Clinical aspects of impulsive-compulsive behaviors under dopaminergic treatment-A scoping review.

ObjectiveDopaminergic medications are commonly prescribed for Parkinson's disease (PD), restless legs syndrome, pituitary and psychotic disorders. Accumulating evidence indicates that dopaminergic treatment triggers impulsive-compulsive behaviors (ICBs), yet systematic synthesis of clinical evidence remains limited.MethodThis scoping review summarizes original research directly clinically relevant to ICBs under dopaminergic treatment, selected through systematic searches of MEDLINE PubMed, Web of Science, Embase and PsycInfo.ResultsOf 349 included studies, most focused on PD, with limited evid

Third-Generation Cognitive and Behavioral Therapies for Obsessive-Compulsive Disorder: A Narrative Review.

Obsessive-compulsive disorder (OCD) is a chronic and disabling condition associated with significant distress and functional impairment. First-line treatments rely primarily on pharmacotherapy and cognitive-behavioral therapy (CBT), particularly exposure and response prevention (ERP). Although effective, a substantial proportion of patients continue to show clinically significant symptoms after treatment. In recent decades, alternative psychological therapies have been investigated to complement CBT and address its limitations in OCD management. This narrative review, based on the relevant lit

Enhanced conditioning and disrupted extinction processes in men struggling with compulsive sexual behaviors.

BACKGROUND AND AIMS: Despite a previously reported connection between compulsive sexual behaviors (CSB), such as problematic pornography use, and heightened cue-reactivity, empirical evidence of the alteration of processes responsible for increased salience attribution to erotic cues remains sparse. Drawing on similarities with addiction models, this study explores the neuronal mechanisms of CSB through the use of appetitive conditioning and extinction with erotic and monetary rewards. METHODS: Thirty-two heterosexual males struggling with CSB (age: 28.9 ± 7.1), and 31 healthy matched partici

Metacognition in Children and Adolescents With Obsessive-Compulsive Disorder Treated With Cognitive Behavioral Therapy.

According to the metacognitive theory, maladaptive metacognition is associated with the development and maintenance of emotional disorders. This study is the first to explore maladaptive metacognition in a sample of children and adolescents (7-17 years) with obsessive-compulsive disorder (OCD) in the context of cognitive behavioral therapy (CBT). A total of 114 children and adolescents were included in the study. Of these 56 were OCD patients who were assessed on metacognition (Metacognitions Questionnaire-Child Version) and OCD symptom severity (Children's Yale-Brown Obsessive Compulsive Scal

Cognitive-Behavioral Treatment of Obsessive-Compulsive Disorder: The Results of a Naturalistic Outcomes Study.

Cognitive−behavioral therapy is a well-established treatment for obsessive−compulsive disorder (OCD). There are a variety of cognitive and behavioral strategies, and it is necessary to analyze the outcomes of the treatments. The aim of the present study is to verify the effectiveness of a treatment that combines evidence-based procedures and specific cognitive interventions highlighting the issue of acceptance. Forty patients with OCD were recruited and underwent a specific treatment procedure. All patients had a psychodiagnostic assessment for OCD using the Y−BOCS (Yale−Brown obsessive−compul

Neurobiological outcomes of cognitive behavioral therapy for obsessive-compulsive disorder: A systematic review.

INTRODUCTION: Obsessive-compulsive disorder (OCD) is characterized by recurrent distressing thoughts and repetitive behaviors, or mental rituals performed to reduce anxiety. Recent neurobiological techniques have been particularly convincing in suggesting that cortico-striatal-thalamic-cortico (CSTC) circuits, including orbitofrontal cortex (OFC) and striatum regions (caudate nucleus and putamen), are responsible for mediation of OCD symptoms. However, it is still unclear how these regions are affected by OCD treatments in adult patients. To address this yet open question, we conducted a syste

The prevalence of impulsive compulsive behaviors in patients treated with apomorphine infusion: a retrospective analysis.

