Tracking Clostridioides difficile in Germany: Active prospective surveillance progress and molecular epidemiological trends.

BACKGROUND: Clostridioides difficile infections pose significant healthcare challenges in Germany, driven by various ribotypes (RTs) and antimicrobial resistance. This study updates the molecular epidemiology through Germany's active and passive surveillance until the end of 2023, including changes during the COVID-19 pandemic. METHODS: As active surveillance, the first 10 stool samples tested positive for toxigenic C. difficile (one per patient) were collected biannually from 33 German microbiological laboratories. Passive surveillance included samples from severe cases and outbreaks. Isolat

Fate of Clostridioides difficile spores in brain heart infusion broth and selected foods.

This study investigated the potential growth of Clostridioides difficile RT078 and RT126 in brain heart infusion (BHI) broth (without supplements) and in beef, chicken, cottage cheese and spinach incubated at 25°C and 37°C. Cells were enumerated on BHI agar with selective C. difficile supplements including cefoxitin, d-cycloserine and sodium taurocholate (BHIST). Growth was obtained for all ribotype-temperature combinations in BHI. Significantly higher counts, as compared to the initial concentration, were also obtained in chicken for both ribotypes at 37°C but not at 25°C. The counts achieved

Clostridioides difficile water-soluble zwitterionic lipoteichoic acid wsPS-III: structure, synthesis, and immunogenicity.

Clostridioides difficile polysaccharides (PSs) represent useful tools for the study of C. difficile pathogenesis. Water-soluble PS-I (pentaglycosyl-phosphate repeats) and PS-II (hexaglycosyl-phosphate repeats) and phenol-soluble lipoteichoic acid (LTA) PS-III have been previously identified. Most of the LTA PS-III repeats consist of two N-acetyl-glucopyranosamine (GlcNAc) units and glyceric acid (GroA), bridged by an exocyclic phosphodiester: →P-6)-α-D-GlcNAc-(1-3)-α-D-GlcNAc-[1-2-GroA]-(6→. Here, we describe a water-soluble PS from an equine C. difficile strain similar to PS-III, but with 6-s

Clinical impact of toxin detection in children with PCR-confirmed Clostridioides difficile infection.

UNLABELLED: The diagnosis of Clostridioides difficile infection (CDI) in children is challenging due to high rates of asymptomatic colonization and the limited ability of available assays to differentiate colonization from true infection. The relative utility of polymerase chain reaction (PCR) versus stool toxin A/B detection by enzyme immunoassay (EIA) remains uncertain in pediatric practice. This study aimed to evaluate whether stool toxin A/B detection adds incremental clinical value among children with gastrointestinal multiplex PCR positivity for C. difficile toxin genes (tcdA/tcdB) by co

How cryoEM has advanced our understanding of bacteriophages and bacteriocins targeting Clostridioides difficile.

We review the structural and functional characteristics of bacteriophages and bacteriocins (diffocins) that specifically target Clostridioides difficile, a significant healthcare concern due to its role in nosocomial infections. The advent of modern cryogenic electron microscopy (cryoEM) has revolutionized our understanding of these contractile injection systems, providing high-resolution insights into their mechanisms. We compare the structures of C. difficile phages and diffocins, highlighting their adaptations for penetrating the Gram-positive bacterial cell envelope - including the cell me

A TcdB-Targeting Antivirulence Compound Preserves Gut Homeostasis and Ameliorates Clostridioides difficile Infection.

Clostridioides difficile (C. difficile) is a leading cause of antibiotic-associated diarrhea and colitis, with hypervirulent or resistant strains and high recurrence, limiting current therapies. Antivirulence therapy presents a promising alternative by neutralizing toxins without damaging the gut microbiota or promoting resistance. Here, we identified myricetin (MYR) and its derivatives, dihydromyricetin, and myricitrin (MYT), which effectively suppressed TcdB-mediated cell rounding. Mechanistically, MYR inhibits TcdB internalization, self-cleavage, and Rac1 glycosylation, thereby reducing the

Parabacteroides distasonis alleviates Clostridioides difficile infection in mice while modulating secondary bile acids.

Clostridioides difficile infection (CDI) is a major cause of antibiotic-associated diarrhea, with frequent recurrences closely linked to antibiotic-induced dysbiosis of the gut microbiota and bile acid metabolism. Parabacteroides distasonis, a potential probiotic capable of converting primary to secondary bile acids, has shown therapeutic promise in several metabolic and inflammatory diseases. This study evaluated the preventive and therapeutic effects of P. distasonis against CDI and explored the underlying mechanisms. We characterized the probiotic properties of four P. distasonis strains an

Bifidobacterium breve synergizes with Akkermansia muciniphila and Bacteroides ovatus to antagonize Clostridioides difficile.

The development of ecologically based in vivo microecological formulations for treating Clostridioides difficile infection (CDI) is a current research focus. Here, we selected three microorganisms-Akkermansia muciniphila (AM), Bacteroides ovatus (BO), and Bifidobacterium breve (BB)-to formulate a mixed bacterial formulation (ABB). Subsequently, we evaluated the ecological interactions among these three microorganisms and investigated their therapeutic efficacy in a CDI murine model. Our investigation revealed the presence of a commensalism relationship among AM, BO, and BB. These microorganism

Functional redundancy between penicillin-binding proteins during asymmetric cell division in Clostridioides difficile.

Peptidoglycan synthesis is an essential driver of bacterial growth and division. The final steps of this crucial process involve the activity of the SEDS family glycosyltransferases that polymerize glycan strands and the class B penicillin-binding protein (bPBP) transpeptidases that cross-link them. While many bacteria encode multiple bPBPs to perform specialized roles during specific cellular processes, some bPBPs can play redundant roles that are important for resistance against certain cell wall stresses. Our understanding of these compensatory mechanisms, however, remains incomplete. Endos

Penicillin-binding proteins exhibit functional redundancy during asymmetric cell division in Clostridioides difficile.

Peptidoglycan synthesis is an essential driver of bacterial growth and division. The final steps of this crucial process involve the polymerization of glycan strands by shape, elongation, division, and sporulation (SEDS) family glycosyltransferases and the cross-linking of peptide cross-bridges by class B penicillin-binding proteins (bPBP). While many bacteria use distinct bPBPs to perform specialized roles during a given cellular process, some bPBPs can play redundant roles, particularly in the presence of certain cell wall stresses. Genetic insight into the relationship between inflammatory bowel disease and Clostridioides difficile infection.

Patients with inflammatory bowel disease (IBD) are at increased risk of Clostridioides difficile infection (CDI). Herein, we aimed to determine if genetic risk contributes to this observed association. We carried out a genome-wide association study (GWAS) analysis in the Michigan Genomics Initiative and the United Kingdom Biobank for CDI based on ICD codes and meta-analyzed these results with similar publicly accessible GWAS summary statistics from Finngen. Conditional and joint multi-SNP analyses were used to identify independent associations. Imputation of the human leukocyte antigen (HLA) r

Membrane Vesicles of Toxigenic Clostridioides difficile Affect the Metabolism of Liver HepG2 Cells.

Clostridioides difficile infection (CDI) appears to be associated with different liver diseases. C. difficile secretes membrane vesicles (MVs), which may be involved in the development of nonalcoholic fatty liver disease (NALFD) and drug-induced liver injury (DILI). In this study, we investigated the presence of C. difficile-derived MVs in patients with and without CDI, and analyzed their effects on pathways related to NAFLD and DILI in HepG2 cells. Fecal extracellular vesicles from CDI patients showed an increase of Clostridioides MVs. C. difficile-derived MVs that were internalized by HepG2
