Undetermined stroke genesis and hidden cardiomyopathies determined by cardiac magnetic resonance.

OBJECTIVE: To determine whether cardiac magnetic resonance imaging (CMR) could be useful in identifying previously undiagnosed cardiomyopathies in a cohort of patients with ischemic stroke who underwent standard etiologic investigation and to describe the type and frequency of these cardiomyopathies. METHODS: We performed a subanalysis of a previously collected prospective cohort of patients with ischemic stroke. Patients with structural changes on echocardiography that are considered causal for stroke in the Trial of Org 10172 in Acute Stroke Treatment (TOAST) classification were excluded. A

An update on canine cardiomyopathies - is it all in the genes?

Dilated cardiomyopathy is the second most common cardiac disease in dogs and causes considerable morbidity and mortality. Primary dilated cardiomyopathy is suspected to be familial, and genetic loci have been associated with the disease in a number of breeds. Because it is an adult-onset disease, usually with late onset, testing breeding dogs and bitches before breeding for a genetic mutation that could lead to dilated cardiomyopathy would be helpful to prevent disease. There is growing evidence that the genetic basis may be multigenic rather than monogenic in the majority of studied breeds. T

Mitochondrial cardiomyopathies feature increased uptake and diminished efflux of mitochondrial calcium.

Calcium (Ca2+) influx into the mitochondrial matrix stimulates ATP synthesis. Here, we investigate whether mitochondrial Ca2+ transport pathways are altered in the setting of deficient mitochondrial energy synthesis, as increased matrix Ca2+ may provide a stimulatory boost. We focused on mitochondrial cardiomyopathies, which feature such dysfunction of oxidative phosphorylation. We study a mouse model where the main transcription factor for mitochondrial DNA (transcription factor A, mitochondrial, Tfam) has been disrupted selectively in cardiomyocytes. By the second postnatal week (10-15day ol

Sudden cardiac death: focus on the genetics of channelopathies and cardiomyopathies.

Sudden cardiac death (SCD) describes a natural and unexpected death from cardiac causes occurring within a short period of time (generally within 1 h of symptom onset) in the absence of any other potentially lethal condition. Most SCD-related diseases have a genetic basis; in particular congenital cardiac channelopathies and cardiomyopathies have been described as leading causes of SCD. Congenital cardiac channelopathies are primary electric disorders caused by mutations affecting genes encoding cardiac ion channels or associated proteins, whereas cardiomyopathies are related to mutations in g

Coronary Microcirculatory Dysfunction in Human Cardiomyopathies: A Pathologic and Pathophysiologic Review.

Cardiomyopathies are a heterogeneous group of diseases of the myocardium. The term cardiomyopathy involves a wide range of pathogenic mechanisms that affect the structural and functional states of cardiomyocytes, extravascular tissues, and coronary vasculature, including both epicardial coronary arteries and the microcirculation. In the developed phase, cardiomyopathies present with various clinical symptoms: dyspnea, chest pain, palpitations, swelling of the extremities, arrhythmias, and sudden cardiac death. Due to the heterogeneity of cardiomyopathic patterns and symptoms, their diagnosis a

[Myocarditis in the differential diagnosis of cardiomyopathies. Endomyocardial biopsy or MRI?].

Myocarditis is an inflammatory disease of the heart muscle commonly caused by viral pathogens. Dilated cardiomyopathy is a major long-term sequela of myocarditis and at least in part related to post-viral immune-mediated responses. Establishing a diagnosis of myocarditis represents a major challenge because of the variable clinical picture and the lack of readily available, non-invasive diagnostic tests. In recent years, cardiac magnetic resonance imaging (cMRI) has emerged as a promising additional diagnostic tool in patients with suspected myocarditis: cMRI not only provides important insigh

[Characteristics of hemodynamics in cardiomyopathies of newborn infants of diabetic mothers].

The hemodynamics was studied in 42 newborns with diabetic fetal diseases. Clinical and instrumental examinations revealed cardiomyopathy in 28 of these. Hemodynamic disorders were characterized by a low stroke volume resultant from reduced diastolic volume of the left ventricle. The most grave hemodynamic disorders! were detected in patients with the obstructive cardiomyopathy when both diastolic and systolic volumes of the left ventricle were changed, myocardia contractility reduced, and diastolic function suppressed.

Endomyocardial biopsies for early detection of mitochondrial disorders in hypertrophic cardiomyopathies.

Considering the high proportion of unexplained hypertrophic cardiomyopathies on the one hand and the occurrence of cardiomyopathies in several mitochondrial disorders on the other, we hypothesized that isolated hypertrophic cardiomyopathies in infancy could occasionally be the result of defects of oxidative phosphorylation. By means of a scaled-down technique, we were able to investigate oxidative phosphorylation on minute amounts of endomyocardial tissue (1 mg) in three patients with concentric hypertrophic cardiomyopathy (shortening fraction in diameter, 18% to 27%; normal mean +/- 1 SD, 33

[Prevention of myopathies and cardiomyopathies in swine using magnesium aspartate hydrochloride].

