Baylor multipurpose circulatory support system for short- to long-term use.

A multipurpose circulatory support system has been developed as both a temporary and permanent device in total artificial hearts (TAHs) and ventricular assist devices (VADs). The multipurpose concept was derived from the development of a totally implantable electromechanical, one-piece TAH. The blood pump is pneumatically driven in short-term use and is electromechanically driven in long-term or permanent use. Both TAH and VAD versions consist of the same components, except for the actuation mechanism. The common components are a compact pumping chamber with the same configuration, a blood con

Full-Support LVAD Implantation in a C-Pulse Heart Assist System Recipient with Deteriorating Chronic Heart Failure: Is It Feasible and Safe?

Heart failure is a progressive disease with limited treatment options. The C-Pulse Heart Assist System (Eden Prairie, MN) is an extravascular, diastolic counterpulsation circulatory support device for patients with refractory NHYA Class III/ambulatory class IV heart failure. It comprises a balloon-cuff which is implanted around the ascending aorta that is synchronised to inflate during ventricular diastole. The system eliminates the need for systemic anticoagulation and significantly reduces the risk of bleeding and overcomes the problem of device thrombosis. However, clinical efficacy is depe

Echocardiography in experimentally-induced myocardial ischemia.

Using an experimental canine model, we have employed echocardiography to study segmental dyskinesis produced by acute coronary occlusion. Characteristic alterations in posterior wall and septal motion occur after acute coronary ligation; these resemble clinical abnormalities described in acute and old myocardial infarction. The degree of dyskinesis produced is directly related to the severity of the perfusion deficit. Coronary reperfusion had variable effects on dyskinesis; most animals showed improvement but in some the myocardial contraction abnormalities became more severe. A vaviety of int

Anatomical advantage to percutaneous insertion of the intra-aortic balloon through the left brachial artery over the right brachial artery.

Off-label use of the intra-aortic balloon (IAB) is not recommended in ideal situations and certainly not a Food and Drug Administration-approved activity. The instruction-for-use manual for the IAB recommends percutaneous insertion. However, there are certain extreme situations where "thinking outside the box" appears necessary. We have successfully inserted a transthoracic IAB (TIAB) in the operating room where an open sternum is an option. This has been instituted whenever severe peripheral vascular disease (PVD) precludes a percutaneous attempt or when attempted insertion fails. An open che

BioMedicus ventricular assistance for postcardiotomy heart failure: evaluation of univentricular assistance.

Clinical outcome and hemodynamic effects of unilateral mechanical ventricular support (UMVS) were evaluated in 19 patients with postcardiotomy heart failure refractory to conventional treatment. Adequate circulation with UMVS was maintained in about 75% of the patients. UMVS initiated circulatory stabilization in 5 of 6 patients with biventricular failure, in 2 of 3 patients with right ventricular failure, and in 7 of 10 patients with left ventricular failure. Eight (42%) patients were successfully weaned from UMVS and discharged from hospital. Six (32%) patients died despite a prolonged, stab

Transthoracic intraaortic balloon pump support: experience in 39 patients.

From June, 1982, through July, 1985, 39 (1.5%) of 2,570 patients undergoing open-heart procedures required insertion of a transthoracic intraaortic balloon pump (IABP). In all of these patients, a percutaneous attempt failed or was contraindicated. There were 24 (62%) men and 15 (38%) women. The mean age was 64.9 years. Five patients (13%) sustained complications potentially related to the transthoracic IABP. They included mediastinal infection in 1 patient (2.5%), balloon rupture in 2 patients (5%), and cerebrovascular accidents in 4 patients (10%). The overall survival was 44%. Survival for

Enhanced external counter pulsation as a novel therapy to maintain cardiac output during hemodialysis: a preliminary randomized controlled study.

