Flavin Adenine Dinucleotide-Related Adverse Event Necessitating Cardiopulmonary Resuscitation in an Infant.

Mitochondrial rescue drugs, such as vitamin B2, are a treatment modality which can be considered in unexplained cases of cardiac arrest or impaired consciousness in which mitochondrial diseases are considered in the differential. Although case reports exist of children developing a drop in their blood pressure following administration of intravenous flavin adenine dinucleotide (FAD) sodium as vitamin B2, we present the first reported case of a child requiring cardiopulmonary resuscitation (CPR) following FAD sodium infusion for severe bradycardia and hypotension. Intravenous infusion of FAD so

Evaluation of the density spectral array in the Wada test: report of six cases.

Wada test is an invasive procedure used in the preoperative evaluation for epilepsy surgery to determine language lateralization, postoperative risk of amnesia syndrome, and to assess the risk of memory deficits. It involves injection of amobarbital into internal carotid artery of the affected hemisphere followed by the healthy hemisphere to shut down brain function. We performed an observational study evaluating the density spectral array (DSA) of the bilateral bispectral index VISTA™ Monitoring System (BVMS) in 6 patients with drug-resistant epilepsy undergoing Wada test. DSA revealed the pr

Reversible encephalopathy induced by cefoperazone: a case report monitored with EEG.

In patients with an impaired state of consciousness, EEG is fundamental, a correct neurological work-up. Cephalosporins have been identified as a case of triphasic waves' (TW) reversible encephalopathy. We report a case of an acute reversible encephalopathy with TWs during treatment with cefoperazone. We report the occurrence and regression of a confusional state with TWs encephalopathy at EEG after the administration of cefoperazone for urinary tract infection in a patient admitted for syncope. In conclusion, cefoperazone should be considered as a cause of toxic encephalopathy with EEG TWs, w

Episodic Memory Precision and Reality Monitoring Following Stimulation of Angular Gyrus.

The qualities of remembered experiences are often used to inform "reality monitoring" judgments, our ability to distinguish real and imagined events. Previous experiments have tended to investigate only whether reality monitoring decisions are accurate or not, providing little insight into the extent to which reality monitoring may be affected by qualities of the underlying mnemonic representations. We used a continuous-response memory precision task to measure the quality of remembered experiences that underlie two different types of reality monitoring decisions: self/experimenter decisions t

The classification and differential diagnosis of absence seizures with short-term video-EEG monitoring during childhood.

Absence seizures are idiopathic epilepsies characterized by impairment of consciousness and generalized 2.5-4 Hz spike and slow wave discharges. This prospective study was performed to classify and define properties of subgroups of absence epilepsies. We included 31 patients, of whom seven were in the differential diagnosis group. On admission, absence epilepsy provisional diagnosis was considered in 16 patients clinically and in the other 15 patients based on routine EEG findings. Ictal EEGs were recorded by video-EEG monitoring in 23 of the patients (totally 202 ictal recordings). Patients w

Plasma Carbamazepine and Carbamazepine-Epoxide Concentrations as Indicators of Consciousness Impairment in Acute Poisoning: A Sex-Based Analysis.

BACKGROUND: Carbamazepine (CBZ) is a widely used antiepileptic drug with a narrow therapeutic window. Its potential for severe toxicity in overdose is primarily due to its active metabolite, carbamazepine-10,11-epoxide (CBZ-E). In acute CBZ intoxication, sex-specific differences and toxicokinetics are poorly understood, especially the impact on neurological impairment, despite its clinical relevance. OBJECTIVES: The present study aimed to explore the clinical, biochemical, and toxicokinetic data of patients with acute CBZ poisoning to assess the relationship between CBZ exposure and the level

Decreased Thalamic Activity Is a Correlate for Disconnectedness during Anesthesia with Propofol, Dexmedetomidine and Sevoflurane But Not S-Ketamine.

