The fetal cardiovascular function in chronic placental insufficiency is different from experimental hypoxia.

Ten mothers at normal term pregnancy were rendered mildly hypoxic by allowing them to breathe a gas mixture of 10% oxygen and 90% nitrogen. The fetuses were studied with Doppler ultrasound and computer assisted cardiotocography. There was vasodilatation in the cerebral vasculature while the utero-placental blood flow decreased. No changes in fetal haemodynamics occurred. The fetal heart rate was correlated inversely to the heart rate variation. There was a positive correlation between the heart rate variation and the umbilical flow velocity waveform index. In a group of 30 subjects with severe

[Indications for cesarean section].

Following introductory remarks on rates of cesarean section, the relative and absolute indications for the procedure are reviewed. The rise in the rate of cesarean sections is due to widening of the relative indications, such as dystocia, previous cesarean, fetal distress and breech presentation. Since 1974 the cesarean section rate at Aarau Gynecologic and Obstetric Clinic has fluctuated between 6% and 8% and showed an increase for the first time, to 9.3%, in 1988. Breakdown by the main indications shows fetal distress, breech presentation and history of previous cesarean. Other indications,

[Supraventricular tachycardia of the fetus in the 3d trimester of pregnancy following persistent supraventricular extrasystole].

Persistent supraventricular extrasystoles are antepartally, intrapartally and postpartally the most frequent form of arrhythmias, and do not cause fetal congestive heart failure (hydrops fetalis). The premature beats often disappear spontaneously prenatally, but in most cases within the first two weeks of life. The extremely rare observation of the occurrence of a supraventricular tachycardia in the 37th week of gestation in a fetus with persistent supraventricular extrasystoles from the 20th week of gestation onward and with a postnatally diagnosed Wolff-Parkinson-White syndrome is described.

Consumer obstetrics.

OBJECTIVE: To investigate the perceptions of patients about delivery of their babies. METHOD: One hundred and forty four out of 200 surveyed women answered consumer orientated questions before and after delivery. RESULTS: They were on the whole satisfied with antenatal classes (there seemed to be a need for more information in the form of an on-the-ward postnatal class), disliked the practice of perineal shaves (but did not object to enemas or rupture of membranes) and felt they had adequate analgesia (although not for after-pains or the discomfort of haemorrhoids in the puerperium). Women w

Peripartum management of caesarean delivery in type 2 von Willebrand disease.

We report the case of a successful caesarean delivery in a woman with von Willebrand disease (VWD) which poses significant risk of maternal bleeding during childbirth. The patient, a primigravida with known type 2 VWD, was closely monitored throughout gestation with periodic assessment of coagulation profile, von Willebrand factor activity and factor VIII levels. A multidisciplinary team-including obstetricians, haematologists, transfusion medicine specialists and anaesthesiologists-was involved and delivery was planned at 39 weeks' gestation with preparedness for factor replacement. Comparison between Protocols for Management of Fetal Growth Restriction.

This comprehensive review compares clinical protocols of important entities regarding the management of fetal growth restriction (FGR), published since 2015. Five protocols were chosen for data extraction. There were no relevant differences regarding the diagnosis and classification of FGR between the protocols. In general, all protocols suggest that the assessment of fetal vitality must be performed in a multimodally, associating biophysical parameters (such as cardiotocography and fetal biophysical profile) with the Doppler velocimetry parameters of the umbilical artery, middle cerebral arte

Managing fetal growth restriction: surveillance tests and their interpretation.

The main challenges in pregnancies affected by fetal growth restriction consist in identifying signs of disease progression and determining the appropriate timing of delivery. The risk of continuing the pregnancy must be balanced with the risk of prematurity, which depends on gestational age. To allow appropriate monitoring of the growth-restricted fetus, several surveillance tests are available. These include ultrasound Doppler velocimetry of feto-placental vessels, cardiotocography, and amniotic fluid evaluation. It is well known that the combination of tests performs better than each test a

Fetal arrhythmias: Surveillance and management.

Fetal arrhythmias warrant sophisticated surveillance and management, especially for the high-risk pregnancies. Clinically, fetal arrhythmias can be categorized into 3 types: premature contractions, tachyarrhythmias, and bradyarrhythmias. Fetal arrhythmias include electrocardiography, cardiotocography, echocardiography and magnetocardiography. Oxygen saturation monitoring can be an effective way of fetal surveillance for congenital complete AV block or SVT during labor. Genetic surveillance of fetal arrhythmias may facilitate the understanding of the mechanisms of the arrhythmias and provide th

Prospective risk of intrauterine death of monochorionic-diamniotic twins.

OBJECTIVE: The purpose of this study was to calculate the prospective risk of fetal death in monochorionic-diamniotic twins. STUDY DESIGN: We evaluated 193 monochorionic diamniotic twin pregnancies that were followed and delivered after 24 weeks. Surveillance included cardiotocography and sonography performed at least once weekly. The prospective risk of fetal death was calculated as the total number of deaths at the beginning of the gestational period divided by the number of continuing pregnancies at or beyond that period. RESULTS: The fetal death rate was 5 of 193 pregnancies (2.6%; 95% C

Transcervical intrapartum amnioinfusion: a simple and effective technique.

OBJECTIVE: To test a simple method of intrapartum amnioinfusion to replace amniotic fluid during labor in cases of prolonged labor and severe variable fetal heart rate decelerations or thick meconium. STUDY DESIGN: We conducted a prospective study of intrapartum amnioinfusion in sixteen consecutive patients. A simple transcervically placed Foley bladder catheter was used for intrauterine infusion of saline solution, while monitoring was performed by external cardiotocography. RESULTS: In twelve patients, catheter placement was easily performed; the remaining four required stabilization by a

[Difficulties in interpreting the cardiotocogram in atrioventricular arrhythmia of the fetal heart].