The efficiency of magnesium-aspartate-hydrochloride (MAH) in prophylaxis of porcine stress-induced myopathy was investigated. For the examination a conveyor belt was used. MAH was administered orally or intramuscularly. Clinical investigations, blood specimen collections and ECG were carried out before and after running. Stress-induced tachypnoea and tachycardia and hyperthermia were reduced after both forms of administration of MAH. The enzyme activity of CK, LDH and alpha-HBDH increased slighter after oral and i. m. application. The increase of serum glucose concentration as well as serum la

Cellular mechanisms of alterations in myocardial contractile function in experimental cardiomyopathies.

The high-energy-phosphate content, myocardial ultrastructure, left ventricular (LV) pressure, and pump function of isolated rat hearts were determined in four kinds of chronic myocardial damage induced by either autoimmunization, treatment of rats with adriamycin or noradrenaline in increasing doses, or infection with smallpox virus. Mild fibrosis, swollen mitochondria, and hyper-contracted and overdistended sarcomeres were typical ultrastructural alterations. The ATP and phosphocreatine content as well as cardiac output at standard load conditions were substantially lower in all four experime

[Disorders of heart rhythm and conduction in dilated and hypertrophic cardiomyopathies].

Idiopathic cardiomyopathies are characterized by diversity of clinical manifestations, among which heart rhythm abnormalities are the most common. The authors carried out qualitative and quantitative evaluations of heart rhythm abnormalities in patients with dilated and hypertrophic cardiomyopathies (DCMP, HCMP) and compared those abnormalities according to the data of daily ECG monitoring. Forty patients with DCMP and 30 with HCMP were examined. In 8 (25%) patients with DCMP and in 2 (6.7%) with HCMP, permanent atrial fibrillation was recorded. Among patients with sinus rhythm, supraventricul

Impact of 18FDG-Avidity and Immunosuppression on Idiopathic and Genetic Cardiomyopathies.

BACKGROUND: Myocardial 18fluorodeoxyglucose (18FDG)-avidity is frequently seen in patients with genetic cardiomyopathy, as well as a growing "idiopathic 18FDG-avidity" group of genotype-negative patients who do not clearly have cardiac sarcoidosis. The prognostic implications of 18FDG-avidity in patients with and without genetic cardiomyopathy, and the effects of immunosuppression in the latter, remain uncertain. METHODS: This multicenter, retrospective study included all patients who were referred for both 18FDG-PET and cardiomyopathy genetic testing. Patients with acute myocarditis, biopsy-

Managing pregnancy in women with pre-existing cardiomyopathies: A case series.

Women with pre-existing cardiomyopathy experience high maternal and fetal complication rates when undergoing pregnancy. Different cardiomyopathy phenotypes and genetics can influence the presentation during pregnancy and help inform risk profile and management. This case series demonstrates the differing course of pregnancy in dilated, hypertrophic and arrhythmogenic cardiomyopathy. The main complications managed were arrhythmia, rising left ventricular outflow tract obstruction and heart failure. The cases highlight important pre-conception considerations, the use of biomarkers, and pharmacol

Gene-based Therapies for Genetic Cardiomyopathies: Molecular Medicine for Heart Disease.

Pathogenic genetic alterations are a well-recognized mechanism in cardiomyopathies. As such, genetic testing has become an integral component of the diagnostic pathway for cardiomyopathy. Subsequent developments in gene-specific therapies have advanced precision medicine by enabling direct targeting of pathogenic genetic variants, demonstrating promise as the ultimate therapy for cardiomyopathy. Gene therapy can be categorized into 3 main approaches: gene replacement, gene silencing, and direct genome editing. Clustered regularly interspaced short palindromic repeats (CRISPR)/CRISPR-associated

Clinical characteristics and outcomes of cardiomyopathies related to immune-mediated diseases.

AIMS: Dilated cardiomyopathy (DCM) encompasses genetic and acquired aetiologies, yet the impact of immune-mediated mechanisms remains unclear. This study investigates the clinical characteristics and outcomes of immune-mediated DCM (ID-CMP). METHODS: From the Maastricht Cardiomyopathy registry, 194 patients with ID-CMP were compared with 678 non-immune DCM controls over a median follow-up of 5.2 (3.7-10.7) years. ID-CMP was categorized into autoimmune-mediated (adaptive immunity dysfunction), autoinflammatory-driven (innate immunity dysfunction), and mixed-pattern (MHC Class I associations an

Inflammation and Immunogenetics in Cardiomyopathies: From Molecular Mechanisms to Therapeutic Perspectives.

Genetic variants that impair cardiac function or predispose individuals to autoimmune diseases can influence both the risk and severity of inflammatory heart diseases, including lymphocytic myocarditis and cardiac sarcoidosis. We review recent clinical and experimental studies that describe the emerging interplay between genetic susceptibility and active myocardial inflammation. We summarize the current perspective on the mechanisms of inflammatory pathways and gene-immune interactions, and explore the emerging therapeutic approaches aimed at targeting inflammation in genetic cardiomyopathies.
