OBJECTIVES: Recognize posterior fossa anomalies on ultrasound and magnetic resonance imaging and appreciate imaging pitfalls that may lead to misdiagnosis.
MATERIALS AND METHODS: Cases are presented to illustrate normal development and various anomalies. Postnatal studies and autopsy are used for correlation with prenatal imaging.

Lymph was collected from cervical lymphatic trunks of anesthetized cats under conditions of normal cerebrospinal fluid (CSF) pressure and again when the CSF pressure was elevated by infusing artificial CSF into the subarachnoid space at the cisterna magna. Diffuse leptomeningitis did not occur in healthy adult male Wistar rats with transient candidemia produced by the injection of organisms into the internal carotid artery, even though intraparenchymal microabscesses with yeasts and pseudohyphae were seen throughout the brain. We report a cryptic angioma associated with a cisternal hematoma confined to the cerebellopontine cistern occurring in a 29-year-old woman. The angioma was related to the acoustic nerve. The cisternal hematoma appeared to have increased in size gradually with repeated hemorrhages from the angioma. The location of the lesion, in conjunction with the gradual size increase of the cisternal hematoma, resulted in the lesion simulating acoustic neurinomas radiologically as well as neurologically.

A case of choroid plexus papilloma is presented which was extensively investigated. The location of the pedicle of this tumor was not demonstrated by conventional CT, ventriculography of ventriculoscopy. Coronal CT sections, however, clearly showed the pedicle and its location. Dilatation of the cisterns and sulci suggested that there was overproduction of cerebrospinal fluid.

A cisternogram characteristic of normal-pressure hydrocephalus was obtained from a patient on Diamox who was being evaluated for rapid mental deterioration. A repeat cisternogram after Diamox was discontinued was nearly normal. We hypothesize that the initial abnormality resulted from reduced cerebrospinal fluid production caused by carbonic anhydrase inhibition; this defect (reduced flow) led to a net reflux of tracer into the ventricles. The reflux is believed to be due to the reduction of bulk cerebrospinal fluid from the ventricles.

Cisternographic examinations have become a relatively common procedure in the evaluation of patients with suspected communicating hydrocephalus. Adverse reactions to radiopharmaceuticals are relatively uncommon, particularly with the commonly used agents such as 111-In-DTPA and 169-Yb-DTPA. Adverse reactions after lumbar intrathecal instillation of 111-In-DTPA in three patients are described.

Isotope studies are well-tolerated examinations in various congenital CNS anomalies and are valuable in demonstrating the presence of associated hydrocephalus, abnormal accumulation of isotope associated with abnormal CSF flow or a failure of absorption. In some cases these studies may be of differential diagnostic value and a helpful indicator in the choice of surgical treatment. These tests do not disturb the CSF equilibrium which is often observed following air or other contrast studies.
