Possible systolic fascicular potentials in patients with left bundle branch block undergoing left bundle branch area pacing: A case series.

INTRODUCTION: In left bundle branch area pacing (LBBAP), several methods allow determination of lead depth during active fixation inside the septum: among these, visualization of a Purkinje potential indicates that the subendocardial area has been reached. In LBB block (LBBB) patients, fascicular potentials are visible as presystolic only in rare conditions. METHODS AND RESULTS: Since October 2022 until August 2023, LBBAP was attempted in 21 patients with LBBB at our Center: among the 18 consecutive patients (86%) in which it was successful, focusing on the terminal part of the unipolar ventr

Predictors of functional mitral regurgitation improvement in patients with left bundle branch block treated with left bundle branch area pacing.

BACKGROUND: Functional mitral regurgitation (FMR) and its severity are associated with adverse outcomes in heart failure patients. This study aims to analyze the predictors of FMR improvement after successful left bundle branch area pacing (LBBAP) in patients with LVEF < 50% and complete left bundle branch block (CLBBB). METHODS: Consecutive patients with LVEF < 50% and CLBBB who underwent successful LBBAP from July 2018 to July 2023 were retrospectively identified. Significant MR was defined as regurgitation of moderate severity or greater. Patients with significant FMR were included in the

Risk of right bundle-branch block and complete heart block during pulmonary artery catheterization.

The need for the prophylactic insertion of a pacemaker before pulmonary artery catheterization in patients with pre-existing left bundle-branch block (LBBB) is controversial. To determine the incidence of new right bundle-branch block (RBBB) and complete heart block during bedside pulmonary artery catheterization, 293 patients undergoing 307 pulmonary artery catheterizations were prospectively studied. Nine patients had pacemaker rhythms and 19 patients had an RBBB on their precatheterization ECGs and therefore were excluded from analysis. In the remaining 279 pulmonary artery catheterizations

Tachycardia-dependent second degree AV-block in a patient with right bundle branch block.

A 55-year old male patient, with dizzy spells during everyday activity and a complete right bundle branch block as the sole electrocardiographic abnormality, reproducibly demonstrated tachycardia-dependent Mobitz Type II- and 2:1 second degree atrioventricular block. An electrophysiologic study revealed a provocable block within the distal portion of the bundle of His without evidence of a split His potential. Because of the truly tachycardia-dependent AV-block, beta-blocker medication was initiated to prevent high sinus rates during everyday activity. This therapy abolished symptoms totally.

Heart's Hidden Signal: A Rare Case of Alternating Bundle Branch Block and Atrioventricular Block.

INTRODUCTION: Alternating bundle branch block (ABBB) is a rare cardiac rhythm disorder characterized by alternating occurrence of right BBB and left BBB patterns in consecutive beats. The presence of ABBB in conjunction with atrioventricular block is even more rare. CASE PRESENTATION: A 69-year-old man with no significant medical history presented to the emergency department with dyspnea on exertion, lightheadedness, and dizziness for 3 weeks. His heart rate was 31 beats/min, blood pressure was 147/67 mm Hg, and Spo2 was 96% on room air. An electrocardiogram obtained in the emergency departme

Clinical significance of the pattern of incomplete left bundle branch block with bifascicular block.

A comparative analysis was carried out of 126 patients (group I) with chronic right bundle branch block (RBBB) and left anterior (LAFB) or left posterior fascicle block (LPFB), and of 44 patients (group II) with the same bifascicular block associated with an ECG pattern of incomplete left bundle branch block (ILBBB). The two groups were found to be clinically different. In group II, heart failure, arrhythmia and first-degree atrioventricular block occurred significantly more frequently. During a mean follow-up period of 1581 +/- 118 days (85-6260 days), complete heart block (CHB) developed in

Transient right bundle branch block unmasking anteroseptal infarction concealed by left posterior fascicular block.

A case of acute anteroseptal myocardial infarction (MI) associated with transient right bundle branch block (RBBB) and a probably persistent left posterior fascicular block is reported. When the RBBB disappeared, small initial r waves were present in the right precordial leads concealing the evidence of MI. Finally, the electrocardiographic pattern evolved to an atypical form of incomplete left bundle branch block. An hypothetic role of the middle septal fascicle in this electrocardiographic evolution is discussed.

