A Comparison of Otolaryngology Training in Five English-Speaking Countries.

Otolaryngology was conceived at the turn of the twentieth century as a product of the amalgamation of the separate disciplines of the primarily surgeon-led otology and physician-led laryngology. Since its conception, otolaryngology has flourished and continues to, due to the many advancements in medical technologies. We aim in this review to provide a comparison between the postgraduate training pathway for otolaryngology in five English-speaking countries, highlighting the main differences, strengths, and drawbacks of each pathway. This, we hope, will be able to guide future changes in the tr

Strategies for incorporating indigenous placenta disposal methods in public healthcare: A Delphi study in Tshwane District, Gauteng, South Africa.

BACKGROUND: Indigenous placenta disposal strategies are essential to bridge clinical protocols with indigenous worldviews, allowing for safe and respectful handling without compromising health standards. Using Appreciative Inquiry in the Dream and Design phases, stakeholders collaboratively developed draft strategies that honour indigenous values while aligning with clinical protocols. These strategies further need inputs from a wider group of stakeholders to ensure that they fit both hospital rules and indigenous traditions. AIM: To refine and validate the draft strategies that incorporate i

Humanitarian Assistance Lessons Learned From Providing Acute Medical Care for Afghan Nationals on a U.S. Military Base.

Operation Allies Welcome provided a unique opportunity for military medical personnel to engage in humanitarian assistance operations on military bases in the USA. With thousands of Afghan nationals evacuated from Kabul in August 2021 to various military installations across the USA, the Military Health System was tasked with health screening, emergency care, and disease prevention and surveillance in resource-limited settings. Marine Corps Base Quantico served as a "safe haven" site from August to December 2021, providing refuge to nearly 5000 travelers awaiting resettlement. During this time

A Cross-Sectional Survey on Professionals to Assess Health Needs of Newly Arrived Migrants in Spain.

Heightened conflicts and lack of safety due to reasons related to economic, social, ethnic, religious, sexual orientation, political, or nationality matters have increased migratory movements during the last, few decades. Unfortunately, when migrants arrive in new territories, they can face many barriers. For example, in Spain, some migrants have difficulties in accessing health services. The main objective of this study was to describe, from the perspective of social and healthcare professionals, health needs and barriers faced among migrants who recently arrived in Spain when accessing the h

African American Communities Speak to Clinician 1. Program Creation and Implementation Feasibility.

CONTEXT: Lack of appreciation of cultural differences compromises care for seriously ill African American patients, yet training programs for clinicians in providing culturally appropriate care is lacking. The goals of this study were to partner with a southern, African American community to create a training program for clinicians on how to communicate in a culturally congruent and respectful way with older southern, African Americans with serious illness and at end of life, and test program delivery feasibility. METHODS: Community Based Participatory Research guided the equitable partnershi

Equity, inclusion and cultural humility: contemporizing the neonatal intensive care unit family-centered care model.

Existing NICU family centered care models lack the key elements of equity, inclusion and cultural humility. These models were conceived to support families during the stressful life event of an infant's NICU admission. Their development, however, occurred prior to recognition of the medical field's systematic shortcomings in providing equitable care and their impact on outcome disparities for marginalized communities; thus, they do not include cultural or equitable healthcare considerations. Given the significant neonatal care inequities for marginalized groups, incorporating the experience of

International and cross-cultural issues: six key challenges for our professions.

This paper discusses a number of key challenges facing professionals who work with people who have communication disabilities. The challenges are related to working internationally and in a cross-culturally competent way. Each challenge is discussed, drawing on material from a range of sources, including the papers in the final 2003 edition of FOLIA PHONIATRICA ET LOGOPAEDICA ('International and Cross-Cultural Issues'). The challenges discussed cover how experiences from majority world countries can be used to benefit services elsewhere, 'mainstreaming' commitment to cross-cultural competence,

Development and validation of the mental health professional culture inventory.

AIMS: No instrument has been developed to explicitly assess the professional culture of mental health workers interacting with severely mentally ill people in publicly or privately run mental health care services. Because of theoretical and methodological concerns, we designed a self-administered questionnaire to assess the professional culture of mental health services workers. The study aims to validate this tool, named the Mental Health Professional Culture Inventory (MHPCI). The MHPCI adopts the notion of 'professional culture' as a hybrid construct between the individual and the organisat

Comparing neuropsychological and psychiatric evaluation of competency in rehabilitation: a case example.

This report describes the case of a 20-year-old man who sustained a gunshot wound to the forehead, resulting in traumatic brain injury and C2 ventilator-dependent quadriplegia. Issues of personal control and autonomy typified his psychological adjustment. The question of competency to refuse medical treatment arose when he disallowed intervention for a suspected ear infection not confirmed by culture. Subsequently, the patient was alternately described as incompetent by a psychiatrist and competent by a medical ethics review panel. These decisions are interpreted within the context of existing

Equity in concussion assessment: Aboriginal and Torres Strait Islander peoples' perspectives of standardised sport-related concussion assessment.

