Nocturnal urinary protein excretion rates in patients with sleep apnea.

We observed nocturnal urinary protein excretion to be 16.2 +/- 5.5 micrograms/min (mean +/- SE) in 9 healthy control subjects (group I), 29.3 +/- 9.5 micrograms/min in 12 obese patients suspected to have obstructive sleep apnea syndrome (OSAS) but with negative polysomnographic studies (group II), and 94.0 +/- 31.8 micrograms/min in 14 patients with documented OSAS (group III) (II vs. I, NS; III vs. I, p less than 0.05; III vs. II, p less than 0.05). The frequency of abnormal proteinuria, defined as protein excretion greater than the highest rate observed in group I (46 micrograms/min), was 14

Carry That Weight! The Challenge of Managing Weight Changes During Inpatient Admission for Patients With Burn Injuries ≥20% TBSA.

The hypermetabolic state of patients with ≥20% total body surface area (TBSA) causes loss of muscle mass and compromised immune function with delayed wound healing. Weight loss is most severe in patients with ≥20% TBSA with initial weight gain due to fluid resuscitation. The American Burn Association (ABA) proposed quality measures for burn injury admissions, including weight loss from admission to discharge. We assessed how our outcomes adhere to these measures and if they correlate with previously described results. We retrospectively reviewed adult admissions with ≥20% TBSA burn injuries fr

The effect of COVID-19 on burn management and outcomes in a resource-limited setting.

INTRODUCTION: Optimal burn care includes fluid resuscitation and early excision and grafting. During the COVID-19 pandemic, resource-constrained environments were susceptible to interruptions in burn care. We sought to characterize pre- and intra-pandemic burn-associated outcomes at a busy tertiary hospital in Malawi. METHODS: This is a retrospective analysis of burn patients that presented to Kamuzu Central Hospital Lilongwe from 2011 through December 2021. We compared patients based on whether they presented pre- or intra-pandemic, starting on March 11, 2020, the date of official WHO design

Acute Management of Deep Facial Burns.

INTRODUCTION: Management of deep facial burns is a serious challenge for many reasons: a considerable anatomic and functional diversity is concentrated in a small space, a uniform treatment does not exist, late sequelae are frequent and may be severe, and the literature on the subject is ambiguous. AIM: To analyse management of deep facial burns. PATIENTS AND METHODS: A retrospective medical chart review was conducted for 569 patients with deep facial burns hospitalized between January 2005 and January 2015. Demographic data, type, depth and size of burns, chronology and type of surgical tre

A pragmatic approach to assess the exposure of the honey bee (Apis mellifera) when subjected to pesticide spray.

Plant protection spray treatments may expose non-target organisms to pesticides. In the pesticide registration procedure, the honey bee represents one of the non-target model species for which the risk posed by pesticides must be assessed on the basis of the hazard quotient (HQ). The HQ is defined as the ratio between environmental exposure and toxicity. For the honey bee, the HQ calculation is not consistent because it corresponds to the ratio between the pesticide field rate (in mass of pesticide/ha) and LD50 (in mass of pesticide/bee). Thus, in contrast to all other species, the HQ can only

How big is your hand and should you use it to score skin in cutaneous T-cell lymphoma?

Assessing burn surface area is essential to triage patients and determine fluid resuscitation. The three commonly used methods are: (i) the palmar method, where palmar surface area is assumed to equal 1% of body surface area (BSA); (ii) the rule of nines; and (iii) the Lund and Browder method, where body parts are assigned a percentage surface area. Calculation of BSA involvement in dermatological disease is used to measure disease severity and treatment responses. The grid-point counting method may also be used. Mycosis fungoides (MF), a form of cutaneous T-cell lymphoma, typically presents w

Excessive iodine intake and ultrasonographic thyroid abnormalities in schoolchildren.

High nutritional levels of iodine may induce a higher prevalence of autoimmune thyroiditis, hypothyroidism, goiter, as well as hyperthyroidism, mostly in the elderly. This study assessed thyroid volume and ultrasonographic abnormalities as well as urinary iodine excretion (UIE) in 964 schoolchildren living in an iodine-sufficient area in southern Brazil. Thyroid volume correlated with age and body surface area in boys and girls. In 76.8% of the children, UIE was above 300 microg/l, with higher levels among boys compared to girls (484.2 microg/l vs 435.3 microg/l, p < 0.001). Thyroid abnormalit

Correlation of quantitative burn wound biopsy culture and surface swab culture to burn wound sepsis.

The present study includes seventeen patients with second and third degree fresh burns involving 15-50 per cent total body surface area (TBSA). Surface swabs and quantitative burn wound biopsy cultures were obtained during postburn weeks 1, 2 and 3 and correlation was studied. To obtain bacterial counts the technique described by Loebel et al. (1974) was used. The patients were divided in two groups depending upon burn body surface area involved. The first group includes five patients with burns between 15-29 per cent body surface area and the second group includes the rest of the twelve patie

Body weight divided by squared knee height as an alternative to body mass index.

Weight/height(2) (Quetelet's index) is the basis for defining both underweight and obesity. Height, however, is often not precisely measurable in the elderly due to involutional changes such as spinal deformity. Body volume or body surface area are not proportionately decreased even with height loss. Previous reports have shown that Quetelet's index is overestimated in the elderly with height loss. Then we have made a hypothesis described below. Maximal height or height at youth would better represent the subjects' nutritional or clinical status. The distinction of these two heights has not be
