Efficacy of a behavioral self-help treatment with or without therapist guidance for co-morbid and primary insomnia--a randomized controlled trial.

BACKGROUND: Cognitive behavioral therapy is treatment of choice for insomnia, but availability is scarce. Self-help can increase availability at low cost, but evidence for its efficacy is limited, especially for the typical insomnia patient with co-morbid problems. We hypothesized that a cognitive behaviorally based self-help book is effective to treat insomnia in individuals, also with co-morbid problems, and that the effect is enhanced by adding brief therapist telephone support. METHODS: Volunteer sample; 133 media-recruited adults with insomnia. History of sleep difficulties (mean [SD]) 1

The role of the MMPI-2-Restructured Form (MMPI-2-RF) in predicting and better understanding engagement in posttraumatic stress disorder treatment.

The ability of Minnesota Multiphasic Personality Inventory-2-Restructured Form (MMPI-2-RF; Ben-Porath & Tellegen, 2008/2011) scores to predict treatment engagement in veterans referred for empirically supported treatments for symptoms of Posttraumatic Stress Disorder (PTSD) was examined. Early withdrawal from treatment and process factors, such as adherence and therapy session attendance, were examined in 100 veterans seeking treatment for trauma-related symptoms who were referred to a Veterans Health Administration medical center's tertiary care clinic for Prolonged Exposure (PE) or Cognitive

Accept, distract, or reframe? An exploratory experimental comparison of strategies for coping with intrusive body image thoughts in anorexia nervosa and body dysmorphic disorder.

Negative body image is the hallmark of anorexia nervosa (AN) and body dysmorphic disorder (BDD). One aspect of body image, appearance-related thoughts, have shown to be a major contributor to relapse, thus further investigation of successful treatment strategies targeting these maladaptive thoughts are warranted. The present study tested an acceptance/mindfulness (AC), a cognitive restructuring (CR), and a distraction strategy with regard to their short-term effectiveness of reducing the frequency of thought occurrence and associated outcomes in participants with AN (n=20), BDD (n=21), and hea

Variations of lymphocyte subsets associated with stress in depressive populations.

Major depression and dysthymia have been associated with increased perception of day-to-day stressors, greater reliance on emotion-focused coping efforts, and reduced perception of uplifting events. Moreover, it has been observed that levels of circulating natural killer (NK) cells were elevated in depressed patients. Given that mild stressors may increase circulating NK cells, it is conceivable that the elevated NK cells in depression may be secondary to the increased stress perception associated with the illness. In the present investigation a laboratory stressor, comprising a mathematical c

Cognitive-Behavioral Sleep and Nutritional Intervention Enhances Recovery and Survival in Rectal Cancer Patients Undergoing Abdominoperineal Resection: A Randomized Controlled Trial.

OBJECTIVE: Patients with rectal cancer undergoing abdominoperineal resection (APR) frequently experience perioperative sleep disturbance and nutritional decline, both linked to impaired recovery, heightened inflammation, and poorer survival. Evidence for combined behavioral sleep and nutritional interventions in this high-risk population is limited. METHODS: In this single-center randomized controlled trial, 186 stage I-III rectal cancer patients scheduled for APR were assigned (1:1) to receive standard enhanced recovery after surgery (ERAS) or ERAS plus a structured cognitive-behavioral slee

Optimising internet-based cognitive-behavioural therapy programme to prevent perinatal depression: a unified protocol for two harmonised randomised controlled trials.

INTRODUCTION: Perinatal depression poses substantial risks to both mothers and their offspring. Given its chronic and recurrent nature, developing effective prevention strategies is crucial. Internet-based cognitive-behavioural therapy (iCBT) has shown promise. However, the efficacy of specific CBT skills and the influence of individual differences remain unclear. METHODS AND ANALYSIS: This protocol describes two harmonised multicentre, open-label, six-arm randomised controlled trials. Across both trials, a total of 2400 pregnant women between 10 and 20 weeks of gestation will be enrolled. Af

Psychological processes and abilities for ceasing sex as self-injury- a qualitative study.