Establishing the neural mechanisms responsible for the altered global states of consciousness during anesthesia and dissociating these from other drug-related effects remains a challenge in consciousness research. We investigated differences in brain activity between connectedness and disconnectedness by administering various anesthetics at concentrations designed to render 50% of the subjects unresponsive. One hundred and sixty healthy male subjects were randomized to receive either propofol (1.7 μg/ml; n = 40), dexmedetomidine (1.5 ng/ml; n = 40), sevoflurane (0.9% end-tidal; n = 40), S-keta

Unintentional fatal toxicity due to nicotine chewing gum: A case report.

RATIONALE: Since tobacco contains numerous harmful substances, besides nicotine, which is addictive, smoking cessation products and tobacco alternatives, such as electronic (e-) cigarettes, nicotine chewing gums, and patches, are being widely used. Nicotine gums are consumed orally. The nicotine from the gum is absorbed at a slower rate than that from e-cigarettes, and the former remains in the bloodstream for a longer period. In addition, the maximum number of daily doses is high, and it can be purchased without a doctor's prescription in many countries. PATIENT CONCERNS: A 29-year-old male

Pediatric delirium is associated with increased brain injury marker levels in cardiac surgery patients.

Despite global consensus on the importance of screening pediatric delirium, correlations between pediatric delirium during acute brain injury and adult delirium are unclear. Therefore, we hypothesized that similar pediatric biomarkers reflect acute brain injury as in adult delirium. We observed pediatric cardiac surgery patients from neonatal age to 18 years, who were admitted to our pediatric intensive care unit after cardiovascular operations between October 2019 to June 2020, up to post-operative day 3 (4 days total). We recorded age, sex, risk score (Risk Adjustment in Congenital Heart Sur

General Anesthesia for a Patient With Pelizaeus-Merzbacher Disease.

We report the successful management of general anesthesia for a patient with Pelizaeus-Merzbacher disease (PMD). PMD is one of a group of progressive, degenerative disorders of the cerebral white matter. The typical clinical manifestations of PMD include psychomotor retardation, nystagmus, abnormal muscle tone, seizures, and cognitive impairment. General anesthesia for a patient with PMD may be difficult mainly because of seizures and airway complications related to poor pharyngeal muscle control. In addition, the possibility of exacerbation of spasticity should be considered. A 20-year-old ma

Unveiling clouded consciousness: Broad-band EEG slowing tracks recovery from post-traumatic confusional state.

BACKGROUND: Post-traumatic confusional state (PTCS) frequently occurs during the recovery from disorders of consciousness (DoC) following severe traumatic brain injury (TBI). Confusional symptoms span multiple domains influencing consciousness, including impairments in the access and integration of mental contents, distortions in perceptual and emotional experiences, vigilance fluctuations, and deficits in memory, orientation, and executive control. While the clinical presentation can be systematically characterized using the Confusion Assessment Protocol (CAP), the underlying neurophysiologic

Safety, tolerability and subjective effects of vaporized N,N-Dimethyltryptamine: A randomized double-blind clinical trial.

Vaporized N,N-Dimethyltryptamine (DMT) has a short duration and simple application, prompting this technique as a new approach for psychedelics' clinical use. Building on our initial dose-ascending studies and addressing potential confounding effects of dosing order, this study is the first to evaluate inhaled DMT in a randomized, placebo-controlled, double-blind design. Given DMT's therapeutic potential, we aimed to assess its safety, tolerability, and subjective effects compared to an active placebo. Twenty-five healthy participants completed two treatment sessions, receiving DMT (60 mg) or

The Effects of Depth of Anesthesia on Muscle-Recorded Motor Evoked Potentials: A Prospective Observational Study.

BACKGROUND: Intraoperative neurophysiological monitoring is used to assess neurological function during surgeries placing the spinal cord at risk. Transcranial electrical stimulation muscle motor evoked potentials (Tc-mMEPs) are used to monitor motors tracts, but their interpretation is complicated by the large temporal variability which can result in false-positive warnings. Although the choice of anesthetic drug regimen and drug dose are often claimed to be factors causing this variability, the relationship between depth of anesthesia, quantified by processed electroencephalogram (pEEG) para

The Association of Physiological and Pharmacological Anesthetic Parameters With Motor-Evoked Potentials: A Multivariable Longitudinal Mixed Model Analysis.