During a routine screening examination of a pregnancy during the 28th gestational week, a pericardial effusion and ascites were recognized in the fetus. Ultrasound B- and M-mode evaluation (echocardiography) revealed an atrio-ventricular block grade III with commencing cardiac insufficiency. Cardiac monitoring (CTG) by ultrasound (HP 8040A Fetal Monitor) documented primarily the fetal atrial activity when the logic switch was turned off, but after it was turned on there was no continuous recording. When fetal heart action was recorded by abdominal electrocardiogram (AFEKG) the cardiotocography

Betamethasone administration to the mother and problems in foeto-placental monitoring.

The administration of betamethasone to the mother in order to accelerate maturation of foetal lungs induces significant modifications in the foeto-placental hormonal secretion. Both the total estriol and unconjugated estriol present a sharp fall in the days immediately following the administration and a rise 10-12 days later, depending on a rebound effect. The HPL levels do not variate except for a progressive and constant rising due probably to better cardiocirculatory maternal condition with, consequently, a higher utero-placental blood-flow. All these phenomena increase when the administrat

[Development of intensive monitoring in obstetrics].

As early as about the turn of the century the causes for the discharge of the meconium under labour were studied and the attempt was made to record alterations of the cardiac sound under the influence of labour pain as well as the behaviour of the fetal heart by means of the electrocardiogram. In the development of obstetrics certain stages can be observed which in the past few decades entailed a great many changes, such as changing from delivery at home to delivery at hospital, parasitological clarification of intrauterine infections and the revelation of the pathogenesis of the Mhn. Dynamics of T2* and deformation in the placenta and myometrium during pre-labour contractions.

Pre-labour uterine contractions, occurring throughout pregnancy, are an important phenomenon involving the placenta in addition to the myometrium. They alter the uterine environment and thus potentially the blood supply to the fetus and may thus provide crucial insights into the processes of labour. Assessment in-vivo is however restricted due to their unpredictability and the inaccessible nature of the utero-placental compartment. While clinical cardiotocography (CTG) only allows global, pressure-based assessment, functional magnetic resonance imaging (MRI) provides an opportunity to study co

Types of intrapartum hypoxia on the cardiotocograph (CTG): do they have any relationship with the type of brain injury in the MRI scan in term babies?

Electronic foetal monitoring using cardiotocography is aimed at the timely recognition and management of foetal hypoxia. The primary objective of this study was to examine whether a relationship exists between the types of foetal hypoxia (acute, subacute, evolving, chronic), as identified on cardiotocography and the nature of hypoxic ischaemic encephalopathy, as observed on MRI scans after birth. We conducted a retrospective study of 16 babies born (out of 52,187 births) at St George's Hospital in London during 2006-2017 with a postnatal diagnosis of HIE. Of the 16 babies, only 11 had both MRI

Risk factors and risk-indicating model for early-onset neonatal sepsis after preterm prelabor rupture of membranes: A historical cohort study.

INTRODUCTION: Early-onset neonatal sepsis (EONS) is an important cause of neonatal morbidity and mortality and is strongly associated with intra-amniotic infection. The risk of intra-amniotic infection increases after preterm prelabor rupture of membranes (PPROM). Better tools are needed for monitoring women with PPROM for intra-amniotic infection and identifying those at high risk of EONS so that intervention can be made timely. This study aimed to identify antepartum risk factors for EONS in a PPROM population and develop a risk-indicating model. MATERIAL AND METHODS: We performed a histori

Impact of oxytocin discontinuation on fetal heart rate and uterine contractility: A pre-specified ancillary analysis embedded within a randomized trial.

INTRODUCTION: Oxytocin is widely used to augment uterine contractions during labor. However, its use has been associated with fetal heart rate (FHR) abnormalities and neonatal morbidity, which may be reduced by discontinuing oxytocin during labor. We aimed to assess the impact of oxytocin discontinuation at the onset of the active phase of labor on FHR patterns and uterine contractility. MATERIAL AND METHODS: This study is a pre-specified ancillary analysis of the STOPOXY trial, a multicenter, randomized, open-label, controlled superiority trial conducted in 21 French maternity units between

Artificial intelligence-based prediction of fetal hypoxia: a multicenter model development and nationwide AI-human comparison.

BACKGROUND: Fetal hypoxia is a leading cause of neonatal morbidity and mortality. Cardiotocography (CTG) is widely used to predict fetal hypoxia during labor, but its interpretation remains suboptimal. Artificial intelligence (AI) models have been developed for CTG interpretation, but their clinical utility is limited by two major challenges: demonstrating superiority over human experts and ensuring explainability in real-world settings. METHODS: A large dataset containing CTG traces from three tertiary hospitals between January 2014 and May 2022 was built for model development. Deep learning

Late-onset fetal growth restriction management: a national survey.

BACKGROUND: Fetal growth restriction (FGR) is an obstetric condition that increases the risk of adverse neonatal outcomes. FGR antenatal care aims to decrease fetal morbidity and mortality through optimal fetal monitoring. However, no univocal strategies for late-onset FGR outpatient management are available, and this survey investigated gynaecologists' attitudes concerning outpatient frequency tests. METHODS: We mailed a survey to 429 Italian gynaecologists. The primary purpose was the ambulatory care of late-onset FGR without doppler alterations evaluation. The queries estimated the self-re