BACKGROUND: The term sex as self-injury (SASI) refers to sexual behaviors that are used as a means of self-injury, with motives such as emotional regulation comparable to other self-injurious behaviors, including burning or cutting the skin. The aim of this study was to explore which psychological processes and abilities that made it possible to cease SASI, to contribute to the knowledge that underpins psychological interventions and treatments. METHOD: The study was based on an open-ended questionnaire published on the websites of Swedish NGOs offering help and support to women and youths. I

Expanding the Scope of Open Weighing to Target Anxiety About the Consequences of Weight Gain in Cognitive-Behavioral Therapy for Eating Disorders.

OBJECTIVE: Anxiety about weight gain is a central feature of eating disorders (EDs) and plays a key role in maintaining ED symptomatology. Cognitive-behavioral therapy (CBT) experts have observed that patients with EDs often believe regular eating will lead to immediate, dramatic, and/or uncontrollable weight gain, prompting interventions like "collaborative weighing" and "open weighing" that help patients develop a realistic understanding of eating-weight relationships. However, patients also report anxiety about the consequences of weight gain (e.g., social rejection, self-hatred, permanent

Association between depression and hostile attribution bias in hostile and non-hostile individuals: An ERP study.

BACKGROUND: Hostility and aggression have been found to be highly prevalent among depressed patients and are associated with higher comorbidity and illness severity levels. Although negative interpretation biases are a fundamental element of cognitive models of depression, few studies have examined the specific biases in information processing, mainly the hostile attribution bias, found in hostile individuals who present depressive symptoms. METHOD: Using pre-collected data from a sample of 72 (male=41,6%, female=58,3%) undergraduate and community-based hostile (n = 26) and non-hostile (n = 4

Dismantling cognitive-behaviour therapy for panic disorder: a systematic review and component network meta-analysis.

Cognitive-behaviour therapy (CBT) for panic disorder may consist of different combinations of several therapeutic components such as relaxation, breathing retraining, cognitive restructuring, interoceptive exposure and/or in vivo exposure. It is therefore important both theoretically and clinically to examine whether specific components of CBT or their combinations are superior to others in the treatment of panic disorder. Component network meta-analysis (NMA) is an extension of standard NMA that can be used to disentangle the treatment effects of different components included in composite int

Group Therapy for Patients with Adjustment Disorder in Primary Care.

A high percentage of patients have a diagnosis of adjustment disorder (AD) when they arrive at primary care (PC) appointments. However, most of them do not receive adequate psychological treatment. The present study's aim is to determine the efficacy of a group psychological treatment program in patients with AD. The sample consisted of patients with AD from two PC units in Valencia, from which two groups were randomly generated: A treatment group (n = 31) and a waiting-list group (n = 20), homogeneous in terms of socio-demographic and psychometric variables prior to treatment. Treatment consi

Child Maltreatment and Neural Systems Underlying Emotion Regulation.

OBJECTIVE: The strong associations between child maltreatment and psychopathology have generated interest in identifying neurodevelopmental processes that are disrupted following maltreatment. Previous research has focused largely on neural response to negative facial emotion. We determined whether child maltreatment was associated with neural responses during passive viewing of negative and positive emotional stimuli and effortful attempts to regulate emotional responses. METHOD: A total of 42 adolescents aged 13 to 19 years, half with exposure to physical and/or sexual abuse, participated.

Iowa Gambling Task scores predict future drug use in bipolar disorder outpatients with stimulant dependence.

Poor decision-making is associated with poor recovery in persons with bipolar disorder and drug relapse in persons with stimulant dependence. Cognitive predictors of stimulant use in those with comorbid bipolar and stimulant dependence are surprisingly absent. Our goal was to determine if a single baseline assessment of decision-making (Iowa Gambling Task, IGT) would predict future drug use in bipolar disorder outpatients with comorbid stimulant dependence. Ninety-four men and women of multiple race/ethnic origins consented to participate in a 20-week study. Data analyses were performed on 63

Efficacy of body exposure as treatment component for patients with eating disorders.