BACKGROUND: During spinal surgery, the motor tracts can be monitored using muscle-recorded transcranial electrical stimulation motor-evoked potentials (mTc-MEPs). We aimed to investigate the association of anesthetic and physiological parameters with mTc-MEPs. METHODS: Intraoperative mTc-MEP amplitudes, mTc-MEP area under the curves (AUC), and anesthetic and physiological measurements were collected retrospectively from the records of 108 consecutive patients undergoing elective spinal surgery. Pharmacological parameters of interest included propofol and opioid concentration, ketamine and nor

Anti-N-methyl-D-aspartate receptor encephalitis associated with an ovarian teratoma: two cases report and anesthesia considerations.

BACKGROUND: Anti-N-methyl-D-aspartate receptor (NMDAR) encephalitis is an immune-mediated syndrome caused by the production of anti-NMDAR receptor antibodies. The syndrome characterised by psychosis, seizures, sleep disorders, hallucinations and short-term memory loss. Ovarian teratoma is the confirmed tumour associated with anti-NMDAR antibodies. The patients with anti-NMDAR encephalitis complicated by ovarian teratoma require surgical treatment under general anesthesia. NMDARs are important targets of many anesthetic drugs. The perioperative management and complications of anti-NMDAR encepha

[Perioperative management of off-pump CABG for a 93-year-old man].

In recent years, the aging population has been growing, and the operative techniques and anesthetic methods have advanced. With these developments and medical support, the number of operations on very elderly patients has been increasing. We report the perioperative management of off-pump CABG for a 93-year-old man. When the heart was displaced during the operation, hypotension was induced and a marked reduction of his bispectral index (BIS) to "1" appeared. During the perioperative period, the patient developed delirium that was difficult to manage, but he was discharged from the hospital wit

Urea cycle defects and hyperammonemia: effects on functional imaging.

The urea-cycle disorders (UCDs) are a group of congenital enzyme and carrier deficiencies predisposing to hyperammonemia (HA). HA causes changes in the central nervous system (CNS) including alterations of neurotransmitter function, cell volume, and energy deprivation ultimately leading to cerebral edema. Neuropathological findings of UCDs primarily reflect changes in astrocyte morphology. Neurological features accompanying acute HA include changes in behavior and consciousness in the short term, and potential for impairments in memory and executive function as long-term effects. Early EEG Detection of Treatable Non-Convulsive Status Epilepticus in Severe COVID-19-Associated Encephalopathy: A Case Report.

BackgroundPersistent depressed consciousness in severe coronavirus disease 2019 (COVID-19) is commonly attributed to hypoxemia, systemic inflammation, sedative exposure, or metabolic disturbance. Non-convulsive status epilepticus (NCSE), however, may be overlooked without electroencephalography (EEG). This diagnostic principle also applies to persistent unexplained encephalopathy more broadly, particularly when impaired consciousness remains disproportionate to systemic illness.CaseAn 85-year-old woman with Parkinsonism, diabetes mellitus, and prior cerebrovascular disease developed severe COV

Binary Classification of Consciousness Using Cerebral Blood Flow and EEG Features.

BACKGROUND: Assessing consciousness at the bedside in the neurocritical care unit is complicated by sedation and other treatment effects. While EEG is commonly used, it offers a limited view of the neurovascular unit. We evaluated whether combining cerebral blood flow (CBF) features with EEG improves binary classification of consciousness in patients with severe brain injury. METHODS: We retrospectively analyzed 35 adults who underwent multimodal neuromonitoring. Signals were segmented into 30-min windows after each probe recalibration. We used parameters including CBF low-frequency bands (Ba