A Simulation Study of the Effects of His Bundle Pacing in Left Bundle Branch Block.

His bundle pacing (HBP) has emerged as a feasible alternative to right (RVP) and biventricular pacing (BVP) for Cardiac Resynchronization Therapy (CRT). This study sought to assess, in ex-vivo experimental models, the optimal setup for HBP in terms of electrode placement and pacing protocol to achieve superior electrical synchrony in the case of complete His-Purkinje block and left bundle branch block (LBBB). We developed a 3D model of His bundle and bundle branches, embedded in a patient-specific biventricular heart model reconstructed from CT images. A monodomain reaction-diffusion model was

Relation of New Permanent Right or Left Bundle Branch Block on Short- and Long-Term Mortality in Acute Myocardial Infarction Bundle Branch Block and Myocardial Infarction.

The aim of this study was to investigate the prognosis associated with bundle branch block (BBB) depending on location, time of appearance, and duration in patients with myocardial infarction (MI). From January 1998 to January 2008, we recruited 5,570 patients with acute MI. Thirty-day and 7-year all-cause mortality, according to BBB location, time of appearance, and duration were analyzed by multivariable analyses. BBB was present in 964 patients (17.3%); right BBB (RBBB) 10.6% and left BBB (LBBB) 6.7%. Overall mortality rate at 30 days was 13.2% (n = 738) and 7 years was 6.34 deaths per 100

Tailoring cardiac resynchronisation therapy to non-left bundle branch block: Successful cardiac resynchronisation for right bundle branch block with left posterior fascicular block without implantation of a left ventricular lead.

Despite advances, cardiac resynchronisation therapy (CRT) remains fundamentally orientated to the dyssynchrony of left bundle branch block (LBBB), in which septo-lateral electrical and mechanical delays predominate. For non-LBBB patients response rates to conventional CRT are lower and mortality and rehospitalisation rates are not reduced. Despite this, alternative approaches which tailor CRT to the differing dyssynchrony patterns of non-LBBB have yet to be developed. In the specific non-LBBB subgroup of right bundle branch block (RBBB) with left posterior fascicular block (LPFB), ventricular

Bicarbonate refractory QRS prolongation and left bundle-branch block following escitalopram and lamotrigine overdose: A case report and literature review of toxic left bundle-branch block.

WHAT IS KNOWN AND OBJECTIVE: Toxic prolongation of the QRS interval most often results from blockade of cardiac voltage-gated sodium channels and manifests on electrocardiogram with a right bundle-branch block-like morphology. Rarely, a left bundle-branch block (LBBB) morphology has been reported. CASE REPORT: We report a case of transient LBBB resultant from ingestion of lamotrigine and citalopram which was refractory to sodium bicarbonate therapy and eventually resolved spontaneously. WHAT IS NEW AND CONCLUSION: Cases of toxic LBBB are less likely to respond to bicarbonate therapy, suggest

The surface electrocardiogram predicts risk of heart block during right heart catheterization in patients with preexisting left bundle branch block: implications for the definition of complete left bundle branch block.

BACKGROUND: Patients with left bundle branch block (LBBB) undergoing right heart catheterization can develop complete heart block (CHB) or right bundle branch block (RBBB) in response to right bundle branch (RBB) trauma. We hypothesized that LBBB patients with an initial r wave (>or=1 mm) in lead V1 have intact left to right ventricular septal (VS) activation suggesting persistent conduction over the left bundle branch. Trauma to the RBB should result in RBBB pattern rather than CHB in such patients. METHODS: Between January 2002 and February 2007, we prospectively evaluated 27 consecutive pa

Permanent Bi-Bundle Pacing in a Patient With Heart Failure and Left Bundle Branch Block.

Left bundle branch pacing (LBBP) is effective in patients with heart failure, left ventricular ejection fraction (LVEF) of ≤35%, and a widened QRS complex. LBBP leads to iatrogenic incomplete right bundle branch block (iRBBB). Bi-bundle pacing can resolve iRBBB, further narrowing the QRS duration, and may improve LVEF. (Level of Difficulty: Advanced.).
