Vector mapping in localizing the transverse conduction site of the crista terminalis in patients with typical atrial flutter.

BACKGROUND: The difference in the conduction properties of the crista terminalis (CT) along its course, has not been fully clarified. Using the vector mapping method, we localized the transverse conduction (TC) site of the CT and elucidated its conduction capabilities in patients with typical atrial flutter (AF). METHODS: The TC site of the CT was localized by the analysis of the polarity reversal of the double potentials recorded at 10 sites along the CT using a 20-pole deflectable catheter in 17 patients. The conduction capabilities of the TC site were analyzed during incremental pacing del

Characterization of left ventricular activation in patients with heart failure and left bundle-branch block.

BACKGROUND: Conventional activation mapping in the dilated human left ventricle (LV) with left bundle-branch block (LBBB) morphology is incomplete given the limited number of recording sites that may be collected in a reasonable time and given the lack of precision in marking specific anatomic locations. METHODS AND RESULTS: We studied LV activation sequences in 24 patients with heart failure and LBBB QRS morphology with simultaneous application of 3D contact and noncontact mapping during intrinsic rhythm and asynchronous pacing. Approximately one third of the patients with typical LBBB QRS m

Usefulness of body surface mapping to differentiate patients with Brugada syndrome from patients with asymptomatic Brugada syndrome.

We attempted to determine the usefulness of body surface mapping (BSM) for differentiating patients with Brugada syndrome (BS) from patients with asymptomatic Brugada syndrome (ABS). Electrocardiograms (ECG) and BSM were recorded in 7 patients with BS and 35 patients with ABS. Following the administration of Ic antiarrhythmic drugs, BSM was recorded in 5 patients with BS and 16 patients with ABS. The maximum amplitudes at J0, J20, J40 and J60 were compared between the 2 groups, as were 3-dimensional maps. The maximum amplitudes at J0, J20 and J60 under control conditions were larger in patient

[Variation of body surface QRST isointegral map in patients with idiopathic long QT syndrome].

Body surface QRST isointegral maps in patients with long QT syndrome (LQTS) often show multipolar distribution. We recorded body surface maps in eight cases with LQTS. There were same day variations in the distribution of multipolarity and unipolarity in two of seven cases, and day-to-day variation in one of seven cases. In two of four cases, the distribution of the map changed from unipolarity to multipolarity following hand-pushing exercise. When two patients with same day variation or day-to-day variation were administered beta-adrenergic blocking agents, their QRST isointegral maps all sho

[Body surface isopotential T map and ventricular volume characteristics in secundum atrial septal defect].

Body surface isopotential T map was analyzed to detect right ventricular volume and pressure overloads in 25 patients with secundum atrial septal defect. Three patterns were distinguished: the T map resembling normal (type A, n = 9); that with isolated negative area in positive area (type B, n = 11); and that with rightward movement of maximum (type C, n = 5). Right ventricular end-diastolic volumes in type B (161 +/- 19% of normal; %N) and C (175 +/- 40% N) were significantly (p < 0.01) greater than those in control (100 +/- 9% N) and type A (113 +/- 18% N). ST-T isointegral analysis of exercise stress body surface mapping for identifying ischemic areas in patients with angina pectoris.

ST-T isointegral analysis of body surface mapping was used in an attempt to localize ischemic areas on exercise tests. In 28 patients with angina pectoris and 10 healthy subjects, body surface potential was recorded with 87 leads, and ST isopotential and ST-T isointegral maps were constructed. In all 10 healthy subjects, the basic pattern of the ST-T isointegral map showed no significant change after exercise. In 23 of 28 patients with angina pectoris (82%), alterations in the ST-T isointegral map after exercise were observed. They were divided into four types (anterior, inferoposterior, later

Body surface isopotential mapping of the entire QRST complex in the Wolff-Parkinson-White syndrome. Correlation with the location of the accessory pathway.

Body surface potential maps were recorded during sinus rhythm and during atrial pacing at the time of electrophysiologic studies in 42 patients with Wolff-Parkinson-White syndrome. The locations of the accessory pathways were determined by epicardial mapping during surgery in 34 patients and by multicatheter endocavitary electrophysiologic studies in eight additional patients. During delta wave inscription, the shape and extension of areas of the negative and positive potentials on the thorax correlated better with the preexcitation site (69% of patients) than with the localization of the mini

Luminal esophageal temperature rise and esophageal lesion formation following remote-controlled magnetic pulmonary vein isolation.

