BACKGROUND: Esophageal hypersensitivity is considered an important pathophysiological mechanism in refractory gastroesophageal reflux disease (GERD) patients. Serotonin (5-HT) plays an important role in the regulation of GI (gastrointestinal) secretion, motility and sensitivity. Previous studies found that altered 5-HT availability has no clear effects on esophageal/GI sensations. Our aim was therefore to investigate the role of 5-HT in esophageal sensitivity in healthy volunteers (HV).
METHODS: Esophageal sensitivity to thermal, mechanical, electrical, and chemical stimuli was assessed in 3

Responding appropriately to stress is essential for survival, yet in pathological states, these responses can develop into debilitating conditions such as post-traumatic stress disorder and generalized anxiety. While genetic models have provided insight into the neurochemical and neuroanatomical pathways that underlie stress, little is known about how evolutionary processes and naturally occurring variation contribute to the diverse responses to stressful stimuli observed in the animal kingdom. The Mexican cavefish is a powerful system to address how altered genetic and neuronal systems can gi

Body dysmorphic disorder (BDD), a preoccupation with an imagined or slight defect in appearance, has been described for more than a century and reported around the world. However, investigation into the treatment of this distressing and impairing disorder is just beginning. Available data on the pharmacologic treatment of BDD suggest that it often responds to serotonin reuptake inhibitors (SRIs), which may be preferentially effective for this disorder. Augmentation of SRIs with buspirone and neuroleptics, as well as combinations of SRIs, also appear promising. Available data also suggest that

Aripiprazole is the first dopamine D(2)/D(3) receptor partial agonist approved for use in the treatment of psychiatric disorders including schizophrenia, bipolar disorder, and unipolar depression in the US. Aripiprazole has demonstrated a relatively favorable side effect profile compared to other commonly prescribed antipsychotics, including a low propensity for treatment-limiting extrapyramidal symptoms, hyperprolactinemia, and body weight gain. In an effort to elucidate aripiprazole's pharmacological activity in relation to clinically relevant fluctuation of dopamine D(2) receptor reserves,

Serotonin syndrome (SS) is a potentially life-threatening condition caused by excessive serotonergic activity, often due to drug interactions. It classically manifests with autonomic and neuromuscular hyperactivity and by mental status changes that might include restlessness, delirium, and agitation. We present a case of a 76-year-old patient with Parkinson's disease with SS triggered by interaction between rasagiline and buspirone. He presented with altered mental status, hyperreflexia, and muscle rigidity. Due to the severity of symptoms, he was early admitted to the ICU, and his clinical co

With serotonergic antidepressants dominating the treatment of depressive disorders, antidepressant-induced sexual side effects have emerged as a major clinical issue. Surprisingly, the effects of depression on sexuality are less well established and more variable than previously thought. It is likely that antidepressants with strong serotonergic effects--selective serotonin reuptake inhibitors, clomipramine, and monoamine oxidase inhibitors--are associated with higher rates of sexual side effects, compared to other antidepressant classes. Orgasmic and ejaculation difficulties are prominent wit

Several medications may help improve treatment of alcoholic patients. Medications that effectively and specifically reverse symptoms of alcohol intoxication or coma currently are lacking. Benzodiazepines remain the most effective medications for the treatment of alcohol withdrawal and for the prevention of withdrawal seizures and delirium, even though the adrenergic agents clonidine or atenolol may hasten resolution of withdrawal symptoms and facilitate outpatient detoxification. Psychosocial rehabilitation of alcoholics may be improved by judicious addition of the alcohol-sensitizing agents d

Over the past five years, the mechanisms of action of several classes of antianxiety drugs have been clarified. Benzodiazepines, triazolopyridazines, and barbiturates seem to produce their anxiolytic effects by interacting with a specific high affinity receptor (viz. benzodiazepine receptor) in the brain. The benzodiazepine receptor is functionally and perhaps structurally coupled to a receptor for the major inhibitory neurotransmitter GABA as well as a chloride channel or ionophore. Taken together, this receptor "complex" may mediate the behavioral effects of a number of chemically diverse an

Opioids are well-known to cause respiratory depression, but despite clinical evidence of dysphagia, the effects of opioids on swallow excitability and motor pattern are unknown. We sought to test the effects of the clinically-relevant opioid buprenorphine on pharyngeal swallow and respiratory drive in male and female rats. We also evaluated utility of serotonin 5-HT1A agonists (8-OH-DPAT and buspirone) to improve swallowing and breathing outcomes following buprenorphine administration. Experiments were performed on 44 freely breathing Sprague Dawley rats anesthetized with sodium pentobarbital.

