Electrophysiologic Characteristics of Various Degrees of Selective Outer Hair Cell Loss in Rats.

HYPOTHESIS: The characteristics of auditory brainstem response (ABR), electrocochleogram (ECochG), and distortion product otoacoustic emissions (DPOAE) of different degrees of selective outer hair cells (OHCs) loss may be helpful for clinicians to evaluate the pathogeny, diagnosis, and rehabilitation of individuals' hearing loss. BACKGROUND: How many OHCs are necessary to maintain cochlear amplifier function remains unknown. The electrophysiologic characteristics may indicate different degrees of OHCs loss. METHODS: Electrophysiological characteristics were tested using 8-kHz pure-tone stimu

Effects of kynurenic acid as a glutamate receptor antagonist in the guinea pig.

Glutamate excitotoxicity is implicated in both the genesis of neural injury and noise-induced hearing loss (NIHL). Acoustic overstimulation may result in excessive synaptic glutamate, resulting in excessive binding to post-synaptic receptors and the initiation of a destructive cascade of cellular events, thus leading to neuronal degeneration and NIHL. The purpose of this study was to determine whether this apparent excitotoxicity can be attenuated by kynurenic acid (KYNA), a broad-spectrum glutamate receptor antagonist, and protect against noise-induced temporary threshold shifts (TTS). Guinea

Too much of a good thing: long-term treatment with salicylate strengthens outer hair cell function but impairs auditory neural activity.

Aspirin has been extensively used in clinical settings. Its side effects on auditory function, including hearing loss and tinnitus, are considered as temporary. A recent promising finding is that chronic treatment with high-dose salicylate (the active ingredient of aspirin) for several weeks enhances expression of the outer hair cell (OHC) motor protein (prestin), resulting in strengthened OHC electromotility and enhanced distortion product otoacoustic emissions (DPOAE). To follow up on these observations, we carried out two studies, one planned study of age-related hearing loss restoration an

Observations on mastoid versus ear canal recorded cochlear microphonic in newborns and adults.

The cochlear microphonic (CM) may play an important role in the diagnosis of auditory neuropathy (AN) in newborns. However, since the CM tends to mirror the waveform of the acoustic stimulus, conscientious recording methodology must be applied to separate true response from artifact. The difficulty in achieving this separation has limited the clinical usefulness of the CM. In an effort to call attention to the importance of recording protocol when measuring the CM, the present study was designed to optimize CM recordings in humans by investigating the following parameters: (1) secondary minus

The feasibility of evoked otoacoustic emissions as an in-patient hearing check after meningitis.

The desirability of finding children with hearing impairment after bacterial meningitis as soon as possible prompted us to examine the feasibility of using evoked otoacoustic emissions (EOAE) as an in-patient check of hearing status in children recovering from bacterial meningitis. Sixty-six episodes of bacterial meningitis were studied. Traces could be recorded for only 54.7% of ears. All children subsequently found to have sensorineural hearing impairment who were tested with EOAE, failed the screening test. The low coverage of the screening test and the predicted low specificity due to the

Permeability to potassium of the endolymph-perilymph barrier and its possible relation to hair cell function.

The endocochlear potential and potassium concentrations in endolymph and perilymph were simultaneously measured in the basal turn of the guinea pig cochlea with double-barreled K+ selective electrodes. The K+ conductance and K+ permeability coefficient of the endolymph-perilymph barrier were calculated from the rate of change of endolymph K+ concentration relative to the K+ electrochemical potential difference recorded during permanent anoxia. When anoxia was induced in guinea pigs treated with kanamycin, the rate of decline of the electrochemical potential difference for K+ between the endoly

Photochemically induced double lateral wall lesions in the guinea pig cochlea.