BACKGROUND: The Sport Concussion Assessment Tool (SCAT) is widely used to evaluate suspected concussion, yet its cultural suitability for Aboriginal and Torres Strait Islander peoples remains underexplored. METHODS: Guided by a social constructionist framework, this study examined perspectives on the SCAT5 through semi-structured yarns with community members in Perth, Western Australia. RESULTS: Reflexive thematic analysis of data identified concerns related to medical jargon, inconsistent terminology, reliance on written instructions and overall readability of the SCAT5. Centring Aboriginal and Torres Strait Islander Voices: A Framework for Culturally Safe and Racism-Free Healthcare.

Culturally safe, racism-free healthcare is essential in Australia, yet Aboriginal and Torres Strait Islander people continue to face inequitable access, experiences and outcomes. Although cultural safety is widely cited, implementation remains shallow and disconnected from patients' realities. We argue that meaningful cultural safety requires Aboriginal and Torres Strait Islander governance, robust measurement and institutional accountability. Evidence shows that culturally unsafe care, including racism, creates psychological distress, leads to care avoidance and results in poorer outcomes, wh

Beyond the classroom: The transformative experience of short rural immersion programs for health professional students: A narrative review.

BACKGROUND: Rural areas house nearly half the global population yet face a significant shortage of skilled health professionals, exacerbating health inequities. Short-term rural immersion programs offer a cost-effective approach to exposing health professional students to the unique challenges of rural healthcare. Transformative learning theory is well suited to examining how these programs foster critical reflection and perspective shifts in students. OBJECTIVES: To explore the potentially transformative impact of short-term (<6 weeks) rural immersion programs on health professional students

Providing culturally sensitive sexual medicine for persons with disabilities: a scoping review.

INTRODUCTION: Persons with physical disabilities (PWD) are a health care disparity group who face both difficulties accessing and inequalities in care. Although their sexual health is impacted by their disability and important to them, they often receive no or inadequate sexual health care. To address this gap, we sought to summarize evidence-based recommendations for culturally sensitive sexual health care for PWD. OBJECTIVES: The aim of this scoping review was to identify patient-reported preferences for culturally sensitive sexual health care for people with Parkinson's Disease, Multiple S

Unusual case of coexistent pulmonary cryptococcosis and tuberculosis in an immuno-competent host.

Coexistence of pulmonary cryptococcosis with other infections has commonly been described in immuno-suppressed individuals. In immuno-competent hosts, such coexistence is rare and mostly described in disseminated disease or uncommonly involving different sites. The simultaneous coinfection of cryptococcosis and tuberculosis of lung in an immuno-competent host is extremely rare with only one previously reported case in the literature. This is the second such case and the first to be reported in India. We describe a case of a 36-year-old immuno-competent male who presented with haemoptysis and c

IMD/ADM21-47, a factor that improves embryo quality.

Starting in vitro fertilization process with competent oocytes that may endure first cellular divisions is a critical step for obtaining an embryo. To obtain in vitro competent oocytes, culture conditions should emulate the in vivo microenvironment as close as possible. With the aim of improving the in vitro culture medium, the present study evaluated the IMD/ADM21-47 peptide as a factor that promotes oocyte competence and improves embryo quality in bovine systems. The culture supplemented with 153 μg/mL of IMD/ADM21-47 was correlated with the production of healthy oocytes in metaphase II (MII

Culturally appropriate sexual health interventions for STBBI and HIV among racialised immigrant communities in Western nations: a scoping review protocol.

INTRODUCTION: Racialised immigrant communities in Western nations face disproportionate risks for sexually transmitted and blood-borne infections (STBBIs) due to systemic barriers, including racism, stigma and limited access to culturally appropriate care. While the need is well-established, a comprehensive synthesis of effective, culturally responsive sexual health interventions is lacking. This scoping review aims to map the available evidence on sexual health intervention needs and protective factors of racialised immigrants, and to identify and describe existing culturally appropriate prog

Overreliance on cultural doulas: the paradox of entrusting the communication and care of high-risk migrant women to cultural doulas.

BACKGROUND: It is widely recognized that migrant women from low-income countries are considered to be a group with increased obstetric challenges. To address these challenges, cultural doulas were introduced to provide continuous emotional and practical support during childbirth in Sweden. Leveraging their shared cultural background, language skills, and understanding, the idea behind these doulas was supposed to facilitate effective communication between the woman, her partner, and healthcare staff, with the assumption that this would lead to better maternity care for migrants. The aim of thi

Cultural adaptation considerations of a comprehensive housing outreach program for Indigenous youth exiting homelessness.

Generalist health interventions that aim to reduce chronic health disparities between Indigenous and non-Indigenous populations can be culturally adapted to better meet the needs of Indigenous people in Canada; however, little is known regarding best practices in implementing these adaptations. The present study first provides a review of the research process used to adapt a previous evidence-based housing initiative for Indigenous youth in Northwestern Ontario. Second, it includes an overview of the adaptations that were made and the associated rationale for such adaptations. Third, it examin

Developing Physician Educational Competencies for the Management of Female Genital Cutting: A Call to Action.

Female genital cutting (FGC), also called female genital mutilation, is defined as "all procedures involving partial or total removal of the external female genitalia or other injury to the female genital organs whether for cultural, religious, or other nontherapeutic reasons." FGC can have significant health consequences, including multiple physical and psychological complications, throughout the life course. Despite violating numerous human rights and existing legal prohibitions, the practice continues. In the United States, FGC is becoming increasingly prevalent, however, physicians are not