OBJECTIVE: To examine whether the co-administration of hormonal contraceptives (HC) and psychotropic drugs commonly used to treat anxiety and/or depression results in safety or efficacy concerns for either drug.
METHODS: We searched PubMed and Cochrane libraries for clinical or pharmacokinetic (PK) studies that examined co-administration of any HC with psychotropic drugs [selective serotonin reuptake inhibitors (SSRIs), serotonin-norepinephrine reuptake inhibitors (SNRIs), tricyclic antidepressants (TCAs), oral benzodiazepines, bupropion, mirtazapine, trazadone, buspirone, hydroxyzine, monoam

The respiratory effects of several benzodiazepine (BZ) agonists and inverse agonists were compared with those of buspirone and pentobarbital in awake rhesus monkeys. Subjects, fitted with a helmet that served as a pressure displacement plethysmograph, were studied in a ventilated, sound-attenuating chamber; ventilatory frequency, tidal volume (VT) and minute volume (VE) were determined from the plethysmograph signal. During experimental sessions monkeys inhaled either 5% carbon dioxide (CO2) mixed in air or air alone according to a fixed alternating schedule. BZ agonists (alprazolam, 0.01-1.0

The coexistence of trichotillomania and autistic disorder has rarely been reported in psychiatric literature. The current study describes successful treatment of trichotillomania in a young autistic girl, using combined clomipramine and behavioral therapy. Neither behavioral therapy, clomipramine, methylphenidate, buspirone, nor naltrexone was effective in monotherapeutic trials. We postulate a synergistic effect of combined treatment with clomipramine and behavioral therapy, suggesting that both pharmacological and behavioral treatment of hair pulling may be necessary for many patients. BACKGROUND: Generalized anxiety disorder (GAD) is a persistent and common mental disorder that entails significant impairments in role functioning and quality of life. Currently available effective interventions include psychological therapies, self-help approaches, and pharmacological treatments, which do not quite meet clinical needs, and the ideal anxiolytic is still being sought. Shu-gan-qing-re (SGQR) formula, a Chinese patent medicine, has been well received by patients with GAD in Chinese clinical practice for years. The present prospective, double-blind, double-dummy, randomized contro

BACKGROUND: In this report, we describe a case of a patient with substance-induced anxiety disorder occurring after a single dose of 3,4-methylenedioxymethamphetamine. Furthermore, we describe the use and efficacy of the Primary Care Behavioral Health model, a collaborative approach to integrative primary mental health care, in evaluating and treating this rare mental health disorder.
CASE PRESENTATION: Three days following ingestion of one dose of 3,4-methylenedioxymethamphetamine, a 35-year-old Hispanic man with no significant prior mental health history and no history of prior 3,4-methylen

In recent years generalised anxiety disorder (GAD) has become a much better defined disorder, with specific criteria distinguishing it from the other anxiety disorders; however, it still lacks the same public and scientific interests as some of the other anxiety disorders such as panic and social phobia. Nevertheless, refinement in the treatment of GAD is becoming more evident through the conduct of clinical trials. Up until the mid-1980's, treatment consisted primarily of benzodiazepines. However, as a result of growing characterisation of their abuse potential, other therapeutic options were

Pharmacological treatments are one of several strategies used in the treatment of anorexia nervosa and bulimia nervosa. Many studies have found that antidepressants are effective in the treatment of bulimia nervosa and these drugs represent a mainstay of treatment for these patients. Over the past several years, selective serotonin reuptake inhibitors have become perhaps the most commonly used class of drugs. A variety of medications have been investigated for anorexia nervosa, but there is little consistent evidence that medications are effective. In both these illnesses it is important to di

Although selective 5-hydroxytryptamine (5-HT) reuptake inhibitors (SSRIs) are widely used in the chronic treatment of several anxiety disorders, increased anxiety has been observed in some patients at the beginning of treatment with these compounds. Similar increases in anxiety-related behaviors have been observed in animal studies following a single injection with SSRIs. The mechanism underlying this effect is unclear. The aim of the present study was to investigate the effects of a variety of psychoactive compounds on the anxiogenic-like activity of fluoxetine. The drugs used included the be

BACKGROUND: Jails in the United States are required to provide health care to the over 10 million people entering jails each year, a significant portion of whom need medications. Yet little is known about the processes by which medications are prescribed, obtained, and administered to incarcerated persons in jails.
OBJECTIVE: To describe medication access, policy, and procedures in jails.
METHODS: Semi-structured interviews were conducted with administrators and health workers from 34 jails (of 125 contacted) across 5 states in the southeastern United States. The interview guide covered all

The stripping voltammetric behaviour of buspirone hydrochloride (BUS) and piribedil (PIR), as models of pyrimidine-containing compounds, was studied using a hanging mercury drop electrode (HMDE). A sensitive adsorptive stripping voltammetric method for determination of such drugs is described. The voltammetric peaks were obtained at -1.23 and -1.22 V for BUS and PIR. respectively, which correspond to the reduction of the azomethine group of pyrimidine ring in Britton-Robinson buffer (pH 7). Factors such as pH of supporting electrolyte, accumulation potential and time and instrumental parameter
