Cluster analysis to define distinct clinical phenotypes among septic patients with bloodstream infections.

Prior attempts at identifying outcome determinants associated with bloodstream infection have employed a priori determined classification schemes based on readily identifiable microbiology, infection site, and patient characteristics. We hypothesized that even amongst this heterogeneous population, clinically relevant groupings can be described that transcend old a priori classifications. We applied cluster analysis to variables from three domains: patient characteristics, acuity of illness/clinical presentation and infection characteristics. We validated our clusters based on both content vali

Cluster analysis of preoperative echocardiographic findings and outcomes following left ventricular device implantation.

OBJECTIVE: To investigate whether preoperative echocardiography findings determine postoperative continuous-flow left ventricular assist device outcomes. METHODS: From January 2003 to June 2017, 490 patients received a durable, continuous-flow left ventricular assist device. Two-step clustering of parameters including heart rate and preoperative echocardiographic findings (ie, left ventricular [LV] ejection fraction, right ventricular [RV] function, aortic insufficiency, mitral regurgitation [MR], tricuspid regurgitation [TR]) was performed and identified 5 distinct clusters associated with L

Cluster analysis of autoimmune rheumatic diseases based on autoantibodies. New insights for polyautoimmunity.

Autoimmune diseases (ADs) are a chronic and clinically heterogeneous group of diseases characterized by share common immunopathogenic mechanisms and risk factors (i.e., the autoimmune tautology), which explain the fact that one AD may coexist with others (i.e., polyautoimmunity - PolyA). In the present exploratory study, a mixed-cluster analysis of the most common autoimmune rheumatic diseases (ARDs) was done. A total of 187 consecutive women with established systemic lupus erythematosus (n = 70), rheumatoid arthritis (n = 51), systemic sclerosis (n = 35) and Sjögren's syndrome (n = 31) were i

Cluster analysis reveals abnormal hippocampal neurometabolic profiles in young people with mood disorders.

While numerous studies have employed magnetic resonance spectroscopy (MRS) to determine in vivo neurometabolite levels associated with mood disorders the findings in both unipolar depression and bipolar disorder have been mixed. Data-driven studies may shed new light on this literature by identifying distinct subgroups of patients who may benefit from different treatment strategies. The objective of the present study was to utilize hierarchical cluster analysis in order to generate new hypotheses with respect to neurometabolic profiling of mood disorder. Participants were 165 young persons (18

[Sequence analysis of 16S rDNA and pmoCAB gene cluster of trichloroethylene-degrading methanotroph].

Methanotrophs could degrade methane and various chlorinated hydrocarbons. The analysis on methane monooxygenase gene cluster sequence would help to understand its catalytic mechanism and enhance the application in pollutants biodegradation. The methanotrophs was enriched and isolated with methane as the sole carbon source in the nitrate mineral salt medium. Then, five chlorinated hydrocarbons were selected as cometabolic substrates to study the biodegradation. The phylogenetic tree of 16S rDNA using MEGE5.05 software was constructed to identify the methanotroph strain. The pmoCAB gene cluster

Sputum neutrophil counts are associated with more severe asthma phenotypes using cluster analysis.

BACKGROUND: Clinical cluster analysis from the Severe Asthma Research Program (SARP) identified 5 asthma subphenotypes that represent the severity spectrum of early-onset allergic asthma, late-onset severe asthma, and severe asthma with chronic obstructive pulmonary disease characteristics. Analysis of induced sputum from a subset of SARP subjects showed 4 sputum inflammatory cellular patterns. Subjects with concurrent increases in eosinophil (≥2%) and neutrophil (≥40%) percentages had characteristics of very severe asthma. OBJECTIVE: To better understand interactions between inflammation and

Novel objective classification of reactive microglia following hypoglossal axotomy using hierarchical cluster analysis.