OBJECTIVE: Multiple patches of atrophy have been reported in the stria vascularis (SV) in elderly persons with presbycusis The aim of this study was to investigate the correlation between sensorineural hearing loss and this strial condition. MATERIALS AND METHODS: We established a new animal model comprising two small lesions in the SV in the second turn of the cochlea by means of photochemical reaction. Using this model, we investigated morphological and physiological changes in the cochlea at 3, 7 and 14 days after SV damage. RESULTS: Scanning electron microscopy studies revealed that the

Hearing Aid Benefit in Auditory Neuropathy Due to Friedreich Ataxia: A Case Report.

PURPOSE: Friedreich ataxia is one of the most common recessively inherited ataxias, with hearing loss due to auditory neuropathy being a recognized complication. However, there are limited data on the best interventions for hearing loss in these patients. We report the significant behavioral and subjective benefits of binaural amplification in a patient with auditory neuropathy due to Friedreich ataxia. METHOD: Auditory function was assessed in a 44-year-old woman with genetically confirmed Friedreich ataxia using pure-tone audiometry, speech audiometry, and immittance testing, along with aud

Postoperative Electrocochleography from Hybrid Cochlear Implant users: An Alternative Analysis Procedure.

OBJECTIVE: Shorter electrode arrays and soft surgical techniques allow for preservation of acoustic hearing in many cochlear implant (CI) users. Recently, we developed a method of using the Neural Response Telemetry (NRT) system built in Custom Sound EP clinical software to record acoustically evoked electrocochleography (ECoG) responses from an intracochlear electrode in Nucleus Hybrid CI users (Abbas et al., 2017). We recorded responses dominated by the hair cells (cochlear microphonic, CM/DIF) and the auditory nerve (auditory nerve neurophonic, ANN/SUM). Unfortunately, the recording procedu

Activation of medial olivocochlear efferent system in humans: influence of stimulus bandwidth.

The activity of the medial olivocochlear bundle (MOCB) can be studied in humans through variations in the level of evoked otoacoustic emissions (EOAEs) elicited by contralateral acoustic stimuli (CAS). The present study sought to investigate how the activity of the MOC system at a given frequency, as measured through the contralateral suppression of tone-pip EOAEs, depends on the bandwidth of the contralateral stimulus. EOAEs were recorded in 155 normal-hearing subjects, successively with and without contralateral stimuli whose bandwidth, center frequency and level were systematically varied.

Longitudinal endolymph movements and endocochlear potential changes induced by stimulation at infrasonic frequencies.

The inner ear is continually exposed to pressure fluctuations in the infrasonic frequency range (< 20 Hz) from external and internal body sources. The cochlea is generally regarded to be insensitive to such stimulation. The effects of stimulation at infrasonic frequencies (0.1 to 10 Hz) on endocochlear potential (EP) and endolymph movements in the guinea pig cochlea were studied. Stimuli were applied directly to the perilymph of scala tympani or scala vestibuli of the cochlea via a fluid-filled pipette. Stimuli, especially those near 1 Hz, elicited large EP changes which under some conditions

Hereditary auditory, vestibular, motor, and sensory neuropathy in a Slovenian Roma (Gypsy) kindred.

Members of a Roma (Gypsy) family with hereditary motor and sensory peripheral neuropathy (HMSN) and concomitant auditory and vestibular cranial neuropathies were identified in Kocevje, Slovenia. The illness begins in childhood with a severe and progressive motor disability and the deafness is delayed until the second decade. There are no symptoms of vestibular dysfunction. The family structure is consistent with an autosomal recessive pattern of inheritance and the genetic locus for the disorder is linked to the same region of chromosome 8q24 as other Roma families with HMSN and deafness from

EVestG: a measure for Meniere's Disease.

Meniere's Disease is commonly diagnosed using Electrocochleography (ECOG). EVestG is a variant of ECOG utilizing one or more patient tilts as stimuli in place of the ECOG's repeated tonal clicks. The dynamic measures averaged 'background-onAA' (onAA=acceleration phase of tilt) and background-onBB (onBB=deceleration phase of tilt) of excitatory (ipsilateral tilt) vestibular responses are compared for a small group of age matched Controls (n=18) and Meniere's Disease patients (n=11). Preliminary data provides for an apparent clearer demarcation between Controls and Meniere's patients. Meniere's

The acoustic middle ear muscle reflex in albino rats.

