Longitudinal assessment of mental disorders, smoking, and hazardous drinking among a population-based cohort of US service members.

OBJECTIVES: Combat exposure is known to increase the risk for mental disorders; however, less is known about the temporal relationship between mental disorders and alcohol misuse or smoking. To better understand these interrelationships, this study investigated mental disorders in association with hazardous drinking and cigarette smoking. METHODS: Using data from a large population-based military cohort, standardized instruments were used to screen for posttraumatic stress disorder, depression, panic, and other anxiety syndromes. Self-reported use of cigarettes and hazardous drinking was also

[MULTI-DIMENSIONAL APPROACH FOR ASSESSING CREDIBILITY OF INDIVIDUALS CLAIMING COMBAT PTSD COMPENSATIONS: A PRELIMINARY STUDY].

Diagnosis of combat post-traumatic stress disorder (PTSD) is largely based on the subjective reports of the claimant and is therefore prone to over-diagnosis of symptoms. Over-diagnosis of combat PTSD is associated with harm. Therefore, it is important to assess the claimant's credibility in assessing PTSD as accurately as possible.

Morale as a moderator of the combat exposure-PTSD symptom relationship.

We examined morale as a moderator of the relationship between combat exposure and posttraumatic stress disorder (PTSD) symptoms in a longitudinal study of U.S. soldiers who had participated in a deployment to Iraq. Soldiers (N = 636) completed assessments at 4 (Time 1) and 10 (Time 2) months following their combat deployment. Combat exposure (both breadth and perceived stressfulness), morale, and PTSD symptoms were assessed at Time 1, and PTSD symptoms were assessed again at Time 2. Results of multivariate multiple regressions revealed that morale at Time 1 interacted with both the breadth and

Measurement of dissociative states with the Clinician-Administered Dissociative States Scale (CADSS).

The purpose of this study was to develop an instrument for the measurement of present-state dissociative symptoms, the Clinician Administered Dissociative States Scale (CADSS). Reported here are interrater reliability and internal consistency of the CADSS, validity as assessed by comparisons with other instruments for the assessment of dissociation, and sensitivity of the CADSS to discriminate patients with dissociative disorders from patients with other psychiatric disorders and healthy subjects. Initial analyses indicated good interrater reliability and construct validity for the CADSS. Scor

The transformation of affect in posttraumatic nightmares.

The author questions the classical psychoanalytic assumptions that anxiety in posttraumatic nightmares arises from the same conflict represented in the manifest content of the nightmare, that such nightmares have no latent content, and that the day residue is insignificant. Such assumptions obscure the function of these nightmares, in which the dreamwork transforms shame into fear. Case material from two patients highlights the instigating role of posttraumatic dissociative states and shame in the day residue of posttraumatic nightmare sufferers. The author also emphasizes that conflict in the

Implosive therapy in the treatment of combat related fears in a world war II veteran.

A 55-year old veteran with combat related fears of 36 years' duration was successfully treated in three sessions of implosive therapy, each lasting approximately 1 hr. This improvement followed 15 hospitalizations over the preceding 30 month period, and previous unsuccessful treatment efforts including prescription of antabuse, an inpatient alcohol treatment program, and an agoraphobia treatment program conducted on an outpatient basis. Treatment gains were maintained over a 2 year follow-up period.

Clinical detection, diagnosis, and differential diagnosis of post-traumatic stress disorder.

This article has reviewed the detection, diagnosis, and differential diagnosis of PTSD. Recognition of particular effects of traumatic stress on patients' communications and adjustments in interviewing technique facilitate the expression and detection of PTSD when it is present. The phenomenologic boundaries between PTSD and a number of clinical syndromes and disorders are explored in detail. Delineation of PTSD from other disorders even when they co-occur with PTSD is almost always possible with effective rapport and knowledge from clinical research. An understanding of the role of traumatic

The effect of enemy combat tactics on PTSD prevalence rates: A comparison of Operation Iraqi Freedom deployment phases in a sample of male and female veterans.

OBJECTIVE: Research suggests that the nature of combat may affect later development of posttraumatic stress disorder (PTSD) in combat veterans. Studies comparing rates of PTSD across different conflicts indicate that the use of asymmetric or guerilla-style tactics by enemy fighters may result in higher rates of PTSD among U.S. military personnel than the use of symmetric tactics, which mirror tactics used by U.S. forces. Investigations of the association between enemy combat tactics and PTSD across conflicts were limited because of cohort effects and a focus on male veterans. The current study

Shell shock in Ireland: The Richmond War Hospital, Dublin (1916-19).

The history of mental disorders occasioned by World War I is a complex and important history, indelibly linked with social, political and cultural circumstances, and the history of the war itself. The Richmond War Hospital was a 32-bed establishment on the grounds of the large Richmond District Asylum in Dublin which, from 16 June 1916 until 23 December 1919, treated 362 soldiers with shell shock and other mental disorders, of whom more than half were considered to have recovered. Despite the limitations of the Richmond War Hospital, it was a generally forward-looking institution that pointed

The fight for 'traumatic neurosis', 1889-1916: Hermann Oppenheim and his opponents in Berlin.

The concept of traumatic neurosis conceived by Hermann Oppenheim (1858-1919) located post-traumatic nervous symptoms between hysteria and neurasthenia, considering them a consequence of physical reactions to fright and a cause of molecular tissue changes. As early as 1890, his concept was criticized at an international congress in Berlin. In February 1916, there was a significant debate of the issue in Berlin, and eventually Oppenheim's concept was completely defeated at the war meeting of German neuropsychiatrists in September 1916 in Munich. In the Berlin debate, a range of views on war neur

Diseases among men 20 years after exposure to severe stress: implications for clinical research and medical care.

OBJECTIVE: Epidemiologic studies have linked exposure to severe environmental stress, such as natural disasters and combat operations, to the onset of specific psychiatric disorders. Some research also suggests that these exposures may be associated with the onset of chronic diseases as well. However, these chronic disease outcome studies often have been obscured by bias and confounding. METHOD: The medical histories of 1399 male Vietnam veterans approximately 20 years after combat exposure (mean years = 17) were analyzed by lifetime posttraumatic stress disorder (PTSD) status (lifetime PTSD

Characteristics of psychogenic nonepileptic events among veterans with posttraumatic stress disorder: an association of semiology with the nature of trauma.

Various psychiatric disorders can contribute to psychogenic nonepileptic events (PNEEs), including posttraumatic stress disorder (PTSD). We hypothesize that among patients with PNEEs and PTSD, the nature of the traumatic experience leading to PTSD has an association with the clinical manifestations of PNEEs. We found that the presence of PTSD, in general, did not demonstrate significant association with any particular PNEE semiology in a veterans population. However, 11 of 12 (91.7%) subjects with PNEEs and specifically combat-related PTSD showed significant predilection for hypomotor or nonmo

Spinal Injuries and Spine Care in the U.S. Military Health System (2001-Present).

STUDY DESIGN: Literature review. OBJECTIVE: The goal of this study was to provide a comprehensive outline of spinal injuries that may transpire over the course of military service from traumatic to repetitive stress injuries and chronic sequelae. We considered studies that assessed spinal injuries in combat and non-combat settings as reported in the literature over the last 15 to 20 years. BACKGROUND: Military service places servicemembers under substantial physical demands, while also exposing them to dangerous, unpredictable environments. As a result, servicemembers are at an increased ris
