Incorporating novel solutions to lower-limb problems into compression formularies.

This study investigates the effect of a new short-stretch two-layer compression system in eight patients with common chronic lower-limb conditions in three locations in the UK. Chronic leg ulcers are the most common type of lower-limb ulceration with 70% caused by chronic venous hypertension. An appropriate level of compression is proven to heal chronic venous leg ulcers. The study was only a small sample of patients; however, the underlying conditions included chronic venous eczema, diabetes, sarcoma, cellulitis and mixed-aetiology ulcers. During the study, the UK experienced some of the hott

Evaluation of sequential intermittent pneumatic compression for filarial lymphoedema.

BACKGROUND: Lymphoedema is a major cause of morbidity in patients with lymphatic filariasis. There is no effective medical treatment and the results of surgery are uncertain. There are very few published studies assessing the volumetric response to the use of sequential intermittent pneumatic compression (SIPC) in patients. METHODS: A 12-celled instrument capable of providing sequential compression from the distal to proximal direction was used in 28 patients with unilateral grades II (n=17) and III (n=11) filarial lymphoedema in a planned 4-week session. The patients were followed up for 6 m

A French Practice Survey of Diagnostic Assessment and Compression Therapy for Patients with Venous Leg Ulcers and Peripheral Artery Disease.

OBJECTIVE: The primary objective was to observe how vascular physicians diagnose peripheral artery disease (PAD) in patients with venous leg ulcers (VLUs) and the type of compression used in patients presenting VLU-PAD without critical limb threatening ischaemia (CLTI). Secondary objectives were to assess the safety and efficacy of compression. METHODS: This was a prospective, multicentre survey of physician practices completed by a prospective patient cohort study with a 6 month follow up. Data were collected on VLU-PAD diagnosis and management by physicians, patient and ulcer characteristic

One contract, one Wales: standardising lymphoedema garment provision through a national procurement model.

Before 2014, lymphoedema compression garment provision in Wales was fragmented, with wide variation in access, prescribing routes, product quality and cost, resulting in inequity for patients and inefficiency. Given the chronic nature of lymphoedema and the central role of compression therapy, a more consistent national approach was required. This commentary describes the rationale, development and impact of implementing a single national procurement contract and standardised garment formulary for lymphoedema care in Wales. Between 2014 and 2023, a clinically led, collaborative procurement pro

The science of compression textiles and garments for upper-body lymphoedema.

Compression therapy is a cornerstone in the management of upper-body lymphoedema. Compression helps reduce oedema, restore function and improve tissue integrity, contributing to enhanced quality of life and more efficient use of healthcare resources. To achieve this, compression garments must be appropriately designed, selected and applied to meet diverse patient needs. Therapeutic effect is determined by the garment's textile properties, including resting pressure, stiffness and gradient, as well as containment, fatigue and moisture wicking. This article synthesises the evidence for how these

Using the STRIDE algorithm for compression selection in upper-body lymphoedema.

This second iteration of STRIDE is an evidence-based algorithm for compression selection, extended to cover the upper limb, trunk and breast. The STRIDE algorithm is patient-centred and complexity-informed, encompassing the site, shape and size of oedematous swelling; impact of tissue texture on textile types; 24-hour refill patterns; patient-specific issues; pressure dosage among other textile characteristics; and oedema etiology and staging. This article details the elements of the STRIDE algorithm and presents practical tools for its application.

Case studies of the STRIDE algorithm for compression selection in upper-body lymphoedema.

This article presents six case studies demonstrating use of the STRIDE algorithm for selecting compression garments for upper-body lymphoedema. STRIDE offers a structured, evidence-informed and individualised approach to assessment and selection. It incorporates patients' multifactorial and evolving needs, as well as textile properties beyond pressure dosage alone to enhance long-term management and overall quality of life.

Creative compression treatment in challenging situations.

There are globally accepted insights and country-specific standards, preferences, and traditions regarding compression therapy. Although there is a wide choice of bandaging and compression materials, the use of these materials must be planned to meet individual patient's temporary needs using the most appropriate technique and optimal timing. Even though indications and contraindications exist for elastic and nonelastic materials and techniques, professionals must use the technical advantages of the several systems for the benefit of the patient. Whatever method is chosen, the effectiveness of

What is the recommended compression pressure for different clinical indications?

IntroductionCompression pressure is the key factor determining effectiveness in compression therapy for venous and lymphatic disorders. Despite its clinical importance, few studies report the actual applied pressure, and national standards for compression classes differ. This review aims to identify the optimal compression pressure at different stages of venous disease.MethodsA literature search was conducted in PubMed, Scopus, and Web of Science (January 1980-October 2025) using MeSH terms related to compression therapy and chronic venous disease, edema, thrombosis, post-thrombotic syndrome,

Case study: night compression use in a patient with Milroy's disease.

AIM: Lymphoedema is associated with dysfunctional lymphatics, tissue fibrosis and inflammatory changes in the skin and local tissue. Ensuring compression supports tissue health is crucial to managing lymphoedema. Providing patients with safe compression which enhances their tissue health is paramount when supporting their 24-hour self-management regimens. This case study explores the use of a new compression garment in two sitting positions in an adult with primary lymphoedema. METHOD: An 18-year-old female (body mass index 25.2 kg/m2) with Milroy's disease was recruited. She attended two sep

Tacky but refined: a "slick" technique for dressings that hold better.

For decades benzoin has been used to enhance the "stickiness" of the skin for improved adherence complementing the protective and functional aspects of well-constructed surgical bandages/dressings. The fully adherent and longer-lasting pressure dressing is both aesthetically pleasing to the patient (and surgeon) and, at the same time, functions to decrease seroma or hematoma formation, enhances antisepsis by blocking transient bacterial contamination, and inhibits traumatic sabotage of surgical wounds due to patient overactivity or inadvertent collision with a door jam, car door, table leg, or

The multiple benefits of accurate assessment. Effective management of leg ulcers.

1. To be effective, the treatment of leg ulcers should be directed at the underlying cause. 2. If the underlying cause is chronic venous insufficiency, the most effective treatment is compression therapy. 3. There are a number of effective bandaging systems in use; the choice of any particular regimen is of less importance than the skill of the practitioner in applying it. 4. The prevention of recurrence should form a significant part of the care plan.

Introducing the British Lymphology Society position paper on ankle brachial pressure index.

Prompt application of appropriate compression therapy is essential for effective treatment of lymphoedema. However, it is accepted that prior to the application of compression to the lower limbs, either with bandaging or compression garments, patients should demonstrate a satisfactory vascular status, as assessed via axillary brachial pressure index (ABPI). Unfortunately, the presence of peripheral oedema may render a reading impossible or grossly inaccurate. Relying solely on ABPI assessment is potentially harmful to patients, who may be denied appropriate treatment or experience complication

Management of venous ulcers.

Chronic venous insufficiency (CVI) results from venous hypertension secondary to superficial or deep venous valvular reflux, as well as venous obstruction. The most severe clinical manifestation of CVI is venous leg ulceration that can result in significant morbidity, including venous gangrene and amputation, albeit rare. Treatment modalities are aimed at reducing venous hypertension. Diuretic therapy, although widely used, only provides short-term improvement of the edema but provides no long-term benefit. Compression therapy is the cornerstone in the management of CVI. Compression can be ach