BACKGROUND: Impulsive compulsive behaviors (ICBs) can affect a significant number of Parkinson's disease (PD) patients. OBJECTIVE: We have studied brain samples from a brain bank of PD patients who received apomorphine via continuous infusion in life to assess the prevalence and outcome of ICBs. METHODS: A search on the Queen Square Brain Bank (QSBB) database for cases donated from 2005 to 2016 with a pathological diagnosis of idiopathic PD was conducted. Notes of all donors who used apomorphine via continuous infusion for at least three months were reviewed. Clinical and demographic data we

Cerebellin-2 regulates a serotonergic dorsal raphe circuit that controls compulsive behaviors.

Cerebellin-1 (Cbln1) and cerebellin-2 (Cbln2) are secreted glycoproteins that are expressed in distinct subsets of neurons throughout the brain. Cbln1 and Cbln2 simultaneously bind to presynaptic neurexins and postsynaptic GluD1 and GluD2, thereby forming trans-synaptic adhesion complexes. Genetic associations link cerebellins, neurexins and GluD's to neuropsychiatric disorders involving compulsive behaviors, such as Tourette syndrome, attention-deficit hyperactivity disorder (ADHD), and obsessive-compulsive disorder (OCD). Extensive evidence implicates dysfunction of serotonergic signaling in

Neurocognitive assessment in obsessive compulsive disorder patients: Adherence to behavioral decision models.

In economics, models of decision-making under risk are widely investigated. Since many empirical studies have shown patterns in choice behavior that classical models fail to predict, several descriptive theories have been developed. Due to an evident phenotypic heterogeneity, obsessive compulsive disorder (OCD) patients have shown a general deficit in decision making when compared to healthy control subjects (HCs). However, the direction for impairment in decision-making in OCD patients is still unclear. Hence, bridging decision-making models widely used in the economic literature with mental

Persistent Complications of Child Sexual Abuse: Sexually Compulsive Behaviors, Attachment, and Emotions.

Child sexual abuse has the potential to cause distress for the victim across the lifespan. Romantic relationships may be particularly difficult for victims of child sexual abuse. This retrospective study examined differences in adult romantic attachment, sexually compulsive behaviors, and emotion regulation by history of child sexual abuse in a large, nonclinical sample. Those with a history of child sexual abuse reported more attachment anxiety in romantic relationships and engaged in more sexually compulsive behaviors. Overall, males displayed more sexually compulsive behaviors than females

From impulses to maladaptive actions: the insula is a neurobiological gate for the development of compulsive behavior.

Impulsivity is an endophenotype of vulnerability for compulsive behaviors. However, the neural mechanisms whereby impulsivity facilitates the development of compulsive disorders, such as addiction or obsessive compulsive disorder, remain unknown. We first investigated, in rats, anatomical and functional correlates of impulsivity in the anterior insular (AI) cortex by measuring both the thickness of, and cellular plasticity markers in, the AI with magnetic resonance imaging and in situ hybridization of the immediate early gene zif268, respectively. We then investigated the influence of bilatera

Proficient motor impulse control in Parkinson disease patients with impulsive and compulsive behaviors.

BACKGROUND: Parkinson disease (PD) patients treated with dopamine agonist therapy can develop maladaptive reward-driven behaviors, known as impulse control disorder (ICD). In this study, we assessed if ICD patients have evidence of motor-impulsivity. METHODS: We used the stop-signal task in a cohort of patients with and without active symptoms of ICD to evaluate motor-impulsivity. Of those with PD, 12 were diagnosed with ICD symptoms (PD-ICD) and were assessed before clinical reduction of dopamine agonist medication; 12 were without symptoms of ICD [PD-control] and taking equivalent dosages o

Rigid-compulsive behaviors are associated with mixed bowel symptoms in autism spectrum disorder.