Intradialytic cardiac output (CO) decline normally occurs during hemodialysis (HD) and results in short-term intradialytic hypotension to longer-term increased cardiovascular morbidity and mortality in chronic HD patients. Enhanced external counter pulsation (EECP) is a novel non-invasive device that has been demonstrated to improve coronary blood flow and maintain systemic hemodynamics in patients without kidney dysfunction. This study is the first to explore the efficacy and safety of EECP application during HD on intradialytic changes of CO and other hemodynamic parameters. Stable chronic H

Case Report: Intra-aortic balloon pump in a patient with refractory cardiogenic shock complicating severe aortic stenosis-enhanced hemodynamic response with low aortic compliance.

Intra-aortic balloon pump (IABP) counterpulsation generates pressure changes that are primarily influenced by aortic compliance, an index of arterial elasticity that varies widely among patients with cardiovascular disease. However, the potential role of aortic compliance in determining IABP efficacy remains poorly understood. We present the case of an 80-year-old man with severe aortic stenosis and pneumonia who was admitted with generalized fatigue and worsening dyspnea. He developed refractory Society for Cardiovascular Angiography & Interventions stage D cardiogenic shock despite multiple

Latest STEMI treatment: a focus on current and upcoming devices.

INTRODUCTION: Ischemic heart disease is the most common cause of mortality worldwide. In ST elevation myocardial infarction (STEMI) patients, optimization of primary percutaneous coronary intervention (pPCI) is crucial to improve myocardial savage and prevent reperfusion injury. In STEMI patients complicated with cardiogenic shock (CS), percutaneous mechanical circulatory support devices (MCS) have emerged as a therapeutic option in the management of this challenging condition and their use is rapidly increasing. AREAS COVERED: In this review we discuss how pPCI can be optimized with respect

Assisted circulation in postcardiotomy heart failure: experience with the Bio-Medicus centrifugal pump in ten patients.

Between January 1990 and September 1992, ten patients required assisted circulation for postcardiotomy heart failure which was unresponsive to inotropic drugs and aortic counterpulsation. All patients were supported by a Bio-Medicus centrifugal pump (biventricular assist in five, left ventricular in three, right ventricular in two); six had ischaemic heart disease, two a congenital ventricular septal defect, one an acute mitral valve incompetence and one an aortic type 'A' dissection. The mean duration of circulatory support was 5.1 days (range 2 hours to 8 days). Six patients were weaned from

Hemodynamic effects of sulmazol (ARL-115 BS), a new vasodilator and positive inotropic agent, in patients with cardiogenic shock.

The imidazol-pyridine ARL-115 BS (sulmazol) has both positive inotropic and vasodilatory effects. Its hemodynamic effects were studied in 13 patients with shock after myocardial infarction. All patients required dobutamine or dopamine, while nine were treated with intra-aortic balloon counterpulsation. After a loading dose of 50 mg, sulmazol was administered at 50 mg/h followed by 100 mg/h during 30 or 60 minutes. At the highest dose of sulmazol, cardiac output increased from 4.3 +/- 1.1 to 4.9 +/- 1.5 1/min; systemic vascular resistance decreased from 1405 +/- 473 to 1228 +/- 439 dynes . s .

Emergent TandemHeart-ECMO for acute severe mitral regurgitation with cardiogenic shock and hypoxaemia: a case series.

BACKGROUND: Acute severe mitral regurgitation (MR) associated with cardiogenic shock is a life-threatening emergency. Traditional teaching has focused on the need for emergent coronary angiography and/or intra-aortic balloon counterpulsation in preparation for emergent open-heart surgery for repair/replacement. Unfortunately, emergent open-heart surgery in patients with acute MR complicated by cardiogenic shock is associated with 25-46% perioperative mortality. New devices have provided additional options for stabilization prior to emergent surgery which facilitate improved outcomes. CASE SUM

New therapies for cardiogenic shock.

Cardiogenic shock is a serious complication of acute myocardial infarction and occurs as a consequence of acute left ventricular failure and subsequent inappropriate tissue perfusion. While its incidence has been reported to be as high as 10% in the late 90ties, it has been steadily declining to currently less than 6% since the implementation of early revascularization as a class I indication in current guidelines. Despite recent advances in the treatment of cardiogenic shock, mortality is still high at approximately 50%. Current therapeutic approaches include early revascularization, fluid re

Hemodynamically complicated ST-segment elevation myocardial infarction: presentation and treatment.