A total of 136 microglia were intracellularly labeled and their morphological features were evaluated by 3D morphometric measurement. According to hierarchical cluster analysis, microglia were objectively categorized into four groups termed types I-IV. The validity of this classification was confirmed by principal component analysis and linear discriminant analysis. Type I microglia were found in sham-operated mice and in mice sacrificed 28 days (D28) after axotomy. The appearance of type I cells was similar to so-called ramified microglia in a resting state. Type II microglia were mainly seen

Delineating psychological and biomedical profiles in a heterogeneous fibromyalgia population using cluster analysis.

The heterogeneity of patients meeting American College of Rheumatology (ACR) criteria for a diagnosis of fibromyalgia (FM) challenges our ability to understand the underlying pathogenesis and to optimize treatment of this enigmatic disorder. Our goal was to discern clinically relevant subgroups across multiple psychological and biomedical domains to better characterize the phenomenology of FM. Women meeting 1990 ACR criteria for FM (N = 107) underwent psychological (childhood trauma, mood, anxiety, and stress) and biomedical (neuroendocrine, immune, and metabolic) testing. Cluster analysis ide

Decompression illness medically reported by hyperbaric treatment facilities: cluster analysis of 1929 cases.

INTRODUCTION: The term decompression illness (DCI) describes maladies resulting from inadequate decompression, but there is little consensus concerning clinically useful DCI subclasses. Our aim was to explore an objective DCI classification using multivariate statistics to assess naturally associated clusters of DCI manifestations. We also evaluated their mapping onto other DCI classifications and investigated the association with therapeutic outcome. METHODS: We defined the optimal number of clusters using "two-step" cluster analysis and Bayesian information criterion with confirmation by hi

Cluster analysis of the metastatic patterns of human immunodeficiency virus-associated Kaposi's sarcoma.

Kaposi's sarcoma (KS) is a pleomorphic spindle cell lesion whose incidence has markedly increased among patients infected with the human immunodeficiency virus (HIV), especially among those whose primary risk factor is homo/bisexual transmission. The question as to whether KS is even a true neoplasm still remains largely unsettled due to the body of epidemiologic and histologic evidence suggesting an infectious etiology of the lesion. Accordingly, very few studies have been published regarding systemic distribution or patterns of metastatic progression of the lesion. In the past, such studies

Identifying optimal dose head CT acquisition techniques through cluster analysis from 904,209 scans.

OBJECTIVES: To identify clinically established and frequently utilized routine head computed tomography (CT) acquisition techniques in children and adults that achieve lower radiation. MATERIALS AND METHODS: The level of the analysis is at the CT scan (series) level. CT acquisition parameters were analyzed from a large, international CT dose registry. Scans were analyzed separately in adults and children, and by approach (helical or axial), resulting in four strata. K-means clustering, an unsupervised machine learning approach, assigned the scans into clusters based on the following acquisiti

A novel approach to quantifying mammal locomotor repertoires using scoring and cluster analysis.

Describing behaviour using qualitative categories is a staple of studies on tetrapod functional morphology, ecology and evolution. However, such categorization has several issues, primarily subjectivity and the loss of important behavioural repertoire information. Here, we propose a novel method for quantifying behaviour, using mammal locomotion as a case study to demonstrate its utility and efficacy. Species are scored from 0 to 4 on their proficiency in five locomotor modes (swimming, climbing, digging, running and aerial movement), then Ward's hierarchical clustering is used on the resultin

Bidirectional Mendelian Randomization Analysis of 338 Cerebrospinal-Fluid Metabolites and Cluster-Headache Risk.

BACKGROUND: Cluster headache (CH) is a rare but highly disabling primary headache disorder characterized by severe unilateral attacks and autonomic symptoms. The metabolic mechanisms underlying CH remain poorly understood. OBJECTIVE: To investigate the potential causal effects of cerebrospinal fluid (CSF) metabolite levels on CH risk, and to explore possible reverse causal effects of CH on CSF metabolites, using a bidirectional Mendelian randomization (MR) approach. METHODS: We performed a bidirectional two-sample Mendelian randomization (MR) analysis integrating genome-wide association stud
