Electric injury, Part II: Specific injuries.

Electric injury can cause disruption of cardiac rhythm and breathing, burns, fractures, dislocations, rhabdomyolysis, eye and ear injury, oral and gastrointestinal injury, vascular damage, disseminated intravascular coagulation, peripheral and spinal cord injury, and Reflex Sympathetic Dystrophy. Secondary trauma from falls, fires, flying debris, and inhalation injury can complicate the clinical picture. Diagnostic and treatment considerations for electric injuries are described in this article, which is the second part of a three-part series on electric injuries.

Epidemiology and prognosis of burn injuries in China: a meta-analysis.

BACKGROUND: Burns are a significant cause of disability and death. Nonfatal burns have drastic impacts on survivors' lives. This study examines the epidemiological characteristics and prognostic factors of burn injuries of hospitalized patients in China. METHODS: Literature search was conducted in online databases and study selection was based on precise eligibility criteria. Meta-analyses of proportions were performed to estimate the relative incidence rates of burn injuries between 1980 and 2022 by age, total body surface area (TBSA), cause, and season. Odds ratios (ORs) were pooled and met

Predicting the proportion of full-thickness involvement for any given burn size based on burn resuscitation volumes.

INTRODUCTION: The depth of burn has been an important factor often overlooked when estimating the total resuscitation fluid needed for early burn care. The goal of this study was to determine the degree to which full-thickness (FT) involvement affected overall 24-hour burn resuscitation volumes. METHODS: We performed a retrospective review of patients admitted to our burn intensive care unit from December 2007 to April 2013, with significant burns that required resuscitation using our computerized decision support system for burn fluid resuscitation. We defined the degree of FT involvement as

High frequency percussive ventilation in pediatric patients with inhalation injury.

The objective of this study was to present data that showed high frequency percussive ventilation (HFPV) was superior to traditional mechanical ventilation for the treatment of children with inhalation injuries. Inhalation injuries continue to be the number one cause of death of patients with thermal injuries in the United States. Therapy for this condition has consisted of conservative pulmonary toilet and mechanical ventilation. Despite improvements in the management of burn injury, patients with inhalation injury develop pneumonia and pneumothorax, leading to adult respiratory distress synd

[Sulfur dioxide-induced bronchial constriction in asthmatics and its prophylaxis by salbutamol].

Patients with bronchial asthma often develop acute attack in kitchen while burning honeycomb briquet which is widely used for cooking in southern China. Burning of a honeycomb briquet produces 0.6-4.3 ppm sulphur dioxide (SO2) in the kitchen air. To assess whether the above concentration of SO2 can induce bronchial constriction, we performed SO2 bronchial provocation test on 21 asthmatics (male 11, female 10, average age 28.8 +/- 2.0 years), 10 patients with allergic rhinitis (male 3, female 7.28 +/- 1.6 years) and 20 healthy non-smokers (male 13, female 7, 28.1 +/- 1.5 years). FEV1 showed no

Outcomes in Ventilated Burn Patients With Acute Respiratory Distress Syndrome: An Evaluation of Early High-PEEP Strategy Using Berlin Criteria.

Acute respiratory distress syndrome (ARDS) remains a formidable sequela, complication, and mortality risk in patients with large burns with or without inhalation injury. Alveolar recruitment using higher Positive end expiratory pressures (PEEP) after the onset of ARDS has been tried with varying success. Studies have identified benefits for several rescue maneuvers in ARDS patients with refractory hypoxemia. A prophylactic strategy utilizing an early recruitment maneuver, however, has not, to our knowledge, been explored in ventilated burn patients. This study was designed to evaluate the natu

Adjunctive hyperbaric oxygen therapy in the treatment of thermal burns: a review.

Hyperbaric oxygen therapy provided in specially designed pressure chambers is currently the treatment of choice for decompression sickness, arterial gas embolism, and serious carbon monoxide poisoning. It is an important adjunct in the treatment of gas gangrene, chronic osteomyelitis, radiation injury, and indolent wounds. The potential benefit in the treatment of thermal burns is not well appreciated. Hyperbaric oxygen therapy may significantly reduce morbidity, mortality, and cost of care. The application of this technology based on the demonstrated beneficial effects on the pathophysiology

Association of early qualitative myoglobinuria with in-hospital mortality and time to hospital discharge in burn patients: A 5-year retrospective cohort study.

