Breast infarction due to calciphylaxis after coronary artery bypass grafting.

The use of the internal mammary artery for coronary artery bypass grafting is common. We describe a patient with chronic renal insufficiency who had no need for dialysis, and who suffered from breast necrosis after coronary artery bypass grafting with internal mammary artery harvesting due to calciphylaxis. The histology report of the breast tissue showed mural vascular calcification and intima proliferation of small-sized to medium-sized vessels. This causes ischemic necrosis of the skin and septal panniculitis. We believe that this is the first case report of breast necrosis after coronary a

Penile calciphylaxis: rare necrotic lesions in chronic renal failure patients.

PURPOSE: Calciphylaxis is a rare devastating condition of cutaneous necrosis resulting from calcification of small blood vessels in patients with end stage renal failure and secondary hyperparathyroidism. We describe 5 patients with penile calciphylaxis at a single institution. MATERIALS AND METHODS: From 1992 to 1996, 5 patients had penile calciphylaxis. The charts of these patients were reviewed to determine the nature of the lesions, association with hyperparathyroidism and treatment outcomes. RESULTS: All 5 patients had chronic renal failure, secondary hyperparathyroidism and painful nec

Histologic comparison of normal and diseased skin in patients with suspected calciphylaxis.

BACKGROUND: The pathophysiology of calciphylaxis and histologic criteria for diagnosis are uncertain. OBJECTIVE: To address this, histology was compared in both diseased and normal skin from patients with suspected calciphylaxis. METHODS: Biopsies from 18 patients with suspected calciphylaxis were examined for vascular calcification, thrombosis, and intimal hyperplasia as well as extravascular calcification. RESULTS: All but 2 patients were receiving dialysis, half had diabetes, half were treated with warfarin, and almost all had involvement of the thigh. Small artery or arteriole calcifica

Multi-site Calciphylaxis With Pulmonary Calcification in a Young Haemodialysis Patient.

Calciphylaxis is a rare and life-threatening complication of end-stage renal failure (ESRF), typically presenting with painful cutaneous necrosis but occasionally involving the visceral organs. Metastatic pulmonary calcification (MPC) results from calcium deposition within the alveolar walls due to severe derangements in mineral metabolism, whereas pulmonary calciphylaxis is even rarer and characterised by vascular calcification with thrombosis and tissue ischaemia. We describe a 34-year-old man with congenital horseshoe kidney and recurrent papillary urothelial carcinoma who developed ESRF se

Calciphylaxis Episodes in the Australia and New Zealand Dialysis and Transplant Registry.

INTRODUCTION: Calciphylaxis is a rare disorder associated with significant morbidity and mortality. Data registries are an invaluable source of information for rare diseases. We reviewed cases of calciphylaxis recorded in the Australia and New Zealand Dialysis and Transplant Registry (ANZDATA) and evaluated associations and outcomes of this condition. METHODS: Data was obtained on all cases of calciphylaxis reported between 2019 and 2022 in Australian and New Zealand patients on kidney replacement therapy (KRT). This cohort was compared to all patients in the registry who received KRT from 20

Penile calciphylaxis with penoscrotal necrosis: A case report with literature review.

Penile calciphylaxis, also known as calcific uremic arteriolopathy is an uncommon condition of the penile vessels due to its extensive vascular network. The aim of this report is to report a very rare case of penile calciphylaxis with penoscrotal necrosis. A 54-year-old male patient presented with progressive penoscrotal necrosis within a duration of one month. He had a history of diabetes mellitus and stage 5 chronic kidney disease. Under spinal anesthesia, partial penectomy and excision of the necrotic scrotum were performed. Histopathological examination was consistent with calciphylaxis. D

A novel model of dermatomyositis induced by membrane antigen and calciphylaxis in rats.