The acoustic middle ear muscle reflex was studied in albino rats anesthetized with chloralose. The best frequency of the reflex and the threshold at this frequency were on average about 3 kHz and 57 dB SPL, respectively. The threshold increased as frequency increased above, and decreased below, the best frequency at a rate of about 20 dB/octave. Above about 12 kHz, the muscular response showed instability and habituation. Thresholds were similar between stapedius and tensor tympani reflexes and between ipsilateral and contralateral reflexes. The middle ear transmission loss due to the reflex w

Cochlear morphology in relation to loss of behavioural, electrophysiological, and middle ear reflex thresholds after exposure to noise.

Loss of auditory function was correlated with different pathological findings in the inner ear of the rabbit after 15 or 30 min exposure to high level broad band (2-7 kHz) noise. The ears from animals with post-exposure times ranging from 1 to 23 months were analysed by scanning electron microscopy (SEM). Complete cochleograms with quantification of loss of hair cells and damage to stereocilia were produced. Loss of function was defined by determining behavioural threshold, threshold of auditory brainstem response (ABR) to narrow-band stimuli and threshold of the middle ear muscle reflex (MER)

The frequency selectivity of the normal and pathological human cochlea.

Human AP tuning curves (tone on tone simultaneous and forward masking curves) were measured during transtympanic electrocochleography. For subjects with near normal thresholds, average Q10 dB values (simultaneous masking) were 2.3 at 2 kHz, 3.6 at 4 kHz, and 4.7 at 8 kHz. Patients with threshold elevations of more than 40 dB, resulting from sensorineural hearing loss of cochlear origin, had tuning curves less sharply tuned by a factor of 2-3, with Q10 dB values of 1-2 at 2 kHz and at 4 kHz, and 1-2.3 at 8 kHz. AP tuning curves and single fibre tuning curves (frequency threshold curves) were me

Increased otoacoustic-emission amplitude secondary to cochlear lesions.

OBJECTIVE: The measurements of transient evoked otoacoustic emissions and distortion-product otoacoustic emissions are being used increasingly, both as an objective hearing test clinically, and as a research tool to investigate the micromechanical aspects of cochlear function. We hypothesized that localized damage in the apical or middle cochlear turns may have an influence on the micromechanics and the function of adjacent, apparently normal cochlea. For that purpose, we used an animal model of localized apical and middle-turn cochlear lesions. METHOD: Extent of damage was assessed by scanni

Clinical and audiological features in auditory neuropathy.

OBJECTIVE: To medically and audiologically characterize a population of children diagnosed as having auditory neuropathy (AN). STUDY DESIGN: Retrospective medical chart review. SETTING/SUBJECTS: We identified 22 patients from a pediatric otology clinic in a tertiary care pediatric hospital setting. RESULTS: A genetic factor in AN is suggested by our identification of 3 families with 2 affected children and 2 other children with family histories that were positive for hearing loss. Clinical features common among our population included a history of hyperbilirubinemia (n = 11 [50%]), prematur

Direct effects of intraperilymphatic reactive oxygen species generation on cochlear function.

Reactive oxygen species (ROS) generation may play a role in ototoxicity, however, the specific effects of ROS generation upon cochlear function are unstudied. Therefore, guinea pig cochleas were instilled with artificial perilymph (AP), H2O2, or confirmed generating systems for the superoxide anion (O2-) or the hydroxyl radical (OH.), or with an ROS system plus its respective scavenger -catalase (CAT), superoxide dismutase (SOD) or deferoxamine (DEF). O2- generating system instillation led to significantly greater mean high frequency compound action potential (CAP) threshold shifts at 10 and 1