Based on clinical experience, we hypothesized that rigid-compulsive behaviors are associated with severe constipation and co-occurring diarrhea or underwear staining in children with autism spectrum disorder. Using data from the Autism Treatment Network, we evaluated the association between these gastrointestinal symptoms and measures of rigid compulsive behavior in children ages 2-17. Following statistical correction, four of five primary measures were significantly associated with constipation and diarrhea or underwear staining, including parental report of repetitive behavior, parental repo

Impulsive and compulsive behaviors in Parkinson's disease: a one-year follow-up study.

Impulsive and compulsive behaviors (ICBs) have been reported to occur frequently in Parkinson's disease (PD) and include impulse control disorders (ICDs), punding and dopamine dysregulation syndrome (SSD). We report on the outcomes of 25 PD patients who developed ICBs. Information was collected on changes in parkinsonian and psychiatric medication follow-up (median=12.1 months). At time 1, only 18 patients (72%) were taking dopamine agonists (DA). At time 2, fifteen patients (83.33%) either discontinued or decreased their DA treatment. Of these patients, thirteen (86.67%) reported experiencing

Conditioned accumbal dopamine transients forecast individual preference for drug versus natural rewards and compulsive behavior.

Addicted individuals prefer drug rewards over natural rewards. Dysregulated dopamine (DA) signaling in the nucleus accumbens is thought to trigger this phenomenon, but there is little supporting evidence. Using a genetically encoded sensor, we monitored DA dynamics in mice in an operant associative task with natural (fat solution) or artificial (cocaine or optogenetic DA self-stimulation) rewards. During learning, DA transients emerged at predictive cues for both reward types, vanished at natural reward delivery, but persisted at artificial reward delivery. In choice sessions, animals' prefere

A randomized clinical trial of app cognitive behavior therapy vs. HealthWatch for obsessive compulsive disorder.

App-based cognitive behavioral therapy (app-CBT) addresses many treatment barriers. We randomized 120 U.S. adults with primary OCD (82.4% female, 77.5% White, mean age 40.1) between 7/2021 and 7/2023 to 12-weeks of coach-guided Perspectives (n = 60; app-CBT) or coach-guided HealthWatch (n = 60; an online well-being program). OCD severity (Yale-Brown Obsessive Compulsive Scale; primary outcome) analyzed using a pre-registered mixed linear model showed a trend level group difference at post-treatment (ES = -0.47; p = 0.079). However, results from secondary and sensitivity analyses showed signifi

An Unusual Compulsive Behavior Related to Scalp Psoriasis and Hair Follicle Anxiety in Young Men.

INTRODUCTION: Patients suffering from androgenetic alopecia may experience obsessive behaviors on observing their own scalp. CASE PRESENTATION: This article describes a series of three cases of young men presenting with an unusual, seemingly compulsive behavior related to mild scalp psoriasis and a family history of androgenetic alopecia. Contrary to the typical goal of psoriasis treatment, these patients exhibit an intense fixation on scalp scales containing embedded hairs. They actively avoid dislodging these scales during routine hygiene or therapeutic efforts, fearing that their removal m

Recommendations for Diagnosing and Quantifying treatment outcomes in clinical trials of compulsive sexual behavior disorder.

This article proposes minimum requirements for reporting efficacy in treatment studies of compulsive sexual behavior (CSB). CSB disorder (CSBD) is a condition whose diagnostic criteria were only recently defined by the World Health Organization. Multiple primary and secondary outcomes have been used in treatment trials of CSB, and possible neuropsychological measures have been considered. We suggest including clinical interviews, specifying the nature of the problem behavior and ensuring that the concerns are not entirely related to distress linked to moral judgments or paraphilic interests ex

Maximizing translational value in models of compulsive behavior: A commentary on Pickenhan et al. (2024).

In this issue of Cognitive, Affective, and Behavioral Neuroscience, Pickenhan et al. (2024) discuss the need for translational studies to understand features underlying obsessive-compulsive disorder (OCD). They highlight the translational value of the observing-response task (ORT) for modeling functional and maladaptive checking behaviors, a common symptom of OCD.