BACKGROUND: Remote magnetic pulmonary vein isolation (PVI) demonstrates comparable success rate to manual ablation, but thermal effects on esophageal tissue remain undefined. OBJECTIVE: The purpose of this study was to evaluate the potential thermal effects on esophageal tissue during remote magnetic wide-area circumferential pulmonary vein isolation (CPVI). METHODS: Twenty-five patients (23 men, age 57 ± 10 years) in the magnetic navigation system (MNS) group and 25 control subjects (15 men, age 61 ± 12 years) with drug-refractory paroxysmal or persistent atrial fibrillation underwent CPVI.

Catheter configuration for mapping micro-anatomic reentries sustaining atrial fibrillation: A simulation study.

Atrial fibrillation (AF) can be sustained by intramural reentrant circuits within three-dimensional arrhythmogenic hubs formed by fibrotically-insulated myobundles. However, the efficacy of different multi-electrode mapping (MEM) to identify the micro-reentrant pathways sustaining AF remains undefined. An anisotropic atrial tissue structure (30 × 30 × 4 mm), incorporating a sub-endocardial laterally-insulated myobundle (15 × 2.5 × 1.5 mm) was simulated reflecting persistent AF conditions. Simulations included endocardial unipolar, bipolar, and omnipolar electrograms, with local activation time

Evaluation of noninvasive isochronal late activation mapping in scar-related ventricular tachycardia with electrocardiographic imaging against contact mapping.

BACKGROUND: Deceleration zones (DZs) represent important ablation targets in scar-related ventricular tachycardias (VTs). Novel electrocardiographic imaging (ECGI) techniques could identify DZs instantly and noninvasively. OBJECTIVE: We aimed to evaluate a novel ECGI last deflection detection algorithm for noninvasive isochronal late activation substrate mapping in scar-related VT procedures and compare against electroanatomic mapping (EAM) as gold-standard. METHODS: Prospectively recruited scar-related VT ablation patients underwent contact and ECGI mapping. Sinus rhythm or right ventricula

Omnipolar versus bipolar mapping to guide ventricular tachycardia ablation.

BACKGROUND: Omnipolar technology (OT) was recently proposed to generate electroanatomic voltage maps with orientation-independent electrograms. We describe the first cohort of patients undergoing ventricular tachycardia (VT) ablation guided by OT. OBJECTIVE: The purpose of this study was to compare omnipolar and bipolar high-density maps with regard to voltage amplitude, late potential (LP) annotation, and isochronal late activation mapping distribution. METHODS: A total of 24 patients (16 [66%] ischemic cardiomyopathy and 12 [50%] redo cases) underwent VT ablation under OT guidance. Twenty-

Modeling the His-Purkinje Effect in Non-invasive Estimation of Endocardial and Epicardial Ventricular Activation.

Inverse electrocardiography (iECG) estimates epi- and endocardial electrical activity from body surface potentials maps (BSPM). In individuals at risk for cardiomyopathy, non-invasive estimation of normal ventricular activation may provide valuable information to aid risk stratification to prevent sudden cardiac death. However, multiple simultaneous activation wavefronts initiated by the His-Purkinje system, severely complicate iECG. To improve the estimation of normal ventricular activation, the iECG method should accurately mimic the effect of the His-Purkinje system, which is not taken into

Cavotricuspid isthmus is constantly a zone of slow conduction: Data from ultra-high-resolution mapping.

BACKGROUND: Whether cavotricuspid isthmus (CTI) is a region of conduction slowing during typical flutter has been discussed with conflicting results in the literature. We aimed to evaluate conduction velocity (CV) along the different portions of the typical flutter circuit with a recently proposed method by means of ultra-high-resolution (UHR) mapping. METHODS: Consecutive patients referred for typical atrial flutter (AFL) ablation underwent UHR mapping (Rhythmia, Boston Scientific). CVs were measured in the CTI as well as laterally and septally, respectively, from its lateral and septal bord

Comparison of the arrhythmogenic substrate between men and women with nonischemic cardiomyopathy.

BACKGROUND: Outcomes of ventricular tachycardia (VT) ablation in structural heart disease have been reported to differ by sex. Whether this is due to differences in the underlying arrhythmogenic substrates among patients with nonischemic cardiomyopathy (NICM) remains unclear. OBJECTIVE: The purpose of this study was to compare the characteristics of arrhythmogenic substrates between women and men with NICM. METHODS: We analyzed 160 consecutive patients (26 women) with NICM who were undergoing VT ablation at the Hospital of the University of Pennsylvania. Of these 160 patients, 59 (13 women)