BACKGROUND: Early urinary myoglobin may indicate extensive tissue injury after burns, but its prognostic value remains uncertain. We evaluated its association with in-hospital mortality and time to hospital discharge. METHODS: This single-centre retrospective cohort included 721 burn patients with an available urinary myoglobin result among 1005 potentially eligible admissions from 2020 to 2024. Myoglobinuria was assessed during the initial hospital evaluation using a qualitative ammonium sulphate precipitation test. Multivariable logistic regression evaluated in-hospital mortality. Early protein C activation is reflective of burn injury severity and plays a critical role in inflammatory burden and patient outcomes.

BACKGROUND: Navigating the complexities of a severe burn injury is a challenging endeavour where the natural course of some patients can be difficult to predict. Straddling both the coagulation and inflammatory cascades that feature strongly in the burns systemic pathophysiology, we propose the pleiotropic protein C (PC) system may produce a viable biomarker to assist traditional evaluation methods for diagnostic and prognostic purposes. METHODS: We enrolled 86 patients in a prospective observational cohort study. Over three weeks, serial blood samples were taken and measured for PC, activate

Invasive Fungal Infection with Absidia Corymbifera in Immunocompetent Patient with Electrical Scalp Burn.

Invasive fungal infection in burn injury is caused by inoculation of fungal spore from patient skin, respiratory tract or from care giver. The risk factors for acquiring fungal infection in burns include age of burns, total burn size, full thickness burns, inhalational injury, prolonged hospital stay, late surgical excision, open dressing, central venous catheters, antibiotics, steroid treatment, long-term artificial ventilation, fungal wound colonization, hyperglycemic episodes and other immunosuppressive disorders. Invasive fungal infection with Absidia corymbifera is rare opportunistic infe

Using the injury severity score to adjust for comorbid trauma may be double counting burns: implications for burn research.

BACKGROUND: The injury severity score considers burn size and inhalation injury in estimating overall anatomical injury severity. Models that adjust for injury severity score in addition to total burn size and inhalation injury may therefore be double counting the risk from these individual burn characteristics, and obscuring (or overemphasizing) the contribution of risk from each source. The primary aim of this study was to compare differences in the estimated mortality risk of burn trauma using the traditional injury severity score (ISS) calculation and the non-burn injury severity score (NB

Temperature Derangement on Admission is Associated With Mortality in Burn Patients-A Nationwide Analysis and Opportunity for Improvement.

Although single-institution studies have described the relationship between hypothermia, burn severity, and complications, there are no national estimates on how temperature on admission impacts hospital mortality. This study aims to evaluate the relationship between admission temperature and complications on a national scale to expose opportunities for improved outcomes. The US National Trauma Data Bank (NTDB) was analyzed between 2007 and 2018. Mortality was modeled using multivariable logistic regression including burn severity variables (% total burn surface area (TBSA), inhalation injury,

A novel scale for predicting delayed intubation in patients with inhalation injury.

BACKGROUND: Strategies to predict delayed airway obstruction in patients with inhalation injury have not been extensively studied. This study aimed to develop a novel scale, predicting the need for Delayed Intubation after inhalation injury (PDI) score. METHODS: We retrospectively identified patients with inhalation injury at four tertiary care centers in Japan between 2012 and 2018. We included patients aged 15 or older and excluded those intubated within 30 min after hospital arrival. Predictors for delayed intubation were identified with univariate analyses and scored on the basis of odds

Delivery and management of a preterm infant in the burn unit: a multidisciplinary approach.

Burns during pregnancy can have profound effects on both the mother and her fetus. While the mother can suffer cardiovascular collapse, infection, hemorrhage and inhalation injury with respiratory failure, the fetus is affected through the placenta by all of these changes as well as by the transfer of drugs administered to the mother. We report a case of severely burned female patient at 29 weeks gestation, who, due to deteriorating maternal condition, was delivered and managed at 32 weeks gestation by a multidisciplinary team. To the best of our knowledge this is the first reported case of a