BACKGROUND AND OBJECTIVES: Body exposure and cognitive restructuring are well known interventions in cognitive-behavioral therapies for patients with eating disorders. Though promising, little is known about the impact of body exposure on body dissatisfaction, overvaluation of weight and shape and other aspects of eating disorder psychopathology. Thus, the aim of the two studies presented here is to evaluate the efficacy of mirror exposure and video exposure on body dissatisfaction, weight and shape concerns and other aspects of eating disorder psychopathology. Long-term impact of role stress and cognitive rumination upon morning and evening saliva cortisol secretion.

The long-term impact of role stress (conflict and ambiguity), cognitive rumination and their interaction were analysed upon morning and evening saliva cortisol secretion. The sample consisted of 52 male and 24 female British white-collars who had participated in a survey study on psychosocial working conditions 3.5 years earlier. Saliva cortisol secretion was measured over seven consecutive days with two measures: in the morning on awakening and at 22.00 hours. Stepwise linear multiple regression analyses was used for the statistical analyses. Role ambiguity at baseline and the interaction bet

Cognitive, Regulatory, and Interpersonal Mechanisms of HIV Stigma on the Mental and Social Health of Men Who Have Sex With Men Living With HIV.

Public stigma surrounding HIV is related to heightened emotional distress, poor psychological functioning, and reduced subjective well-being in people living with HIV. For men who have sex with men (MSM) living with HIV, they may also face stigmatizing attitudes within the gay community, which create an additional burden to their health. Grounded in the psychological mediation framework, the present study examined the underlying psychological processes through which HIV stigma from the public and within the gay community influences the mental and social health of MSM living with HIV. Findings

Psycho-social factors and coping strategies as predictors of chronic evolution and quality of life in patients with low back pain: a prospective study.

UNLABELLED: Low back pain (LBP) is a major problem of public health. Chronic pain is the most difficult to treat and the most expensive. The way patients cope with their pain may influence its outcome. AIM: To identify coping strategies of LBP patients, and their influence on LBP evolution. METHODS: Ninety nine patients were assessed just after an acute LBP episode and one year later. Assessment tools included medical and social reports, scales of anxiety, depression, quality of life, locus of control (LOC), social support and coping strategies. RESULTS: One year after the initial episode,

Cognitive-behavioral therapy induces sensorimotor and specific electrocortical changes in chronic tic and Tourette's disorder.

BACKGROUND: Tic disorders, such as the Gilles de la Tourette syndrome and persistent tic disorder, are neurodevelopmental movement disorders involving impaired motor control. Hence, patients show repetitive unwanted muscular contractions in one or more parts of the body. A cognitive-behavioral therapy, with a particular emphasis on the psychophysiology of tic expression and sensorimotor activation, can reduce the frequency and intensity of tics. However, its impact on motor activation and inhibition is not fully understood. METHODS: To study the effects of a cognitive-behavioral therapy on el

Adverse Childhood Experiences and Mental Health in Women: Pathways of Influence in a Clinical Sample.

BACKGROUND: While the negative impacts of Adverse Childhood Experiences (ACEs) on mental health have already been established in previous research, the pathways through which these events affect psychological well-being are still unclear. This cross-sectional study analyzed the relationship between ACEs and psychological distress. It also examined the role of coping strategies, emotion regulation, and adult attachment in this association. METHOD: The sample consisted of 207 women receiving psychotherapeutic support from Public Mental Health Services in Spain. Just under half (46.4%) being tre

The Direct and Indirect Influences of Adverse Childhood Experiences on Physical Health: A Cross-Sectional Study.

A growing and significant body of research has documented the close relationship between adverse childhood experiences (ACEs) and health outcomes in adults. Less is known about the complex pathways through which ACEs exert their influence. This article examines the direct relationship between the quality of perceived physical health and childhood adversities. The association between the adversities and the physical health with other psychological and social variables is also analyzed. Data were collected from a sample of 170 subjects, using tools to assess adverse childhood experiences, physic