When ST-segment elevation myocardial infarction is complicated by hemodynamic instability, clinical evaluation alone cannot be relied upon for accurate assessment. Invasive hemodynamic monitoring with a pulmonary artery catheter and intra-arterial catheter should, therefore, be performed to gauge impaired hemodynamics in such patients and make necessary changes in therapy. There are several hemodynamic disturbances that can occur after ST-segment elevation myocardial infarction including pulmonary congestion, cardiogenic shock and mechanical complications such as left-ventricular free-wall rup

Safety and feasibility of elective high-risk percutaneous coronary intervention procedures with left ventricular support of the Impella Recover LP 2.5.

Currently, the most used left ventricular (LV) support device is intra-aortic balloon counterpulsation. The percutaneous implantable Impella Recover LP 2.5 system is a novel LV (unloading) assist device. We studied the feasibility and safety of LV support with the percutaneous implantable Impella Recover LP 2.5 system in 19 consecutive high-risk patients with percutaneous coronary intervention. Procedural success using the device and percutaneous coronary intervention was achieved in all 19 patients, who were very poor candidates for surgery. The patients were elderly (84% were >60 years of ag

Left atrial-to-aortic assistance with in-line left ventricular venting.

Left atrial-to-aortic assistance (LA-A) has been employed for left ventricular failure (LVF) refractory to intra-aortic balloon counterpulsation. Although clinical success has been reported, LA-A is limited by incomplete left ventricular (LV) decompression. We have developed a method to augment LA-A with an in-line LV apical vent (LA-lv-A). LVF was produced in 12 dogs by ligation of the circumflex coronary artery. Circulatory assistance was established with LA (28-32 Fr), LV apex (12-14 Fr) and femoral artery cannulation and a centrifugal pump. LA-A assistance was compared to LA-lv-A. Left atr

Veno-arterial extracorporeal membrane oxygenation (VA ECMO) in postcardiotomy cardiogenic shock: how much pump flow is enough?

Post-cardiotomy cardiogenic shock (PCCS) is a complication of heart surgery associated with a poor prognosis: veno-arterial extracorporeal membrane oxygenation (VA ECMO) ensures end-organ perfusion while fully replacing heart and lung function, though it is associated with unsatisfactory results. Few studies have identified reliable predictors of poor prognosis early in the course of extracorporeal support. A recent study showed the strong prognostic power of urine output in the first 24 hours of VA ECMO in predicting early and late mortality of PCCS. Urine output is a commonly collected param

Chronic effect of left ventricular assist pumping on right ventricular function.

We evaluated right ventricular (RV) function during left ventricular assist device (LVAD) pumping during acute and chronic experiments in both normal hearts and hearts with left ventricular failure. An LVAD was implanted between the left atrium and the descending aorta of goats. The LVAD was pumped in a counterpulsation mode and at maximum bypass flow for 4-6 weeks in chronic experiments. The following results were obtained: 1) LVAD pumping itself did not affect RV function, even after 6 weeks of pumping, 2) the dimensions of the RV free-wall segments measured by sonomicrometers showed a gradu

Supported percutaneous coronary intervention using a novel 6-Fr intra-aortic balloon pump catheter via the brachial artery in a nonagenarian patient with an abdominal aortic aneurysm.

Intra-aortic balloon pump (IABP) counterpulsation is a useful hemodynamic assist device during complex percutaneous coronary intervention (PCI) in patients with poor left ventricular function; however, the presence of an abdominal aortic aneurysm poses a problem, because insertion via the femoral artery may cause distal embolism and aneurysm rupture. A 92-year-old man with unstable angina was admitted to our hospital. Coronary angiography revealed chronic total occlusion of the proximal left anterior descending artery and severe stenosis of the left circumflex artery (LCX). The left ventricula