Dermatomyositis (DM) is a severe autoimmune disease of the connective tissue characterized by inflammatory and degenerative changes in the skin and muscle. However, the lack of experimental models of DM represents a challenge for the development of effective drugs. The aim of the present study was to establish a pharmacodynamic rat model of DM that would recapitulate the clinical manifestation seen in patients. The DM model was established using membrane antigen-induced autoimmune injury, followed by toxin-induced subcutaneous calciphylaxis. The rats were divided into five groups and were subc

Vascular calcifications and calciphylaxis in a patient on concurrent haemodialysis and Coumadin therapy.

Calciphylaxis is a rare life-threatening condition, with calcification of small and medium-sized vessels leading to skin necrosis. It has a high morbidity and mortality, and most of the patients die from wound superinfection and sepsis. A 48-year-old man with a history of end-stage renal disease on haemodialysis and Coumadin therapy for venous thromboembolism presented with pulmonary oedema after missing two haemodialysis treatment. At examination, he had bilateral lower extremity dark brown, possibly necrotic, painful ulcers. He was diagnosed with calciphylaxis and treated with sevelamer hydr

Partial Penectomy for Dry Penile Gangrene in a Patient With Penile Calciphylaxis.

Penile calciphylaxis is a difficult urologic condition to manage. First described by Wood et al in 1997, penile calciphylaxis is a rare cause of penile gangrene.1 One study found the overall mortality rate associated with penile calciphylaxis to be 64%, with a mean time to death of 2.5 months.2 Additionally, no statistically significant difference in survival has been seen between patients treated with penectomy and those treated nonoperatively.3 We report a case of penile calciphylaxis in a patient treated with partial penectomy with scrotoplasty who has survived greater than 1 year without a

Multidisciplinary management of calciphylaxis: a series of 5 patients at a single facility.

Calciphylaxis is a rare and severe disease that manifests with painful skin ulceration and necrosis. Herein, we report five patients of hemodialysis patients with skin biopsy-proven calciphylaxis at a single facility. One patient had undergone parathyroidectomy (PTx) due to severe secondary hyperparathyroidism, four had been treated with vitamin D receptor activators, and two were on warfarin therapy. All patients had hyperphosphatemia, and one had hypercalcemia. The intact parathyroid hormone level at diagnosis was 2 pg/ml in the patient after PTx, while three patients were within the target

Sodium thiosulfate for the treatment of warfarin-induced calciphylaxis in a nondialysis patient.

Calciphylaxis or uremic arteriolopathy is a complex process typically seen in patients with end-stage renal disease, but has also been reported in patients with normal renal function. However, therapies for calciphylaxis are based on reports of traditional patients (i.e., end-stage renal disease). A mainstay of therapy, sodium thiosulfate (STS), has been shown to be effective for the treatment of calciphylaxis. Without a standardized therapy reported for nondialysis patients there is a need for evidence-based therapy. Here, we report a case of a 63-year-old woman with an acute injury on chroni

Simulation-based sodium thiosulfate dosing strategies for the treatment of calciphylaxis.

BACKGROUND AND OBJECTIVES: Calciphylaxis remains a poorly understood life-threatening disorder with limited therapeutic options. Sodium thiosulfate (STS) has reported efficacy, thought to be because solubilizing calcium deposits promote clearance by hemodialysis (HD). Lack of rigorous pharmacokinetic studies makes it problematic for determining proper STS dosing given the expanding range of dialysis prescriptions and intensities. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: The purpose of this study was to determine the dosing strategies for STS during different dialysis regimens. Given rep

Calciphylaxis of the breast: a rare disease in the differential diagnosis of breast cancer.

Calciphylaxis is a rare, but potentially life-threatening condition usually observed in patients with end-stage chronic renal disease and characterized by small- and medium-sized blood vessel calcification leading to tissue ischemia. The clinical features are primarily cutaneous, consisting of skin necrosis and ulceration, mostly located in the lower extremities. We report a case of a 56-year-old woman with a previous history of renal disease and atypical lobular hyperplasia of the right breast, who developed painful skin necrosis on both breasts. Differential diagnosis comprises acute bacteri
